이 논문에서는 3 차원 CT/CTA 영상 데이터에 대하여 고속 자동 정합 기법을 제안한다. 제안하는 기법은 다해상도 (multi-resolution) 구조의 정규 상호 정보량(normalized mutual information) 을 최대화하는 정합 방식에서, 정합 유사도를 계산하는 볼륨 영역을 효율적으로 줄여 정합 속도를 증가시키는 방법이다. 제안된 정합방식을 CT/CTA (CT angiography) 팬텀 데이터와 7 세트의 실제 CT/CTA 임상 데이터에 적용하여 테스트하였다. 이로부터 제안하는 방식이, 정합 정확도를 유지하는 동시에 정합 속도를 10 ∼ 60% 로 감소시킴을 확인 할 수 있었다. 또한 제안된 정합 방식을 DS-CTA (digital subtraction CT angiography) 에 적용하여, CT/CTA 영상으로부터 혈관 영상을 성공적으로 추출하였다.
관상동맥 질환은 죽상동맥경화(atherosclerosis)로 인해 혈관의 내강이 좁아지면서 심근허혈 또는 경색까지 유발할 수 있는 질병이다. 이런 관상동맥 질환은 조기에 진단해서 치료하면 그만큼 예후가 좋기에 정확한 진단이 환자 관리에서 매우 중요하다. 전산화단층촬영 혈관조영술(CT angiography; 이하 CTA)은 높은 해상도와 대조도를 통해 혈관의 구조 및 협착 정도를 세밀하게 평가할 수 있는 비침습적 영상 진단법이다. 여러 임상시험들이 관상동맥 질환에 대한 조기 진단과 평가에 있어 관상동맥 CTA의 유용성을 보고하였다. 최근에 관상동맥 질환이 의심되는 환자들에 대한 보다 효과적인 처치를 위해 CTA에 기반한 관상동맥 질환 진단 분류 체계인 관상동맥 질환 판독과 자료 체계(coronary artery disease-reporting and data system; 이하 CAD-RADS)가 만들어졌다. 이런 CAD-RADS는 관상동맥 CTA를 기반으로 하지만 CAD-RADS는 CTA 결과만으로 관상동맥의 정확한 상태를 해석하는 것이 어려운 경계선 범주를 포함하고 있다. 본 종설은 침습적 관상동맥 조영술 진행 여부를 결정하기에 앞서 추가검사가 필요한 경계선상 CAD-RADS범주들의 CTA의 소견과 이들에 대한 대처를 논하고자 한다.
Objectives : Three-dimensional computed tomographic angiography(3D-CTA) is recently developed diagnostic imaging modality. We have studied this noninvasive method for possible role in replacing conventional angiography( CA) in the detection of aneurysms of the circle of Willis in patients with subarachnoid hemorrahge(SAH). Methods : We studied retrospectively, the 100 patients with SAH or unruptured aneurysms admitted to our hospital from October 1997 to December 1998. Among there, 85 patients underwent CTA, 82 patients underwent CA and 67 patients underwent both of CTA and CA. 3D-CTA was obtained using maximum intensity projection(MIP) and shaded-surface display(SSD) reconstruction. Results : Total 107 aneurysms were detected in 92 patients, and 64 aneurysms were detected in 67 patients underwent both CTA and CA. In five cases of those 67 cases, aneurysms were detected by CA but not by 3D-CTA. The detection rate of aneurysms(91.8%) and the detection rate of parent artery in cases of anterior communicating artery aneurysms(86.9%) with total 3D-CTA were relatively compatible with that of CA. But 3D-CTA was not enough in detection of posterior communicating artery aneurysms, internal carotid artery aneurysms as well as small sized aneurysm(<3mm). Conclusion : We consider CTA is valuable in as a screening test for cerebral aneurysm and follow-up test. And it is also valuable in early surgery for patients with aneurysmal rebleeding because of simple, quick, non-invasive method.
This study investigated the accuracy of magnetic resonance angiography (MRA) and computed tomography angiography (CTA) in terms of reflecting the actual vascular length. Three-dimensional time of flight (3D TOF) MRA, 3D contrast-enhanced (CE) MRA, volume-rendering after CTA and maximum intensity projection were investigated using a flow model phantom with a diameter of 2.11 mm and area of $0.26cm^2$. 1.5 and 3.0 Tesla devices were used for 3D TOF MRA and 3D CE MRA. CTA was investigated using 16 and 64 channel CT scanners, and the images were transmitted and reconstructed by volume-rendering and maximum intensity projection, followed by conduit length measurement as described above. The smallest 3D TOF MRA measure was $2.51{\pm}0.12mm$ with a flow velocity of 40 cm/s using the 3.0 Tesla apparatus, and $2.57{\pm}0.07mm$ with a velocity of 71.5 cm/s using the 1.5 Tesla apparatus; both images were magnified from the actual measurement of 2.11 mm. The measurement with the 16 channel CT scanner was smaller ($3.83{\pm}0.37mm$) than the reconstructed image on maximum intensity projection. The images from CTA from examination apparatus and reconstruction technique were all larger than the actual measurement.
