응급실 화상환자의 역학적 요인에 대한 고찰: 추이변화와 예후에 미치는 영향을 중심으로 (A Study on Epidemiological Factors of Burn Patients in Emergency Rooms)

  • Purpose: It is important to consider both clinical factors and epidemiological factors in treating burn patients in emergency rooms. However, many emergency medical staffs happen to miss their chances of treating burns based on these considerations. This study is designed to find a better treatment for burn patients in emergency rooms along this approach. Methods: This study was conducted based on the data of the burn patients visiting the emergency room of a single general hospital from January 2015 to December 2019. The epidemiological and clinical factors were extracted out of the data, then the relationship between the prognosis and these factors were analyzed. Results: Most of burn accidents occurred at home, and were caused by hot water, soup, drinks, oil, etc. Especially, flame burns showed high hospitalization rate, surgical rate and mortality. In addition, their prognosis was poor when the affected area included facial, limb and perineal areas etc., or any inhalation burn co-existed. Also, the hospitalization rate and period increased when the treatment time was delayed or the pre-treatment was taken. There was a strong relationship between prognosis and the period of follow-up when patients were admitted during the period. Conclusion: It is difficult for medical staffs to evaluate prognosis of burns in emergency rooms due to progressive damages. Precise treatment and disposition are essential for patients' good prognosis. Therefore, medical staffs should establish treatment plans by identifying the patient's epidemiological and clinical factors, rather than giving prescriptions based on fragmentary and superficial symptoms.

매몰재의 혼수비가 국부의치 금속구조물의 Fin에 미치는 영향에 관한 실험적 연구 (THE EFFECT OF INVESTMENT'S W/P RATIO ON THE FIN OF PARTIAL DENTURE METAL CASTINGS)

  • We use investment to retain the mould of the wax pattern in making dental metal castings. A fin on the metal casting is occasionally formed due to several factors. The factors making the fin are improper burn out time and temperature, improper W/P ratio of investment and lining of asbestos in casting ring. The purpose of this study is to investigate the effect of W/P ratio of investment on the fin formation. Except the W/P ratio of investment which used to invest the wax pattern, the study was done under same condition; burn out time and temperature, W/P ratio of refractory cast(W/P=0.12) and asbestos lining in casting ring. The obtained result is that the fin is more likely to be formed on the casting which invested with higher W/P ratio of investment to that of refractory cast.

창상 치료 시스템에 대한 선호도 및 만족도 조사 (A Preference and Satisfaction Survey on the Wound Management System)

  • Purpose: The biggest problem of wound healing is a possible occurrence of lesion. Especially, in the case of patients who have a skin injury around exposed body parts, if their treatment period drag on for long time, they can suffer from aftereffects and the costs can be passed on to a society. Therefore, in this research, we investigated the need to develop the effective medicine and appliances for the patients by examining which therapy methods are being applying to the skin damage and what is the advantage and limit by evaluating the patient's satisfaction level. Methods: We carried out an online and offline survey targeting medical teams in order to analyze device for wound care. A total of 125 medical teams applied to the research, and investigate the level of customer satisfaction. Results: The moist dressings are the most used method for wound healing. When it comes to the level of customer satisfaction, biological dressing product also has a high satisfaction level. However its high cost tends to limit the use. Conclusion: This research reached a conclusion that it is need to develop a low cost and high efficiency wound care product considering the fact that its high cost and low efficiency induced economic problems. Generally, it is needed to develop a product for skin regeneration based on biological technologies, not a product just for damage cure.

폐기물 소각로 베드에서의 연소현상 관찰을 위한 실험적 연구 (An Experimental Study on the Bed Combustion Phenomena in MSW(Municipal Solid Waste) Incinerator)

  • Experimental studies have been performed to observe the basic phenomena of waste bed combustion in MSW incinerator. A reduced scale apparatus was utilized to simulate the combustion behavior in real plant with 1-dimensional transient behavior at the experimental setup, which uses wet cubic wood with ash content as simulated waste. LHV (lower heating value) of solid fuel, fuel particle size and flow rate of combustion air were taken as important parameters of the bed combustion. For the quantitative analysis, FPR (flame propagation rate), TBT (total burn-out time) and PBT (particle burn-out time) was defined. LHV represent the capability of heat release of the fuel, so that a higher LHV results in faster reaction rate of the fuel bed, which is shown by higher FPR. Fuel particle size is related with surface area per unit mass as well as heat and mass transfer coefficient. As the particle size increases the FPR decreases owing to decreasing specific surface area. Air injection supplies oxygen to the reaction zone. However oversupply of combustion air increases convection cooling of the bed and possibly extinguishes the flame.

  • As the 4th industrial age approaches, the demand for semiconductors is increasing enough to be used in all electronic devices. At the same time, semiconductor technology is also developing day by day, leading to ultraprecision and low power consumption. Semiconductors that keep getting smaller generate heat because the energy density increases, and the generated heat changes the shape of the semiconductor package, so it is important to manage. The temperature change is not only self-heating of the semiconductor package, but also heat generated by external damage. If the package is deformed, it is necessary to manage it because functional problems and performance degradation such as damage occur. The package burn in test in the post-process of semiconductor production is a process that tests the durability and function of the package in a high-temperature environment, and heat dissipation performance can be evaluated. In this paper, we intend to review a new material formulation that can improve the performance of the adapter, which is one of the parts of the test socket used in the burn-in test. It was confirmed what characteristics the basic base showed when polyamide, a high-molecular material, and alumina, which had high thermal conductivity, were mixed for each magnification. In this study, functional evaluation was also carried out by injecting an adapter, a part of the test socket, at the same time as the specimen was manufactured. Verification of stiffness such as tensile strength and flexural strength by mixing ratio, performance evaluation such as thermal conductivity, and manufacturing of a dummy device also confirmed warpage. As a result, it was confirmed that the thermal stability was excellent. Through this study, it is thought that it can be used as basic data for the development of materials for burn-in sockets in the future.

