인체 각 부위의 PET/MRI와 PET/CT의 SUV 변화 (Comparison of SUV for PET/MRI and PET/CT)

  • Purpose: Due to developed simultaneous PET/MRI, it has become possible to obtain more anatomical image information better than conventional PET/CT. By the way, in the PET/CT, the linear absorption coefficient is measured by X-ray directly. However in case of PET/MRI, the value is not measured from MRI images directly, but is calculated by dividing as 4 segmentation ${\mu}-map$. Therefore, in this paper, we will evaluate the SUV's difference of attenuation correction PET images from PET/MRI and PET/CT. Materials and Methods: Biograph mCT40 (Siemens, Germany), Biograph mMR were used as a PET/CT, PET/MRI scanner. For a phantom study, we used a solid type $^{68}Ge$ source, and a liquid type $^{18}F$ uniformity phantom. By using VIBE-DIXON sequence of PET/MRI, human anatomical structure was divided into air-lung-fat-soft tissue for attenuation correction coefficient. In case of PET/CT, the hounsfield unit of CT was used. By setting the ROI at five places of each PET phantom images that is corrected attenuation, the maximum SUV was measured, evaluated %diff about PET/CT vs. PET/MRI. In clinical study, the 18 patients who underwent simultaneous PET/CT and PET/MRI was selected and set the ROI at background, lung, liver, brain, muscle, fat, bone from the each attenuation correction PET images, and then evaluated, compared by measuring the maximum SUV. Results: For solid $^{68}Ge$ source, SUV from PET/MRI is measured lower 88.55% compared to PET/CT. In case of liquid $^{18}F$ uniform phantom, SUV of PET/MRI as compared to PET/CT is measured low 70.17%. If the clinical study, the background SUV of PET/MRI is same with PET/CT's and the one of lung was higher 2.51%. However, it is measured lower about 32.50, 40.35, 23.92, 13.92, 5.00% at liver, brain, muscle, fat, femoral head. Conclusion: In the case of a CT image, because there is a linear relationship between 511 keV ${\gamma}-ray$ and linear absorption coefficient of X-ray, it is possible to correct directly the attenuation of 511 keV ${\gamma}-ray$ by creating a ${\mu}$map from the CT image. However, in the case of the MRI, because the MRI signal has no relationship at all with linear absorption coefficient of ${\gamma}-ray$, the anatomical structure of the human body is divided into four segmentations to correct the attenuation of ${\gamma}-rays$. Even a number of protons in a bone is too low to make MRI signal and to localize segmentation of ${\mu}-map$. Therefore, to develope a proper sequence for measuring more accurate attenuation coefficient is indeed necessary in the future PET/MRI.

F-18 FDG PET/MRI와 PET/CT 전신 영상에서 VOI를 이용한 정상부위의 $SUV_{LBM}$-최대치에 의한 영상평가 (Image Evaluation Via $SUV_{LBM}$ for Normal Regions of VOI by Using Whole Body Images Obtained from PET/MRI and PET/CT)

  • 본 연구의 목적은 26명의 건강한 검진자들을 대상으로 PET/CT와 PET/MRI의 전신 영상에서 조기 정밀/ 진단 수준을 향상하고자 관심부피를 이용하여 정상부위의 $SUV_{LBM}$-최대치를 이용하여 PET/CT와 PET/MRI를 조영제 사용 유무와 관계없이 동시 평가할 수 있는 데이터를 구축하고자 하였다. 전신 F-18 FDG PET/CT와 전신 F-18 FDG PET/MRI의 정상부위의 VOI를 이용한 $SUV_{LBM}$-최대치평가는 PET/CT를 기준으로 PET/MRI의 영상은 매우 유의한 차이를 보였다(p<0.0001). 그러나 통계학적으로 높은 상관관계를 가진다(R>0.8). PET/MRI의 $SUV_{LBM}$ 평가 시 26.3% 감소하여 판단할 것과 조영제를 사용할 경우는 29.3% 감소하여 판단할 것으로 생각한다. PET/CT와 PET/MRI의 모든 영상의 판독에서는 $SUV_{LBM}$-최대치를 사용하는 것이 편리성과 효율성을 고려하여 임상의나 연구자들에게 많은 도움이 되리라 판단된다.

