Effects of Daily Chlorhexidine Bathing on the Acquisition of Multidrug-resistant Organisms and Healthcare-associated Infection in an Intensive Care Unit (중환자실에서 클로르헥시딘 목욕이 다제내성균 획득과 의료관련감염에 미치는 효과)

  • Purpose: The aim of this study was to verify the effects of daily 2% chlorhexidine gluconate (CHG) bathing on the acquisition of multidrug-resistant organisms (MDRO) and healthcare-associated infection (HAI) in a medical intensive care unit (MICU). Methods: The study was a randomized controlled group posttest only design, involving 91 patients in MICU at a tertiary hospital (47 patients in the experimental group and 44 patients in the control group). The 2% CHG bathing was performed daily according to bathing protocol to the patients in the experimental group, and traditional bath was performed every three days to those in the control group. Fisher's exact test and x2 test were used to analyze the data. Results: MDRO were found in 6 patients of the experimental group and in 15 patients of the control group. The difference was statistically significant (p= .016). HAI occurred in 2 patients of the experimental group and in 7 patients of the control group. The difference was not statistically significant (p= .084). Conclusion: The results confirmed that daily bathing with CHG was effective in reducing the incidence of MDRO acquisition. Therefore, it is expected that daily bathing with CHG will be used as an effective nursing intervention to reduce the incidence of MDRO acquisition.

Effects of perineal care in preventing catheter associated urinary tract infections (CAUTI) in intensive care units (ICU) (회음부 간호시 클로르헥시딘과 생리식염수 적용에 따른 유치도뇨관 관련 요로감염 발생률 비교)

  • Purpose: The purpose of this study was to compare the effects of preventing CAUTIs through the practice of using 0.05% chlorhexidine gluconate (CHG) versus normal saline for perineal care in ICU patients. Methods: A randomized controlled trial was used, and participants were randomly allocated to either the CHG group (n=79) or normal saline group (n=81). CAUTI was diagnosed following the criteria of the Centers of Disease Control and Prevention (CDC). Incidence of CAUTI and characte oftics of infection were evaluated. Results: In the CHG group, 8 epofodes of CAUTI were obsevend in 79 patients and 875 foley catheter e ys. Tnce ate of CAUTI was 9.14 per 1,000 foley catheter e ys. Tncre were 2 epofodes of CAUTI in the 81 patients and 837 foley catheter e ys of the normal saline group. Tnce ate of CAUTI was 2.39 per 1,000 foley catheter e ys. Tnc difference between both groups was not significant (p=.1e ConConclusevaluUsing normal saline to provide perineal care to ICU patients with a foley catheter inserted will not increase the risk for CAUTI.

  • The effects of casein hydrolysate on blood pressure (BP) and serum lipid profiles were investigated in spontaneously hypertensive rats (SHR). Twenty-four 6-wk old male SHRs were assigned to 3 groups in a completely randomized design. Experimental groups were as follow: control, market milk group (MLG) and casein hydrolysate group (CHG). Reference blood pressure (RBP) showed average $198.94{\pm}1.46mmHg$. Blood pressure (BP) of CHG reduced 24 mmHg after 2 wk of treatment, but these increased after 3 wk. BP of CHG was significantly lower than BP of Control in experimental time (p<0.05). Serum lipid profiles were not differ significantly among groups (p<0.05). These data suggest that casein hydrolysate may beneficially improvement of blood pressure level in SHR.

A Comparison of Antimicrobial Effect of Two Waterless Alcohol-based Hand Rubs with a Povidone-Iodine Hand Scrub for Surgical Hand Antisepsis (두 가지 알코올제제 손마찰과 포비돈 아이오다인의 외과적 손소독 효과 비교)

  • Purpose: The purpose of this study was to compare the 1% chlorhexidine gluconate/61% ethanol (CHG/ethanol), 45% ethanol/18% 1-propanol (ethanol/propanol) and 7.5% povidone-iodine (PVI) scrub with brush to evaluate their antimicrobial effect. Method: Utilizing repeated measures design, 9 nurses participated in the study. Glove juice sampling procedure was used to evaluate microbial hand counts before the surgical hand antisepsis, one minute after hand wash, and after the surgery. Results: Waterless rub using CHG and ethanol combination resulted in a 3.94 log reduction at 1 min and 2.78 log reduction at 3 hrs. Ethanol/propanol resulted in a 2.42 at 1 min and 2.22 at 3 hrs. The traditional scrub using PVI with brush resulted in a 0.94 at 1 min and 0.95 at 3 hrs (p=.003) and 3 hrs (p=.026) after the surgical hand antisepsis. Repeated measures ANOVA results showed that there was a statistically significant difference among group (p=.002). Duncan post hoc test result showed that the PVI was less effective (p<.05) in sterilizing microbials on hands than CHG/ethanol or ethanol/propanol. Conclusion: Both of the two alcohol-based antiseptic rubs are acceptable alternatives to the PVI with brush for surgical hand antisepsis.

