To determine the clinical usefulness of Immuno Blot test, 160 samples from the patients with chronic HCV infection were analyzed. And serotyping and line probe assay were performed to evaluate the distribution of hepatitis C virus genotypes in Korean isolates. In this group, as a result of genotyping type 1 band 2a, the serotype I and II were the most common source of HCV infection. There were no significant difference in response to the alpha-interferon HCV infection treatment with the subtype 1 b or 2a. And the serotypes of NS4 peptides were compared with the genotypes to evaluate their clinical usefulness. Among 49 cases studied for genotypes and serotype, genotype 1 b, 1 b/2b, 2a, 2a/2c and 2b were 51.0%, 2.0%, 34.6%, 8.1% and 4.0%, respectively. The serotypes I and II were 57.1% and 42.8%, respectively; they were matched with genotypes in 85.7% and seemed to be easy to perform. To monitor their performing progress or treatment response, serotype test was made before the genotype test. The Result showed that there was no significant difference in response to the alpha-interferon HCV infection treatment with the subtype 1 b or 2a in Korea.
Hepatocellular carcinoma (HCC) related to hepatitis B virus (HBV) and hepatitis C virus (HCV) infections is thought to account for more than 80% of primary liver cancers. Both HBV and HCV can establish chronic liver inflammatory infections, altering hepatocyte and liver physiology with potential liver disease progression and HCC development. Cyclophilin A (CypA) has been identified as an essential host factor for the HCV replication by physically interacting with the HCV non structural protein NS5A that in turn interacts with RNA-dependent RNA polymerase NS5B. CypA, a cytosolic binding protein of the immunosuppressive drug cyclosporine A, is overexpressed in many cancer types and often associated with malignant transformation. Therefore, CypA can be a good target for molecular cancer therapy. Because of antiviral activity, the CypA inhibitors have been tested for the treatment of chronic hepatitis C. Nonimmunosuppressive Cyp inhibitors such as NIM811, SCY-635, and Alisporivir have attracted more interests for appropriating CypA for antiviral chemotherapeutic target on HCV infection. This review describes CypA inhibitors as a potential HCC treatment tool that is contrived by their obstructing chronic HCV infection and summarizes roles of CypA in cancer development.
This study established a new polymerase spiral reaction (PSR) that combines with reverse transcription reactions for HCV detection targeting 5'UTR gene. To avoid cross-contamination of aerosols, an isothermal amplification tube (IAT), as a separate containment control, was used to judge the result. After optimizing the RT-PSR reaction system, its effectiveness and specificity were tested against 15 different virus strains which included 8 that were HCV positive and 7 as non-HCV controls. The results showed that the RT-PSR assay effectively detected all 8 HCV strains, and no false positives were found among the 7 non-HCV strains. The detection limit of our RT-PSR assay is comparable to the real-time RT-PCR, but is more sensitive than the RT-LAMP. The established RT-PSR assay was further evaluated for detection of HCV in clinical blood samples, and the resulting 80.25% detection rate demonstrated better or similar effectiveness compared to the RT-LAMP (79.63%) and real-time RT-PCR (80.25%). Overall, the results showed that the RT-PSR assay offers high specificity and sensitivity for HCV detection with great potential for screening HCV in clinical blood samples.
The hepatitis C virus (HCV) is a major causative agent of chronic hepatitis and hepatocellular carcinoma. The development of alternative antiviral therapies is warranted because current treatments for the HCV infection affect only a limited number of patients and lead to significant toxicities. The HCV genome is exclusively present in the RNA form; therefore, ribozyme strategies to target certain HCV sequences have been proposed as anti-HCV treatments. In this study, we determined which regions of the internal ribosome entry site (IRES) of HCV are accessible to ribozymes by employing an RNA mapping strategy that is based on a trans-splicing ribozyme library. We then discovered that the loop regions of the domain IIIb of HCV IRES appeared to be particularly accessible. Moreover, to verify if the target sites that were predicted to be accessible are truly the most accessible, we assessed the ribozyme activities by comparing not only the trans-splicing activities in vitro but also the trans-cleavage activities in cells of several ribozymes that targeted different sites. The ribozyme that could target the most accessible site identified by mapping studies was then the most active with high fidelity in cells as well as in vitro. These results demonstrate that the RNA mapping strategy represents an effective method to determine the accessible regions of target RNAs and have important implications for the development of various antiviral therapies which are based on RNA such as ribozyme, antisense, or siRNA.
