Reverse Anterolateral Thigh Flap to Reconstruct a Below-Knee Amputation Stump: Two Case Reports

  • During below knee amputation, the amputation stump must be covered with well-vascularized and sensate soft tissue. Many flaps can be used for this purpose, but available reconstructive options are limited. We performed reverse flow ALT flap elevation on two patients with below knee amputations to reconstruct defects in the stumps. The sizes of the defects in the stumps were $4{\times}16$ cm and $5{\times}5$ cm, respectively. The most distal portion of the defects were located 20 cm and 16 cm lateral to the knee joint in a curve, respectively. The size of the elevated flap was $5{\times}18$ cm for case 1 and $18{\times}10$ cm for case 2. The respective pivot points of the pedicles were 7 cm and 6 cm above the patella and the respective lengths of the pedicles were 17 cm and 16 cm. In both cases, venous congestion occurred on the second postoperative day and the flap distal to 10 cm or more from below the knee joint was necrotized at the second postoperative week. Surgeons should be cautious when using a reverse ALT flap to reconstruct a soft tissue defect located 10 cm or more distal from below the knee joint. Since a pedicle longer than 15 cm may develop partial necrosis of the flap, simultaneous application of antegrade venous drainage is recommended.

The Effects of Chelidonium majus on NO and $TNF-{\alpha}$ Production in Macrophages (백굴채가 대식세포의 NO 및 $TNF-{\alpha}$ 생성에 미치는 영향)

  • Objectives : In this study, we investigated the mechanism by which Chelidonium majus (CM) regulates nitric oxide (NO) production. Methods : Using mouse peritoneal macrophages, the mechanism by which CM regulates NO or tumor necrosis $factor-{\alpha}(TNF-{\alpha})$ production was examined. NO release was measured by the Griess method. $TNF-{\alpha}$ production was measured by the ELISA method. The protein extracts were prepared and samples were analyzed for the inducible NOS(iNOS) expression and nuclear factor kappa $B(NF-{\kappa}B)$ activation by Western blotting. Results : When CM was used in combination with recombinant $interferon-{\gamma}{\;}(rIFN-{\gamma})$, there was a marked cooperative induction of NO production. CM had an effect on NO production by itself. The expression of the iNOS gene was increased in $rIFN-{\gamma}$ plus CM-stimulated peritoneal macrophages and almost completely inhibited by pre-treatment with pyrrolidine dithiocarbamate (PDTC), an inhibitor of $NF-{\kappa}B$. The $NF-{\kappa}B$ activation was increased in rIFN-{\gamma} plus CM-induced peritoneal macrophages. The increased production of NO from $rIFN-{\gamma}$ plus CM-stimulated peritoneal rnacrophages was decreased by the treatment with $N^{G}-monomethyl-{_L}-arginine{\;}(N^{G}MMA){\;}N^{\alpha}-Tosyl-Phe$ chloromethyl ketone (TPCK) , and was almost completely inhibited by pre-treatment with PDTC. Furthermore, treatment with CM alone or rIFN-{\gamma} plus CM in peritoneal macrophages caused a significant increase in $TNF-{\alpha}$ production. PDTC decreased CM-induced $TNF-{\alpha}$ production significantly. After CM treatment in HT-29 or AGS cells, cell viability decreased. Conclusions : These findings demonstrate that CM increases the production of NO and $TNF-{\alpha}{\;}by{\;}rIFN-{\gamma}-primed$ macrophages and suggest that NF-B plays a critical role in mediating these effects of CM.

Infection Status with Clonorchis sinensis Metacercariae in Pseudorasbora parva from a Tributary Stream of Naktonggang (River) in Pusan, Korea

  • This study was performed to investigate the infection status with Clonorchis sinensis metacercariae in Pseudorasbora parva from a tributary stream of Naktonggang (River) in Kangso-gu, Pusan. A total of 325 P. parva was collected in 4 sites using fish traps. They were transferred in the laboratory, measured the length and examined by artificial digestion method. In the site I, a total of 72 (80.0%) out of 90 fish examined was infected with 1~665 (112 in average) metacercariae of C. sinensis. The average number of metacercariae infected by the size of fish was 146 in the group of over 8 cm, 61 in 7.0~7.9 cm and 86 in below 6.9 cm. In the site II, a total of 50 (83.3%) out of 60 fish examined was infected with 1~178 (31 in average) metacercariae. The average number of metacercariae was 40 in the group of over 8 cm 35 in 7.0~7.9 cm, 23 in 6.0~6.9 cm and 25 in below 5.9 cm. In the site III, a total of 68 (97.1%) out of 70 fish examined was infected with 1~2,662 (508 in average) metacercariae. The average number of metacercariae was 679 in the group of over 7.0 cm, 444 in 6.0~6.9 cm and 426 in below 5.9 cm. In the site IV, a total of 103 (98.1%) out of 105 fish examined was infected with 1~1,536 (416 in average) metacercariae. The average number of metacercariae was 532 in the group of over 7 cm, 353 in 6.0~6.9 cm and 262 in below 5.9 cm. From the above results, it is confirmed that there are wide differences in the infection status of C. sinensis metacercariae in P. paeva according to investigation sites, and P. parva collected from the lower stream are more heavily infected than those from the upper stream.

