지대치 변연 형태에 따른 수작업과 CAD/CAM으로 제작한 coping 패턴의 적합도 비교 (Comparison of the fit of the coping pattern constructed by manual and CAD/CAM, depending on the margin of the abutment tooth)

  • 본 연구는 수작업으로 제작된 metal coping과 CAD/CAM(Computer Aided Design/Computer Aided Manufacturing)으로 제작된 지르코니아 coping을 제작하여 변연과 내면 적합도를 비교 분석하고자 하였다. 모형은 우레탄 모형재를 사용하여 knife, chamfer변연 두 종류의 지대치를 제작하였다. 수작업과 CAD/CAM으로 제작된 보철물의 변연 적합도를 실리콘 리플리카 테크닉방법을 사용하여 측정하였다. 적합도 측정은 현미경의 CCD카메라로 캡쳐하였다. 이때 지대치와 보철물의 거리는 이미지분석 소프트웨어에서 거리 조정이 된 상태에서 실시하였다. 측정부위는 marginal opening을 MO, marginal gap을 MG, internal gap을 IG, Axial gap을 AG, occlusal gap을 OG로 하였다. 제작법과 변연 형태에 따른 비교분석을 위해 이원배치분산분석을 실시하였으며 서로 다른 평균값들의 비교 분석을 위해서 일원배치분산분석과 Scheffe's 사후 검정을 실시하였다. 그 결과 CAD/CAM의 OG와 knife 변연의 MO을 제외하고는 < $120{\mu}m$ 적합도를 나타냈다. CAD/CAM으로 제작된 coping은 chamfer 변연 MO에서 높은 적합도를 보였으나 MG에서는 knife변연이 chamfer변연 보다 더 좋은 적합도를 보였다. AG의 내면 적합도는 가장 좋은 값을 나타내었다(< $38{\mu}m$).

Trueness and precision of scanning abutment impressions and stone models according to dental CAD/CAM evaluation standards

  • PURPOSE. The purpose of the present study was to compare scanning trueness and precision between an abutment impression and a stone model according to dental computer-aided design/computer-aided manufacturing (CAD/CAM) evaluation standards. MATERIALS AND METHODS. To evaluate trueness, the abutment impression and stone model were scanned to obtain the first 3-dimensional (3-D) stereolithography (STL) file. Next, the abutment impression or stone model was removed from the scanner and re-fixed on the table; scanning was then repeated so that 11 files were obtained for each scan type. To evaluate precision, the abutment impression or stone model was scanned to obtain the first 3-D STL file. Without moving it, scanning was performed 10 more times, so that 11 files were obtained for each scan type. By superimposing the first scanned STL file onto the other STL files one by one, 10 color-difference maps and reports were obtained; i.e., 10 experimental scans per type. The independent t-test was used to compare root mean square (RMS) data between the groups (${\alpha}=.05$). RESULTS. The $RMS{\pm}SD$ values of scanning trueness of the abutment impression and stone model were $22.4{\pm}4.4$ and $17.4{\pm}3.5{\mu}m$, respectively (P<.012). The $RMS{\pm}SD$ values of scanning precision of the abutment impression and stone model were $16.4{\pm}2.9$ and $14.6{\pm}1.6{\mu}m$, respectively (P=.108). CONCLUSION. There was a significant difference in scanning trueness between the abutment impression and stone model, as evaluated according to dental CAD/CAM standards. However, all scans showed high trueness and precision.

치과 캐드캠 ISO평가 기준에 준한 지대치 경석고 모형 및 인상체 스캐닝의 반복측정안정성 비교 평가 (Comparative evaluation of repeatability of scanning abutment tooth stone model and impression : Applied assessment of CAD/CAM ISO standard in dentistry)

  • Purpose: The purpose of this in vitro study compared to evaluation of repeatability of scanning abutment tooth stone model and impression applied CAD/CAM ISO standard in dentistry. Methods: To evaluate repeatability of scanning abutment tooth stone model, were repeatedly scanned to obtain 11 data via 3D stereolithography (STL) files. 10 data (STL files) were compared with the first 3D data (STL file), and the error sizes were measured by using 3D superimposing software(n=10). Also, the repeatability of scanning abutment tooth impression was evaluated with the same procedure. Independent t test was performed to evaluate the repeatability of scanning abutment tooth stone model versus impression through root mean square(RMS) and standard deviation(SD)(${\alpha}=0.05$). Results: $RMS{\pm}SD$ with regard to repeatability were $14.7{\pm}2.5{\mu}m$, $17.1{\pm}4.0{\mu}m$, respectively, with scanning abutment tooth stone model and impression(p=0.129). Conclusion: This study results showed a little different repeatability of scanning abutment tooth stone model and impression applied CAD/CAM ISO standard in dentistry, will suggest futures good studies and clinical advantages.

