Ge-68/Ga-68 Rod Sources을 이용한 방사능측정기의 정도관리 (Quality Management of Radionuclide Activity Meter using Ge-68/Ga-68 Rod Sources)

  • PET와 PET/CT의 검사 영상품질관리를 위해서는 F-18 방사능측정기의 정도관리가 필요하다. Ge-68/Ga-68 Rod Sources을 선원을 이용하여 의료기관에서 사용하고 있는 5종류의 방사능측정기의 방사능 측정의 정확도와 정밀도를 확인하였다. CRC-15R, CRC-15 PET, CRC-712M, CRC-15 베타 및 CRC-25PET에 대한 측정값과 기준값 사이의 방사능 양에 따른 상관관계는 0.99999 (P <0.0001)로 매우 높은 선형성을 보였다. 정확도 테스트에서 CRC-15R, CRC-15 PET, CRC-712M, CRC-15 베타 및 CRC-25PET 모델은 각각 -3.232 %, -1.342 %, -2.815 %, -2.913 % 및 -3.089 %를 이었다. 국내와 국제기준인 정확도 ${\pm}10%$, 정밀도 ${\pm}5%$ 이내로 기준을 만족하였고 반감기가 짧은 F-18에 비하여 경제성 있고 비교적 긴 기간 동안 안정적으로 정도관리가 가능하다는 것을 확인하였다.

대장암 동물 모델에 대한 이해 (Understanding animal models on colorectal cancer)

  • Colorectal cancer (CRC) is a third leading cause of cancer-related death in cancer patients. Sporadic and inflammation-related colon carcinogenesis are major mechanism of colorectal cancer. In vivo CRC models have been developed and implicated to understand their mechanisms upon a different type of CRC. Moreover, recently animal models have played important roles in chemopreventive and preclinical trials over the years. In this mini-review, the aim is to introduce various animal models of CRC and help the understanding to establish in vivo experimental plans according to the cancer type of CRC.

FDG 사용 시 Dose Calibrator에 따른 SUV에 미치는 영향 (The Effect of Using Two Different Type of Dose Calibrators on In Vivo Standard Uptake Value of FDG PET)

  • 방사성핵종과 방사능을 측정하는 dose calibrator의 서로 다른 두개의 기기에서 F-18 FDG을 각각 측정하고, 이 측정값에 의한 인체 내 SUV (Standard Uptake Value)에 미치는 영향은 없는지에 대하여 알아보고자 하였다. 이 연구에서 두개의 다른 dose calibrator는 CRC-15 Dual PET, CRC-15R을 사용하였다. 각 dose calibrator에서 F-18 FDG를 2 mL 주사기 3개에 볼륨 1 mL, 2 mL, 3 mL를 만들어 각각 초기 radio activity를 측정한 후 4시간 30분(270분)까지 30분 간격으로 radio activity를 각각 측정하고 기록한다. 초기 radio activity 값을 기준으로 방사선붕괴 공식으로 산출된 값(decay factor)과 dose calibrator로 측정된 값 간의 직선성을 단순선형을 통하여 분석하였다. CRC-15 Dual PET에서 가장 이상적인 값에 가까운 측정값을 볼륨을 기준으로 하여, CRC-15R에서 측정된 값을 최적화하기 위한 선형회귀식을 회귀분석을 통하여 구한다. 선형회귀식의 산출값을 적용하여 PET/CT 검사를 시행한 50명을 대상으로 lung, liver, region 부위에 ROI을 그려 SUV를 구한다. CRC-15 Dual PET, CRC-15R에서 측정한 radio activity와 그 값을 이용한 SUV를 paired t-test로 통계적으로 유의한 차이가 있는지 알아보았다. CRC-15 Dual PET과 CRC-15R에서 측정한 radio activity의 상관분석결과 1 mL의 경우 상관성을 보여주는 r값은 r=0.999이었으며, 회귀분석에 위한 선형식은 y=1.0345x+ 0.2601이었다. 2 mL의 경우 r=0.999이며, 선형회귀식은 y=1.0226x+0.1669이었다. 3 mL의 경우 r=0.999이며, 선형회귀식은 y=1.0094x+0.1577이었다. 각 세 가지 볼륨에서 얻어진 선형회귀식의 산출 값을 이용하여 구한 lung, liver, region 부위에 ROI의 SUV에서의 차이는 1 mL의 경우, lung, liver, region은 (p<0.0001)로 t-test 결과 모두 유의한 차이가 있었다. 2mL의 경우, lung (p<0.002), liver, region은 ROI의 SUV에서 유의한 차이(p<0.0001)를 보였다. 또한 3 mL의 경우, lung(p<0.044), liver, region의 유의한 차이(p<0.0001)가 있었다. F-18 FDG 검사에서 사용되는 두 개의 dose calibrator CRC-15 Dual PET, CRC-15R의 radioactivity에 대한 측정치에서는 상관관계의 차이가 없다는 것을 알 수 있었다. 그러나 이 두 값을 통해 SUV는 인체 내의 uptake 정도에 있어서는 유의한 차이를 보인다는 것을 알 수 있었다. 따라서 이 두개의 dose calibrator를 사용함에 있어서, 이 두 값이 인체 내 SUV의 차이를 고려하여 사용하여야 할 필요성이 요구된다.

