임파부종 환자의 보존적 치료법 (Conservative Treatment of Lymphedema)

  • The purpose of this article was to provide basic knowledge and treatment principles of patient with lymphedema, which was usually not treated at all, or the treatment given didn't work efficiently. Lmphedema is defined as an abnormal accumulation of protein-rich fluid, edema, and chronic inflammation within an extremity. Lmphedema may be classified as either primary results from defects with aplasia, hypoplasia, and hyperplasia in the lymphatic system at birth or secondary is caused by known precipitating factors such as cancer, infection, inflammation, radiation, surgery, or trauma etc. There are essentially several conservative treatment methods which has been utilized successfully to treat lymphedema in Samsung Medical Center. We used following procedures: CPT (Complex Physical therapy) or CDP (Complex Decongesitive Physical therapy) such as skin care, MLD (Manual Lymph Drainage), compression with short-stretch bandage, exercise, elevation, elastic stocking, and pneumatic compression. Our experiences shows that conservative treatments can significantly reduce lymphedema and prevent different complications.

개심술후 혈청효소 변화에 관한 연구 (A clinical study on the changes of serum enzymes after open heart operation under the cardiopulmonary bypases)

  • The changes of serum creatine phosphokinase [CPK], serum lactic dehydrogenase [LDH], serum glutamic oxaloacetic transaminase [GOT] and serum glutamic pyruvic transaminase [GPT] were studied in a total of 82 cases who underwent open heart operation under the cardiopulmonary bypass at the Department of Thoracic and Cardiovascular Surgery, Chungnam National University Hospital. The results were as follows 1. The average values of CPK and COT after the operation were reached to the maximal values on the first postoperative day, which were returned to the normal range until the fifth postoperative day. The average values of LDH and CPT after the operation were reached to the maximal values on the first and second postoperative day respectively, which were significantly increased until the seventh postoperative day. 2. In the relationship of the serum enzymes and duration of the extracorporeal circulation, the values on the group over 90 minute of the extracorporeal circulation were more increased than on the group below 90 minute of the extracorporeal circulation. 3. In the relationship of the changes of the serum enzymes and congenital heart diseases and acquired heart diseases, there were no significantly differences in the values between the two groups.

The effect of different functionalities of FGM and FGPM layers on free vibration analysis of the FG circular plates integrated with piezoelectric layers

  • The present paper deals with the free vibration analysis of the functionally graded solid and annular circular plates with two functionally graded piezoelectric layers at top and bottom subjected to an electric field. Classical plate theory (CPT) is used for description of the all deformation components based on a symmetric distribution. All the mechanical and electrical properties except Poisson's ratio can vary continuously along the thickness direction of the plate. The properties of plate core can vary from metal at bottom to ceramic at top. The effect of non homogeneous index of functionally graded and functionally graded piezoelectric sections can be considered on the results of the system. $1^{st}$ and $2^{nd}$ modes of natural frequencies of the system have been evaluated for both solid and annular circular plates, individually.

  • 소형 루비듐 원자시계를 만들기 위해 원자에서의 CPT(coherent population trapping) 현상, 유도 라만 산란(SRS: Stimulated Raman Scattering), 루비듐 메이저 등 다양한 방법들이 소개되고 있다$^{(1, 3)}$ . 본 연구에서는 루비듐 원자 주파수 표준기를 위해 루비듐 셀을 이용한 마이크로파 주파수 표준기(즉, 원자시계)에 관한 연구를 수행하였다. 루비듐 셀을 이용한 원자시계는 가격이 저렴하고 부피가 작아 휴대용으로 적합하다. 따라서 정보통신 분야에서 동기 신호 발생기에 사용되는 등 다양한 응용분야를 갖고 있다. 갈근이 비만 랫드 간조직의 비만관련 유전자 발현에 미치는 영향 (The effects of Pueraria lobata extract on gene expression in liver tissue of rat with estrogen-deficient obesity)