핵의학 영상 물리 및 기기의 최신 동향 (Current Status of Imaging Physics & Instrumentation In Nuclear Medicine)

  • 지난 몇 년 동안 핵의학 영상 물리 및 기기 관련 융합영상기기 분야에 많은 발전이 있었다. 최근 몇 년 동안 국내 PET/CT 설치의 급속한 증가는 이들의 임상에서의 역할 및 그 중요성을 입증하고 있다. 그러나 PET/CT는 고가이고, 임상적으로 아직도 그 유용성이 입증되는 과정에 있으며, 특히 치료계획을 위한 장비로는, 미국의 경우 다소 많이 설치되었다는 평가가 공존하고 있다. 또한 저가로 해 결할 수 있는 소프트웨어 융합도 가능하기 때문에 PET 대신 반드시 PET/CT가 필요하다는 주장에는 위의 관점에서 그 한계가 있을 수 있다. 그림에도 불구하고 PET/CT 설치는 계속 증가하고 있어 영상획득 또는 영상판독 모두 PET과 CT 분야의 훈련과 교육이 매우 중요하며 이에 대한 지침서가 발간되었다. PET/CT에 비하여 SPECT/CT는 발전속도가 다소 떨어지지만 이에 대한 지침서도 발간되었다. PET/CT와 SPECT/CT 모두 CT는 앞 부분에 PET과 SPECT는 뒷부분에 설치된다. 이는 환자 영상을 얻을 때 동시에 얻는 것이 아니고 CT영상을 얻은 후 PET과 SPECT영상을 얻게되는 단점이 있다. 이와는 달리 MRI/PET경우 뇌영상을 얻을 때 동시에 MRI와 PET영상을 얻을 수 있어 뇌영상 연구에 큰 도움이 될 것으로 예상하고 있다. 앞으로 기술이 발전함에 따라 전신용 MRI/PET과 유방영상을 위한 PET/US 융합영상장치의 개발 가능성이 매우 높다. 이와 함께 SPECT/CT, PET/CT 및 MRI/PET 기기의 발전도 계속될 것으로 판단된다. 핵의학 영상 물리 및 기기 분야는 최첨단 융합영상 장치의 개발과 함께 하드웨어, 소프트웨어 물리적 특성에 대한 보정기술, 영상 재구성 분야의 다양한 연구도 많은 진전이 필요하다. 또한 영상기기의 설계 및 정량화 기법을 연구하기 위한 몬테카를로 시뮬레이션, 영상장비의 성능평가, 동력학 분석, 선량평가, 움직임 보정 및 융합영상의 정량화 및 분석 기법에 대한 연구도 더욱 활발히 진행될 것으로 예측된다.

두경부 편평상피암 환자에서 수술 전 경부림프절 전이 평가에 대한 F-18 FDG PET/CT의 정확도: CT/MRI와의 비교 (Accuracy of F-18 FDG PET/CT in Preoperative Assessment of Cervical Lymph Nodes in Head and Neck Squamous Cell Cancer: Comparison with CT/MRI)

  • 목적: 두경부 편평상피암 환자에서 경부림프절전이에 대한 F-18 FDG PET/CT의 진단성적을 평가하고 CT/MRI와 비교하였다. 대상 및 방법: 수술전 조직검사로 편평상피암으로 진단받은 17명(남: 15명, 여: 2명, 평균나이: $59{\pm}11$세)의 환자를 대상으로 F-18 FDG PET/CT를 시행하였다. 경부림프절의 F-18 FDG섭취 정도는 세 등급(1=배후방사능, 2=간방사능보다 낮거나 같은 경우, 3=간방사능보다 높은 경우)으로 나누었고, 3등급일 때 전이로 진단하였다. 경부림프절전이에 대한 F-18 FDG PET/CT의 예민도와 특이도를 CT/MRI와 비교하였다. 결과: 17명의 환자에서 123개의 경부림프절이 절제되었고 이 중 29개가 전이로 확진되었다. F-18 FDG PET/CT의 예민도는 69%(20/29), 특이도는 99%(93/94), CT/MRI의 예민도는 62%(18/29), 특이도는 96%(90/94)로 경부림프절전이에 대한 두 검사의 진단성적은 유의한 차이가 없었다. 그러나 F-18 FDG PET/CT는 각각의 환자에서 간에 생긴 이차성 원발암과 늑골전이를 발견할 수 있었다. 결론: 두경부 편평상피암에서 경부림프절 전이에 대한 F-18 FDG PET/CT의 특이도는 매우 높았지만 예민도는 비교적 낮았고 CT/MRI와 유의하게 다르지 않았다. F-18 FDG PET/CT의 비교적 낮은 예민도의 원인은 최근 많이 절제되고 있는 잠재 전이의 가능성이 있는 작은 경부림프절을 발견하지 못하는 F-18 FDG PET/CT 의 한계 때문이었다. 그러나 F-18 FDG PET/CT는 이차성 원발암과 원격전이를 부가적으로 평가할 수 있어 환자의 치료 방침 결정에 유용하였다.

