The Affecting Factor to Magnification Ratio from Alveolar Crest To Inferior Mandibular Canal in Diagnosis of Implant Site of Mandibular Molar Region with Panoramic Radiography (파노라마방사선사진에서 하악 구치부의 임플란트 매식부위 평가시 치조정-하악관간 거리의 확대율에 영향을 미치는 요소에 관한 연구)

  • 최근 임플란트를 원하고, 필요로 하는 환자가 증가하면서 술전의 정확한 진단이 보다 중요해졌고 다양한 진단방법이 소개되고 이용되어져 왔다. 그 중 보편적으로 가장 널리 사용되어지는 방법이 방사선 촬영법이고 그 중에서도 파노라마방사선사진이다. 하지만 파노라마방사선사진의 용이함과 간편성에도 불구하고 그 방법 자체가 가지고 있는 한계점 때문에 CT/MPR(Computer Tomography/Multiplanar Reformatting)이 일부 환자에서는 반드시 필요하다는 주장이 대다수이다. 본 연구의 목적은 하악 구치부에 임플란트 시술을 받은 환자를 대상으로 파노라마방사선사진의 확대율을 알아보고 파노라마방사선사진과 CT/MPR에서 치조정-하악관간 거리가 실제 어느 정도 차이를 보이는지, 그리고 CT/MPR에 대한 파노라마방사선사진의 확대율에 환자의 하악골의 경사도와 하악관의 해부학적 위치가 어떤 영향을 미치는지를 알아보는 것이다. 본 연구는 서울대학 치과병원 치주과를 내원하여 하악구치부에 Dental Implant 시술을 받은 15명의 환자, 32개 부위를 대상으로, 임플란트 시술을 위한 술전 파노라마방사선사진과 CT/MPR, 술후의 파노라마방사선사진, CT/MPR상을 자료로, 임플란트가 식립될 위치에서 치조정에서 하악관의 길이, 하악골의 협설폭경, 하악골의 설측면에서 하악관의 수평길이, 임플란트 매식체의 설측에서 하악관까지의 수평거리를 계측하였고 이 해부학적 계측치들이 CT/MPR에 대한 파노라마방사선사진의 확대율과 어떤 상관관계에 있는지 Pearson's correlation coefficient test를 이용해 살펴보았다. 1. CT/MPR에 대한 파노라마방사선사진의 확대율은 CT/MPR상에서 하악골 단면의 협설폭경(Pearson test에서의 상관관계 분석시, 상관계수 r=-0.604), 임플란트 매식체의 설측에서 하악관까지의 수평거리(r=-0.515), 하악골의 설측면에서 하악관까지의 수평거리(r=-0.640)와 뚜렷한 음적 선형관계가 있었다. 2. CT/MPR에 대한 파노라마방사선사진의 확대율은 CT/MPR상에서 하악골의 장축의 각도와는 뚜렷한 양적 선형관계(Pearson test에서의 상관관계 분석시, 상관계수 r=-0.446)가 있었다. 3. 여러 요소중 CT/MPR에 대한 파노라마방사선사진의 확대율과 가장 유의성있는 연관성을 보이는 것은 CT/MPR상에서 하악골의 설측면에서 하악관까지의 수평거리였다(r=-0.640) 4. CT/MPR에 대한 파노라마방사선사진의 확대율은 일반적으로 생각되어지는 파노라마방사선사진의 확대율 125%보다 큰, 약 135%로 나왔다. 이상의 결과로 볼 때, 하악골의 설측면에서 하악관까지의 수평거리가 CT/MPR에 대한 파노라마방사선사진의 확대율에 가장 영향을 미치는 요소로 추정되며 확대율은 125%보다 높은 것으로 나타났다.

  • In this study, we introduce robust face recognition system with illumination variation realized with the aid of CT preprocessing method. As preprocessing algorithm, Census Transform(CT) algorithm is used to extract locally facial features under unilluminated condition. The dimension reduction of the preprocessed data is carried out by using $(2D)^2$PCA which is the extended type of PCA. Feature data extracted through dimension algorithm is used as the inputs of proposed radial basis function neural networks. The hidden layer of the radial basis function neural networks(RBFNN) is built up by fuzzy c-means(FCM) clustering algorithm and the connection weights of the networks are described as the coefficients of linear polynomial function. The essential design parameters (including the number of inputs and fuzzification coefficient) of the proposed networks are optimized by means of artificial bee colony(ABC) algorithm. This study is experimented with both Yale Face database B and CMU PIE database to evaluate the performance of the proposed system.