Influence of the implant abutment types and the dynamic loading on initial screw loosening

  • PURPOSE. This study examined the effects of the abutment types and dynamic loading on the stability of implant prostheses with three types of implant abutments prepared using different fabrication methods by measuring removal torque both before and after dynamic loading. MATERIALS AND METHODS. Three groups of abutments were produced using different types of fabrication methods; stock abutment, gold cast abutment, and CAD/CAM custom abutment. A customized jig was fabricated to apply the load at $30^{\circ}$ to the long axis. The implant fixtures were fixed to the jig, and connected to the abutments with a 30 Ncm tightening torque. A sine curved dynamic load was applied for $10^5$ cycles between 25 and 250 N at 14 Hz. Removal torque before loading and after loading were evaluated. The SPSS was used for statistical analysis of the results. A Kruskal-Wallis test was performed to compare screw loosening between the abutment systems. A Wilcoxon signed-rank test was performed to compare screw loosening between before and after loading in each group (${\alpha}$=0.05). RESULTS. Removal torque value before loading and after loading was the highest in stock abutment, which was then followed by gold cast abutment and CAD/CAM custom abutment, but there were no significant differences. CONCLUSION. The abutment types did not have a significant influence on short term screw loosening. On the other hand, after $10^5$ cycles dynamic loading, CAD/CAM custom abutment affected the initial screw loosening, but stock abutment and gold cast abutment did not.

  • Purpose: The aim of this study is to make some basic materials to find retention force difference based on the total height of CAD/CAM zirconia abutment used for implant, retention force difference based on how to regulate the height of the abutment, retention force difference based on the size and retention force difference based on the design group. Methods: The retention force was measured by being pulled at the speed of 1mm/min after being combined with zirconia block and abutment using Temp-BOND of Kerr. The experiment was done at the research lab of OSTEM in Busan by useing UNIVERSAL TESTING MACHINE on March 3rd, 2010. Results: After analysing the total height and the retention force, p-value had minor difference by 0.01 statistically. Namely, 3mm, 4mm, 5mm had the retention force difference and we could see retention force difference on 3mm and 5mm at the post test. After analyzing how to regulate the height and retention force, p-value had minor difference by 0.000 statistically. Namely, 1mm and 2mm had the retention difference and we could see that 1mm and 2mm with the total height had retention difference. After analyzing the retention force based on the size, p-value had minor retention force difference by 0.000 statistically. Namely, 7 different size had retention force difference and we could see the size 21.9mm, 32.9mm, 32.9mm, 38.4mm, 48.9mm and 54.9mm had retention force difference. Conclusion: Namely 9 different design group had retention difference and we could see that 9 design group with 5.6.7.8 design group and 9 design group with 1.2.3.4. design group had retention force.

CAD/CAM 맞춤형 지대주를 이용한 milled-bar 피개의치 증례 (Rehabilitation of edentulous maxilla with implant-supported milled bar overdenture using CAD/CAM customized abutment: A case report)

  • 상악 완전 무치악에 6개 이상의 임플란트가 식립된 경우 임플란트-지지형 피개의치 제작이 가능하다. 임플란트-지지 피개의치에 사용되는 다양한 부착장치 중에 milled-bar의 경우 의치의 회전을 허용하지 않고, 안정성이 뛰어나며, 여러 개의 임플란트를 연결 고정하는 효과를 가진다는 장점이 있다. 본 증례에서는 전치부에 심한 골소실을 동반한 상악 완전 무치악 환자에서 세 가지 다른 시스템으로 구성된 7개의 임플란트 상부에 CAD/CAM 맞춤형 지대주와 시멘트 접착형의 milled bar를 이용한 피개의치를 제작하여 상실된 심미와 기능을 효과적으로 회복하였다.

CAD/CAM으로 제작한 코누스내관의 변연적합 (MARGINAL ADAPTATION OF THE CONICAL INNER CROWN FABRICATED WITH CAD/CAM)

  • This study was to evaluate the fabrication method and marginal adaptation of the conical inner crown fabricated with CAD/CAM. The informations on abutment teeth were transferred to a computer with a micro contact digitizer, which had a $50{\mu}m$ accuracy on the master die. A conical inner crown was designed on a computer and a real crown was machined based on this design using CAM. The marginal fit of a computer-machined conical inner crown was assessed using electron microscopy Measurement of the marginal gap between the conical inner crown and the abutment was performed on four different locations (mesial, distal, buccal, and lingual surfaces) of the finish line. The evaluation was based on 10 test specimens. The results were as follow. 1 The mean marginal gap between the conical inner crown and abutment tooth was $83.2{\pm}43{\mu}m$, 28.9% of the specimen showed marginal gap over $100{\mu}m$. 2. The fabrication method using CATRS and CAM provided clinically acceptable marginal fitness compared to conventional casting method (P<0.05).