핵의학 융합영상의 표준섭취계수 차이에 관한 연구 (Study on the Difference of Standardized Uptake Value in Fusion Image of Nuclear Medicine)

  • PET-CT and PET-MRI which integrates CT using ionized radiation and MRI using phenomena of magnetic resonance are determined to have the limitation to apply the semi-quantitative index, standardized uptake value (SUV), with the same level due to the fundamental differences of image capturing principle and reorganization, hence, their correlations were analyzed to provide their clinical information. To 30 study subjects maintaining pre-treatment, $^{18}F-FDG$ (5.18 MBq/㎏) was injected and they were scanned continuously without delaying time using $Biograph^{TM}$ mMR 3T (Siemens, Munich) and Biograph mCT 64 (Siemens, Germany), which is an integral type, under the optimized condition except the structural differences of both scanners. Upon the measurement results of $SUV_{max}$ setting volume region of interest with evenly distributed radioactive pharmaceuticals by captured images, $SUV_{max}$ mean values of PET-CT and PET-MRI were $2.94{\pm}0.55$ and $2.45{\pm}0.52$, respectively, and the value of PET-MRI was measured lower by $-20.85{\pm}7.26%$ than that of PET-CT. Also, there was a statistically significant difference in SUVs between two scanners (P<0.001), hence, SUV of PET-CT and PET-MRI cannot express the clinical meanings in the same level. Therefore, in case of the patients who undergo cross follow-up tests with PET-CT and PET-MRI, diagnostic information should be analyzed considering the conditions of SUV differences in both scanners.

Effect of Gd-based MR contrast agents on CT attenuation of PET/CT for quantitative PET-MRI study

  • We evaluate the influence of MR contrast agent on positron emission tomography (PET) image using phantom, animal and human studies. Phantom consisted of 15 solutions with the mixture of various concentrations of Gd-based MR contrast agent and fixed activity of [$^{18}F$]FDG. Animal study was performed using rabbit and two kinds of MR contrast agents. After injecting contrast agent, CT or MRI scanning was performed at 1, 2, 5, 10, and 20 minutes. PET image was obtained using clinical PET/CT scan, and attenuation correction was performed using the all CT images. The values of HU, PET activity and MRI intensity were obtained from ROIs in each phantom and organ regions. In clinical study, patients (n=20) with breast cancer underwent sequential acquisitions of early [$^{18}F$]FDG PET/CT, MRI and delayed PET/CT. In phantom study, as the concentration increased, the CT attenuation and PET activity also increased. However, there was no relationship between the PET activity and the concentration in the clinical dose range of contrast agent. In animal study, change of PET activity was not significant at all time point of CT scan both MR contrast agents. There was no significant change of HU between early and delayed CT, except for kidney. Early and delayed SUV in tumor and liver showed significant increase and decrease, respectively (P<0.05). Under the condition of most clinical study (< 0.2 mM), MR contrast agent did not influence on PET image quantitation.

내시경, 초음파, CT, MRI, PET 검사와 민간의료보험의 관련성 연구 (An Association between Private Health Insurance and Use of Endoscopy, Sonography, CT, MRI, and PET)

  • This paper examined the effects of private health insurance (PHI) on the use of endoscopy, sonography, CT, MRI, and PET among Koreans. The data used was the four waves of the Korea Health Panel (2008-2011), and the number of subjects was 141,862 visits. We employed a pooled logistic regression mode which was corrected with cluster errors. The results showed that PHI was positively related to the use of endoscopy, but not related to the use of sonography, CT, MRI, and PET. It is, however, recommended that health policy makers pay attention to the effects of PHI on the use of sonography, CT, MRI, and PET.

