A Survey on Patient's Experience on Health Care Utilization and Perception of Health Care System for Developing of a Korean Medicine Clinical Practice Guideline for Traffic Injuries (교통사고상해증후군 한의표준임상진료지침 개발을 위한 환자의 의료이용 경험과 의료체계 인식도 설문조사 연구)

  • Objectives To explore the traffic injury patients' experience on health care utilization of Korean Medicine (KM) practice and perception of health care system for developing a Korean medicine clinical practice guideline (CPG) of traffic injuries by survey method. Methods Two hundred ten patients in suffering from traffic injuries were surveyed from September 1st, 2019 to January 31th, 2020 at 2 University Hospitals (Pusan National Korean Medicine Hospital and Kyung Hee Korean Medicine Hospital at Gangdong) and 1 Spine Specialty Hospital (Jaseng Hospital of Korean Medicine). A structured questionnaire of experience on health care utilization of KM practice and perception of health care system of was distributed to responders by visits and all data were statistically analysed. Results Survey results showed high satisfaction of patients with the experience of KM treatments in order of daoyin exercise (7.8±2.3), chuna manual therapy (7.7±4.0), pharmacoacupuncture (7.4±3.0) etc. Safety concerns were reported in 9.1% subjects and 205 (97.6%) patients answered that collaboration with KM and western medicine is necessary for patients with traffic injuries. For the patients' requirement for extending insurance coverage, the most required therapy was chuna manual therapy (57.5%) and pharmacoacupuncture (42.0%). Conclusions This study presented the realistic patient-centered perception of KM practice and health care system in Korea. These results will provide basic data to be reflected in the process of adaptation for the revision of Korean Medicine CPG for traffic injuries.

A Study on the Cognition of Workers with Postpartum Care Home and Korean Medicine Doctor about Postpartum Care (일부지역 산후조리원 종사자와 한의사의 산후조리에 대한 인식조사)

  • Purpose : This study was carried out to analyze the cognition and realities of postpartum care and to aid the spread of oriental medicine in postpartum care and to get a basic guideline of postpartum care home and Korean Medicine Doctor(K.M.D.)'s role model of postpartum care. Methods : We has made questions about the cognition and realities of postpartum care to 2 groups they are workers with postpartum care Home at Gangdong-gu or Songpa-gu puerperants and K.M. doctors working at Gangdong-gu from December 2005 to April 2006. And then we analyzed collected data by using statistics analysis program, SPSS. Results : Almost of 2 group felt sympathy for importance of postpartum care. 5.9% of workers with postpartum care home had maternity nurse licence, 67.6% of them had nurse license and 26.5% of them had nurse's aide qualification. 97.1% of them had experience at medical institution. Most of K.M.D. used Herb-medication for postpartum care with acupuncture and Moxibustion additionally. Conclusion : Workers with postpartum care home had a comparatively good record of medical care or postpartum care. There was difference between 2 groups about proper management model of postpartum care home.

The Influence of Parenting Attitudes on Adolescents' School Adjustment : Mediating Effects of Ego Resilience and Care (부모양육태도가 청소년의 학교적응에 미치는 영향 : 자아탄력성과 배려심의 매개효과)

  • The purpose of the study is to explore how parenting attitudes, ego resilience, and care affects adolescents' adjustment to school so as to provide a necessary basic guideline needed to seek various intervention methods. To achieve this end, the study referred to the 2014 Annual Report of the Korean Children and Youth Panel Survey. By using both SPSS 19.0 and AMOS 19.0, the study yielded the following results. First, the study showed that parenting attitudes, ego resilience, care, and school adjustment all have a positive correlation. Second, by conducting path analysis in order to verify the mediating effect of ego resilience and care with regards to the relationship between parenting attitudes and school adjustment, the direct influence that parenting attitudes had on school adaptation was shown to be relevant. Third, adolescents' ego resilience and care have a partial mediating effect on the relationship between parenting attitudes and school adaptation. Therefore, together with intervention in actual school settings, assessing various factors that would aid the recovery of adolescents' characters and pursuing a positive reinforcement of character by finding protective factors would enable this study to be used as a basic guideline that would prevent adolescents' maladjustment to school and social deviance as well as aid the growth of their characters.

Development of Nursing Practice Guideline on Intermittent Urinary Catheterization by Using Remake Process (수용개작방법을 활용한 간헐도뇨 간호실무지침 개발)

  • Purpose: The purpose of this study was aimed to develop evidence-based nursing practice guideline for preventing intermittent urinary catheterization (IUC) related complications using guideline remake process. Methods: Guideline remake process was conducted according to guideline adaptation manual developed by Gu et al (2012) which consisted of three main phases and 9 modules including a total of 24 steps. Results: Newly developed IUC guideline consists of introduction, overview of intermittent catheterization, summary of recommendations, recommendations, references, and appendices. There were 50 recommendations in 5 sections including assessment, equipments, catheterization, complications management, and education/consult. Three recommendations (6%) were graded A, and five (10%) and 41 recommendations(82%) were B and C, respectively. Conclusion: The IUC remade-guideline was developed based on evidence-based nursing and therefore, this guideline is recommended to be disseminated and utilized by nurses nationwide to improve the quality of care for IUC and to decrease the IUC related complications.