철도에서 병렬 순환 잉여 기법을 이용한 차세대 무선인식 시스템에 관한 연구 (A Study on the Advanced RFID System in Railway using the Parallel CRC Technique)

  • This paper has presented the parallel cyclic redundancy check (CRC) technique that performs CRC computation in parallel superior to the conventional CRC technique that processes data bits serially. Also, it has showed that the implemented parallel CRC circuit has been successfully applied to the inductively coupled passive RFTD system working at a frequency of 13.56㎒ in order to process the detection of logical faults more fast and the system has been verified experimentally. In comparison with previous works, the proposed RFID system using the parallel CRC technique has been shown to reduce the latency and increase the data processing rates about 15% In the results. Therefore, it seems reasonable to conclude that the parallel CRC realization in the RFID system offers a means of maintaining the integrity of data in the high speed RFID system.

Accuracy of Self-Checked Fecal Occult Blood Testing for Colorectal Cancer in Thai Patients

  • Purpose: Colorectal cancer (CRC) screening with fecal occult blood testing (FOBT) has been associated with a reduction in CRC incidence and CRC-related mortality. However, a conventional FOBT requires stool collection and handling, which may be inconvenient for participants. The EZ-Detect$^{TM}$ (Siam Pharmaceutical Thailand) is a FDA-approved chromogen-substrate based FOBT which is basically a self-checked FOBT (no stool handling required). This study aimed to evaluate the accuracy of EZ-Detect for CRC detection. Methods: This prospective study was conducted in the Faculty of Medicine, Siriraj Hospital, Bangkok, Thailand between November 2013 and May 2014. Some 96 patients with histologically-proven CRC and 101 patients with normal colonoscopic findings were invited to perform self-checked FOBT according to the manufacturer's instructions. Results were compared with endoscopic and pathologic findings. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for CRC detection were calculated. Results: The present study revealed the sensitivity, specificity, PPV and NPV of this self-checked FOBT for CRC detection to be 41% (95% CI: 31-51), 97% (95% CI: 92-99), 93% (95% CI: 81-98) and 63% (95% CI: 55-70), respectively. The overall accuracy of the self-checked FOBT for identifying CRC was 70%. The sensitivity for CRC detection based on 7th AJCC staging was 29% for stage I, 32% for stage II and 50% for stage III/IV (P=0.19). The sensitivity was 33% for proximal colon and 42% for distal colon and rectal cancer (P=0.76). Notably, none of nine infiltrative lesions gave a positive FOBT. Conclusions: The self-checked FOBT had an acceptable accuracy of CRC detection except for infiltrative tumors. This home-administrated or 'DIY' do-it-yourself FOBT could be considered as one non-invasive and convenient tool for CRC screening.

컨볼루션 부호를 적용한 산업용 무선 콘트롤러에 관한 연구 (A Study on the Radio Controller with Convolution Coding for Industry application)

  • 본 논문에서는 현재 산업현장에서 사용되고 있는 유선 콘트롤러와 무선 콘트롤러의 문제점을 보완하기 위해 컨볼루션 부호와 CRC부호를 적용한 무선 콘트롤러를 구현하였다. 콘트롤러의 제어를 위해 마이크로프로세서를 이용하였으며, 송수신 프로그램에 컨볼루션 부호를 적용하였을 때와 적용하지 않았을 때의 각각의 경우에 데이터 수신율을 테스트하여 비교해 보았다. 무선 콘트롤러의 신뢰도 향상을 위해 적용한 컨볼루션 부호는 구속장 k=3, 부호율 1/2인 부호를 사용하였고, CRC 부호는 POLYNOMIAL함수를 /Χ$^{15}$ +Χ$^{2}$+1로 사용하여 총 CRC비트는 16비트(2바이트)가 되게 하였다.

Characteristics of Young Colorectal Cancer in Brunei Darussalam: an Epidemiologic Study of 29 Years (1986-2014)

  • Background: Colorectal cancer (CRC) is the most common gastrointestinal cancer and the incidence is increasing. CRC is more common with increasing age, but a proportion occurs in young adults, termed young CRC. This study assessed the incidence and the demographic of young CRC in Brunei Darussalam. Materials and Methods: All histologically proven CRC between 1986 and 2014 registered with the Department of Pathology cancer registry were reviewed and data extracted for analyses. Young CRC was defined as cancer in patients aged less than 45 years. The various population groups were categorized into locals (Malays, Chinese and Indigenous) and expatriates. Results: Over the study period, there were 1,126 histologically proven CRC (mean age $59.1{\pm}14.7$ years, Male 58.0%, Locals 91.8% and 8.2% expatriates). Young CRC accounted for 15.1% with the proportion declining over the years, from 29% (1986-1990) to 13.2% (2011-2014). The proportion of young CRC was highest among the indigenous (30.8%), followed by the expatriates (29.3%), Malays (14.3%) and lowest among the Chinese (10.8%). The mean age of young CRC was $35.9{\pm}6.2$; lowest among the indigenous ($33.5{\pm}6.7$), expatriate ($34.9{\pm}6.0$) groupd and the Malays ($35.6{\pm}6.5$) compared to the Chinese ($38.6{\pm}4.6$), a similar trend being observed in the non-young CRC groups. There were no difference between the genders and tumor locations (rectum or colon) between the young and the non-young CRC cases. Female young CRC was significantly younger than male (p<0.05) without any significant variation between the various population groups (p>0.05). Conclusions: Our study showed that the young CRC accounted for 15.1% of all CRC with declining trend observed over recent years. Young CRC was more common among indigenous, expatriates and Malays and least common among the Chinese. There were no differences in the gender and tumor locations.

