In vitro experimental study on flow characteristics of abdominal aorta aneurysm (복부대동맥 동맥류의 유동특성에 관한 in vitro 실험적 연구)

  • Hemodynamic features of blood flow in the abdominal aorta aneurysm (AAA) are very important, because they are closely related with the rupture of aneurysm to death. It has been considered that the wall shear stress of blood flows influences the formation, growth, and rupture of AAA. On this account, it is important to understand the flow structure of blood in the aneurysm. In this study, the whole velocity field information inside a typical AAA was measured using an in vitro AAA model under the pulsatile flow condition. The vessel geometry was reconstructed based on the computerized tomography (CT) data of a patient. The AAA model was made by using a rapid prototyping (RP) method, based on the reconstructed vessel geometry. Velocity fields in the AAA model were measured at different pulsatile phases using a PIV (particle image velocimetry) system. As experimental results, a large-scale vortex is formed inside the AAA model and the vortices located near the AAA wall are supposed to increase the local pressure and wall shear stress. In this study, the AAA wall stress found to be was one of the most important governing parameters giving rise to the ruptured aneurysm.

  • An efficient 3D finite element walking model that considers the detailed shapes of muscles, ligaments, bones, skin, and soles was developed based on a real computed tomography (CT) scan image of a foot, and nonlinear contact analyses were performed to investigate pressure changes. The highest pressure occurs at the rear bottom of the foot when standing and walking. The pressure on the outsole with a curved foot bottom surface is lessened and distributed over a wider area than in the case of a flat outsole. The result shows that a shoe sole shape optimized for diabetes patients can relieve the foot pressure concentration and prevent further worsening of symptoms.

  • Schwannomas of the jugular foramen, originating from the glossopharyngeal nerve, vagus and accessory nerve represent approximately 0.17-0.72% of all intracranial tumor, and consists of 1.4-2.9% of all intracranial schwannomas. The clinical presentation of these tumors varies significantly according to originated nerve and it's growth pattern. Magnetic resonance(MR) image and temporal bone computed tomography(CT) scan have a major role for diagnosis of such tumor. The treatment of choice is total resection whenever possible. Generally, suboccipital approach is sufficient for the removal of the tumor, but in case with large size, combination of resection of petrous part of temporal bone with or without transection of sigmoid sinus is may be necessory. We have recently experienced one case of giant jugular foramen schwannoma and postoperative fatal complication in a 34-year-old male who was treated with combined posterior petrous and suboccipital approach with transection of sigmoid sinus

Ga-67 SPECT Finding in Tuberculous Pericarditis with Mediastinal Mass: A case report (종격동 종괴를 수반한 결핵성 심낭염 1예의 Ga-67 SPECT 소견)

  • We present a case of a 31 year-old male patient with tuberculous pericarditis with mediastinal mass that showed increased uptake on Gallium-67 image. Gallium-67 scan was performed to evaluate the activity of the superior mediastinal mass, which was detected on chest CT. A rim of intense activity around the heart was observed, but increased uptake was not seen in the mediastinum. However, on maximal contrast-enhanced SPECT images, a small focus of faint uptake was observed in the superior mediastinum. This finding implied that there was an active tuberculosis in the pericardium and inflammation in the superior mediastinal mass. This case demonstrated that Gallium-67 scinitigraphy was helpful for the diagnosis of tuberculous pericarditis.

  • In plastic and reconstructive craniomaxillofacial surgery, careful preoperative planning is essential to get a successful outcome. Many craniomaxillofacial surgeons have used imaging modalities like conventional radiographs, computed tomography(CT) and magnetic resonance imaging(MRI) for supporting the planning process. But, there are a lot of limitations in the comprehension of the surgical anatomy with these modalities. Medical models made with rapid prototyping (RP) technique represent a new approach for preoperative planning and simulation surgery. With rapid prototyping models, surgical procedures can be simulated and performed interactively so that surgeon can get a realistic impression of complex structures before surgical intervention. The great advantage of rapid prototyping technique is the precise reproduction of objects from a 3-dimensional reconstruction image as a physical model. Craniomaxillofacial surgeon can establish treatment strategy through preoperative simulation surgery and predict the postoperative result.