The Trend of CM at Risk in the U.S Construction Market (미국 CM at Risk 시장 동향 및 시사점)

  • Alternative project delivery methods including design-build and CM at Risk have evolved as a means of meeting owner's diverse needs and implementing project efficiently with limited budget and time in more competitive construction environment. It is time to consider adopting alternative project delivery methods, especially CM at Risk, for domestic construction industry to enhance competitiveness and to provide various options for owners so that they can select more appropriate project delivery methods for the facility being planned. This research report derives suggestions from investigating how CM at Risk has been evolved in the US construction market. Besides the analysis of CM at Risk market in the US construction industry, the characteristics of companies whose market share are in Top 100, the type of facility usually built by this method were investigated. Based on the analysis of CM at Risk in the US, the prepositions in adopting CM at Risk to domestic construction industry and the possibility of adopting this method have been identified.

Relationship between Vertical Root Distribution and Yield Traits in IRRI's New Plant Type Rice

  • This experiment was conducted to elucidate the relation-ship between vertical distribution of rice roots and yield traits under field conditions. Eight IRRI's new plant type rices (NPTRs) were tested in a volcanic ash soil paddy field under dense (IO 10 cm) and common (20 20 cm) planting densities. These lines were evaluated to have more spikelet numbers per panicle (SNP), lower filled grain rate (FGR), and lower rough grain weight per hill (RGWH). In dense planting, rough grain weight per stem (RGWS) was increased due to heavier culm and leaf dry weight (CLDW), and both RGWS and CLDW were related with the percentage of root distribution (%RWI) in the 10~30 cm soil layer, while in common planting, RGWS was not closely related with CLDW. SNP was highly related with root dry weight (RDW) in the 0~10cm soil layer. FGR was mainly affected by ROW in the 10~30 cm soil layer under both planting densities. RGWS was positively correlated with top dry weight (TDW) and harvest index (HI), and TDW was positively correlated with RWI under common planting or %RWI under dense planting, and HI was positively correlated with RWI in the 10~30 cm soil layer only under dense planting. RGWS was closely related with root weight index by dry weight (RWI) in the 10~30 cm soil layer and %RWI in the 0~30 cm or 10~30 cm soil layer under dense planting, and with only RWI in the 10~30 cm soil layer under common planting. But RGWH showed the close positive relationship with RDW and RWI in the 10~30 cm soil layer under dense planting, while under common planting, it showed the close positive relationship with RWI and %RWI in the 10~30 cm soil layer or %RWI in the 0~30 cm soil layer. The deeper root system in rice, especially under dense planting, is important for high yield of NPTRs focusing on the increment of top mass production and harvest index.

The Critical Concentration and Flow Equation of Aqueous Carboxymethyl Chitin Solution (Carboxymethyl Chitin 수용액의 임계농도와 유동방정식)

  • To elucidate the flow properties of carboxymethyl chitin (CM-chitin), the critical concentration and flow equation of aqueous CM-chitin solution were investigated. The concentration of $0.8\%$ appeared to be the critical concentration. So interaction occurred between polymer chains in the CM-chitin solutions had the higher concentration than $0.8\%$ but not in lower than $0.8\%.\;0.5\%$ CM-chitin solution was revealed as a newtonian flow but $1.0\%$ CM-chitin solution showed a pseudoplastic flow. Flow constants of $3.0\%$ CH-chitin solution were 0.0908cp for $\eta_\infty$, 770cp for $\eta_0$, 0.81 for $\beta$ and 0.36 for n. Therefore, flow equation of $3.0\%$ CM-chitin solution was as follow; $$\eta=0.1+\{{770/(1+0.81D^{0.36})\}$$.

Chemical Composition and Acaricidal Activities of Constituents Derived from Eugenia caryophyllata Leaf Oils

  • The acaricidal activities of 12 commercial constituents derived from Eugenia caryophyllata leaf oils against Dermatophagoides farinae, D. pteronyssinus and Tyrophagus putrescentiae adults were examined using an impregnated fabric disk application and compared with that of the commercial benzyl benzoate as synthetic acaricide. On the basis of $LD_{50}$ values, the most toxic compound was methyl eugenol ($4.13\;{\mu}g/cm^2$), followed by methyl isoeugenol ($4.19\;{\mu}g/cm^2$), isoeugenol ($4.29\;{\mu}g/cm^2$), eugenol ($4.94\;{\mu}g/cm^2$), and acetyl eugenol ($13.91\;{\mu}g/cm^2$) against D. farinae. In the case of D. pteronyssinus, isoeugenol ($2.93\;{\mu}g/cm^2$) was the most toxic, followed by methyl isoeugenol ($3.28\;{\mu}g/cm^2$), methyl eugenol ($3.87\;{\mu}g/cm^2$), eugenol ($3.92\;{\mu}g/cm^2$), and acetyl eugenol ($7.21\;{\mu}g/cm^2$). These results suggest that D. pteronyssinus may be controlled more effectively by the application of eugenol congeners than D. farinae. In comparison with synthetic acaricides, the acaricidal activities of eugenol, isoeugenol, methyl eugenol, and methyl isoeugenol were about 1.9-2.2 times more toxic than benzyl benzoate. Furthermore, the most toxic constituent against T. putrescentiae was exhibited on eugenol ($10.11\;{\mu}g/cm^2$), followed by methyl eugenol ($38.67\;{\mu}g/cm^2$) and acetyl eugenol ($70.09\;{\mu}g/cm^2$), but no activity was observed for isoeugenol and methyl isoeugenol. The results suggested that eugenol congeners may be useful as a new source for selective control of house dust mites and stored food mites.

