Comparison of Radiation Dose and Image Quality between the 2nd Generation and 3rd Generation DualSource Single-Energy and Dual-Source Dual-Energy CT of the Abdomen (2세대와 3세대 이중 소스 단일 에너지와 이중 소스 이중 에너지를 이용한 복부 컴퓨터단층촬영의 방사선량 및 영상 품질 비교)

  • Purpose We compared the radiation dose and image quality between the 2nd generation and the 3rd generation dual-source single-energy (DSSE) and dual-source dual-energy (DSDE) CT of the abdomen. Materials and Methods We included patients undergoing follow-up abdominal CT after partial or radical nephrectomy in the first 10 months of 2019 (2nd generation DS CT) and the first 10 months of 2020 (3rd generation DS CT). We divided the 320 patients into 4 groups (A, 2nd generation DSSE CT; B, 2nd generation DSDE CT; C, 3rd generation DSSE CT; and D, 3rd generation DSDE CT) (n = 80 each) matched by sex and body mass index. Radiation dose and image quality (objective and subjective qualities) were compared between the groups. Results The mean size-specific dose estimation of 3rd generation DSDE CT group was significantly lower than that of the 2nd generation DSSE CT (42.5%, p = 0.013) and 2nd generation DSDE CT (46.9%, p = 0.015) groups. Interobserver agreement was excellent for the overall image quality (intraclass correlation coefficient [ICC]: 0.8867) and image artifacts (ICC: 0.9423). Conclusion Our results showed a considerable reduction in the radiation dose while maintaining high image quality with 3rd generation DSDE CT as compared to the 2nd generation DSDE CT and 2nd generation DSSE CT.

  • Cardiac CT is the most accurate tool for diagnosing and evaluating coronary artery anomalies. Coronary anomalies can often be observed as the number of cardiac CT scans increases. In this review article, we described the CT findings and clinical significance of coronary anomalies that radiologists should know. In particular, we described the dangerous anatomical findings of coronary anomalies on CT images in detail.

Study on Methods to Improve Image Quality of Abdominal CT Images (복부 CT 영상의 화질 개선 방법에 대한 연구)

  • Liver disease is highly associated with death, and other abdominal diseases are also important causes affecting a person's lifespan, and a CT scan is essential when treating abdominal diseases. High radiation exposure is essential to create images that are good for reading, but managing the patient's radiation exposure is also essential. In this study, a post-processing wavelet algorithm was proposed to improve the image quality of abdominal CT images. Wavelets have the disadvantage of having to set a threshold value depending on the type of input image. Therefore, we experimentally proposed the threshold value of the wavelet and evaluated whether the image quality was effective. As a result of the experiment, the optimal threshold value for abdominal CT images was calculated to be 50. In the case of image 1, noise was improved by 49% and in the case of image 2, by 29%, and the contrast also increased. if the results of this study are applied for post-processing after abdominal CT, image quality can be improved and it will be helpful in disease diagnosis.

Diagnostic Performance of Rectal CT for Staging Rectal Cancer: Comparison with Rectal MRI and Histopathology (직장암 병기결정에서 직장 CT의 진단능: 직장 MRI 및 병리결과와의 비교분석)

  • Purpose To compare the diagnostic performance of rectal CT with that of high-resolution rectal MRI and histopathology in assessing rectal cancer. Materials and Methods Sixty-seven patients with rectal cancer who underwent rectal CT with rectal distension using sonographic gel and high-resolution MRI were enrolled in this study. The distance from the anal verge/anorectal junction, distance to the mesorectal fascia (MRF), extramural depth (EMD), extramesorectal lymph node (LN) involvement, extramural venous invasion (EMVI), and T/N stages in rectal CT/MRI were analyzed by two gastrointestinal radiologists. The CT findings of 20 patients who underwent radical surgery without concurrent chemoradiotherapy were compared using histopathology. Interclass correlations and kappa statistics were used. Results The distance from the anal verge/anorectal junction showed an excellent intraclass correlation between CT and MRI for both reviewers. For EMD, the distance to the MRF, presence of LNs, extramesorectal LN metastasis, EMVI, T stage, and intermodality kappa or weighted kappa values between CT and MRI showed excellent agreement. Among the 20 patients who underwent radical surgery, T staging, circumferential resection margin involvement, EMVI, and LN metastasis on rectal CT showed acceptable concordance rates with histopathology. Conclusion Dedicated rectal CT may be on par with rectal MRI in providing critical information to patients with rectal cancer.

Incidental Breast Lesions on Chest CT: Clinical Significance and Differential Features Requiring Referral (흉부 전산화단층촬영에서 우연히 발견된 유방 병변: 임상적 중요성 및 진료 의뢰가 필요한 특징적 영상 소견)

  • Purpose: To evaluate the CT features of incidental breast lesions on chest CT and to suggest useful criteria for referral to a specialized breast unit. Materials and Methods: Between May 2009 and April 2014, enhanced chest CT examination reports containing the key word 'breast' were reviewed retrospectively. Patients who had incidental breast lesion and were referred to a specialized breast unit and then underwent pathological confirmation or follow-up over a 1-year period were included. Finally, 86 patients (all female, mean age, $48.9{\pm}12.6years$) were enrolled. Two radiologists evaluated lesion characteristics, including size, shape, margins, and enhancement. The correlations between the CT features and pathologies were evaluated, and the diagnostic accuracy of CT features in various combinations was assessed. Results: Among the CT features, irregular shape, non-circumscribed margin, and high contrast enhancement were different between malignant and benign lesions (p < 0.05). The combination of non-circumscribed margin and high contrast enhancement had the highest accuracy (97.7%). Conclusion: Reliable CT features for incidental malignant breast masses are irregular shape, non-circumscribed margin, and high contrast enhancement. The combination of non-circumscribed margin and high contrast enhancement could help distinguish incidental malignant breast lesions and indicate referral to a specialized breast unit.

