Biomechanical Evaluation of SMA Dynamic Stabilization for Spinal fusion (척추고정용 형상기억합금 동적안정기기의 생체역학적 성능 평가)

  • In this study, a commercial fixation device, BioFlex, which was designed with shape memory alloy(SMA) for dynamic stabilization of spine was biomechanically evaluated. The finite element model of intact lumbar spine from L1 to S was developed using CT images. Also, low FE models of 2-level(L4-L5-S) and 3-level(L3-L4-L5-S) posteriori fixation using titanium(Ti) rod and BioFlex(SMA) rod. The rotations of bone segments in the intact model and four models were predicted. Although the rotations of the BioFlex fixation model were smaller than those of the intact model, they were relatively larger than those of Ti fixation. The present can be applied for not only evaluation of the stability of interbody fixator, but also development of new implant.

Ga-67 SPECT Finding in Tuberculous Pericarditis with Mediastinal Mass: A case report (종격동 종괴를 수반한 결핵성 심낭염 1예의 Ga-67 SPECT 소견)

  • We present a case of a 31 year-old male patient with tuberculous pericarditis with mediastinal mass that showed increased uptake on Gallium-67 image. Gallium-67 scan was performed to evaluate the activity of the superior mediastinal mass, which was detected on chest CT. A rim of intense activity around the heart was observed, but increased uptake was not seen in the mediastinum. However, on maximal contrast-enhanced SPECT images, a small focus of faint uptake was observed in the superior mediastinum. This finding implied that there was an active tuberculosis in the pericardium and inflammation in the superior mediastinal mass. This case demonstrated that Gallium-67 scinitigraphy was helpful for the diagnosis of tuberculous pericarditis.

WASHINGTON PHOTOMETRY OF THE GLOBULAR CLUSTERS IN THE VIRGO GIANT ELLIPTICAL GALAXY M86

  • We present a photometric study of the globular clusters (GCs) in the Virgo giant elliptical galaxy M86 based on Washington $CT_1$ images. The colors of the GCs in M86 show a bimodal distribution with a blue peak at ($C-T_1$) = 1.30 and a red peak at ($C-T_1$) = 1.72. The spatial distribution of the red GCs is elongated similar to that of the stellar halo, while that of the blue GCs is roughly circular. The radial number density profile of the blue GCs is more extended than that of the red GCs. The radial number density profile of the red GCs is consistent with the surface brightness profile of the M86 stellar halo. The GC system has a negative radial color gradient, which is mainly due to the number ratio of the blue GCs to the red GCs increasing as galactocentric radius increases. The bright blue GCs in the outer region of M86 show a blue tilt: the brighter they are, the redder their mean colors get. These results are discussed in comparison with other Virgo giant elliptical galaxies.

Assessment of accessory mandibular canal in mandibular third molars using cone-beam computed tomography (콘빔 CT를 이용한 하악 제3대구치부의 부하악관 평가)

  • Purpose: This study was performed to examine distribution of accessory mandibular canal and its characteristics in mandibular third molars. Materials and methods: A total of 251 subjects (166 males and 85 females) having mandibular third molars bilaterally were included in the study. Cone-beam computed tomographic images were reviewed for bifid or trifid accessory mandibular canal. The prevalence of accessory mandibular canal was evaluated according to gender, side and its branching type. Proximity and crosssectional position of accessory mandibular canal to mandibular third molar was analyzed. Results: Accessory mandibular canals were found in 66 (26.3%) of 251 patients and 86 (17.1%) of 502 hemi-mandibles. Gender and sides showed no statistically significant differences in prevalence. Retromolar canal (46.1%) was the most common branching type. Proximity of accessory canal to mandibular third molars showed mean distance of 2.8 mm from third molar and a statistically significant difference was found among types of accessory canal. Dental canal was the closest to tooth among branching types and closer to tooth than main canal. On cross-sectional view, accessory canal was generally located on buccal side of mandibular third molar. Conclusion: Accessory mandibular canal was common and well detected with cone-beam computed tomography. Their localization is significant in all anesthetic and surgical procedures involving mandibular third molars.

Stiffness loss in enzyme-induced carbonate precipitated sand with stress scenarios

  • The enzyme-induced carbonate precipitation (EICP) method has been investigated to improve the hydro-mechanical properties of natural soil deposits. This study was conducted to explore the stiffness evolution during various stress scenarios. First, the optimal concentration of urea, CaCl2, and urease for the maximum efficiency of calcite precipitation was identified. The results show that the optimal recipe is 0.5 g/L and 0.9 g/L of urease for 0.5 M CaCl2 and 1 M CaCl2 solutions with a urea-CaCl2 molar ratio of 1.5. The shear stiffness of EICP-treated sands remains constant up to debonding stresses, and further loading induces the reduction of S-wave velocity. It was also found that the debonding stress at which stiffness loss occurs depends on the void ratio, not on cementation solution. Repeated loading-unloading deteriorates the bonding quality, thereby reducing the debonding stress. Scanning electron microscopy and X-ray images reveal that higher concentrations of CaCl2 solution facilitate heterogeneous nucleation to form larger CaCO3 nodules and 11-12 % of CaCO3 forms at the interparticle contact as the main contributor to the evolution of shear stiffness.