  • PACS(Picture Archiving Communication System)를 바탕으로 하는 DICOM(Digital Imaging and Communications in Medicine)은 주요 의료영상장비들 사이에 데이터와 영상을 효율적으로 교환하고 전송할 수 있도록 마련한 표준안으로 현재 대부분의 최신형 의료영상장비(CT, MR, DSA, CR(Computed Radiology), 초음파검사, 핵의학검사, 내시경검사, 조직병리검사, 등)들은 의료 영상 분야의 국제 표준인 DICOM 표준방식에 의해 영상을 제공하고 있다. 최근 이러한 의료영상장비들은 독립적으로 사용하기보다는 의료수술 모니터링 장비 등의 비 의학영상장비와 서로 연계하여 사용하는 경우가 많아졌다. 그러나 이러한 의료수술 모니터링 장비들은 DICOM 표준 데이터를 고려하지 못하므로 PACS를 통한 데이터 연계에 어려움이 있다. 따라서 본 논문에서는 표준 DICOM 포맷과 의료수술 모니터링 장비의 데이터 구조를 분석하여 non-DICOM 의료수술 모니터링 장비의 PACS 연동을 위한 방안을 제안하였다.

Study in vivo metabolism using nuclear medical examination of the dose rate reduction (체내대사를 이용한 핵의학적 검사의 선량률 저감에 관한 연구)

  • The purpose of this study was administered to the body for examination and treatment, high-energy radioactive isotope(F-18, I-131) in vitro discharge experiments. Increasing exposed dose of radiation to health professionals is caused by the increase of PET/CT use and a radioactive isotope. Therefore, the high-energy isotope F-18 and I-131 after administration about using Metabolite excretion was studied. As a results of this study, patients had plenty of fluids for testing and treatment alone administered radiopharmaceuticals can be more than twice as fast excretion induced emissions. Also was able to get a better image space to reduce the dose rate.

  • Primary localized amyloidosis of the ureter and bladder is a very rare condition. CT and cystoscopic findings are indistinguishable from malignant neoplasm or inflammatory lesions. We report a case of unilateral distal ureteral thickening caused by primary localized amyloidosis. MR image features of hypointensity on T2-weighted images and mural enhancing pattern on contrast enhanced T1-weighted images can be helpful for diagnosis of ureteral amyloidosis.

Case Report of Facial Nerve Paralysis (안면신경마비의 치험례)

  • Facial nerve paralysis(or Bell's palsy) which commonly occurs unilaterally, gives rise to paralysis of facial expression muscle. This condition is classified into symptomatic facial nerve paralysis due to intracranial tumor, post operative trauma, etc. and idiopathic facial nerve paralysis. To explain the etiology of idiopathic facial nerve paralysis, many hypothesis including ischemic theory, viral infection, exposure to cold, immune theory etc. were suggested, but there is no agreement at this point. The method to evaluate the facial nerve paralysis, when it occurs, consists of three stage scale method, image thechnics like CT and MRI, laboratory test to examine the antibody titers of viral infection, neurophysiologic test to evaluate the degree and prognosis of paralysis. Treatment includes medication, stellate ganglion block(SGB), surgery, physical therapy and other home care therapy. In medication, systemic steroids, vitamins, vasodilating-drug and ATP drugs were used. SGB was also used repeatedly to attempt the improvement of circulation and to stimulate the recovery of nerve function. Physical therapy including electric acupuncture stimulation therapy(EAST) and hot pack was used to prevent the muscle atrophy. When No response was showed to this conservative therapies, surgery was considered. After treating two patients complaining of Bell's palsy with medication(systemic steroids) and EAST, favorable result was obtained. so author report the case of facial nerve paralysis.

Diagnosis and Treatment of Patient with Radiopaque Material in Temporomandibular Joint : Case Report (측두하악관절 내에 방사선 불투과성 물질을 가지 환자의 진단과 치료 : 증례보고)

  • A 69-year-old male pateint was admitted for discomfort on right temporomandibular joint during opening, closing and chewing that started few months ago. The patient had no special medical history except for lung tuberculosis approximately 30 years ago and nothing specific appeared on a physical exam taken 2 months ago. Clinical tests show that mouth opening of 53mm which was normal and no joint sound, deviation, pain during opening. But tenderness to palpation on Rt masseter muscle and pain existed on Rt temporomandibular joint during loading test on the right joint. No pain existed during resistance test and protrusion and range of lateral movement was normal. Rt temporomandibular joint was not swollen and no palpable mass was observed. No previous trauma history to the face existed. On X-ray calcific material existed in the joint cavity and on CT image, approximately 2mm sized calcific material appeared on the Rt temporomandibular joint but no change in bone appeared on the condyle nor the temporal bone. The patient was diagnosed as loose body, and the symptoms were relived after 2 physical therapies and is under regular check ups. The purpose of this case is to review disease that cause loose bodies.
