복합부위 통증증후군의 치료 (Treatments of Complex Regional Pain Syndrome(CRPS))

  • 복합부위 통증증후군은 하나 혹은 그 이상의 사지에 극심한 통증과 장애를 일으키는 질환이다. 하지만 질환의 원인과 경과에 대한 지식이 매우 한정되어 있어, 복합부위 통증증후군의 치료에 대한 최신의 이론들은 다른 신경병증성 통증의 치료에 많은 부분 의존하고 있다. 본문에서는 복합부위 통증증후군의 다양한 치료에 대해 소개하고 있으나, 그 효과에 대해 잘 설계된 논문은 극히 드문 실정이다. 따라서 복합부위 통증증후군에 대한 조기 진단과 다과적인 치료적 접근이 좋은 결과를 얻는데 필수적인 것으로 사료된다.

Concomitant Glomus Tumor with CRPS in the Hand

  • Glomus tumors are benign tumors that account for 1% to 5% of all soft tissue tumors of the hand and are characterized by a triad of sensitivity to cold, localized tenderness and severe paroxysmal pain. Paroxysmal pain is a symptom common not only in glomus tumors but also in CRPS, and the hand is one of the commonly affected sites in patients with both glomus tumors and CRPS. Therefore, it is not easy to clinically diagnose glomus tumors superimposed on already affected region of CRPS patients. We report a case of glomus tumor concomitantly originating with CRPS at the hand.

  • Complex regional pain syndrome (CRPS) is a chronic regional pain disorder that most frequently affects the limbs. It is characterized by hyperalgesia, allodynia, edema, motor disturbance, and vasomotor instability, and typically occurs following surgery or trauma. In type-I CRPS there is no confirmed nerve injury, while peripheral nerve injury is present in type-II CRPS. The multifactorial pathophysiological etiology of CRPS includes inflammation, autoimmune responses, abnormal cytokine production, autonomic dysfunction, altered blood flow, psychological factors, and central cortical reorganization. There are no specific laboratory diagnostic tools for CRPS, and so it is diagnosed clinically. The Budapest criteria are currently the most-accepted diagnostic criteria.

  • Objectives : To report the R/O CRPS patient after Korean medicine treatment, suffering severe buring pain and insomnia. Methods : The patient was a 56-year-old female patient and treated with herbal medicines and acupuncture in 32 days. The treatment effect was evaluated by measuring VNRS of pain, sleeping time and change in mood status. Results : The Korean Medical therapy had good effect on the patient. Especially, relaxing mental treatment is effective in improving and maintaining symptoms. Conclusions : The Korean medical treatments appeared to be effective in reducing R/O CRPS symptoms. Further clinical research of patients with CRPS is needed.

통합적인 한방치료로 호전된 복합부위통증증후군(CRPS) type1 환자 1례 보고 (A Case Report of a Patient Diagnosed with Complex Regional Pain Syndrome (Type 1) Improved by Integrative Korean Medical Treatment)

  • Objectives: This study reports on the efficacy of using integrative Korean medical treatments for Type 1 complex regional pain syndrome (CRPS). Methods: A 48-year-old female patient with Type 1 R/O CRPS was treated with herbal medicines, acupuncture, and pharmacopuncture for 38 days. The chief complaints were severe burning pain, general weakness, sleep disorder, and aggressive and depressed mood. The treatment effect was evaluated by measuring the numerical rating scale (NRS) of pain, improvement of the quality of sleep, and change in mood status. Results: After the hospital treatment, the patient's pain was controlled and the NRS score was decreased. Sleep and mood disorder also improved. Conclusions: The integrative Korean medical treatments appeared to be effective in reducing Type 1 CRPS symptoms. Further clinical research of patients with CRPS is needed.

A Korean nationwide investigation of the national trend of complex regional pain syndrome vis-à-vis age-structural transformations

  • Background: The present study employed National Health Insurance Data to explore complex regional pain syndrome (CRPS) updated epidemiology in a Korean context. Methods: A CRPS cohort for the period 2009-2016 was created based on Korean Standard Classification of Diseases codes alongside the national registry. The general CRPS incidence rate and the yearly incidence rate trend for every CRPS type were respectively the primary and secondary outcomes. Among the analyzed risk factors were age, sex, region, and hospital level for the yearly trend of the incidence rate for every CRPS. Statistical analysis was performed via the chi-square test and the linear and logistic linear regression tests. Results: Over the research period, the number of registered patients was 122,210. The general CRPS incidence rate was 15.83 per 100,000, with 19.5 for type 1 and 12.1 for type 2. The condition exhibited a declining trend according to its overall occurrence, particularly in the case of type 2 (P < 0.001). On the other hand, registration was more pervasive among type 1 compared to type 2 patients (61.7% vs. 38.3%), while both types affected female individuals to a greater extent. Regarding age, individuals older than 60 years of age were associated with the highest prevalence in both types, regardless of sex (P < 0.001). Conclusions: CRPS displayed an overall incidence of 15.83 per 100,000 in Korea and a declining trend for every age group which showed a negative association with the aging shift phenomenon.