Performance Evaluation of U-net Deep Learning Model for Noise Reduction according to Various Hyper Parameters in Lung CT Images (폐 CT 영상에서의 노이즈 감소를 위한 U-net 딥러닝 모델의 다양한 학습 파라미터 적용에 따른 성능 평가)

  • In this study, the performance evaluation of image quality for noise reduction was implemented using the U-net deep learning architecture in computed tomography (CT) images. In order to generate input data, the Gaussian noise was applied to ground truth (GT) data, and datasets were consisted of 8:1:1 ratio of train, validation, and test sets among 1300 CT images. The Adagrad, Adam, and AdamW were used as optimizer function, and 10, 50 and 100 times for number of epochs were applied. In addition, learning rates of 0.01, 0.001, and 0.0001 were applied using the U-net deep learning model to compare the output image quality. To analyze the quantitative values, the peak signal to noise ratio (PSNR) and coefficient of variation (COV) were calculated. Based on the results, deep learning model was useful for noise reduction. We suggested that optimized hyper parameters for noise reduction in CT images were AdamW optimizer function, 100 times number of epochs and 0.0001 learning rates.

Impact of Photon-Counting Detector Computed Tomography on Image Quality and Radiation Dose in Patients With Multiple Myeloma

  • Objective: Computed tomography (CT) is an established method for the diagnosis, staging, and treatment of multiple myeloma. Here, we investigated the potential of photon-counting detector computed tomography (PCD-CT) in terms of image quality, diagnostic confidence, and radiation dose compared with energy-integrating detector CT (EID-CT). Materials and Methods: In this prospective study, patients with known multiple myeloma underwent clinically indicated whole-body PCD-CT. The image quality of PCD-CT was assessed qualitatively by three independent radiologists for overall image quality, edge sharpness, image noise, lesion conspicuity, and diagnostic confidence using a 5-point Likert scale (5 = excellent), and quantitatively for signal homogeneity using the coefficient of variation (CV) of Hounsfield Units (HU) values and modulation transfer function (MTF) via the full width at half maximum (FWHM) in the frequency space. The results were compared with those of the current clinical standard EID-CT protocols as controls. Additionally, the radiation dose (CTDIvol) was determined. Results: We enrolled 35 patients with multiple myeloma (mean age 69.8 ± 9.1 years; 18 [51%] males). Qualitative image analysis revealed superior scores (median [interquartile range]) for PCD-CT regarding overall image quality (4.0 [4.0-5.0] vs. 4.0 [3.0-4.0]), edge sharpness (4.0 [4.0-5.0] vs. 4.0 [3.0-4.0]), image noise (4.0 [4.0-4.0] vs. 3.0 [3.0-4.0]), lesion conspicuity (4.0 [4.0-5.0] vs. 4.0 [3.0-4.0]), and diagnostic confidence (4.0 [4.0-5.0] vs. 4.0 [3.0-4.0]) compared with EID-CT (P ≤ 0.004). In quantitative image analyses, PCD-CT compared with EID-CT revealed a substantially lower FWHM (2.89 vs. 25.68 cy/pixel) and a significantly more homogeneous signal (mean CV ± standard deviation [SD], 0.99 ± 0.65 vs. 1.66 ± 0.5; P < 0.001) at a significantly lower radiation dose (mean CTDIvol ± SD, 3.33 ± 0.82 vs. 7.19 ± 3.57 mGy; P < 0.001). Conclusion: Whole-body PCD-CT provides significantly higher subjective and objective image quality at significantly reduced radiation doses than the current clinical standard EID-CT protocols, along with readily available multi-spectral data, facilitating the potential for further advanced post-processing.

