지혈대 내측의 포비돈-요오드 용액에 의한 화학 화상 (1예 보고) (Povidone-Iodine related Chemical Burn under the Tourniquet (A Case Report))

  • In the extremity surgery, pneumatic tourniquet and povidone-iodine solution are commonly used to provide an aseptic, bloodless field, and their complication rate has remained low. However, chemical burn under tourniquet has been rarely reported. Patients sustained burn injuries over the dependent, weight-bearing regions such as posterior neck, back, buttocks and posterior thighs. This rare adverse complication occurred in a 22-year-old man who underwent modified Brostrom operation with arthroscopic os trigonum excision. 10% povidone-iodine was used as topical antiseptic, and full thickness burn occurred underneath the area of tourniquet application. Main causes of povidone-iodine related chemical burn are considered maceration, irritation of the skin, long term use of the tourniquet and pressure. To reduce the complications like chemical burn, awareness of the risk and the possible pathogenesis as well as the preventive measures is important in surgical practice.

화상환자의 외상후 스트레스 장애 증상에 대한 영향 요인 : 울분, 우울, 삶의 의미 (Factors Influencing Posttraumatic Stress Disorder Symptoms in Burn Injured Patients : Embitterment, Depression and Meaning in Life)

  • Objective : To determine predictors of posttraumatic stress disorder (PTSD) symptoms in burn injured patients and evaluate factors for identifying high risk group of PTSD. Methods : This study examined sixty one patients aged in the range of 19-65 years with burn injuries. All subjects completed self-assessment inventories about PTSD (The PTSD Check List for DSM-5. PCL-5), depression (Patient Health Questionnaire-9, PHQ-9), embitterment (Posttraumatic embitterment disorder self-rating scale, PTED scale) and meaning of life (Meaning in Life Questionnaire, MLQ). Stepwise multiple regression and ROC curve analysis were the tools used for analysis. Results : The results revealed higher depression, embitterment and lower presence of meaning in life predicted severe PTSD symptoms. ROC analysis indicated PTED scale and PHQ-9 were useful for discriminating high risk group of PTSD. Conclusion : The present study established that the need to consider embitterment, depression and meaning of life for alleviation and prevention of PTSD symptoms in burn patients.

Thermal Burn Injury from a Forced-Air Warming Device in an Anesthetized Dog with Peripheral Hypoperfusion

  • A 13-year-old female Poodle presented with gallbladder rupture caused by a mucocele and hypoperfusion. The animal had a thermal burn injury caused by a forced-air warming device used for hypothermia during surgery. This could be attributed to two causes. First, the forced-air warming device was used with direct hosing, without attachment to an air blanket, and the heat was concentrated in a single area. Second, perioperative peripheral hypoperfusion hampered heat dissipation and increased the susceptibility to a burn injury. These findings suggest that an air blanket should be used with a forced-air warming device according to the manufacturer's instructions. Furthermore, patients with peripheral hypoperfusion are at a higher risk of burn injuries and require close monitoring.

  • Fires in operating rooms rarely occur. However, this type of disaster can complicate almost any surgical procedure. Fuel, heat and oxygen are related with fire outbreak. When ignition sources such as alcohol-based surgical preparation solutions are present, the risk of an operating room fire increases, and burns are more severe in such conditions. Many manufacturers recommend waiting at least three minutes after application to allow complete drying for reduce fire risk. There are a few studies regarding flame burns in the operation room, although most of these studies are related to preoperative skin preparation. However, alcohol containing solutions can be used occasionally for cleansing of the operation field after the surgery, therefore, the surgical team should pay attention to surgical fires, even if they have completed the operation successfully. We present our case of a post-operative flame burn and introduce some precautions that will reduce the risk of alcohol burns.

소아 화상 환자 2,795례에 대한 연령별 임상 통계학적 분석 (Clinical and Statistical Analysis with Age in Cases of Pediatric Burn Patients)

  • Purpose: The objective of this study is to analyze the epidemiological characteristics of pediatric burn patients and to determine the targets for a pediatric burn prevention program. Methods: A retrospective review of all medical records of acute pediatric burn patients (age < 15 years old) admitted to our hospital between January 2005 and December 2009 was performed. Results: 1472 males and 1323 females were investigated, with a male to female ratio of 1.11 : 1. The greatest number of burn patients were those with an age of 1~2 years (1,463, 52.3%). Scalding burn was the most common cause of injury, which accounted for 2183 (78.1%) patients, followed by contact burns (10.5%), flame burn (4.9%), steam burn (3.6%). Especially steam burn was the second cause of injury in the age under 1 year, while flame burn was the second cause of injury in the age over 7 years. During recent 5 years, incidence of flame burn, steam burn, electrical burn gradually decreased. Variation of seasonal incidence is minimal and most of the patients (2,716 cases, 97.2%) had burns less than 20% TBSA (Total body surface area). The median hospital stay was 18.79 days, and the rate of operation was 28.6% with a high rate in electrical burn (76.2%), flame burn (50.0%), steam burn (46.1%). 6 patients died in this series, which yielded a mortality rate of 0.2%. Conclusion: Prevention efforts should reflect recent study results. Focused prevention program and campaign to make people aware of risk factors and their avoidance is required to reduce the number of burn accidents in children.