부모의 양육지식과 실천 도구 개발 예비연구 (A Pilot Study on Korean Version Development of the Rearing Knowledge and Practice for Infant Parents)

  • 양육지식은 양육실천에 중요한 관련요인이며 양육지식과 양육실천 정도 사정은 영아기 부모에게 중요하다. 저자는 기 개발된 타당성이 검증된 양육지식 척도(CRKS)와 양육실천 척도(CRPS)의 자국내 도입을 위하여 Saramma와 Thomas가 기 개발한 도구들을 채택하였다. 예비연구가 국제적 언어학적 검증단계에 따라서 수행되었다. 조사대상자는 A의 보건소와 B의 오케타니 마사지실을 방문한 영아 부모 20명이었다. 수집된 자료는 기술통계와 Wilcoxon Rank Sum Test가 도구의 이해도와 일반적 특성에 따른 도구의 평균 정도를 비교하기 위해서 사용되었다. 도구의 신뢰도와 타당도는 Cronbach's alpha, Spearman Correlations을 사용하였다. 대상자의 평균 나이는 33.94(${\pm}2.99$)세였고 영아의 평균 월령은 3.35(${\pm}0.58$)개월였다. CRKS와 CRPS의 이해도는 각각 1.80(${\pm}0.65$)점과 1.33(${\pm}0.54$)점으로 사용하기 쉬웠다. CRKS의 평균점수는 22.50(${\pm}4.89$)점으로 중간점수였고 CRPS의 평균점수는 30.75(${\pm}2.04$)점으로 높았다. CRPS의 Cronbach's alpha는 식이영역이 0.71, 성장과 발달 영역이 0.64, 청결과 보호영역이 0.68, 영아자극 영역이 0.77였다. CRPS의 영아자극과 CRKS의 성장과 발달 영역은 유의한 상관관계가 있었다(r=.530, p=.016). 일반적 특성 중에서 의료인이 비의료인에 비해서 유의하게 CRKS 점수가 높았다(p=0.04). CRKS와 CRPS 척도는 지역사회와 임상실무에서 사용이 간편하고 쉽게 응답할 수 있는 도구로 사용되기를 기대한다.

팔체질침(八體質鍼)으로 치료한 복합부위통증증후군 1형 환자 1례에 대한 증례보고 (A Clinical Case Study of a Patient with Complex Regional Pain Syndrome (CRPS) Type I Treated by 8 Constitution Acupuncture)

  • Objectives : The purpose of this study is to report the clinical effects of 8 constitution acupuncture on a patient with complex regional pain syndrome (CRPS) type I. Methods : In this study, a patient with CRPS was treated by 8 constitution acupuncture therapy. He was diagnosed in the Cholecystonia of 8 constitution diagnosis. The progress of symptoms were evaluated by visual analog scale (VAS) and sleeping time. VAS and sleeping time were evaluated at 7:00 am daily. Results : VAS decreased from 10 to 4, while sleeping time increased from 30 minutes to 3 hours. Conclusion : The results suggest that 8 constitution acupuncture is effective for treating symptoms of CRPS type I.

Physical Therapy and Rehabilitation of Complex Regional Pain Syndrome in Shoulder Prosthesis

  • We report a 66-year-old woman with complex regional pain syndrome (CRPS) 1 treated with combined medical and active physical therapy. She was diagnosed with CRPS 1 following partial shoulder prosthesis due to proximal humerus fracture. Despite continuous medication and physical therapy, there was no improvement in her pain and functional outcome. Her overall pain was decreased by stellate ganglion block 3 times in two weeks conducted during the second month of the follow-up period. Following the ganglion blockades, pain and the other symptoms were decreased intermittently but range of motion (ROM) and functional status were not satisfied as much as expected. After the third month of follow-up, her passive and active ROM of the shoulder joint was increased after application of manipulation under general anesthesia. In conclusion, because CRPS 1 remains one of the most difficult pain syndromes, early diagnosis and treatment are important to have adequate functional results from physical therapy. Manipulation under general anesthesia may be an additional effective treatment tool to obtain functional improvement in some patients diagnosed with CRPS 1.

주민주도적 마을만들기의 동태적 측면에 관한 연구 (A Study of Dynamic Characteristics in the Process of Community Building Projects in Korea)

  • Recently, Community Rebuilding Projects (hereinafter CRPs) in Korea have been driven and motivated by community inhabitants by themselves, and highlighted as an alternative approach to overcome the limitation revealed normally in top-down projects of community vitalization by the Government. The CRPs encourage community-lead development because it happens to integrate multiple stakeholders and to help inhabitants to participate in the process of the CRPs by accumulating and enlarging social capital and network among inhabitants, and the social gain consequently raises the efficiency of public investment in revitalizing communities. In this study, the series of Causal Loop Diagrams (CLDs) is to be elaborated and the System-Thinking (ST) approach is to be presented to understand the dynamic characteristics residing in the above development process of rebuilding communities. The ST approach refers to the CLDs analyzing 19 remarkable Korean CRP cases, and leads to a conclusion including some of policy instruments to support the bottom-up CRPs in Korea. This study contributes a theoretical framework to understand the dynamic relation between social capital and community revitalization as well as empirical suggestions to respond to the issues of the CRPs in Korean local governments.