Hepatitis C virus (HCV) has become a major public health issue and is prevalent in most countries. HCV infection starts frequently without clinical symptoms, and progresses in the majority of patients (70 to 85%) to persistent viremia and chronic hepatitis including cirrhosis and hepatocellularcarcinoma (HCC) [1]. Alcohol is one of the independent cofactors accelerating the development of HCC in chronic hepatitis C patients. This is of great interest because a synergy between excessive alcohol intake and HCV infection has been documented in the development of HCC in chronic hepatitis C patients [2]. E6AP (E6-associated protein)는 C형 간염바이러스(hepatitis C virus, HCV)의 코어 단백질 유비퀴틴화와 프로테오좀 분해를 유도하여 캡시드 조립을 저해함으로써 HCV 복제를 억제하는 것으로 알려져 있다. 반면에 HCV 코어 단백질은 숙주의 항바이러스 방어계에 대항하고 자신의 유비퀴틴-의존적 프로테아좀 분해를 막기 위하여 DNA 메틸화를 통하여 E6AP 발현을 저해하는 전략을 진화과정에서 획득하였다. 본 연구에서는 HCV 코어 단백질이 E6AP 발현을 저해하는 기전을 밝혀내고자 하였다. HCV 코어 단백질은 HepG2 세포에서 DNA 메틸화 효소들인 DNMT1, 3a 및 3b의 단백질 수준과 효소 활성을 증가시켜 프로모터 과메틸화를 통하여 E6AP 발현을 저해하였지만 p53를 발현하지 않는 Hep3B 세포에서는 이러한 효과들이 관찰되지 않았다. 흥미롭게도 Hep3B 세포에 p53만 과발현시키면 HCV 코어 단백질이 없더라도 DNMT가 활성화되고 프로모터 과메틸화를 통하여 E6AP 발현이 저해되었다. 또한 p53 녹다운 및 과발현 실험을 통하여 p53 활성화가 HCV 코어 단백질의 효과에 필수적임을 알 수 있었다. 이로 인하여 Hep3B 보다 HepG2 세포에서 낮은 수준의 유비퀴틴화된 HCV 코어 단백질이 검출되었다. 따라서 HCV 코어 단백질은 p53-의존적으로 자신의 유비퀴틴-매개성 프로테아좀 분해를 저해한다.
직접적인 근거를 제시하기는 힘드나 정신장애에서 자가면역적 요소가 병인적으로 중요하다는 단편적인 보고들이 있다. 특히 바이러스 감염은 정신장애를 유발하거나 나중에 정신장애에 대한 소지를 증가시킬 가능성이 높다. 저자들은 최근에 많은 관심을 끌고 있는 C형 간염 항체(이하 anti-HCV)의 양성율이 자가면역적 관점 및 수혈 외에도 성행위 또는 약물의 존자에서 많다는 전파경로상의 특정 때문에 정신과 환자에서 일반 인구군보다 높을 것으로 추정되어 이를 확인해 보고자 본 연구를 시행하였다. 1992년 12월 초부터 1994년 5월 말까지 정신과에 입원한 환자 중에서 무작위로 효소면역측정법 (Abbott HCV EIA kit) 에 의해서 혈청내 anti-HCV를 검사하였으며, anti-HCV 양성 환자와 anti-HCV 음성인 환자를 구분하여 여러 변인별로 비교 분석하였다. 정신과 입원환자 113명중 12명(10.6%) 에서 anti-HCV 양성이었다. anti-HCV 양성자중 간기능검사상 이상이 있는 경우가 50.0% 로서 이중 83.0%는 주정 의존자였으며, 간기능검사상 정상인 환자의 83.3%는 비주정의존자였다. 정신과 진단별 anti-HCV 양성율은 주정의존 환자의 22.2%, 정신증 환자의 2.3% (주로 양극성장애), 신경증(불안장애, 적응장애)환자의 22.2%에서 anti-HCV 양성이 나타났다. 연령, 출생계절, 임파구(%), 간기능 등 변인에 대한 유의한 상관성은 관찰되지 않았다. 결론적으로 정신과 입원환자는 정상 대조군 (3.0%)에 비해 최소한 3.5배 이상 anti-HCV 양성율이 높으므로 (P<0.05), 이에 대한 주의를 환기시킬 필요가 있다.