  • Objective : It is known that Pueriaria lobata has an anti-osteoporetic effect, anti-cancer effect, anti-pyretic effect, and anti-diabetic effect. The aim of this study was to evaluate anti-obesity effect of Pueriaria lobata extract (PLE), and elucidate the effect of it on gene expression related to lipid metabolism. Method : The experiments were performed with the use of ovariectomized rats as estrogen-deficient obesity model. They were grouped NC (normal control), OC (estrogen-deficient control), PLH (100mg/kg of PLE), PLL (20mg/kg). PLE was orally administered for 6 weeks. Body weights and serum lipid level were estimated, and real-time PCR was performed to investigate the effect of PLE on gene expression in liver. Results : PLE decreased the body weight and serum cholesterol and triglyceride, but increased HDL-cholesterol. And PLE increased leptin, CYP27, CPT1, CYP8B1, ACAT2, LDLR, and SCD1, but reduced $PPAR{\gamma}$, PGC1A, HMG-CoA-R, ACAT1, SCD1, and APoB gene expression in liver tissue of rat with estrogen-deficient obesity. Conclusion : It is concluded that Pueriaria lobata reduced body weight, and its effect was expressed by regulation of gene expression related to lipid metabolism in rats with estrogen-deficient obesity.

Choline supplementation improves the lipid metabolism of intrauterine-growth-restricted pigs

  • Objective: The objective of this study was to investigate the effects of dietary choline supplementation on hepatic lipid metabolism and gene expression in finishing pigs with intrauterine growth retardation (IUGR). Methods: Using a $2{\times}2$ factorial design, eight normal birth weight (NBW) and eight IUGR weaned pigs were fed either a basal diet (NBW pigs fed a basal diet, NC; IUGR pigs fed a basal diet, IC) or a diet supplemented with two times more choline than the basal diet (NBW pigs fed a high-choline diet, NH; IUGR pigs fed a high-choline diet, IH) until 200 d of age. Results: The results showed that the IUGR pigs had reduced body weight compared with the NBW pigs (p<0.05 from birth to d 120; p = 0.07 from d 120 to 200). Increased (p<0.05) free fatty acid (FFA) and triglyceride levels were observed in the IUGR pigs compared with the NBW pigs. Choline supplementation decreased (p<0.05) the levels of FFAs and triglycerides in the serum of the pigs. The activities of malate dehydrogenase and glucose 6-phosphate dehydrogenase were both increased (p<0.05) in the livers of the IUGR pigs. Choline supplementation decreased (p<0.05) malate dehydrogenase activity in the liver of the pigs. Gene expression of fatty acid synthase (FAS) was higher (p<0.05) in the IC group than in the other groups, and choline supplementation decreased (p<0.05) FAS and acetyl-CoA carboxylase ${\alpha}$ expression in the livers of the IUGR pigs. The expression of carnitine palmitoyl transferase 1A (CPT1A) was lower (p<0.05) in the IC group than in the other groups, and choline supplementation increased (p<0.05) the expression of CPT1A in the liver of the IUGR pigs and decreased (p<0.01) the expression of hormone-sensitive lipase in both types of pigs. The gene expression of phosphatidylethanolamine N-methyltransferase (PEMT) was higher (p<0.05) in the IC group than in the other groups, and choline supplementation significantly reduced (p<0.05) PEMT expression in the liver of the IUGR pigs. Conclusion: In conclusion, the lipid metabolism was abnormal in IUGR pigs, but the IUGR pigs consuming twice the normal level of choline had improved circulating lipid parameters, which could be related to the decreased activity of nicotinamide adenine dinucleotide phosphate-generating enzymes or the altered expressions of lipid metabolism-related genes.

db/db 마우스 동물모델에서 Vigna nakashimae 클로로포름층 분획물의 항당뇨 효능 (Antidiabetic Activity of a Chloroform Fraction of Vigna nakashimae in db/db mice)

  • 최근 식생활의 변화에 따라 당뇨병, 비만 등의 만성대사질환의 발병률이 증가함에 따라 예방과 치료를 위한 물질연구가 많이 진행되고 있다. 본 연구에서는 선행 연구에서 항당뇨 효능이 확인된 Vigna nakashimae (VN) 메탄올 추출물로부터 VN 클로로포름층 분획물과 물층 분획물을 얻어 각 분획물의 항당뇨 효능과 분자적 기전을 살펴보았다. 각각의 VN 분획물을 제 2형 당뇨병질환모델인 db/db 마우스에 경구투여하여 VN 분획물의 혈당 및 지질 대사 변화 등을 측정하였다. VN 클로로포름층 분획물은 공복 혈당과 당화혈색소를 물층 분획물보다 더 효과적으로 감소시켰다. 또한 내당능도 개선하였으며, 혈중 지방산과 중성지방도 감소시켰다. VN 클로로포름층 분획물은 HepG2와 C2C12세포에서 인산화된 AMPK와 AMPK 하위 유전자인 CPT-1의 유전자 발현을 증가시켰다. VN 클로로포름층 분획물은 AMPK의 활성화와 일치하게 HepG2에서 당 생성 효소인 PEPCK와 G-6Pase의 발현을 감소시켰으며, C2C12에서는 당 유입을 증가시켰다. 이상의 결과는 VN 클로로포름층 분획물은 AMPK의 활성화를 통해 공복혈당을 감소시켰으며, 이러한 효과는 물층 분획물보다 더 효과적이었다.

