In nowdays many dental CAD/CAM system were developed. Among those only Cerec and Celay were used globally as clinical application. Celay is a machinable ceramic system that is capable of milling inlays, onlays, and veneers from prefabricated industrial ceramic blocks(Vita Celay Blanks). The advatages of Celay are to simplify the manufactures and to save the processing time. For esthetics In-Ceram Alumina bridges were introduced into maxillary anterior restoration. They have a high strength, a high translucency and an excellent marginal adapation. But the laboratory processes are very difficult and complicated. So the construction of In-Ceram Alumina bridge combined with celay system was desgined. The patient is a 28 year old age male. The chief complain is missing of maxillary left central incisor. He wants to restore anterior bridge for esthetically. The Alumina bridge framework was constructed easily by celay system. Glass ilfiltration was occurred. After that, vitadura-${\alpha}$porcelain build up was occurred by conventional method. The translucency of In-Ceram Alumina 3 unit bridge revealed to be superior to that of porcelain fused to metal bridge. So we report it with clincal case and literature reviews.
PURPOSE: The application of computer-aided technology to implant dentistry has created new opportunities for treatment planning, surgery and prosthodontic treatment, but the correct selection and combination of available methods may be challenging in times. Hence, the purpose of this case report is to present a combination of several computer-aided tools as approaches to manage complicated implant case. MATERIAL AND METHODS: A 47 year-old female patient with severe dental anxiety, high expectations, financial restrictions and poor compliance presented for a fixed rehabilitation. A CT scan with a radiographic template obtained with software (SimPlant, Materialize, Leuven, Belgium) was used for treatment planning. The surgical plan was created and converted into a stereolithographic model of the maxilla with bone-supported surgical templates (SurgiGuide, Materialise, Leuven, Belgium), that allowed for the precise placement of 7 implants in a severely resorbed edentulous maxilla. After successful osseointegration, an accurate scan model served as the basis for the fabrication of a one-piece milled titanium framework using the Procera (Nobel Biocare, Gothenburg, Sweden) technology. The final rehabilitation of the edentulous maxilla was rendered in the form of a screw-retained maxillary metal-reinforced resin-based complete prosthesis. RESULTS: Despite challenging circumstances, 7 implants could be placed without bone augmentation in a severely resorbed maxilla using the SimPlant software for pre-implant analysis and the SurgiGuide-system as the surgical template. The patient was successfully restored with a fixed full arch restoration, utilizing the Procera system for the fabrication of a milled titanium framework.
점진적인 마모는 정상적인 과정이지만 교합면의 마모가 병적으로 과도하게 진행되어 교합평면이 붕괴된 경우 교합, 저작근, 치수병변 등의 문제가 나타날 수 있다. 본 증례에서는 광범위한 마모를 보이는 환자에서 수직고경을 증가하여 임시 수복물을 제작한 후 약 12주간 환자의 적응 여부와 심미성을 평가하였다. 임시 수복물을 바탕으로 CAD/CAM 단일구조 지르코니아를 이용해 전악 고정성 보철물로 수복 하였으며, 기능적인 면과 심미적인 면에서 만족스러운 결과를 얻었기에 이를 보고하는 바이다.
With the development of computer-aided design/computer-aided manufacturing (CAD/CAM) technology, it has been possible to reconstruct the cranio-maxillofacial defect with more accurate preoperative planning, precise patient-specific implants (PSIs), and shorter operation times. The manufacturing processes include subtractive manufacturing and additive manufacturing and should be selected in consideration of the material type, available technology, post-processing, accuracy, lead time, properties, and surface quality. Materials such as titanium, polyethylene, polyetheretherketone (PEEK), hydroxyapatite (HA), poly-DL-lactic acid (PDLLA), polylactide-co-glycolide acid (PLGA), and calcium phosphate are used. Design methods for the reconstruction of cranio-maxillofacial defects include the use of a pre-operative model printed with pre-operative data, printing a cutting guide or template after virtual surgery, a model after virtual surgery printed with reconstructed data using a mirror image, and manufacturing PSIs by directly obtaining PSI data after reconstruction using a mirror image. By selecting the appropriate design method, manufacturing process, and implant material according to the case, it is possible to obtain a more accurate surgical procedure, reduced operation time, the prevention of various complications that can occur using the traditional method, and predictive results compared to the traditional method.