Analysis of Research Trend by Technical Field of Construction Management Using Subject Classification Code (주제분류코드에 의한 국내외 건설사업관리(CM) 기술 분야별 연구 현황분석)

  • Recently, the application of construction management system is being increased and various research activities by each specific field are going on many universities and research institutes. It is necessary to understand the latest research trends by each subject for keeping up excellent products in CM field. This study suggests a subject breakdown structure for classifying detailed technologies of CM field. And over 2,000 domestic and international papers in the last five years are analyzed for analogizing research trends by each subject. The analyzed results include research trends by year and strong and weak fields by each research subject of CM technology. Finally, this study suggests an improved countermeasure for guaranteeing sustainable research activation in CM field.

A Study of the Development of Formal Pants Patterns for Women in Their 20s and 30s with Lower-body Obesity (20~30대 하체 비만형 여성을 위한 정장 바지 패턴 연구)

  • The purpose of this study is to provide a basic pattern to assist in the development of formal pants that reflect the physical characteristics of women with lower-body obesity. The subjects selected for this study were women in their 20s and 30s with a Rohrer index of 1.6 or higher, a waist circumference 78.5cm to 83.5cm, and a lower drop of at least 18cm. The experimental pants patterns were developed by varying the waist line position, the waist ease, the ease in the hip area, the crotch length, the front and rear crotch width, the leg width, and the total length. Data analysis was performed using the SPSS statistics program(version 18.0). To verify the difference, this study used a variance analysis and Duncan's test. The conclusions drawn by the study are as follows. To reflect the form element of an abdomen with fatty deposits, the waist circumference should have an ease of 2cm. The front waist result was W/4+0.5cm+0.75cm and the rear waist result was W/4-0.5cm+0.25cm. The hip circumference showed a total ease of 4.5cm. The front hip result was H/4+0.75cm and the rear hip result was H/4+1.5cm. The crotch width set to H/16-1cm and the applied rear crotch width was H/16+2cm. The rear hip area was given more ease to reflect the protrusion of the hip. Along the line extending from the knee area to the crotch line, the outer seam line was designed with a gradual S-shaped curve to accommodate the protrusion from the maximum thigh width.

Dyeing Properties of Silk Fabrics with Pteridium aquilinum Extract (고사리잎 추출액을 이용한 견직물 염색성)

  • In this study, the characteristics of catechol tannin and pyrogallol tannin contained in Pteridium aquilinum were analysed by It spectrum. Silk fabrics were dyed with Pteridium aquilinum extracts using various mordants, and their dyeing properties were discussed. Additionally the fastness the water digestion, perspiration liquid digestion and light irradiation were investigated. IR spectrum of catechol tannin showed bands of O-H at $3417cm^{-1}$, C-H at $2930cm^{-1}$, C=0 at $1722cm^{-1}$, C=C at $1644cm^{-1},\;CH_2\;at\;1402cm^{-1}$. And IR spectrum of pyrogallol tannin showed bands of O-H at $3409cm^{-1}$, C-H at $3003cm^{-1}\;and\;2933cm^{-11}$, C=0 at $1701cm^{-1}$, C=C at $1582cm^{-11},\;CH_2\;at\;1410cm^{-1}$, CO at carboxylic acid and carboxylic acid ester at $1287cm^{-1}\;and\;1135cm^{-1}$. The maximum absorption wavelength of the extracts appeared at 270.0nm and 311.5nm. The optimum conditions for dyeing silk fabric with Pteridium aquilinum extracts were $80^{\circ}C$, 60min. Surface color of the silk fabric dyed with Pteridium aquilinum extracts was 2.7Y Surface color of the pre-mordanted fabrics with Al, Cu and Fe were 4.3Y, 2.5Y and 4.7Y, respectively. And Surface color of the post-mordanted fabrics with Al, Cu and Fe were 3.7Y, 2.8Y and 0.2GY. The water fastness and the alkaline perspiration fastness were improved in the Al-mordanted silk fabrics. By acidic and alkaline perspiration treatment, ${\Delta}E of the unmordanted fabrics was lower than hat of pre- and post- mordanted fabrics. Also after 40hour irradiation, ${\Delta}E of the unmordanted fabrics was lower than that of pre- and post- mordanted fabrics.