COVID-19 Vaccine-Related Axillary and Cervical Lymphadenopathy in Patients with Current or Prior Breast Cancer and Other Malignancies: Cross-Sectional Imaging Findings on MRI, CT, and PET-CT

  • Breast radiologists are increasingly seeing patients with axillary adenopathy related to COVID-19 vaccination. Vaccination can cause levels I-III axillary as well as cervical lymphadenopathy. Appropriate management of vaccine-related adenopathy may vary depending on clinical context. In patients with current or past history of malignancy, vaccine-related adenopathy can be indistinguishable from nodal metastasis. This article presents imaging findings of oncology patients with adenopathy seen in the axilla or neck on cross-sectional imaging (breast MRI, CT, or PET-CT) after COVID-19 vaccination. Management approach and rationale is discussed, along with consideration on strategies to minimize false positives in vaccinated cancer patients. Time interval between vaccination and adenopathy seen on breast MRI, CT, or PET-CT is also reported.

Diagnosis of Hepatocellular Carcinoma Using C-11 Choline PET/CT: Comparison with F-18 FDG, Contrast-Enhanced MRI and MDCT

  • Purpose: The aim of this study was to compare C-11 choline and F-18 FDG PET/CT, gadoxetic-enhanced 3-T MRI and contrast-enhanced CT for diagnosis of hepatocellular carcinoma (HCC). Materials and Methods: Twelve chronic hepatitis B patients suspected of having HCC by abdominal ultrasonography received all diagnostic modalities performed within a one-week timeslot. PET/CT results were analyzed visually by two independent nuclear medicine physicians and quantitatively by tumor to background ratio (T/B). Nine patients then had histopathological confirmation. Results: Six patients had well differentiated HCC, while two and one patient(s) were noted with moderately and poorly differentiated HCC, respectively. All were detected by both CT and MRI with an average tumor size of $5.7{\pm}3.8cm$. Five patients had positive C-11 choline and F-18 FDG uptake. Of the remaining four patients, three with well differentiated HCC showed negative F-FDG uptake (one of which showed negative results by both tracers) and one patient with moderately differentiated HCC demonstrated no C-11 choline uptake despite intense F-18 FDG avidity. The overall HCC detection rates with C-11 choline and F-18 FDG were 78% and 67%, respectively, while the sensitivity of F-18 FDG for non-well differentiated HCC was 100%, compared with 83% of C-11 choline. The average T/B of C-11 choline in well-differentiated HCC patients was higher than in moderately and poorly differentiated cases (p=0.5) and vice versa with statistical significance for T/B of F-18 FDG (p = 0.02). Conclusions: Our results suggested better detection rate in C-11 choline for well differentiated HCC than F-18 FDG PET. However, the overall detection rate of PET/CT with both tracers could not compare with contrast-enhanced CT and MRI.

구강암의 진단에 사용되어지는 PET/CT: 임상 증례 (THE PET/CT IN THE DIAGNOSIS OF ORAL CANCER: CLINICAL CASES)

  • With the development of systemic diagnostic technique in cancer, the diagnostic methods of head and neck region are developing, also. Now, it is usually used computed tomography(CT), magnetic resornance image(MRI) in head and neck cancer and positron emission tomography(PET) is being increased in diagnostic use because of tumor specificity and accuracy. However, CT and MRI show the advantage of showing precise anatomical landmarks, but the disadvantage of these methods is much affecting by anatomical variations and changes. Otherwise, PET presents the imaging of physiologic and biochemical phenomenon and the disadvantage is the difficult differentiation of normal physiologic uptake, the lack of normal anatomical landmarks. PET/CT, the combination of clinical PET and CT imaging in a single unit is introduced recently, and it helps to get more accurate diagnostic interpretation and to improve in evaluating response to therapy, in management of patients with malignant tumors. So, we report the advantages of PET/CT in the diagnosis of oral cancer with review of literatures.