본 논문에서는 CT 및 CT 혈관 조영 영상에서 강체 정합 및 뼈 소거법을 이용한 하지 혈관 자동 추출 방법을 제안한다. 첫째, 뼈의 부분적인 움직임을 반영하기 위하여 해부학 정보를 바탕으로 하지를 자동 구역화하고, 둘째, CT와 CTA 영상간 움직임을 산정하기 위하여 거리지도 기반의 강체 정합을 수행한다. 셋째, CTA 영상에서 복잡한 구조를 갖는 뼈를 제거하고 뼈에 인접한 혈관이 깎이는 것을 방지하기 위하여 뼈 소거법과 혈관 마스킹 기법을 제안한다. 넷째, 정합오차 및 연골 등의 잡음을 줄이기 위하여 혈관 추적 기반의 후 처리 과정을 통하여 보정한다. 제안 방법의 평가를 위해 육안 평가와 정확성 평가 그리고 수행시간을 측정하였다. 육안 평가를 위해 차감 기법, 정합 후 차감 기법, 제안 방법을 적용한 결과를 볼륨렌더링과 최대 강도 투영영상을 사용하여 비교하였다. 정확성 평가를 위해 CTA 영상과 차감 기반 기법 및 제안 방법을 적용한 결과의 밝기값 분포도를 분석하였다. 실험 결과 뼈는 제거되고 가는 혈관 및 다른 조직의 손실 없이 혈관이 정확하게 추출되었음을 볼 수 있었고, 13명의 환자 데이터 전채에 대한 전체 수행시간은 약 40포 정도로 측정되었다.
본 연구는 검사 기법에 따른 영상의 질과 방사선 피폭의 정도를 제시하여 검사자가 받는 방사선 피폭을 저감시키는 데 목적이 있다. 관상동맥 전산화 단층 혈관조영(Coronary CTA, coronary computed tomography angiography) 검사자를 대상으로 SnapShot Pulse의 전향적 동조화 기법과 SnapShot Segment의 후향적 동조화 기법으로 coronary CTA 검사에서 검사자가 받는 단면 피폭선량(CTDIvol, volume computed tomography dose index), 총 피폭선량(dose-length product; DLP)을 각각 측정하였다. 또한, Coronary CTA 촬영조건을 동일하게 팬텀을 이용하여 CT감약계수, 노이즈 및 균일도, 공간분해능을 측정하였다. 연구 결과 두기법에서 CT감약계수, 노이즈 및 균일도, 공간분해능의 질적 수준이 비슷하게 나타났으며, CTDIvol, DLP는 SnapShot Segment의 후향적 동조화 기법에 비해 피폭선량이 약 37.5%, 40.3% 감소되었다. 임상에서 coronary CTA 검사 시 SnapShot Pulse의 전향적 동조화 기법을 적극 채택하여 검사자가 받는 방사선 피폭을 감소시키기 위한 노력이 절실히 요구된다.
We report a case of fatal duret hemorrhage (DH) in a patient with acute tentorial subdural hematoma and bilateral chronic subdural hematoma along the cerebral hemispheres. Preoperative CT angiography (CTA) revealed prominent parenchymal enhancement in the ventral pontomesencephalic area. After burr-hole drainage, a large hemorrhage developed in this area. The parenchymal enhancement in the CTA may reflect the pontomensencephalic perforating vessel injury, and may be a sign of impending DH of acute transtentorial downward herniation. Previous use of aspirin and warfarin might have potentiated the process of DH and increase the extent of the bleed.