Developmental Study of Korean Frontal Sinus by using 3D CT Scan (3차원 컴퓨터단층촬영 영상을 이용한 전두동 발달의 연구)

  • Purpose: Although there are many reports on the development of paranasal sinuses, few studies were performed with respect to the frontal sinus with 3D CT, especially in Korean children. In this study, we evaluated the normal development of frontal sinus in Korean children with 3D CT imaging. Methods: A total of 301 patients under the age of 20 were reviewed retrospectively. Maximal AP (anteroposterior) length, height and width were measured with the aid of computer device. The volume of frontal sinus was also evaluated. Results: At the age of more than 4, the pneumatization of frontal sinus was detected. Frontal sinus developed continuously and was observed in almost every children at the age of more than 17. The AP length, height, width and volume of frontal sinus were enlarged continuously until the age of 20. And the maximal growth of frontal sinus was observed at puberty. The width and volume of frontal sinus showed high correlation coefficient (r=0.8) relative to height or AP distance. Conclusion: The results of this study may be helpful in the comprehension of normal development of frontal sinus in Korean children.

Performance analysis of improved hybrid median filter applied to X-ray computed tomography images obtained with high-resolution photon-counting CZT detector: A pilot study

  • We evaluated the performance of an improved hybrid median filter (IHMF) applied to X-ray computed tomography (CT) images obtained using a high-resolution photon-counting cadmium zinc telluride (CZT) detector. To study how the proposed approach improves the image quality, we measured the noise levels and the overall CT-image quality. We established a CZT imaging system with a detector length of 5.12 cm and thickness of 0.3 cm and acquired phantom images. To evaluate the efficacy of the proposed filter, we first modeled two conventional median filters. Subsequently, we were able to achieve a normalized noise power spectrum result of ~10-8 mm2, and furthermore, the proposed method improved the contrast-to-noise ratio by a factor of ~1.51 and the coefficient of variation by 1.55 relative to the counterpart values of the no-filter image. In addition, the IHMF exhibited the best performance among the three filters considered as regards the peak signal-to-noise ratio and no-reference-based image-quality evaluation parameters. Thus, our results demonstrate that the IHMF approach provides a superior image performance over conventional median filtering methods when applied to actual CZT X-ray CT images.

Usefulness of intraoperative transcranial sonography in patients with traumatic brain injuries: a comparison with postoperative computed tomography

  • Purpose: The aim of this study was to assess the agreement between intraoperative transcranial sonography (TCS) and postoperative computed tomography (CT) in patients with traumatic brain injuries. Methods: We performed a retrospective cross-sectional study of 35 patients who underwent TCS during surgery, among those who presented to a regional trauma center and underwent decompressive craniectomy between January 1, 2017 and April 30, 2020. Results: The mean difference between TCS and CT in measuring the midline shift was -1.33 mm (95% confidence interval, -2.00 to -0.65; intraclass correlation coefficient [ICC], 0.96; P<0.001). An excellent correlation was found between TCS and CT in assessing contralateral subdural hematomas (ICC, 0.96; P<0.001) and focal hematoma lesions (ICC, 0.99; P<0.001). A very good correlation between TCS and CT was found for measurements of ventricle width (ICC, 0.92; P<0.001). Conclusions: TCS during surgery is considered an effective diagnostic tool for the detection of intraoperative parenchymal changes in patients with traumatic brain injuries.

  • The primary aim of this study is to investigate the probabilistic characteristics of the fatigue parameters that describe the fatigue crack growth behavior in magnesium alloy. Statistical fatigue crack propagation experiments have been performed on rolled AZ31 magnesium alloy CT specimens with different specimen thickness, load ratio, and maximum load at ambient temperature in a laboratory. Using the statistical fatigue data obtained from these experiments, the goodness-of-fit of the probability distribution of the fatigue behavior parameters is evaluated in this study by performing statistical analyses. The crack growth rate coefficient is a fatigue parameter having a very large COV(Coefficient of Variation), but the variation of a crack growth rate exponent is not substantial. It is considered that a crack growth rate exponent can be a material constant. It is also found that the best fit probability distribution of the parameters such as the crack growth rate coefficient and crack growth rate exponent for a magnesium alloy is a three-parameter Weibull distribution, and two-parameter Weibull distribution is a good distribution only for the crack growth rate coefficient.

