Survey on the Actual Condition of Injured Wildlife Animals in Gangwon-Do (강원도내 야생동물의 부상실태 조사)

  • This research was conducted to investigate the actual condition of injured wild animals in Gangwon-do between January and December in 2002. The subject of this study was the wounded wild animals on the receipt of the governmental and the civil organizations for medical treatment. Especially, the natural monuments among the wounded were included in this research. 363 cases in birds and 274 cases in mammals were saved from injuries. In the case of the natural monuments, 140 and 7 cases got help in birds and mammals, respectively. Wild animals were injured particularly in summer and the ultimate causes were trauma and dehydration. More than 50% out of the animals under medical treatment from the organizations were dead. It was clear that most causes of the animal injuries were artificial owing to the breakdown of the ecosystem. Consequently, the consistent observation on the control system of the wild animals will be needed. And it will be also necessary to increase research works and the nationwide public city activities on the wild animals.

Clinical experience of ventilator therapy in chest trauma (인공호흡기 치료를 받은 흉부외상 환자의 임상적 고찰)

  • A clinical evaluation was performed with a population of 49 patients of chest trauma, who were diagnosed to undergo ventilator therapy, and had gone through ventilator therapy at the Department of Thoracic and Cardiovascular Surgery, Kyungpook University Hospital. One of most common causes of chest trauma was vehicle accidents [77.5%] with the prevalent age group being their forties. The common findings were multiple rib fractures [89.8%], hemopneumothrax [81.6%], lung contusion [61.2%] and flail chest [44.9%]. Their common combined injuries were the orthopedics and neurosugical injuries [86.7%]. Complications caused by chest trauma were pneumonia, respiratory failure, atelectasis, barotrauma and empyema. Pulmonary infections were commonly associated with mechanical ventilation in the long term group and were best prevented by using bronchial hygiene therapy.The mortality rate was 5.8% of the total patients and that was 38.8% of the patients, who needed ventilator therapy. The causes of death were pneumonia, respiratory failure, acute renal failure and hypovolemic shock. Mechanical ventilation has an important place in the treatment of patients with severe chest trauma.

Oral and maxillofacial injuries in children: a retrospective study

  • Objectives: The purpose of this retrospective epidemiological study was to determine the etiology and pattern of maxillofacial injuries in a pediatric population. Materials and Methods: Data for pediatric maxillofacial trauma patients aged 12 years and younger who were registered at the Department of Pediatric and Preventive Dentistry, Dr. R. Ahmed Dental College and Hospital, Kolkata, India, were reviewed and examined. Patients who were treated between October 2016 and September 2018 were analyzed according to age, sex, cause of injury, frequency and site of facial fractures, and soft tissue injuries. The chi-square tests were carried out for statistical analyses with a significance level of 5%. Results: Of 232 patients with a mean age of 6.77±3.25 years, there were 134 males (57.8%) and 98 females (42.2%). The overall male to female ratio was 1.39:1. The most common causes of injuries were falls (56.5%) and motor vehicle accidents (16.8%). Incidence of falls decreased significantly with age (P<0.001). Dentoalveolar injuries (61.6%) and soft tissue injuries (57.3%) were more common than facial fractures (42.7%). Mandibular fractures (82.8%) were the most common facial fractures, and perioral or lip injuries were the most prevalent injuries in our patient population. There was a positive association between facial fractures and soft tissue injury (P<0.01) (odds ratio 0.26; confidence interval 0.15-0.46). Conclusion: Falls were the leading cause of maxillofacial trauma in our sample of children, and the most common site of fractures was the mandible.

