무치악 환자에게서 의치의 제작은 통상적이고 전통적인 방법을 따를 경우 최종 의치 장착까지 여러 단계를 거쳐야 하며 이로 인한 환자의 잦은 내원이 불가피하다. 또한 기공 과정 상에서 나타나는 중합수축에 의한 오차나, 의치의 파절이나 분실 시 빠른 재제작의 어려움 등의 단점이 존재하였다. 하지만 최근 치과 분야에서 computer-aided design and computer-aided manufacturing (CAD/CAM) System이 사용되기 시작하였고, 의치 제작에까지 그 영역을 넓히기 시작하였다. CAD/CAM System을 이용하여 의치를 제작하면 첫 번째 내원 시에 인상채득과 악간관계의 기록, 인공치의 선택 등이 동시에 이루어지고 두 번째 내원 시 의치의 장착이 가능하기 때문에 환자의 내원 횟수와 진료시간을 감소시킬 수 있다. 또한 3D-Printing 기술을 이용하기 때문에 중합수축에 의한 기공 오차를 감소시킬 수 있다. 현재 상용화 되어 있는 CAD/CAM System을 사용한 의치로는 DENTCA CAD/CAM denture (DENTCA Inc., Los Angeles, CA, USA)가 있다. 본 증례에서는 상, 하악 완전 무치악 환자에게서 conventional 한 방법과 DENTCA CAD/CAM Denture System을 사용하는 방법을 동시에 사용하여 두 쌍의 총의치로 수복함으로써 발음, 저작, 연하 등의 기능적인 측면뿐만 아니라 심미적인 측면에서도 양호한 결과를 얻었기에 이를 보고하고자 한다.
Fabricating a surveyed prosthesis beneath an existing partial removable dental prosthesis (PRDP) is a challenging and time-consuming procedure. The computer-aided design/computer-assisted manufacturing (CAD/CAM) technology was applied to fabricate a retrofitted, surveyed zirconia prosthesis to an existing PRDP. CAD/CAM technology enabled precise and easy replication of the contour of the planned surveyed crown on the existing abutment tooth. This technology ensured excellent adaptation and fit of newly fabricated crown to the existing PRDP with minimal adjustments. In this case report, a seventy-year-old male patient presented with fractured existing surveyed crown. Because the existing PRDP was serviceable, new crown was fabricated to the existing PRDP.

PURPOSE. The aim of this study was to investigate shade changes in fully- and pre-crystalized CAD-CAM lithium disilicate crowns after the required and additional firing processes. MATERIALS AND METHODS. One hundred and five crowns of shade A1 with high translucency were milled out of CAD-CAM lithium disilicate blocks and categorized as follows (n = 15): (1) restorations fabricated from Straumann n!ce with no additional sintering process; (2) restorations fabricated from Straumann n!ce with one additional sintering process; (3) restorations fabricated from Straumann n!ce with two additional sintering processes; (4) restorations fabricated from Amber Mill with one sintering process; (5) restorations fabricated from Amber Mill with two sintering processes; (6) restorations fabricated from IPS e.max CAD with one sintering process; (7) restorations fabricated from IPS e.max CAD with two sintering processes. All restorations were evaluated with a color imaging spectrophotometer. RESULTS. All restorations presented some color alteration from the original shade both after a single and after two firing processes. CONCLUSION. The required and additional sintering processes for restorations fabricated with chairside CAD-CAM lithium disilicate blocks cause an alteration of the original shade selected. Shade A1 high translucency restorations tend to change to a more yellowish B1 shade after a sintering process.
PURPOSE. This study evaluated the color stabilities of two computer-aided design and computer-aided manufacturing (CAD/CAM) blocks and a nanofill composite resin and the microtensile bond strength (µTBS) between the materials. MATERIALS AND METHODS. Twelve specimens of 4 mm height were prepared for both Lava Ultimate (L) and Vita Enamic (E) CAD/CAM blocks. Half of the specimens were thermocycled (10,000 cycle, 5° to 55℃) for each material. Both thermocycled and non-thermocycled specimens were surface treated with one of the three different methods (Er,Cr:YSGG laser, bur, or control). For each surface treatment group, one of the thermocycled and one of non-thermocycled specimens were restored using silane (Ceramic Primer II), universal adhesive (Single Bond Universal), and nanofill composite resin of 4-mm height (Filtek Ultimate). The other specimens were restored with the same procedure without using silane. For each group, 1 × 1 × 8 mm bar specimens were prepared using a microcutting device. Bar specimens were thermocycled (10,000 cycle, 5° to 55℃) and microtensile tests were performed. Staining of the materials in coffee solution was also compared using a spectrophotometer. Data were analyzed using one-way ANOVA, t-test and post-hoc Scheffe tests. RESULTS. µTBS were found similar between the thermocycled and non-thermocycled groups (P>.05). The highest µTBS (20.818 MPa) was found in the non-thermocycled, bur-ground, silane-applied E group. Silane increased µTBS at some E groups (P<.05). Composite resin specimens showed more staining than CAD/CAM blocks (P<.05). CONCLUSION. CAD/CAM blocks can be repaired with composite resins after proper surface treatments. Using silane is recommended in repair process. Color differences may be shown between CAD/CAM blocks and the nanofill composite after a certain time period.
