CA 19-9 방사면역측정법의 기본적 검토 및 악성종양환자에서의 혈청 CA 19-9치의 임상적 의의에 관한 연구 (Performance Characteristics of CA 19-9 Radioimmunoassay and Clinical Significance of Serum CA 19-9 Assay in Patients with Malignancy)

  • To evaluate the performance characteristics of CA 19-9 radioimmunoassay and the clinical significance of serum CA 19-9 assay in patients with malignancy, serum CA 19-9 levels were measured by radioimmunoassay using monoclonal antibody in 135 normal controls, 81 patients with various untreated malignancy, 9 patients of postoperative colon cancer without recurrence and 20 patients with benign gastrointestinal diseases, who visited Seoul National University Hospital from June, 1984 to March, 1985. The results were as follows; 1) The CA 19-9 radioimmunoassay was simple to perform and can be completed in one work day. And the between-assay reproducibility and the assay recovery were both excellent. 2) The mean serum CA 19-9 level in 135 normal controls was $8.4{\pm}4.2U/mL$. Normal upper limit of serum CA 19-9 was defined as 21.0 U/mL. 4 out of 135(3.0%) normal controls showed elevated CA 19-9 levels above the normal upper limit. 3) One out of 20(5.0%) patients with benign gastrointestinal diseases showed elevated serum CA 19-9 level above the normal upper limit. 4) In 81 patients with various' untreated malignancy, 41 patients(50.6%) showed elevated serum CA 19-9 levels. 66.7% of 18 patients with colorectal cancer, 100% of 2 patients with pancreatic cancer, 100% of 3 patients with common bile duct cancer, 47.1% of 17 patients with stomach cancer, 28.6% of 28 patients with hepatoma and 60.0% of 5 other gastrointestinal tract cancers showed elevated serum CA 19-9 levels. 5) The sensitivities of serum CA 19-9 related to resectability in colorectal and stomach cancer were 33.3% in resectable colorectal cancer, 83.3% in unresectable colorectal cancer, 41.7% in resectable stomach cancer, 60.0% in unresectable stomach cancer respectively. 6) The sensitivity of serum CA 19-9 in 9 patients of postoperative colorectal cancer without recurrence were 33.3% and significantly decreased compared with that of untreated colorectal cancer, 66.7% (p<0.05). 7) In patients with colorectal cancer, simultaneous measurement of serum CA 19-9 and serum CEA levels increased sensitivities. From above results, we concluded that serum CA 19-9 radioimmunoassay is simple to perform and reproducible, and is a useful indicator reflecting tumor extent and responses to the treatment in patients with malignancy.

Normalization of CA19-9 Following Resection for Pancreatic Ductal Adenocarcinoma is not Tantamount to being Cured?

  • Background: Postoperative carbohydrate antigen 19-9 (CA19-9) is an independent predictor of survival for pancreatic ductal adenocarcinoma (PDAC), and more powerful than preoperative CA19-9. However, making decisions just dependent on postoperative CA19-9 may result in necessary treatments not being performed. Materials and Methods: A total of 178 patients with resected PDAC were eligible for this retrospective study, classified into two corresponding subgroups according to postoperative CA19-9. Prognostic significance of all clinicopathologic factors was evaluated by univariate and multivariate analyses. Results: Postoperative CA19-9, preoperative CA125 and lymph node status were independent predictors. Better predictive performances for overall survival (OS) and recurrence-free survival (RFS) were achieved by postoperative CA19-9 compared to preoperative CA125 and lymph node status. Particularly, preoperative CA125 was associated with poor OS (p<0.001 for the normalized CA19-9 patients, p=0.012 for the elevated) and RFS (p=0.005 for the normalized, p=0.004 for the elevated). Moreover, preoperative CA125 levels related with survival in double-negative patients. Conclusions: Normalization of CA19-9 is not tantamount to be cured. Preoperative CA125 is a critical predictor for PDAC patients, especially in double-negative patients.

Applicative Value of Serum CA19-9, CEA, CA125 and CA242 in Diagnosis and Prognosis for Patients with Pancreatic Cancer Treated by Concurrent Chemoradiotherapy