Detection of MicroRNA-21 Expression as a Potential Screening Biomarker for Colorectal Cancer: a Meta-analysis

  • Background: Colorectal cancer (CRC) is a major cause of cancer-related death and cancer-related incidence worldwide. The potential of microRNA-21 (miR-21) as a biomarker for CRC detection has been studied in several studies. However, the results were inconsistent. Therefore, we conducted the present meta-analysis to systematically assess the diagnostic value of miR-21 for CRC. Materials and Methods: Using a random-effect model, the pooled sensitivity (SEN), specificity (SPE), positive likelihood ratio (PLR), negative likelihood ratio (NLR), and diagnostic odds ratio (DOR) were calculated to evaluate the diagnostic performance of miR-21 for CRC. A summary receiver operating characteristic (SROC) curve and an area under the curve (AUC) were also generated to assess the diagnosis accuracy of miR-21 for CRC. Q test and I2 statistics were used to assess between-study heterogeneity. Publication bias was evaluated by the Deeks' funnel plot asymmetry test. Results: A total of 986 CRC patients and 702 matched healthy controls from 8 studies were involved in the meta-analysis. The pooled results for SEN, SPE, PLR, NLR, DOR, and AUC were 57% (95%CI: 39%-74%), 87% (95%CI: 78%-93%), 4.4 (95%CI: 2.4-8.0), 0.49 (95%CI: 0.32-0.74), 9 (95%CI: 4-22), and 0.83 (95%CI: 0.79-0.86), respectively. Subgroup analyses further suggested that blood-based studies showed a better diagnostic accuracy compared with feces-based studies, indicating that blood may be a better matrix for miR-21 assay and CRC detection. Conclusions: Our findings suggest that miR-21 has a potential diagnostic value for CRC with a moderate level of overall diagnostic accuracy. Hence, it could be used as auxiliary means for the initial screening of CRC and avoid unnecessary colonoscopy, which is an invasive and expensive procedure.

MicroRNAs in Colorectal Cancer: from Diagnosis to Targeted Therapy

  • Colorectal cancer (CRC) is one of the major healthcare problems worldwide and its processes of genesis include a sequence of molecular pathways from adenoma to carcinoma. The discovery of microRNAs, a subset of regulatory non-coding RNAs, has added new insights into CRC diagnosis and management. Together with several causes of colorectal neoplasia, aberrant expression of oncomiRs (oncogenic and tumor suppressor miRNAs) in cancer cells was found to be indirectly result in up- or down-regulation of targeted mRNAs specific to tumor promoter or inhibitor genes. The study of miRNAs as CRC biomarkers utilizes expression profiling methods from traditional tissue samples along with newly introduced non-invasive samples of faeces and body fluids. In addition, miRNAs could be employed to predict chemo- and radio-therapy responses and be manipulated in order to alleviate CRC characteristics. The scope of this article is to provide a comprehensive review of scientific literature describing aberrantly expressed miRNAs, and consequently dysregulation of targeted mRNAs along with the potential role of miRNAs in CRC diagnosis and prognosis, as well as to summarize the recent findings on miRNA-based manipulation methods with the aim of advancing in anti-CRC therapies.

Methylenetetrahydrofolate Reductase Gene Germ-Line C677T and A1298C SNPs are Associated with Colorectal Cancer Risk in the Turkish Population

  • Colorectal cancer (CRC) is the third most common cause of death due to cancer in the worldwide and the incidence is also increasing in Turkey. Our present aim was to investigate any association between germ-line methylenetetrahydrofolate reductase (MTHFR) C677T and A1298C polymorphisms and CRC risk in Turkey. A total of 86 CRC cases and 212 control individuals of the same ethnicity were included in the current study. Peripheral blood-DNA samples were used for genotyping by StripAssay technique, based on the reverse-hybridization principle and real-time PCR methods. Results were compared in Pearson Chi-square and multiple logistic regression models. The MTHFR 677TT (homozygous) genotype was found in 20.9% and the T allele frequency 4.2-fold increased in CRC when compared with the control group.The second SNP MTHFR 1298CC (homozygous) genotype was found in 14.0% and the C allele frequency 1.4-fold elevated in the CRC group. The current data suggest strong associations between both SNPs of germ-line MTHFR 677 C>T and 1298 A>C genotypes and CRC susceptibility in the Turkish population. Now the results need to be confirmed with a larger sample size.