골흡수가 심한 환자에서 임플란트를 이용한 전악 수복의 경우 치은과 치아의 외형을 심미적이면서도 기능적으로 회복하기 위해서는 보철물 제작에 있어 신중을 기해야 한다. 이러한 전악 수복 증례에서 CAD/CAM 시스템이 높은 정확성을 가지는 보철물 제작을 가능하게 함으로써 사용이 증가하고 있다. 최근에 임상에서 티타늄이나 지르코니아의 단점을 보완하면서 높은 생체적합성을 가지고 CAD/CAM 시스템으로 제작할 수 있는 팔라듐-은(Pd-Ag) 합금이 개발되어 사용되고 있다. 본 증례에서는 상악 전부 무치악 환자에서 임플란트를 식립하고 도재의 chipping이나 파절의 위험을 감소시키기 위해 Pd-Ag 합금을 이용해 제작된 임플란트 고정성 보철물로 수복한 환자에 대해 보고하고자 한다.
목적: 이번 연구에서, 다양한 상황에서 골소실양을 평가하기 위하여 맞춤형 지대주를 사용한 환자에서 4년간의 후향적 방사선학적인 연구를 시행하였다. 대상 및 방법: 이번 연구의 대상은 CAD/CAM 맞춤형 지대주를 이용한 임플란트 고정성 보철물이다. 2011년 8월 1일부터 2012년 7월 31일 까지 조선대학교 치과병원 보철과에서 제작하여 고정성 보철물로 사용된 CAD/CAM 맞춤형 지대주와 상부 보철물들을 대상으로 하였으며, 고정성 보철물을 제작한 환자에 대하여 방사선학적 평가를 시행하여 보철물 장착 후 3개월, 6개월, 1년, 2년, 3년, 4년 단위로 총 4년 간의 후향적 연구를 시행하였다. 결과:연구 결과, CAD/CAM을 이용한 맞춤형 지대주는 기존 연구 결과에 비해 골소실 양이 적었다. 연결 고정된 그룹과 연결 고정되지 않은 그룹 간에 골소실 양(각각 0.27 mm, 0.5 mm)에 통계적으로 유의한 차이가 없었다. 또한 상악 전치부, 상악 구치부, 하악 전치부, 하악 구치부의 부위별 골소실양(각각 1.37 mm, 0.39 mm, 0.00 mm, 0.30 mm)에 통계적으로 유의한 차이가 있었다. 결론:CAD/CAM을 이용한 맞춤형 지대주는 연결 고정 여부에 따른 골소실 양에서 통계적으로 유의한 차이가 없었으며, 임플란트 식립 부위에 따른 골소실 양에서 통계적으로 유의한 차이가 있었다.
최근 CAD/CAM 시스템을 이용하여 제작된 지르코니아 코어의 사용이 증가하고 있다. 하지만 국내 기술력을 이용한 제품에 대한 연구는 아직 미흡한 단계이다. 본 연구의 목적은 3종의 CAD/CAM 시스템에서 단일 전부 도재관을 위한 지르코니아 코어의 파절 강도를 비교하는 것이다. 본 연구를 위해 상악 중절치 부위의 전부도재관을 위한 30개의 지르코니아 코어를 제작하였다. 각 군은 다음과 같다. 1. $Adens^{(R)}$ CAD/CAM 시스템 군, 2. $Cerasys^{(R)}$ CAD/CAM 시스템 군, 3. $Lava^{(R)}$ CAD/CAM 시스템 군. 모든 시편은 두께 0.5mm, relief $40{\mu}m$로 일관성 있게 제작하였다. 시편은 Z250/SN5S을 이용하여 파절 강도를 측정하였다. 각 군의 평균과 표준편차를 구하고, 각 군간의 유의성을 알아보기 위해 Kruskal waillis test와 Wilcoxon Rank Sum test를 시행하였다. 본 연구 결과, 각 군의 파절 강도는 $Adens^{(R)}$ 시스템은 $615.89{\pm}156.1N$, $Cerasys^{(R)}$ 시스템은 $863.98{\pm}151.5N$, 3M $Lava^{(R)}$ 시스템은 $1143.1{\pm}286.6N$ 이었다. 각 군의 파절 강도는 3M $Lava^{(R)}$ 시스템, $Cerasys^{(R)}$ 시스템, $Adens^{(R)}$ 시스템 순으로 통계학적으로 유의한 수준의 차이를 보였으나, 세 그룹 모두 임상적으로 사용 가능한 결과를 나타냈다. 실제 임상에서 장기간의 사용에 대한 연구가 필요할 것이다.