  • Objective: To evaluate the application value of serum CA19-9, CEA, CA125 and CA242 in diagnosis and prognosis of pancreatic cancer cases treated with concurrent chemotherapy. Materials and Methods: 52 patients with pancreatic cancer, 40 with benign pancreatic diseases and 40 healthy people were selected. The electrochemiluminescence immunoassay method was used for detecting levels of CA19-9, CEA and CA125, and a CanAg CA242 enzyme linked immunoassay kit for assessing the level of CA242. The Kaplan-Meier method was used for analyzing the prognostic factors of patients with pancreatic cancer. The Cox proportional hazard model was applied for analyzing the hazard ratio (HR) and 95% confidential interval (CI) for survival time of patients with pancreatic cancer. Results: The levels of serum CA19-9, CEA, CA125 and CA242 in patients with pancreatic cancer were significantly higher than those in patients with benign pancreatic diseases and healthy people (P<0.001). The sensitivity of CA19-9 was the highest among these, followed by CA242, CA125 and CEA. The specificity of CA242 is the highest, followed by CA125, CEA and CA19-9. The sensitivity and specificity of joint detection of serum CA19-9, CEA, CA125and CA242 were 90.4% and 93.8%, obviously higher than single detection of those markers in diagnosis of pancreatic cancer. The median survival time of 52 patients with pancreatic cancer was 10 months (95% CI7.389~12.611).. Patients with the increasing level of serum CA19-9, CEA, CA125, CA242 had shorter survival times (P=0.047. 0.043, 0.0041, 0.029). COX regression analysis showed that CA19-9 was an independent prognostic factor for patients with pancreatic cancer (P=0.001, 95%CI 2.591~38.243). Conclusions: The detection of serum tumor markers (CA19.9, CEA, CA125 and CA242) is conducive to the early diagnosis of pancreatic cancer and joint detection of tumor markers helps improve the diagnostic efficiency. Moreover, CA19-9 is an independent prognostic factor for patients with pancreatic cancer.

  • Carbohydrate antigen 19-9 (CA19-9) is a specific tumor marker of the biliary, pancreatic and gastrointestinal tracts. CA19-9 is occasionally elevated in serum in patiens with benign pulmonary diseases such as bronchiectasis, idiopathic interstitial pneumonia or collagen disease-associated pulmonary fibrosis. Intralobar pulmonary sequestration is an uncommon congenital lung anomaly. It is dissociated from the normal tracheobronchial tree and is supplied by an anomalous systemic artery. There have been some reports of elevation of CA19-9 in this lesion. We report a case of intralobar pulmonary sequestration with elevated serum CA19-9 in a 29-year-old man who was diagnosed with bronchiectasia of left lower lung field on general check up. He had no evidence of any malignant disease in pancreatobiliary or gastrointestinal tracts. Elevated serum CA19-9 level might be encountered with benign pulmonary disease such as pulmonary sequestration.

The Comparisons between RIA and CIA in CA19-9 Monitoring for Diagnosis of Pancreaticobiliary Tumors

  • The CA(carbohydrate antigen)19-9 is complex protein that can be used as an important marker which aids the clinical diagnosis and prognosis of various pancreaticobiliary tumors. However, it was also reported that there were some CA19-9 positive patients with benign disease as using RIA method. The purpose of this study is to evaluate the clinical usefulness of serum level of CA19-9 with RIA(radioimmuno assay), CIA(chemiluminescence immuno assay), and conventional liver function tests. The correlation between CIA and RIA in CA19-9 of pancreatobiliary disease was 0.9833(P<0.01). Also, the correlations between CIA and RIA in CA19-9 of benign and malignant pancreaticobiliary tumor patients was 0.8714(P<0.01) and 0.9727(P<0.01) respectively. The correlation between CA19-9 and ALP was 0.5140(P<0.01) and CEA was 0.3385(P<0.05) as using CIA. The measurement of serum CA19-9 levels by CIA method may be useful in differentiating patients with malignant disease from those with benign disease in pancreaticobiliary tumors.

Preoperative Serum CEA and CA19-9 in Gastric Cancer - a Single Tertiary Hospital Study of 1,075 Cases

  • To evaluate the clinical impact of preoperative serum CEA and CA19-9 on resectable gastric cancer (GC), a total of 1,075 consecutive cases with gastric adenocarcinoma were obtained retrospectively from January 2012 and December 2013 in a single tertiary hospital, and the relationships between serum CEA, CA19-9 and clinicopathologic features were investigated. Positive preoperative serum rates of CEA and CA19-9 were 22.4% and 12.3% respectively, levels significantly correlating with each other and depth of invasion, lymph node involvement, pTNM and stage. The CEA level also presented a remarkable association with lymphovascular invasion. Both CEA and CA19-9 positivity significantly and positively correlated with depth of invasion, nodal involvement, pTNM stage, lymphovascular invasion, tumor size and tumor location. Stratified analyses according to gender or tumor location showed preoperative CEA or CA19-9 had different associations with clinicopathologic features in different gender subgroups or location subgroups. Preoperative serum CA19-9 positivity may be more meaningful for tumor size rather than CEA. In conclusion, preoperative serum CEA and CA19-9 correlate with disease progression of GC, and may have applications in aiding more accurate estimation of tumor stage, decision of treatment choice and prognosis evaluation.