3도 화상에 대한 동종 상피세포 치료제의 효과 보고 (Feasibility of Cultured Allogenic Keratinocyte Treatment for Third Degree Burns)

  • Allogenic keratinocyte application is widely used for treatment of second degree burns. However, there is no significant body of report on application of allogenic keratinocyte to third degree burns. A geriatric patient visited our burn center showing second to third-degree burn on dorsum of her left hand. Considering the surface area and wound depth, surgery was indicated but her medical condition and age made the surgery high risk for a long operation. Therefore, chemical escharolysis, serial bedside debridement, and cultured allogenic keratinocyte (Kaloderm®, Tegoscience, Seoul, Korea) application was done. The wound was completely epithelialized after four rounds of Kaloderm® application. For third-degree burns where definitive surgical reconstruction is precluded due to medical comorbidity of the patient, we investigated the possibility of allogenic keratinocyte treatment which may allow to avoid high-risk anesthesia and surgery.

셀룰로오스 혼방 니트 편포의 착색번아웃 날염복합기술에 관한 연구 (A Study on the Cellulose Blend Knit Fabrics using Burn-out Printing Convergence Technology)

  • This study conducted a research on burn-out printing convergence technology for cellulose blend knit fabrics. Printing technology, which forms color pattern on the fabric, can be generally classified into four according to printer or printing method, e.g. screen printing, roller printing, rotary printing, digital printing. However, these printing methods are flat in design or pattern, which have limitation to overcome monotonousness of fabric, so that recently burn-out process method, which expresses three-dimensional pattern effect by treating chemical on the surface of fabric as the method to appeal its esthetics to the customers. Particularly, in case of cellulose/polyester composite material, first, it is proceeded in 2 processes, by dyeing cellulose or polyester fabric and burning out cellulose fabric, in this process, due to pollution caused by disperse dye migration, color of polyester fabric part could be discolored, which has high falt risk. This research considered coloring burn-out technique, which simultaneously proceed dyeing and burn-out by reducing dyeing and burn-out process to 1 stage, which were proceeded in 2 stages previously. As the research result, it was confirmed that reasonable depth of roller was 0.04~0.06mm in roller printing process, heat treatment condition of burn-out far-infrared radiation was $185^{\circ}C{\times}30m/min$. Color fastness to washing was confirmed to be 4-5 grade, color fastness to rubbing, 3-4 grade, color fastness to light, 4 grade. Also, it was confirmed that energy reduction effect appeared 38.19%, in case of energy cost per yard compared to the existing production, also, 19.74%, in case of production cost.

산림 공간구조 특성과 산불 연소강도와의 관계에 관한 연구 (Linking Spatial Characteristics of Forest Structure and Burn Severity)

  • Because fire has significant impacts on fauna and flora in forest ecosystems, as well as socioeconomic influences to local community, it has been an important field of study for decades. One of the most common ways to reduce fire risk is to enhance fire-resilience of forest through fuel treatments including thinning and prescribed burning. Since fuel treatment can't be practiced over all forested areas, appropriate and effective strategies are needed. The present study aims to look at the relationship between spatial characteristics of forest structure measured with landscape pattern metrics and burn severity to provide guidelines for effective fuel treatments. Samchuck fire was selected for the study, and 232 grids covering the study areas were generated, and the grid size was 1km. The burn severity is measured with dNBR derived from satellite imagery, and spatial characteristics of forest structure were measured using FRAGSTATS for both landscape and class levels for each 1km grid. The results of this study strongly indicated that heterogeneity in composition and configuration of forests may significantly reduce burn severity. By enhancing heterogeneity of forests, fuel treatments for fire-resilience forest could be more effective.

  • Objectives: The purpose of this study is to understand the factors affecting the dual smoking experience of conventional and heat-not-burn tobacco among adolescents with smoking experience. Methods: The study selected 8,691 people with experience in smoking. Data analysis used SPSS 25.0. Logistic regression was performed to identify the factors affecting the dual smoking experience. Results: In this study, the dual smoking experience rate of conventional and heat-not-burn tobacco was 16.3%. The dual smoking experience rate was 5.09 times higher than the number of smokers among friends. The lower the smoking age, the higher the smoking rate, and the longer the smoking days, the higher the dual smoking rate of heat-not-burn cigarettes. Conclusion: We need a peer-to-peer group arbitration program rather than a separate anti-smoking arbitration program for smokers.

Cardiorespiratory Fitness Is Associated with Gait Changes among Firefighters after a Live Burn Training Evolution

  • Background: Recommendations have been proposed for minimum aerobic fitness among firefighters but it is unclear if those criteria relate to performance on the fireground. Less fit individuals fatigue more quickly than fit individuals when working at comparable intensity and may have gait changes, increasing risk of falls. We evaluated the effect of fatigue during a live burn evolution on gait parameters and functional balance comparing them to aerobic fitness levels. Methods: A total of 24 firefighters had gait and balance tested before and after a live burn evolution. Data were stratified by aerobic fitness of greater/less than 14 metabolic equivalents (METs). Results: Analysis of gait cycles measurements before and after the live burn evolution revealed that single leg stance, cycle, and swing time decreased (p < 0.05) but there were no differences in the other measures. There were no differences in time to complete the functional balance test, or errors committed before or after a live burn evolution. When firefighters were sorted by fitness level of 14 METs, there were no differences for errors or time before or after the live burn evolution. Balance data were analyzed using a linear regression. Individuals with lower fitness levels required more time to complete the test. Conclusion: A 14-MET criterion failed to distinguish gait or balance characteristics in this group. However, less fit firefighters did require more time to complete the balance test (p = 0.003). Aerobic fitness alone does not predict gait changes among firefighters following a live burn evolution but does appear to influence functional balance.