Accuracy of simulation surgery of Le Fort I osteotomy using optoelectronic tracking navigation system (광학추적항법장치를 이용한 르포씨 제1형 골절단 가상 수술의 정확성에 대한 연구)

  • Introduction: The aim of this study was to demonstrate that the simulation surgery on rapid prototype (RP) model, which is based on the 3-dimensional computed tomography (3D CT) data taken before surgery, has the same accuracy as traditional orthograthic surgery with an intermediate splint, using an optoelectronic tracking navigation system. Materials and Methods: Simulation surgery with the same treatment plan as the Le Fort I osteotomy on the patient was done on a RP model based on the 3D CT data of 12 patients who had undergone a Le Fort I osteotomy in the department of oral and maxillofacial surgery, Seoul National University Dental Hospital. The 12 distances between 4 points on the skull, such as both infraorbital foramen and both supraorbital foramen, and 3 points on maxilla, such as the contact point of both maxillary central incisors and mesiobuccal cuspal tip of both maxillary first molars, were tracked using an optoelectronic tracking navigation system. The distances before surgery were compared to evaluate the accuracy of the RP model and the distance changes of 3D CT image after surgery were compared with those of the RP model after simulation surgery. Results: A paired t-test revealed a significant difference between the distances in the 3D CT image and RP model before surgery.(P<0.0001) On the other hand, Pearson's correlation coefficient, 0.995, revealed a significant positive correlation between the distances.(P<0.0001) There was a significant difference between the change in the distance of the 3D CT image and RP model in before and after surgery.(P<0.05) The Pearson's correlation coefficient was 0.13844, indicating positive correlation.(P<0.1) Conclusion: Theses results suggest that the simulation surgery of a Le Fort I osteotomy using an optoelectronic tracking navigation system I s relatively accurate in comparing the pre-, and post-operative 3D CT data. Furthermore, the application of an optoelectronic tracking navigation system may be a predictable and efficient method in Le Fort I orthognathic surgery.

Study of 68Ga Labelled PET/CT Scan Parameters Optimization (68Ga 표지 PET/CT 검사의 최적화된 매개변수에 대한 연구)

  • Purpose: Gallium-68 (68Ga) is increasingly used in nuclear medicine imaging for various conditions such as lymphoma and neuroendocrine tumors by labeling tracers like Prostate Specific Membrane Antigen (PSMA) and DOTA-TOC. However, compared to Fluorine-18 (18F) used in conventional nuclear medicine imaging, 68Ga has lower spatial resolution and relatively higher Signal to Background Ratio (SBR). Therefore, this study aimed to investigate the optimized parameters and reconstruction methods for PET/CT imaging using the 68Ga radiotracer through model-based image evaluation. Materials and Methods: Based on clinical images of 68Ga-PSMA PET/CT, a NEMA/IEC 2008 PET phantom model was prepared with a Hot vs Background (H/B) ratio of 10:1. Images were acquired for 9 minutes in list mode using DMIDR (GE, Milwaukee WI, USA). Subsequently, reconstructions were performed for 1 to 8 minutes using OS-EM (Ordered Subset Expectation Maximization) + TOF (Time of Flight) + Sharp IR (VPFX-S), and BSREM (Block Sequential Regularized Expectation Maximization) + TOF + Sharp IR (QCFX-S-400), followed by comparative evaluation. Based on the previous experimental results, images were reconstructed for BSREM + TOF + Sharp IR / 2 minutes (QCFX-S-2min) with varying β-strength values from 100 to 700. The image quality was evaluated using AMIDE (freeware, Ver.1.0.1) and Advanced Workstation (GE, USA). Results: Images reconstructed with QCFX-S-400 showed relatively higher values for SNR (Signal to Noise Ratio), CNR (Contrast to Noise Ratio), count, RC (Recovery Coefficient), and SUV (Standardized Uptake Value) compared to VPFX-S. SNR, CNR, and SUV exhibited the highest values at 2 minutes/bed acquisition time. RC showed the highest values for a 10 mm sphere at 2 minutes/bed acquisition time. For small spheres of 10 mm and 13 mm, an inverse relationship between β-strength increase and count was observed. SNR and CNR peaked at β-strength 400 and then decreased, while SUV and RC exhibited a normal distribution based on sphere size for β-strength values of 400 and above. Conclusion: Based on the experiments, PET/CT imaging using the 68Ga radiotracer yielded the most favorable quantitative and qualitative results with a 2 minutes/bed acquisition time and BSREM reconstruction, particularly when applying β-strength 400. The application of BSREM can enhance accurate quantification and image quality in 68Ga PET/CT imaging, and an optimization process tailored to each institution's imaging objectives appears necessary.