CEA and CA 19-9 are Still Valuable Markers for the Prognosis of Colorectal and Gastric Cancer Patients

  • Background: The purpose of this study was to assess the predictive effect of preoperative CEA and CA 19-9 levels on the prognosis of colorectal and gastric cancer patients. Materials and Methods: CEA and CA 19-9 were evaluated preoperatively in patients undergoing surgery for colorectal cancer (n=116) and gastric cancer (n=49). Patients with CEA levels <5 ng/mL were classified as CEA Group 1, 5-30 ng/mL as CEA Group 2 and >30 ng/mL were classified as CEA Group 3. Similarly the patients with a CA 19-9 level <35 U/mL were classified as CA 19-9 Group 1, with 35-100 U/mL as Group 2 and with >100 U/mL as Group and 3. TNM stages and histologic grades were noted according to histopathological reports. Patients with a TNM grade 0 or 1 were classified as Group A, TNM grade 2 patients constituted Group B and TNM grade 3 and 4 patients constituted Group C. Demographic characteristics, tumor locations and blood types of the patients were all recorded and these data were compared with the preoperative CEA and CA19-9 values. Results: A significant correlation between CA 19-9 levels (>100 U/mL) and TNM stage (in advanced stages) was determined. We also determined a significant correlation between TNM stages and positive vlaues for both CEA and CA 19-9 in colorectal and gastric cancer patients. In comparison between CEA and CA 19-9 levels and age, gender, tumor location, ABO blood group, and tumor histologic grade, no significant correlation was found. Conclusions: Positive levels of both CEA and CA 19-9 can be considered to indicate an advanced stage in colorectal and gastric cancer patients.

일부 농촌지역 주민에서 혈청 CA19-9 및 CA125 농도에 영향을 미치는 인자에 관한 연구 (Factors Related to Serum Level of Carbohydrate Antigen 19-9 and Cancer Antigen 125 in Healthy Rural Populations in Korea)

  • 우리 나라의 일부 지역 주민에서 혈청 CA19-9과 CA125 농도의 분포를 관찰함으로써 경계치 설정 이전에 미리 예비적 자료를 확보하고, 두 종양표지자의 측정치에 영향을 미치는 인자들, 특히 음주와 흡연 습관에 대한 통계적 유의성을 평가하기 위해 단시적 조사연구를 시도하였다. 1995년에 일부 농촌지역 성인 171명(남자 76명, 여자 95명)을 대상으로 하여 설문조사와 혈청 검사를 통해 개개인의 음주, 흡연습관 및 생식요인에 관한 정보 및 혈청 CA19-9 및 CA125 농도를 분석한 결과는 다음과 같다. 1) 혈청 CA19-9 농도의 평균 및 표준편차는 남녀 각각 $10.4{\pm}11.1{\mu}/ml$, $10.1{\pm}10.0{\mu}/ml$로 남녀간의 차이가 없었으나, 혈청 CA125 농도의 평균 및 표준편차는 각각 $2.5{\pm}12.2{\mu}/ml$, $11.2{\pm}7.4{\mu}/ml$로 여자에서 높았다. 한편 현재 임상적으로 적용되고 있는 양성판정치를 근거로 하여 판정한 혈청 CA19-9 및 CA125 양성률은 각각 2.9%, 1.8%이었으며, 각각 남녀간에 통계적으로 유의한 차이는 없었다. 2) 혈청 CA19-9 농도는 일일 흡연량(r=0.23, p=0.054) 및 총 흡연량(r=0.28, p=0.017)과 양의 상관관계를 보였으며, 성별로는 여자에서만 유의한 상관관계를 나타내었다(일일흡연량 r=0.59, p=.0.026 ; 총흡연량 r=0.74, p=0.003). 3) 혈청 CA125 농도는 흡연시작연령(r=0.51, p=0.000) 과는 양의 상관관계를, 총흡연기간(r=-0.23, p=0.051) 및 총흡연량(r=-0.23, p=0.057)과는 음의 상관관계를 보였는데, 성별로는 남자에서만 흡연시작연령(r=0.28, p=0.031)과 유의한 상관관계를 나타내었다. 4) 혈청 CA125 농도는 여자에서 연령(r=-0.20, p=0.058), 생리중단 연령(r=-0.28, p=0.030), 총수유기간(r=-0.29, p=0.014)과 음의 상관관계를 보였다. 5) 혈청 CA19-9 및 CA125 농도는 비만, 학력, 그리고 음주와 유의한 상관관계가 없었으며, 여자에서는 혈청 CA19-9 및 CA125 농도간에 양의 상관관계를 보였다(r=0.20, p=0.047). 6) 연령 및 비만의 교란효과를 통제하기 위한 중회귀분석 결과, 혈청 CA19-9 농도와 유의한 회귀관계를 보이는 변수는 없었다. 이에 비해 혈청 CA125 농도는 전체대상례에선 흡연시작연령이 빠르거나(p=0.000), 총 흡연기간이 길거나(p=0.001), 총흡연량이 많거나(p=0.020), 생리중단연령이 늦거나(p=0.035), 수유를 오래 할수록(p=0.050) 감소하는 것으로 나타났으나, 성별로 분석하였을 때 여자에서 흡연인자들의 유의한 회귀관계가 모두 소실되었다. 결론적으로, 혈청 CA19-9 농도는 흡연, 음주, 월경-출산관련 변수들에 의해 영향을 받는 않는 것으로 보여 실제 임상에 적용할 때 이들 변수를 고려하지 않고도 이용될 수 있는 비교적 안정된 종양표지자로 생각된다. 이에 비해 혈청 CA125 농도는 흡연량이 증가할수록 감소하는 경향을 보였으나 연령과 비만지수에 따라 흡연량이 교란영향을 받고 있는 지표임을 알 수 있었다. 한편, 여성에서는 폐경연령과 총수유기간이 유의한 관계를 보여 향후 정상치역 결정시 생리, 수유 등의 변수를 고려해야 할 것으로 사료된다.