Photometric properties of the globular cluster system of the massive elliptical galaxy M86

  • We present a photometric study of the globular clusters (GCs) in the giant elliptical galaxy M86 in the Virgo Cluster, using the Washington $CT_1$ images taken at the KPNO 4 m telescope. The color distribution of the GCs in M86 is bimodal. The radial number density profile of the blue GCs decreases more slowly as the galactocentric distance increases than that of the red GCs. The density profile of the red GCs is similar to the surface brightness profile of M86 stellar halo. The blue GCs have a roughly circular spatial distribution, while the red GCs have a spatial distribution somewhat elongated, which is consistent with the distribution of the galaxy stellar light. M86 GCs have the negative radial color gradient because the number ratio of the blue GCs to the red GCs increases as galactocentric radius increases. The mean color of the red GCs is similar to that of the stellar halo. The bright blue GCs in the outer region of M86 reveal a blue tilt that the mean colors of the blue GCs get redder as they get brighter. We discuss these results in comparison with other giant elliptical galaxies in the Virgo Cluster.

Giant plunging ranula: a case report

  • A ranula is a bluish, transparent, and thin-walled swelling in the floor of the mouth. They originate from the extravasation and subsequent accumulation of saliva from the sublingual gland. Ranulas are usually limited to the sublingual space but they sometimes extend to the submandibular space and parapharyngeal space, which is defined as a plunging ranula. A 21-year-old woman presented with a complaint of a large swelling in the left submandibular region. On contrast-enhanced CT images, it dissected across the midline, and extended to the parapharyngeal space posteriorly and to the submandibular space inferiorly. Several septa and a fluid-fluid level within the lesion were also demonstrated. We diagnosed this lesion as a ranula rather than cystic hygroma due to the location of its center and its sublingual tail sign. As plunging ranula and cystic hygroma are managed with different surgical approaches, it is important to differentiate them radiologically.

  • Alveolar soft-part sarcoma(ASPS) is a rare, aggressive malignancy of uncertain histologic origin with a propensity for vascular invasion and distant metastasis. ASPS may mimic benign vascular neoplams of malformation but careful evaluation of the unique imaging features on CT scans, MR images, and angiograms lead to the correct diagnosis. ASPS of the tongue is slow-growing, painless mass, especially ASPS of the base the tongue is difficult to be noticed by patient, dentists or oral and maxillofacial surgeons on oral examintion because of its location and clinical resemblance to a benign lesion. And it leads to delayed or inadequate diagnosis. We report radiologic and clinical features of an ASPS of the basal portion of the tongue in a 17-year-old boy, showing normal appearance, but palpation of the tongue and floor of the mouth reveals the tumor. Among the 23 cases of a primary ASPS of tongue reported, 7 cases occured on the basal region of the tongue, inculding the present one. There has been no recurrence or metastasis as of 3 years postoperatively.

Finite Element Analysis of Lumbar Spine under Surgical Condition (척추 수술시 요추의 유한요소해석)

  • We study the fracture behavior of the lumbar No.4 and No.5 vertebra subjected to posteroanterior (PA) forces, a three dimensional finite element method (FEM). The lumbar spine was modeled 3-dimensionally using commercial software based on the principle of convert stacked two dimensional CT scan images into three dimensional shapes. Determination of the boundary conditions corresponding to actual surgical conditions was not easy, so that the simplified spine beam analyses were performed. The results were used in three dimensional finite element (FE) analysis. This FE analysis, indicates that the fracture loads of the lumbar No.4 and No.5 vertebra are respectively 1550 N and 1500 N. These fracture loads are for static loading, but in actual conditions the load on the lumbar spine varies dynamically. We found that the fracture load of lumbar No.4 vertebra is larger than that of lumbar No.5 vertebra, as a result of the total stress difference by the moment.

Verrucous carcinoma arising from a previous cystic lesion: a case report

  • Background: Verrucous carcinoma (VC) accounts for 1-10% of cases of squamous cell carcinoma (SCC) in the oral cavity, and 75% of VC occur in the oral cavity. Only 3% of primary intraosseous squamous cell carcinomas (PIOSCC), which means SCC occurring primarily in the bone, are VC. Verrucous carcinoma arising from odontogenic cysts (OC) is very rare, with only seven cases reported to date. Case presentation: This study reported a case of a patient who underwent partial maxillectomy and neck dissection for VC that occurred in the right anterior maxilla. The patient was admitted to the emergency department at our institution 8 years ago and showed cystic lesions in the anterior maxilla on facial computed tomography (CT) images. Treatment through other departments including assessment of laceration in the mental region and only suture was performed. This report highlights a very rare case of VC in the right anterior maxilla arising from a previous cystic lesion. Conclusions: Since PIOSCC can arise from OC, appropriate treatment of intraosseous cysts and regular radiologic evaluation are necesssary. Surgical exicision of the primary lesion without neck dissection can lead to good prognosis for patients with primary intraosseous verrucous carcinoma.