Anti-HCV 양성인 만성간질환 환자 80명의 가족중 3가족에 걸쳐 3명이 anti-HCV 양성이었고 양성율 3.5%로서 HBsAg 양성 만성간질환 환자 60명의 가족중 40가족에서 HBsAg 양성자가 나타난 결과가 양성을 66.7%보다 월등히 낮은 수치를 보였다. 이 결과를 볼때 HCV는 HBV와는 다른 정도의 감염력을 가지거나 또는 다른 형태의 개체면역반응을 유도하여 항체 형성이 낮은 것이 아닌지 고려해 보아야겠다. 가족중 배우자나 자녀에 있어서의 감염율은 HBV와 비교될 정도는 아니었으나 anti-HCV 양성인 사람이 배우자나 자녀였으므로 향후 성적접촉이나 수직감염에 의한 감염 가능성에 관해 더욱 조사가 필요할 것으로 사료된다. 그러나 아직 HCV RNA를 직접검사하는 PCR검사가 보편화되어 있지 못하며 HCV 감염의 진단을 위한 검사법이 여러가지 난무하는 이유로 이후 정밀도가 높으며 쉽게 이용될 수 있는 HCV 감염진단을 위한 검사법이 확립되어 정확한 HCV 환자의 파악이 먼저 되어야 HCV 감염의 역학적 조사가 정확히 이루어지며 HCV 감염관리에 도움이 될 것으로 생각된다.
3세대 EIA를 이용한 anti-HCV 검사는 2세대 EIA에 비해 예민도와 특이도가 향상되었으나, 여전히 제조회사간에 서로 상이한 결과를 보이는 경우가 많았다. 저자들은 국산 3세대 anti-HCV 검사 시약인 LG HCD 3.0과 기존 영남대학교 의과대학 부속병원에서 사용하고 있는 Axsym HCV version 3.0, Cobas Core anti-HCV EIA와 비교하였다. Cobas 키트가 분별력과 민감도는 높았으나 상대적으로 위양성률이 높은 경향을 보였다. LG 키트는 희석 검체에서 1례를 제외하고는 1:160까지 양성을 보였으며, 다른 두 종의 anti-HCV 키트와 우수한 일치율을 보였으나, 다른 두 키트에 비해 비교적 분별력이 낮아 양성의 50%에서 S/C가 5이하의 값을 보였다. 위양성을 보인 검체의 S/C는 1에서 4사이이고, 위음성을 보인 1례에서는 0.91로 나타나 S/C가 0.8에서 4사이인 경우 면역블로팅검사로 확인할 필요가 있다고 생각되므로 검체의 양성율과 사용목적을 고려하여 시약을 선정하여야 할 것이다.

Hepatitis C virus (HCV) is remarkably efficient at establishing chronic infection. One of the reasons for this appears to be the suppression of the accessory cell function of professional antigen presenting cells. In the present study, the immunosuppressive activity of HCV protein was examined on dendritic cells (DCs) generated from mouse bone marrow progenitor cells in vitro. We found that the DCs forced to express HCV protein have defective allostimulatory ability. DCs expressing HCV protein were phenotypically indistinguishable from normal DCs. However, they were unable to produce IL-12 effectively when stimulated with lipopolysaccharide. The functional domain of the HCV protein essential for immunosuppression was determined using a series of ${NH_2}-and$ C-terminal deletion mutants of HCV core protein. We found that amino acid residues residing between the 21 st and the 40th residues from the ${NH_2}-terminus$ of HCV core protein are required for immunosuppression. These findings suggest that HCV core protein suppresses the elicitation of protective Th1 responses by the inhibition of IL-12 production by DCs.