Purpose. To investigate the fracture resistance of monolithic CAD-CAM all-ceramic surveyed crowns with two different occlusal rest seat designs. Materials and Methods. Two maxillary first premolar were prepared for all-ceramic surveyed crowns with wide (2/3rd of buccolingual width of an unprepared tooth) or narrow (1/3rd of buccolingual width of an unprepared tooth) disto-occlusal rest seat (ORS) designs. Eighty monolithic CAD-CAM all-ceramic surveyed crowns were prepared and divided into 4 groups - Group CR, Composite resin material as a control; Group LDS, Lithium disilicate based material; Group ZIPS, zirconia-material (IPS ZirCAD); and Group ZLHT, zirconia- material (CeramillZolidht+). Crowns were cemented on an epoxy resin die with adhesive resin cement. The fracture resistance of crowns was tested with the universal machine. Univariate regression analysis was used. Results. The mean ± standard deviation of maximum failure force values varied from 3476.10 ± 285.97 N for the narrow ORS subgroup of group ZIPS to 687.89 ± 167.63 N for the wide ORS subgroup of group CR. The mean ± standard deviation of maximum force was 1075 ± 77.0 N for group CR, 1309.3 ± 283.9 N for group LDS, 3476.1 ± 285.97 N for group ZIPS, and 2666.7 ± 228.21 N for group ZLHT, with narrow occlusal rest seat design. The results of the intergroup comparison showed significant differences in fracture strength with various material groups and occlusal rest seat designs (P<.001). Conclusion. The zirconia-based all-ceramic surveyed crowns fractured at more than double the load of Lithium disilicate based crowns. The crowns with narrow base occlusal rest seat design had statistically significantly higher fracture resistance than surveyed crowns with wide occlusal rest seat design. The use of narrow occlusal rest seat design in CAD-CAM all ceramic surveyed crowns provides higher fracture resistance, and therefore narrow occlusal rest design can be used for providing esthetics with high strength.
With the explosive increase of esthetic demands by patients, many dental materials for the esthetic restoration have been introduced. Recently, zirconia based restorations are using for the cases of single crown, bridges, implant prostheses etc. Zirconia have superior mechanical properties and biocompatibility. Owing to the properties of high strength, zirconia has to be manufactured by CAD/CAM system. Dental CAD/CAM system is a futuristic treatment and technical system which makes it possible to produce the precision and uniform prosthesis and also standardize the treatments. This article introduces the characteristics of zirconia, fabrication procedure using CAD/CAM system and procedure for the cementation of zirconia based restoration.
PURPOSE. To evaluate the effect of surface treatments and repair materials on the shear bond strength and to measure the fracture toughness of CAD/CAM provisional restoration materials. MATERIALS AND METHODS. Four CAD/CAM (3D printing: Nextdent C&B and ZMD-1000B Temporary, CAD/CAM resin block: Yamahachi PMMA disk and Huge PMMA block) and four conventional (monometacrylate: Jet and Alike, dimetacrylate: Luxatemp and Protemp 4) materials were selected to fabricate disk-shaped specimens and divided into six groups according to surface treatment (n=10). CAD/CAM materials were repaired with Jet or Luxatemp, while conventional materials were repaired with their own materials. The shear bond strength was measured by using universal testing machine. Ten rectangular column-shaped specimens for each material were fabricated to measure the fracture toughness by single edge v notched beam technique. Statistical analysis was performed by one-way ANOVA. RESULTS. The highest shear bond strength of CAD/CAM materials was achieved by SiC paper + sandblasting. It was also accomplished when repairing 3D printing materials with Luxatemp, and repairing CAD/CAM resin blocks with Jet. Yamahachi PMMA disk showed the highest fracture toughness. Nextdent C&B showed the lowest fracture toughness value but no statistically significant difference from Alike and Luxatemp (P>.05). CONCLUSION. In order to successfully repair the CAD/CAM provisional restoration, mechanical surface treatment and appropriate repair material according to the CAD/CAM material type should be selected. The CAD/CAM provisional materials have proper mechanical properties for clinical use as compared to conventional materials.