The restoration of endodontically treated teeth (ETT) with more than one cusp missing and thin remaining walls is challenging for the general practitioner. The use of posts combined with full coverage restorations is a well-established approach, yet not following the minimal invasive principles of adhesive dentistry. Endocrowns are indirect monoblock restorations that use the pulp chamber of the ETT for retention. In this study the fabrication of 4 endocrowns and their clinical performance will be discussed. Two clinical cases include computer-aided design/computer-aided manufacturing manufactured molar endocrowns (one feldspathic ceramic and one hybrid composite-ceramic restoration) and the other two are dental laboratory manufactured resin composite premolar endocrown restorations. The modified United States Public Health Service criteria were used to assess the clinical behavior of the restorations at different follow up periods. Endocrown restorations present a satisfactory clinical alternative, either by the use of resin composite or glass ceramic and hybrid materials. Specific guidelines with minimal alterations should be followed for an endocrown restoration to be successful. Due to limited evidence regarding the long term evaluation of this restorative technique, a careful selection of cases should be applied.
고기능성 폴리머 중 하나인 Polyetherketoneketone (PEKK)는 좋은 생체적합성과 우수한 물성을 가져 치과 영역을 포함한 다양한 분야에서 사용되고 있다. 이러한 PEKK를 프레임워크로 하는 임플란트 지지 고정성 보철물에 관한 성공적인 증례들이 많이 보고되어 왔으나, 보고된 증례에 대한 장기간의 관찰과 합병증에 관한 고찰은 부족한 상태이다. 이에 본 증례보고에서는 PEKK 프레임워크를 composite resin, lithium disilicate crown, high-impact polymethyl methacrylate PMMA로 각각 veneering하여 임플란트 지지 고정성 보철 수복을 한 세 가지 증례와 각각의 6년간의 경과관찰 결과를 소개하고 이를 고찰해 보고하고자 한다.
치과의사가 수복재료를 선택하고 환자에게 추천할 때는 수복할 치아 위치, 결손 정도, 환자의 심미적 요구도, 저작력, 나이, 경제적 여건 등 여러 가지의 요인을 고려한다. 그렇지만, 수복재료를 선택할 때의 일차적인 기준은 구강 내에서 기능 시 작용하는 교합력을 최우선으로 고려해야 한다. 미국 국립보건원(NIH) 지원으로 조사된 연구 결과에 의하면, 전치부 수복재료로서는 리튬 디실리케이트계 글라스-세라믹이 54%, layered zirconia가 17%, 루사이트 강화 글라스-세라믹이 13% 순을 보여, 전치부 수복재료로서 리튬 디실리케이트계 글라스-세라믹이 가장 많이 사용었다. 또한 구치부 수복재료로서는 단일구조 지르코니아가 32%, 금속-세라믹이 31%, 리튬 디실리케이트계 글라스-세라믹이 21% 순을 나타냈다. 세라믹 수복 재료의 특성을 살펴보면, 단일구조 지르코니아는 강도는 높지만 명도가 높고 저온열화로 인한 강도 저하가 일어날 수 있다. layered zirconia는 심미성은 우수하지만 강도가 낮은 비니어 세라믹의 칩핑과 박리가 문제가 되고 있다. 리튬 디실리케이트계 글라스-세라믹은 심미성은 우수하지만 지르코니아에 비해 강도가 낮으므로 구치부에 적용 시 파절이 일어날 위험성이 있다. 루사이트계 글라스-세라믹은 심미성은 우수하지만 기본적으로 강도가 낮기 때문에 전치부에 한정하여 적용한다. 세라믹 크라운이 구치부 교합력에 저항하기 위해서는 350 MPa 이상의 굴곡강도를 가져야 한다. 리튬 디실리케이트계 글라스-세라믹은 광투과성이 양호한 심미성이 있는 재료이므로 비니어 없이 전치부에 적용할 수 있고, 굴곡강도가 400 MPa 이상이므로 구치부 교합력에 저항할 수 있고, HF에 의한 산부식과 실란(silane) 처리가 가능하므로 레진과 강한 결합력을 얻을 수 있고, 또한 포세린에 비해 대합치의 마모가 적다. 이러한 점들을 고려하면 리튬 디실리케이트계 글라스-세라믹 재료는 전구치부의 단일치 수복에 적합한 성질을 갖고 있음을 알수 있다. 그렇지만 그의 임상 응용이 더욱 증가하기 위해서는 이들 재료에 대한 보다 많은 연구와 이해가 필요하리라고 생각된다.