Background: Bone subtraction computed tomography angiography (BSCTA) is a useful alternative technique for improving visualization of vessels surrounded by skull bone. However, no studies have compared computed tomography angiography (CTA) and BSCTA for improving the visibility of canine cerebral blood vessels. Objectives: To evaluate the potential benefit of BSCTA for better delineation of brain arteries of the circle of Willis (CoW) in dogs by comparing BSCTA with non-subtraction computed tomography angiography (NSCTA). Methods: Brain CTA was performed for nine healthy beagle dogs using a bolus tracking method with saline flushing. A total dose of 600 mgI/kg of contrast agent with an iodine content of 370 mgI/mL was injected at a rate of 4 ml/s. Bone removal was achieved automatically by subtracting non-enhanced computed tomography (CT) data from contrast CT data. Five main intracranial arteries of the CoW were analyzed and graded on a scale of five for qualitative evaluation. Results: Scores of basilar artery, middle cerebral artery, and rostral cerebral artery in the BSCTA group were significantly higher than those in the NSCTA group (p = 0.001, p = 0.020, and p < 0.0001, respectively). Scores of rostral cerebellar artery (RcA) and caudal cerebral artery (CCA) did not differ significantly between the two groups. However, scores of RcA and CCA in the BSCTA group were higher than those in the NSCTA group. Conclusions: BSCTA improved visualization of intracranial arteries of the CoW with close contact to bone. Thus, it should be recommended as a routine scan method in dogs suspected of having brain vessel disease.
Purpose: This study was conducted to evaluate the effectiveness of the treatment strategy of transcatheter arterial embolization after pelvic CT angiography (CTA) in cases of traumatic pelvic hemorrhage. Methods: This is a retrospective analysis of pelvic hemorrhage patients who underwent transcatheter arterial embolization after pelvic CTA at our regional emergency center during a 31-month period. We reviewed the medical records and imagings of all these patients. Results: Transcatheter arterial embolization was performed in 17 patients (M:F=7:10, mean age=53.9) who underwent pelvic CTA for the evaluation of traumatic pelvic hemorrhage. Arterial bleeding was demonstrated on pelvic CTA in all patients, and the combined injury was also noted in 13 patients. The admission-to-CTA time was $84.53{\pm}66.92$ minutes, and the CTA-to-embolization time was $147.65{\pm}99.97$ minutes. Extravasation of contrast media or pseudoaneurysm was demonstrated on conventional angiography in all patients. Unilateral iliac artery embolization was performed in 8 patients, and bilateral iliac artery embolization was performed in 9 patients. Additional embolizations other than in the iliac arteries were performed in 7 patients. Initial hemostasis was achieved in 16 patients. One patient died of ongoing pelvic bleeding. Rebleeding occurred in only one patient and hemostasis was achieved with the second embolization. Another patient died of intracranial and facial bleeding in spite of pelvic hemostasis. The overall mortality was 11.8%, and there was no significant adverse effects in the other patients. Conclusion: Transcatheter arterial embolization after pelvic CTA is an effective treatment strategy in the management of traumatic pelvic hemorrhage patients.
MDCT의 시간분해능 향상과 등방성 해상능(isotrophic resolution) 영상의 획득, 그리고 지능적인 심전도 동조를 바탕으로 하여 심혈관 질환의 효과적인 진단검사로 인정받고 있는 후향적 심전도 동조화(retrospective ECG gating) 하의 coronary CT angiography는 상대적으로 많은 환자선량을 제공함으로 인해 우수한 진단방법으로서의 장점을 반감시키고 있다. 이에 각 장치 제조사에서는 환자선량을 감소시키는 방법의 연구가 활성화되어 왔으며, 이의 일환으로 지능적인 cardiac dose modulation 기술과 전향적 심전도 동조화(prospective ECG gating)를 사용한 sequential scan이 도입되고 있다. 이에 본 연구에서는 64 채널 MDCT에서 54 kg, 163 cm인 여성 인체모형팬텀을 대상으로 하고 형광유리선량계를 사용하여 후향적 심전도 동조화 하의 coronary CT angiography 프로토콜에서 환자선량의 정량적 평가와 환자선량 감소를 위해 본원에서 선택적으로 적용하고 있는 5가지 검사 프로토콜을 적용하였을 경우의 effective dose와 중요 부위의 organ dose를 측정 비교하여 다음과 같은 결과를 얻었다. 1) Dose modulation없이 120 kVp와 210 mAs의 노출조건으로 retrospectively ECG gated helical scan으로 시행한 conventional coronary CT angiography 프로토콜의 effective dose는 17.8 mSv였으며, 심장의 organ dose는 103.8 mGy였다. 2) 관전압을 120 kVp에서 100 kVp로 낮추었을 경우 effective dose는 11.0 mSv로 conventional coronary CT에 비해 38.2%가 감소하였으며, 심장은 67.3 mGy로 45.2%가 감소하였다. 3) Cardiac dose modulation을 적용한 경우 effective dose는 13.3 mSv로 conventional coronary CT에 비해 25.3%가 감소하였으며, 심장은 80.0 mGy로 22.9%가 감소하였다. 4) 100 kVp의 저관전압과 cardiac dose modulation을 적용한 경우 effective dose는 8.1 mSv로 conventional coronary CT angiography에 비해 54.5%가 감소하였으며, 심장은 49.5 mGy로 52.3%가 감소하였다.