The Relationship between Facial Fractures and Radiologically-proven Cranial Injuries (안면부 골절과 전산화 단층 촬영으로 진단된 두부 손상의 연관성)

  • Purpose: In this study, we retrospectively investigated the medical records of patients with facial fractures and suspected cranial injuries in order to determine if there was any relationship between various facial fracture patterns and cranial injuries. Methods: Medical records were reviewed to identify patients diagnosed with facial fractures who underwent cranial computed tomography (CT) scans. Records were reviewed for gender, age, injury mechanism, facial fracture pattern, and presence or absence of cranial injuries. Facial fracture patterns were classified as isolated fractures (tripod, zygomatic arch, maxilla, orbit, and mandible), combined fractures, or total fractures. Cranial injuries included skull fractures, traumatic subarachnoid hemorrhages, subdural hemorrhages, epidural hemorrhages, and contusional hemorrhages. All cranial injuries were established by using cranial CT scans, and these kinds of cranial injuries were defined radiologically-proven cranial injuries (RPCIs). We evaluated the relationship between each pattern of facial fractures and the incidence of RPCIs. Results: Of 132 eligible patients with facial fractures who underwent cranial CT scans, a total of 27 (20.5%) patients had RPCIs associated with facial fractures. Falls and slips were the most common causes of the fractures (31.8%), followed by assaults and motor vehicle accidents (MVAs). One hundred one (76.5%) patients had isolated facial fractures, and 31 (23.5%) patients had combined facial fractures. Fractures were found most commonly in the orbital and maxillary bones. Patients with isolated maxillary fractures had a lower incidence of RPCIs than those with total mandibular fractures. RPCIs frequently accompanied combined facial fractures. Conclusion: Combined facial fractures had a significant positive correlation with RPCIs. This means that facial fractures caused by stronger or multidirectional external force are likely to be accompanied by cranial injuries.

Clinical Analysis of the Patients with Isolated Low-Velocity Penetrating Neck Injury

  • Purpose: Although there has been substantial progress for the treatment of thoracic trauma, the mortality of the penetrating neck injury is still high, has been reported about 10-15%. However, there has not been a report which is reflecting Korean medical present. We retrospectively analyzed the penetrating neck injury patients based on the Korean Trauma Data Base. Methods: Between December 2013 and June 2017 at the trauma center of the Pusan National University Hospital, Busan, Korea, total of 36 patients with isolated low-velocity penetrating neck injuries were included. We analyzed the patients' age, gender, injury mechanism and causes by medical chart review. Results: Among total of 36 patients, 26 (72.2%) were male and 10 (27.8%) were female. Homicidal neck injuries were most common, followed by accidental and suicidal injuries (47% vs. 33% vs. 19%, respectively). All penetrating injuries in our study were low-velocity trauma such as following: knife (n=16, 44.4%); glass or glass bottle (n=11, 30.6%); scissors (n=4, 11.1%); grinder (n=2, 5.6%); and three (8.3%) of miscellaneous injuries. Twenty-seven (75.0%) patients underwent emergency surgery, and only one (2.8%) patient underwent elective surgery. Eleven (30.6) patients were diagnosed with superficial injuries, including six patients who had conservative treatment. Twelve (33.3%) patients had arterial injuries and 10 (27.8%) patients had venous injuries. The patients who had deep injuries showed significant difference against the patient with superficial injury (98.0 vs. 129.1, p=0.008). Conclusions: Low velocity penetrating injury confined to the neck is able to be successfully treated with prompt surgical management. Regardless of the conditions which are evaluated at emergency department, all penetrating neck injury patients should be regarded as urgent surgical candidates.

Alternative Prevention on Human Error of Fatal Injuries by the Folk Lifts (지게차 사망재해의 인적오류에 대한 대안)

  • According to the statistics, occupational fatal injuries by the fork lifts were about 30 per year in whole industrial. Fork lifts are widely used in various parts of industries to improve the efficiency of the work. In this study, the current regulations to be adequate in industrial site have to be renew in order to prevent the fatal injuries by the fork lifts. Fatal injury analysis were conducted with several accident cases by the fork lifts. For each accident, the causes of the injuries were examined and proper safety measures were proposed. In this study, the fork lift showed a high fatality rate in industrial accidents and no detailed cause analysis of fatal accidents was conducted in terms of unsafe acts or conditions. First, fork lifts were the highest of the machines caused the accidents. In order to prevent fatal injuries by the fork lifts, the tarket was manufacturing industry. Second, the order of the cause of cognitive engineering agenda in the manufacture industrial was visibility, responsibility and affordance, and revision of acts was proposed. Third, there was not a lots of different points of human error between occurrence types and business sizes. Forth, number of fatalities by the attacker was more than by the inducer.