Affecting Factors on Hospital Nurses' Practice of Disinfection: Focused on Alcohol, Chlorhexidine Gulconate, and Povidone Iodine (병원 간호사의 주요 피부소독제 인지도와 수행도에 영향을 미치는 요인 - 알코올, 클로르헥시딘, 베타딘을 중심으로 -)

  • Purpose: This study was performed to identify the affecting factors on hospital nurses' practice of disinfection focused on alcohol, chlorhexidine gluconate (CHG), and povidone iodine (PVI). Methods: The participants in this study were 196 nurses of 3 general hospitals and 2 upgrade general hospitals in 5 cities in Korea. Data were collected through self-reported questionnaires in January, 2011. The collected data were analysed by ANOVA, Pearson correlation and stepwise multiple regression using SPSS/WIN 18.0. Results: The subjects used alcohol and PVI daily for intravenous site care and surgical wound dressing respectively. The mean percentage of awareness in practicing main disinfectants were 80.0% and 72.5% respectively. The awareness and practice were highest in alcohol, but lowest in CHG. The mean percentage of practice of disinfectants was 72.5%. There was positive correlation among awareness and practice of main disinfectants. The awareness and nurses' salary explained 34.6% of variance in practice of disinfectants. Conclusion: An educational program focusing on strategy to enhance hospital nurses' awareness would be effective in improving their practice of disinfectants.

The effect of chlorhexidine on reduction of viable organisms in aerosol produced by ultrasonic scaler (클로르헥시딘이 초음파 치석제거기에 의해 발생된 에어로졸 내 세균 수에 미치는 영향)

  • Purpose: The aerosol generated by ultrasonic scaler can contain bacteria or virus which can penetrate into body through respiratory systems of dentists, dental hygienist or patients. The aim of this study is to evaluate the effect of chlorhexidine digluconate as preoperative mouthrinse or lavage for ultrasonic scaler on the reduction of viable organisms in aerosol produced during periodontal treatment using ultrasonic scaler. Methods: 30 patients with moderate chronic periodontitis were included and divided into 3 groups: Control (no preoperative mouthrinse and tap water as lavage), CHG (preoperative mouthrinse with 0.1% chlorhexidine digluconate and tap water as lavage), CHL (no preoperative mouthrinse and 0.1% chlorhexidine digluconate as lavage). Each patient received scaling or subgingival curettage for 30 min. In CHG group, mouthrinse with chlorhexidine digluconate was performed for 1 min. before treatment. Before, during and after scaling or subgingival curettage, air sampling was performed for 7 min. each (1000 L/7 min.) with trypticase-soy agar plate. Agar plates were incubated in $37^{\circ}C$ aerobically. The numbers of colony-forming units (CFU) were counted and compared. Results: The numbers of CFUs of the samples obtained during treatment were $97{\pm}14.0$ in control, $73.1{\pm}14.9$ in CHG group and $44.5{\pm}9.0$ in CHL group. The difference among the 3 groups was determined to be statistically significant (one-way ANOVA with Bonferroni's correction, p-value: 0.0003). In contrast, the numbers of CFU of samples obtained before and after treatment were not significantly different among the groups. Conclusions: Chlorhexidine digluconate used as preoperative mouthrinse or lavage for ultrasonic scaler can reduce the microorganisms in aerosol produced during periodontal treatment using ultrasonic scaler. Less number of microorganisms were detected when chlorhexidine was used as lavage for ultrasonic scaler.

Hand Hygiene Effects Measured by Hand Culture in Intensive Care Unit (배양검사를 통해 확인한 중환자실 직원의 손씻기 효과)

  • The effect of hand hygiene was measured by hand culture before and after hand hygiene for 86 nurses, doctors, and nurses aide/housekeepers in Surgical Intensive Care Unit. The subjects were asked to press their dominant hand in hand-shaped Mannitol salt agar immediately after patient contact and then washed their hand by preferred hand hygiene agents [soap and water, waterless alcohol gel, or 4% chlorhexidine gluconate detergent (CHG)], and cultured one hand again Amount of isolated microorganism was calculated by counting the number of divided areas ($1{\times}1cm$) which is culture positive in hand culture plate. The amount of microorganisms were significantly reduced from 58.1(${\pm}38.59$) to 27.4(${\pm}30.4$) cells after hand hygiene. The staff nurse's hand hygiene was more effective compared to medical doctors and nurses aide/housekeepers. Methicillin-resistant Staphylococcus aureus(MRSA) was isolated in 41(47.1%) subjects ; but only removed 100% in 28(32.2%) subjects. When the amount of hand microorganisms was compared by subject's preferred hand hygiene agents, it was decreased in order of 4% CHG, waterless alcohol solution, soap and water, and water. The hand hygiene practice was inadequate to reduce hand microorganisms and significantly different by occupations. Further research and development of hand hygiene improvement program which emphasize the quality of hand hygiene is recommended.