위암 환자에서 복강 세척액의 CEA, CA19-9 측정의 임상적 의의 (Clinical Significance of Measuring Levels of CEA, CA19-9 in Peritoneal Washing Fluid in Patients with Gastric Cancer)

  • 목적: 위암 환자에서 복강 내 파종은 대부분 장막을 침범한 원발성 암에서 떨어져 나온 복강 내 유리 암세포에 의해 이루어지는 것으로 가장 흔한 재발의 형태이다. 수술 중 이러한 유리 암세포의 존재를 직접 또는 간접적으로 확인할 수 있는 방법이 있다면 위암의 치료 전략을 정하는데 유용할 것이다. 이에 수술을 시행 받은 위암 환자를 대상으로 복강 세척액에서 CEA와 CA19-9의 검출 정도와 종양의 침윤도, 림프절 전이 및 병기 등과의 연관성을 알아보고자 연구를 시행하였다. 대상 및 방법: 2004년 5월부터 2006년 1월까지 건양대학교병원 외과에서 위암을 진단받고 수술을 시행한 130명의 환자를 대상으로 하였고, 모든 환자에서 복강 세척액을 채집하여 CEA, CA19-9를 측정하였다. 위의 비선암종 환자 14명을 대조군으로 하여 복강 세척액을 분석하였다. 결과: 위암 환자의 복강 CEA, CA19-9의 측정치는 종양의 침윤도, 림프절 전이 및 병기와 상관 관계가 있었으며(P<0.05) 복강 CEA 측정치는 비선암종 환자군과도 유의한 차이를 보였다. 결론: 복강 CEA, CA19-9의 증가는 TNM 병기와 부분적으로 평균치의 차이가 있고 복강 CEA는 다른 측정치에 비해 가치 있는 복막 전이 예측인자로서의 가능성을 확인하였다.

혈중 CA19-9이 지속적으로 상승되었던 기관지 확장증 환자 1예 (A Case of Bronchiectasis with High Serum CA19-9)

  • CA19-9의 상승은 췌장 및 담도계 악성 종양을 시사하는 소견이다. 하지만, 최근에 양성 소화기계 질환 뿐 아니라 특발성 간질성 폐렴, 교원성 폐 질환, 미만성 범세기관지염, 기관지확장증 등 다양한 양성 폐 질환에서도 혈중 CA19-9 수치가 증가되어 있다고 보고된다. 저자들은 최근 지속적으로 혈중 CA19-9 수치는 증가되어 있지만, 내시경적 역류성 담도 췌장 조영술, 복부 전산화 단층 촬영, PET-CT에서 악성 종양이 발견되지 않은 기관지확장증 1예를 경험 하였다. 3년 10개월의 추적 관찰 기간 중에도 췌장암과 같은 악성 종양은 발견되지 않아서, 혈중 CA19-9 수치의 상승은 기관지확장증에 의한 것으로 추정 되었다. 혈중 CA19-9 수치의 상승은 환자의 임상 양상과 연관하여 조심스럽게 판단하여야 될 것으로 생각된다.