Characteristics of Burn Types, Causes, and Treatment in Hospitalized Female Patients with Industrial Injuries (화상으로 입원한 여성 근로자의 화상유형 및 발생원인, 치료적 특성)

  • Purpose: The purpose of this study was to examine the burn characteristics of female patients hospitalized in a burn center. Methods: This is a retrospective descriptive study. The subjects were 222 female patients aged over 19 years old having burn injuries from work sites between January 1, 2012 and December 31, 2014. Data were collected using electronic medical records about the burn-related characteristics. The data were analyzed descriptively. Results: Many of the subjects were in their forties and fifties. Most subjects showed burns with an area of less than 10% of the body surface with a severity of second degree. Scalding burns were the most frequent. Contact burns were usually of third-degree severity and occurred on hands and wrists. Flame burns affected the largest body surface area and frequently occurred on the face and mainly caused by explosion of a stove or kitchen gas. Chemical burns frequently occurred on the lower limbs. Conclusion: Female workers are exposed to the hazards of burn injuries in industrial accidents. Types of burn differ depending on the workplace conditions. Therefore, preventative measures for burn injuries should be established in accordance with the characteristics of each industry to which female workers belong.

A Retrospective Review of Iatrogenic Skin and Soft Tissue Injuries

  • Background Even though the quality of medical and surgical care has improved remarkably over time, iatrogenic injuries that require surgical treatment including injuries caused by cast and elastic bandage pressure, extravasation, and dopamine-induced ischemia still frequently occur. The goal of this study was to estimate the incidence and analyze the distribution of iatrogenic injuries referred to our department. Methods A retrospective clinical review was performed from April 2006 to November 2010. In total, 196 patients (116 females and 80 males) were referred to the plastic surgery department for the treatment of iatrogenic injuries. We analyzed the types and anatomic locations of iatrogenic complications, along with therapeutic results. Results An extravasation injury (65 cases, 37.4%) was the most common iatrogenic complication in our study sample, followed by splint-induced skin ulceration, dopamine-induced necrosis, prefabricated pneumatic walking brace-related wounds and elastic bandage-induced wounds. Among these, prefabricated pneumatic walking brace-related complication incidence increased the most during the 5-year study period. Conclusions The awareness of the very common iatrogenic complications and its causes may allow physicians to reduce their occurrence and allow for earlier detection and referral to a plastic surgeon. We believe this is the first study to analyze iatrogenic complications referred to a plastic surgery department in a hospital unit.

Ligament/muscle strains, nerve entrapment syndromes, sportsman's hernia, symphysitis, stress fractures and various hip pathologies are reviewed as causes of groin pain are reviewed as common causes of groin pain.

Fracture of the Second Rib: An Indirect Sign of Serious Trauma Like Fracture of the First Rib?

  • Background: The first 3 ribs are anatomically well-protected, shielded by the large thoracic muscles, the shoulder girdle, and the arm. A significant force is required to fracture these ribs; thus, such fractures suggest a high-energy trauma and are associated with injuries to vital organs of the thorax, such as the aorta, the heart, the lungs and the great vessels. Methods: A retrospective analysis was conducted over a 10-year period at a single hospital. The study assessed patients with fractures of the second rib, including their concurrent injuries and the overall severity of their trauma. Results: Among the 76 patients included in the study, the average age was 47.35 years, 81.5% were men, and 19.5% were women. Thirteen patients (17.1%) survived their injuries. The most common causes of injury were road traffic accidents (63%) and pedestrian injuries (22%). The patients who did not survive sustained injuries to an average of 5 additional organs, while survivors had injuries to an average of 2.07 additional organs. Left rib fractures were the most frequently observed (46%). The most serious concurrent injuries reported were to the aorta (5.26%), heart (10.52%), lung (52.36%), head (57.89%), liver (30.2%), spleen (26.31%), and kidney (17.1%). Conclusion: As indicators of serious injury to vital endothoracic organs, isolated fractures of the second rib should be considered equal to first rib fractures in clinical importance.