  • 인공위성 기반의 원격탐사자료는 홍수, 가뭄 등 자연재해에 대한 모니터링 및 예측에 활용되어 왔으며, 특히 인공위성을 이용한 광역적 강수량 추정 자료는 지형적 제약을 받는 지상관측자료와 비교하여 시공간적으로 연속적이고 균질한 강수량 자료 취득이 가능하다는 장점이 있다. 우리나라의 경우 상대적으로 조밀한 지상관측망이 구축되어 있어 공간적으로 상세한 강수량 정보를 생산할 수 있는 여건을 갖추고 있지만, 북한 지역의 경우 기상, 수문, 통계자료에 관한 자료의 접근 및 품질의 제한성으로 인해 미계측 지역에 대한 강수량의 추정에 한계가 있다. CHIRPS (Climate Hazards Group InfraRed Precipitation with Stations) 데이터는 1999년부터 미국국제개발처 (U.S. Agency for International Development, USAID), 미국항공우주국 (National Aeronautics and Space Administration, NASA), 미국해양대기청 (National Oceanic and Atmospheric Administration, NOAA)의 지원으로 개발된 전지구 강우데이터 자료이다. CHIRPS는 1981년부터 현재까지 전지구 강우자료를 0.05도 격자 해상도로 제공하고 있으며, 강수량의 추세 분석 및 가뭄 모니터링을 위해 활용되고 있다. 본 연구에서는 CHG (Climate Hazards Group)에서 제공하고 있는 인공위성을 이용한 광역적 강수량 추정 자료인 CHIRPS와 남한 및 북한의 지상관측 강수량 자료와의 비교를 통해 위성으로부터 유도된 격자 강수량자료의 정확도 및 지역적인 강수추정의 불확실성을 평가하고, 수자원 및 재해 분야 이용 가능성을 검토하고자 한다.

Superficial and Deep Skin Preparation with Povidone-Iodine for Ventriculoperitoneal Shunt Surgery : A Technical Note

  • Objective : Ventriculoperitoneal (VP) shunt surgery is a common and effective treatment for hydrocephalus and cerebrospinal fluid disorders. Infection remains a major cause of morbidity and mortality after a VP shunt. There is evidence that a deep skin flora microbiome may have a role to play in post-operative infections. In this technical note, we present a skin preparation technique that addresses the issue of the skin flora beyond the initial incision. Methods : The patient is initially prepped, as standard, with. a single layer of 2% CHG+70% isopropyl alcohol. The novel stage is the 'double incision' whereby an initial superficial incision receives a further application of povidone-iodine prior to completing the full depth incision. Results : Of the 84 shunts inserted using the double-incision method (September 2015 to September 2016), only one developed a shunt infection. Conclusion : The double incision approach to skin preparation is a unique operative stage in VP shunt surgery that may have a role to play in reducing acute shunt infection.

Evaluation of porcine intestinal organoids as an in vitro model for mammalian orthoreovirus 3 infection

  • Background: Mammalian orthoreovirus type 3 (MRV3), which is responsible for gastroenteritis in many mammalian species including pigs, has been isolated from piglets with severe diarrhea. However, the use of pig-derived cells as an infection model for swine-MRV3 has rarely been studied. Objectives: This study aims to establish porcine intestinal organoids (PIOs) and examine their susceptibility as an in vitro model for intestinal MRV3 infection. Methods: PIOs were isolated and established from the jejunum of a miniature pig. Established PIOs were characterized using polymerase chain reaction (PCR) and immunofluorescence assays (IFAs) to confirm the expression of small intestine-specific genes and proteins, such as Lgr5, LYZI, Mucin-2, ChgA, and Villin. The monolayered PIOs and three-dimensional (3D) PIOs, obtained through their distribution to expose the apical surface, were infected with MRV3 for 2 h, washed with Dulbecco's phosphate-buffered saline, and observed. Viral infection was confirmed using PCR and IFA. We performed quantitative real-time reverse transcription-PCR to assess changes in viral copy numbers and gene expressions linked to intestinal epithelial genes and antiviral activity. Results: The established PIOs have molecular characteristics of intestinal organoids. Infected PIOs showed delayed proliferation with disruption of structures. In addition, infection with MRV3 altered the gene expression linked to intestinal epithelial cells and antiviral activity, and these effects were observed in both 2D and 3D models. Furthermore, viral copy numbers in the supernatant of both models increased in a time-dependent manner. Conclusions: We suggest that PIOs can be an in vitro model to study the infection mechanism of MRV3 in detail, facilitating pharmaceutical development.