  • In the United States, The concept of designation for hospital facilities that care for newborn infants according to the level of complexity of care provided was first proposed in 1976. The extent of perinatal health care regionalization varies widely from one area to the other. facilities that provide hospital care for newborn are classified into three categories on the basis of functional capabilities; level I-primary or basic care, level II-secondary or specialty care, level III-tertially or subspecialty care. These facilities should be organized within a regionalized system of perinatal care. The transport system of newborn infants should be organized for referral of high risk newborn to centers with the personnel and resources needed for their degree of risk and severity of illness. In Korea, The korean society of neonatology was established and articulated in the 1994. During the past decade, the number of neonatologist has increased and neonatal intensive care units have proliferated in Korea. However, no standard definitions exist for the graded levels of complexity of care that neonatal intensive care units provide and no uniform guideline or recommendation for regionalization and referral system of high risk neonate have been established. With the rapid changing neonatal care system in Korea, the optimal neonatal care demands regionalization of care in utilization of manpower resources and in efficient use of advanced technology and facility.

User Benefit Characteristics of Customized Housing Design Guideline for Childcare Families (사용자혜택 이론에 따른 신혼·육아가구 맞춤형 주택계획가이드 라인의 특성 연구)

  • Modern society is suffering from the phenomena of low-fertile, ultra-aging, and low-growth. On this social flow, most social classes undergo vulnerable situation and their overall housing and living condition have difficulties. So far many housing guidelines for diverse population were developed, recently, inclusive 'Customized Housing Design Guideline for Childcare Families' in terms of maintaining family and society has been completed. The purpose of this study is to analyze User Benefit Characteristics of 'Customized Housing Design Guideline for Childcare Families'. The research subjects are essential 140 guidelines appropriate to children's development and parents' care. The research method is contents analysis and the analysis criterion are 4 concepts of the User Benefit Criteria- 'Behavioral Facilitation (BF)', 'Physiological Maintenance (PhM)', 'Perceptual Maintenance (PM)', 'Social Facilitation (SF)'. 3 people inter-raters reliability was established. Altogether, the guidelines were rated in the order of PhM>PM>BF>SF, and these characteristics are similar as common universal design guidelines. This showed the supportability of spatial characteristics in the guidelines related to user's special behaviors. Also, the results show the importance of outdoor space planning for safe social interactions. The supportable characteristics of 'Customized Housing Design Guideline for Childcare Families' could be expected to efficiently apply for new housing development in the future.

Development of Nursing Handoff Practice Guideline and Standards for Korean Hospital (국내병원 간호사의 인수인계 실무표준, 표준지침 및 표준화된 항목 개발에 대한 연구)

  • Purpose: Addressing current status of nursing handoff provides information as to where to change practice for improvement in patient care. The aim of this study was to develop nursing a handoff practice guideline and standardsfor Korean hospitals. Methods: A questionnaire was used to collect data in a cross-sectional survey with 207 head nurses and 697 nurses working in general hospitals. Results: The practice guideline consisted of 2 items on purpose of handoff, 5 items on policy, 3 on environment, 5 on process, 7 on content, 3 on evaluation, and 1 on the function of education, for a total 26 of items. Standard instruction was developed related to Patients, Assessment, Situation, Safety concerns, Background, Actions, and Recommendations. Conclusion: The findings of this study indicate that the guideline can be applied in real nursing practice to improve the effectiveness of handoff procedures. It can be used to guide nurses to follow the standardized content, which presents patient details clearly. The guideline provides consistency and structure for the information that is handed over.

  • Background : The atopic dermatitis patient and their families are looking for safer herb-medicine treatments that possess therapeutic effects, but without the recurrence of symptoms and long-term harmful consequences that can result from other treatment. However, for many reasons, including methodological difficulties and lack of high quality study like randomized clinical study and systematic review, there are no consistent clinical guide line for atopic dermatitis in Traditional Korean Medicine. Objectives : Evidence-based clinical practice guidelines support clinical decision-making by making recommendations to guide clinical practice. The purpose of this study was to develop Traditional Korean Medicine clinical practice guideline for atopic dermatitis Result : The future guide should be based on scientific evidence and include the followings: (1) diagnosis of atopic dermatitis (2) the pattern identification of atopic dermatitis (3) classification of Sasang Constitution (4) efficacy assessment (5) treatment guideline (6) education for patients and care givers Conclusion : The development of Traditional Korean Medicine clinical practice guideline for atopic dermatitis is needed.

The Comparison of Cost Analysis Researches for the Home Care Nursing Service (가정간호서비스에 대한 국내 비용분석 연구비교)

  • Purpose: This is a simple survey for discussion about cost analysis methodological issues in home care nursing service studies. Method: The subject of this study were articles published in Korea from 1961 to August, 2002, and searched by key word 'cost' and 'nursing' from various DB(National Assembly Library, The National Library of Korea, RICH etc). Finally, 13 articles were collected. Result: 1) The major type of cost analysis studies was a cost comparison or a simple cost study. 2) The important methodological weaknesses were as followers; (1) few studies were suggested cost analysis framework or analytic perspective, (2) it ,was not enough to describe for basis of selection of cost/effectiveness items, (3) few studies were done by sensitivity analysis. Conclusion: These above results will be used to develop a more proper cost analysis methodological framework in home care nursing services and also to contribute as a guideline for further studies.

Clinical Year in Review 2014: Critical Care Medicine

  • Severe sepsis is the most common cause of death among critically ill patients in non-coronary intensive care units. In 2002, the guideline titled "Surviving Sepsis Campaign" was published by American and European Critical Care Medicine to decrease the mortality of severe sepsis and septic shock patients, which has been the basis of the treatment for those patients. After the first revised guidelines were published on 2008, the most current version was published in 2013 based on the updated literature of until fall 2012. Other important revised guidelines in critical care field such as 'Clinical Practice Guidelines for the Management of Pain, Agitation, and Delirium in Adult Patients in the Intensive Care Unit' were revised in 2013. This article will review the revised guidelines and several additional interesting published papers of until March 2014, including the part of ventilator-induced lung injury and the preventive strategies.