본 연구는 치과용 모형재 중 4형(type IV)석고와 스캔용(scannable)석고를 이용하여 작업모형을 제작한 후, 치과용 백색광 스캐너를 이용하여 3차원 디지털 모형으로 전환한 데이터의 계측지점간 거리를 측정하여, 그 결과 값을 토대로 이종의 치과용 석고가 3차원 디지털 모형의 크기재현 시 정확성여부를 비교하였다. 본 실험을 통하여 치과용 모형재에 따른 3차원 디지털 모형의 정확도를 확인함으로써 임상 적용의 가능성을 평가하였으며, 제한된 조건 하에서 수행된 본 연구에서 다음과 같은 결론을 얻었다. 1. 치과용 백색광 스캐너를 사용하여 주 모형과 두 종류의 모형재로 제작한 작업모형을 3차원 디지털 모형으로 전환한 후, 선계측의 값을 비교한 결과 전체적인 계측지점에서 모두 모형재로 제작된 작업모형이 작게 계측되는 경향을 보였다. 2. 각 모형별 동일한 계측지점의 계측값의 차이가 있는지 알아보기 위해 paired t-test의 시행결과, 모든 계측지점에서 통계적으로 유의한 차이를 보였다(p<0.05). 결론적으로 두 가지 석고 모두 통계적으로 적정한 수준의 정확성을 나타내지 않았으나, 오차 수준이 선행연구에 비추어 볼 때 임상적으로 수용 가능한 수준이라 생각된다. 전악 인상채득을 통한 디지털 모형의 정확성은 검증이 되었으나, 스캔용(scannable)석고에 대한 임상 효용성을 판단하기 위해 타 제품과의 성분 분석 비교연구나 실질적인 보철물 제작을 통한 적합도 평가를 통해 CAD/CAM 보철물 제작 시장점을 부각할 수 있는 연구가 뒷받침되어야 할 것으로 사료된다.
치과에서 수복물제작에는 지난 50년간 금속을 이용한 금속도재관이 많이 사용되어 왔다. 그러다 보니 심미적이고 생체 친화적이며 금속 알러지등의 문제로 인한 환자들의 사고의 변화에 따라서 'metal free restoration'에 대한 관심이 점점 높아지고 있다. 특히 그 동안 많이 사용되어왔던 귀금속 가격이 급등하여 더 이상 구강 내 수복 물로써 제 역할을 하는 것은 불가능하게 되었으며, PFM수복은 술자로 하여금 chipping과 파절로 인한 문제점을 안고 있다. 따라서 구치부 임플란트 수복물로써 보다 심미적이고 강도가 높은 재료의 요구로 인하여 그 어느 때보다 CAD/CAM이 주목을 받고 있다. Full zirconia 수복을 위해서 고려해야 할 사항은 1. 강도 2. 콤비네이션 작업은? 3. 빛 투과성은? 4. crack이 발생한 경우의 처치는? 5. 블럭의 색조 재현상은? 6. 대합치 마모도 7. 저온 열화 현상 등을 들 수가 있는데, 본 연구에서는 블럭의 색조 재현상에 대해서 정리하고자 한다. Full zirconia 수복을 꺼려하는 가장 커다란 이유 중에 하나가 바로 기존의 PFM 수복과 비교했을 때 자연스러운 색조를 재현하기가 어렵다는 점이며, 교합 조정 후에 컬러링한 표면이 삭제된 후 나타나는 보기 싫은 블럭의 노출로 인하여 많은 임상가들이 꺼려하고 있다. 지르코잔의 블럭을 약 4년 여 동안 사용하면서 이러한 점들은 어느 정도 극복되어질 수 있는 문제라고 여겨지며, 어떻게 하면 그 가급적 주변 보철물 또는 자연 치아와의 조화 이룰 수 있는 수복물을 만들 수 있을 것인가에 관해 정리였다.
목적: 본 연구의 목적은 CAD-CAM 방식 중 절삭가공과 적층가공으로 제작한 임시 보철 재료의 색 안정성을 비교 연구하는 것이다. 연구 재료 및 방법: 절삭가공을 통해 제작한 PMMA계열, DLP타입 프린터와 광경화성 수지를 이용해 적층가공으로 제작한 bis-acryl계열 그리고 전통적인 방법으로 제작한 PMMA계열의 시편을 각 20개씩 제작하였다. 시편을 커피용액과 와인용액에 침전시킨 후, 비색계를 이용하여 4주간 매주 색 측정을 시행하였다. 색 변화(ΔE*)값의 비교분석을 위해 One-way ANOVA와 사후 검정으로 Tukey test를 실시하였다(α = 0.05). 결과: 절삭가공을 통해 제작된 PMMA계열의 시편은 적층가공을 통해 제작된 bis-acryl계열의 시편에 비해 뛰어난 색 안정성을 보였고(P < 0.05), 전통적인 방법을 통해 제작된 PMMA계열의 시편과 유사한 색 안정성을 보였다(P > 0.05). 결론: 전치부와 같이 심미가 중요한 부위에서는 절삭가공을 통한 임시 보철물의 제작을 추천하며, 적층가공을 이용한 임시 보철물 제작 시에는 색 안정성에 대한 고려가 필요하다.