A Study on the Use of Active Protocol Using the Change of Pitch and Rotation Time in PET/CT (PET/CT에서 Pitch와 Rotation Time의 변화를 이용한 능동적인 프로토콜 사용에 대한 연구)

  • Purpose: The Change of CT exposure condition have a effect on image quality and patient exposure dose. In this study, we evaluated effect CT image quality and SUV when CT parameters (Pitch, Rotation time) were changed. Materials and Methods: Discovery Ste (GE, USA) was used as a PET/CT scanner. Using GE QA Phantom and AAPM CT Performance Phantom for evaluate Noise of CT image. Images are acquired by using 24 combinations that four stages pitch (0.562, 0.938, 1.375, 1.75:1) and six stages X-ray tube rotation time (0.5s-1.0s). PET images are acquired using 1994 NEMA PET Phantom ($^{18}F-FDG$ 5.3 kBq/mL, 2.5 min/frame). For noise test, noise are evaluated by standard deviation of each image's CT numbers. And then we used expectation noise according to change of DLP (Dose Length Product) to experimental noise ratio for index of effectiveness. For spatial resolution test, we confirmed that it is possible to identify to 1.0 mm size of the holes at the AAPM CT Performance Phantom. Finally we evaluated each 24 image's SUV. Results: Noise efficiency were 1.00, 1.03, 1.01, 0.96 and 1.00, 1.04, 1.02, 0.97 when pitch changes at the QA Phantom and AAPM Phantom. In case of X-ray tube rotation time changes, 0.99, 1.02, 1.00, 1.00, 0.99, 0.99 and 1.01, 1.01, 0.99, 1.01, 1.01, 1.01 at the QA Phantom and AAPM Phantom. We could identify 1.0 mm size of the holes all 24 images. Also, there were no significant change of SUV and all image's average SUV were 1.1. Conclusion: 1.75:1 pitch is the most effective value at the CT image evaluation according to pitch change and It doesn't affect to the spatial resolution and SUV. However, the change of rotation time doesn't affect anything. So, we recommend to use the effective pitch like 1.75:1 and adequate X-ray tube rotation time according to patient size.

  • Computed tomography (CT) adds a new dimension in the study of body contour, organs, and tissues as well as various pathologic conditions. This modality provides a great degree of accuracy in radiation therapy Planning (RTP). However, CT images are usually taken on a small reduced format so that possible errors can be made during inputting the CT data into an automatic planner. Authors have designed a simple inexpensive magnifying device of CT images to obviate errors created by reduced image.

  • As the hand-held mobile devices are widely used, they are recently coming into convergence with medical diagnostic systems. Furthermore, the wireless mobile Internet and the various kinds of communication devices are rapidly coming into wide use converging with medical technology. The mobile communication environments can make people get more health care services beyond space and time. In this paper, we implement and evaluate the mobile client and the medical diagnostic information server for transmitting, searching and updating the medical diagnostic information. The DICOM CT image and the compressed JPEG 2000 CT image are statistically evaluated by t-test performance whether those images are clinically appropriate. In the case of the DICOM CT image, we realize that the average value is relatively more appropriate to the clinical diagnosis than the JPEG 2000 CT image.

Changes in the Standardized Uptake Value According to the Type of Metal of Dental Prosthesis in PET-CT Fusion Image (PET-CT 융합 영상에서 치과보철물의 금속 종류에 따른 표준섭취계수 값의 변화)

  • In this study, HU(hounsfield unit) value of CT generated by dental prosthesis was measured according to the type of metal when PET-CT was performed, and the degree of distortion and standard deviation of SUV(standard uptake value) and to propose a method to reduce errors in image reading. PET-CT was performed using actual teeth, metal crown, gold crown, titanium, and zirconia dental prosthesis. Compared with general teeth, the SUV value increased with increasing HU value. The SUV value of metal crown, titanium, and zirconia was increased by 37% and the gold crown increased by 45.4%. In addition, image distortions were small in general teeth, metal crown, titanium, and zirconia, but hard curing of the gold crown occurred and image distortion occurred. Therefore, since the metal type of the dental prosthesis affects the SUV value, the NAC(non attenuation correction) PET image of the dental prosthesis can be helpful in the diagnosis of the patient using the gold material.

  • The image data amount that used in medical institution with great development of medical technology is increasing rapidly. Therefore, people need automation method that use image processing description than macrography of doctors for analysis many medical image. In this paper. we propose that acquire texture information to using GLCM about liver area of abdomen CT image, and automatically detects liver tumor using PCA from this data. Method by one feature as intensity of existent liver humor detection was most but we changed into 4 principal component accumulation images using GLCM's texture information 8 feature. Experiment result, 4 principal component accumulation image's variance percentage is 89.9%. It was seen this compare with liver tumor detecting that use only intensity about 92%. This means that can detect liver tumor even if reduce from dimension of image data to 4 dimensions that is the half in 8 dimensions.