Effects of abutment diameter, luting agent type, and re-cementation on the retention of implant-supported CAD/CAM metal copings over short abutments

  • PURPOSE. The aim of this study was to evaluate the effects of abutment diameter, cement type, and re-cementation on the retention of implant-supported CAD/CAM metal copings over short abutments. MATERIALS AND METHODS. Sixty abutments with two different diameters, the height of which was reduced to 3 mm, were vertically mounted in acrylic resin blocks with matching implant analogues. The specimens were divided into 2 diameter groups: 4.5 mm and 5.5 mm (n=30). For each abutment a CAD/CAM metal coping was manufactured, with an occlusal loop. Each group was sub-divided into 3 sub-groups (n=10). In each subgroup, a different cement type was used: resin-modified glass-ionomer, resin cement and zinc-oxide-eugenol. After incubation and thermocycling, the removal force was measured using a universal testing machine at a cross-head speed of 0.5 mm/min. In zinc-oxide-eugenol group, after removal of the coping, the cement remnants were completely cleaned and the copings were re-cemented with resin cement and re-tested. Two-way ANOVA, post hoc Tukey tests, and paired t-test were used to analyze data (${\alpha}=.05$). RESULTS. The highest pulling force was registered in the resin cement group (414.8 N), followed by the re-cementation group (380.5 N). Increasing the diameter improved the retention significantly (P=.006). The difference in retention between the cemented and recemented copings was not statistically significant (P=.40). CONCLUSION. Resin cement provided retention almost twice as strong as that of the RMGI. Increasing the abutment diameter improved retention significantly. Re-cementation with resin cement did not exhibit any difference from the initial cementation with resin cement.

Effect of cement type, luting protocol, and ceramic abutment material on the shade of cemented titanium-based lithium disilicate crowns and surrounding peri-implant soft tissue: a spectrophotometric analysis

  • PURPOSE. The objective of the study was to analyze the impact of cement, bonding pretreatment, and ceramic abutment material on the overall color results of CAD-CAM ceramic crowns bonded to titanium-based hybrid abutments. MATERIALS AND METHODS. For single implant restoration of a maxillary lateral incisor a total of 51 CAD-CAM-fabricated monolithic lithium disilicate crowns were fabricated and subsequently bonded onto 24 lithium disilicate Ti-base abutments, 24 zirconia Ti-base abutments and 3 resin abutment replicas as a control group. The 48 copings were cemented with three definitive and one provisional cement on both grit-blasted and non-blasted Ti-bases. The color of each restoration and surrounding artificial gingiva was measured spectrophotometrically at predefined measuring points and the CIELAB (ΔEab) color scale values were recorded. RESULTS. The color outcome of ceramic crowns bonded to hybrid abutments and soft tissues was affected differently by cements of different brands. Grit-blasting of Ti-bases prior to cementing CAD-CAM copings affected the color results of allceramic crowns. There was a significant difference (P = .038) for the median ΔE value between blasted and non-blasted reconstructions at the cervical aspect of the crown. Full-ceramic crowns on zirconia Ti-base abutments exhibited significantly lower ΔE values below the threshold of visibility (ΔE 1.8). In all subcategories tested, the use of a highly opaque temporary cement demonstrated the lowest median ΔE for both the crown and the artificial gingiva. CONCLUSION. Various cements, core ceramic materials and airborne particle abrasion prior to bonding can adversely affect the color of Ti-base supported ceramic crowns and peri-implant soft tissue. However, zirconia CAD-CAM copings and an opaque cement can effectively mask this darkening.

  • 심미의 기준은 시대와 문화 그리고 개개인의 주관적인 관점에 따라 각기 다를 수 있다. 하지만 심미적인 부위를 수복하는 치과의사로서 일반적으로 따라야 할 원칙과 기준들이 있으며 수복물의 형태와 주변치아와의 조화 등을 위해 캐드캠의 몇몇 기능들은 아주 유용한 것으로 보이며, 전통적인 작업 방식에 비해 효율성도 높은 것으로 보인다. 또한 전치부 임플란트에서 치은이 얇은 경우 기계적인 문제를 줄이면서도 지르코니아 등의 도재를 사용하기 위한 한 방법인 링크 지대주에 대해 함께 알아보려 한다.