Fermented Kochujang Supplement Shows Anti-obesity Effects by Controlling Lipid Metabolism in C57BL/6J Mice Fed High Fat Diet

  • The aim of the present study was to assess the anti-obesity effects of fermented kochujang supplement in C57BL/6J mice. Thirty mice were divided into 3 groups; normal diet control group (ND), high fat diet control group (HD), and high fat diet plus kochujang supplemented group (HDK). Results were as follows: 1. Fennented kochujang supplement in high fat diet decreased body weight and epidydimal and back fat weight compared to non-supplement in HD group. 2. Lipid content and blood glucose level were lower in HDK group than HD group. 3. Fermented kochujang supplement increased mRNA level of lipolytic genes such as acyl-CoA synthetase (ACS), carnitine palmitoyltransferase-1 (CPT-1), and uncoupling proteins-1 (UCP-1) expression, whereas decreased mRNA level of adipogenic genes such as acetyl CoA carboxylase (ACC) expression. These findings suggest that fermented kochujang supplement in high fat diet normalized body weight, epididymal and back fat weight, lipid content, and blood glucose levels through controlling lipid metabolism and provides basic information on the control of obesity.

Hepatoprotective Effects of Gardenia jasminoides Ellis Extract in Nonalcoholic Fatty Liver Disease Induced by a High Fat Diet in C57BL/6 Mice

  • This study was carried out to investigate the potential effects of Gardenia jasminoides (GJ) extracts, on hepatic steatosis and lipid metabolism in mice fed with high-fat diet (HFD). GJ extracts (100 mg/kg, ${\times}10$ weeks) fed mice showed reduced body weight, adipose tissue weight, reduced aminotransferase level in plasma and hepatic lipid (triglyceride, total cholesterol) content. These effects were accompanied by decreased expression of lipogenic genes, sterol regulatory element binding protein-1c (SREBP-1c), liver X receptor (LXR), fatty acid synthase (FAS), acetyl-CoA carboxylase (ACC), cluster of differentiation 36 (CD36), lipoprotein lipase (LPL) and decreased lipogenic enzyme FAS and HMG-CoAR enzyme activities while elevating carnitine palmitoyltrasferase-1 (CPT) activity. Based on these results, we speculated that the inhibitory effect on hepatic steatosis of GJ extract containing geniposide is the result of suppression of lipid synthesis in mice fed with HFD, suggesting that GJ extract may be beneficial in preventing hepatic steatosis.

Anti-adipogenic Effect of Hydrolysate Silk Fibroin in 3T3-L1 Cells

  • Hydrolysate silk fibroin (HSF) is a fibrous protein composed of parallel $\beta$-structures and is made from pure silk elements including 18 amino acids, with glycine, alanine, and serine comprising of over 80% of the amino acids. Numerous studies have documented a range of effects of HSF, including moisturizing, antioxidant activity, nervous system disorders, and many more. We investigated whether HSF has anti-obesity effects in vitro. The effects of HSF inhibition on lipid accumulation and acceleration of lipid degradation in 3T3-L1 cells were studied. Treatment of 3T3-L1 cells with HSF caused significant inhibition of cell viability, an increase in glycerol release, and a decreased in adipocyte differentiation. Moreover HSF stimulated downregulated of adipogenic enzyme expressions (PPAR${\gamma}$ and C/EBP${\alpha}$) and up-regulated of fatty oxidation enzyme expressions (CPT-1 and UCP-2). Based on these results, hydrolysate silk fibroin can be suggested as a potential therapeutic substance as part of a prevention or treatment strategy for obesity.