황산구리를 이용한 면/폴리에스터 교직물의 탄화가공 (Burn-out Finishing of Cotton/Polyester Fiber Mixed Fabrics using Cupric Sulfate)

  • This study used cupric sulfate as a bum-out agent. The concentration of cupric sulfate, temperature and time were varied with glycerin included or excluded on the properties of polyester ground fabrics. The results are as follows. The effect of carbonization with glycerin included was decreased, but removal of carbide with glycerin included was easier than that with glycerin excluded. The white index and tensile strength of polyester ground fabrics were decreased as the processing concentration, temperature and time increased. The shrinkage was increased as the processing concentration, temperature and time increased. The optimum conditions with cupric sulfate as the bum-out agent was 5% concentration, 140$^{\circ}C$, 3min., and 3kgf/cm$^2$ and with cupric sulfate added to glycerin was 5% concentration, 150$^{\circ}C$, 5min., and 3kgf/cm$^2$.

Acticoat®를 이용한 불산에 의한 화학 화상의 치료 (Treatment for Hydrofluoric Acid Chemical Burn Using Acticoat®)

  • Purpose: Hydrofluoric acid(HF) is one of the most dangerous mineral acids with the dissociated fluoride ions. The initial corrosive burn is caused by free hydrogen ion, and the second and more severe burn is caused by penetration of fluoride ions into subcutaneous tissues. Silver is a cation producing dressing, an effective antimicrobial agent, but older silver-containing formulations are rapidly inactivated by wound environment, requiring frequent replenishment. But, $Acticoat^{(R)}$ is a relatively new form of silver dressing which helps avoid the problems of earlier agents. The aim of this study is to evaluate effects of $Acticoat^{(R)}$, silver-containing dressing on the treatment for HF injury wound. Methods: From september 2006 to september 2007, the study was carried out with 10 patients who had HF partial thickness burns. $Acticoat^{(R)}$ dressing and 10% calcium gluconate wet gauze dressings in 10 cases. As a principle, in the emergency treatment, partial or complete removal of the nail and early bullectomy along with copious washing with normal saline was done, depending on the degree of HF invasion of the wound. Wound was dressed with $Acticoat^{(R)}$ and 10% calcium gluconate solution. The effect of dressing was investgated by serial bacterial culture and wound exudates assessment. Results: We therefore reviewed 10 cases of HF-induced chemical burns and treatment principle. The 10 cases who came to the hospital nearly immediately after the injury healed completely without sequelae. Conclusion: As the industrial sector develops, the use of HF is increasing more and more, leading to increased incidences of HF-induced chemical burns. The education of patients regarding this subject should be empathized accordingly. In conclusion, $Acticoat^{(R)}$ dressing is a better choice for HF partial thickness burn injuries because of shorter healing time, less pain and more comfortable dressing.

치과용 합금 주조 시의 소환온도와 주조 후 냉각방법이 미세조직과 부식거동에 미치는 영향 (The Effect of Burn-out Temperature and Cooling Rate on the Microstructure and Corrosion Behavior of Dental Casting Gold Alloy)

  • The microstructure and corrosion behavior of commercially dental casting gold alloys were investigated to clarify the effect of burn-out temperature and cooling rate. In the case of water quenching after casting, only the αphase, which is typical dendritic microstructure of golda alloy, was detected. However, the precipitates along the grain boundary were detected only at the slow cooling rate and they increased inversely proportional to the burn-out temperature. This might be due to the time difference which solute atom could diffuse. EPMA and SEM results also demonstrated that the precipitate should be lamellar structure consisted of Ag rich phase(${\alpha}_1$) and Cu rich phase (${\alpha}_2$). In terms of corrosion, the galvanic coupling was formed due to the difference of composition between precipitates and matrix at the slow cooling rate. In the case of water quenching, the critical current density($i_p$) which indicate the degree of corrosion was lowest at $650^{\circ}C$ and below the burnout temperature, $i_p$ increased with it because of the effect of grain boundary segregation. But above the temperature, $i_p$ increased with it. This may be due to the strain field effect by residual thermal stress.

동적 존 할당이 가능한 번인 시험 시스템 (A Burn-in Test System with Dynamic Bone Allocation)

  • 번인 시험(burn-in test)은 초기 장애가 예측되는 반도체 소자들을 제거하기 위한 시험으로서, 보통 소자에 전압, 온도 및 시간 등의 가혹 조건을 가하며 행해진다. 이런 번인 시험을 위해서는 시험할 소자들을 가진 번인 보드들을 해당 슬롯(slot)들에 삽입할 필요가 있다. 이런 슬롯들의 집합을 존(zone)이라 부른다. 한 존을 구성하는 슬롯들은 동종 반도체 소자들을 가진 번인 보드들만을 가질 수 있다. 따라서 많은 이종 반도체 소자들의 시험을 위해서는 존 수가 많도록 번인 시험 시스템을 구성하는 것이 바람직하다 존을 제어하는 존 컨트롤러는 번인 시험을 수행하고 결과를 수집하는 장치이다. 기존 시스템의 경우, 각 존 컨트롤러는 일정 수의 슬롯들로 구성된 한 존의 시험을 담당한다. 대개, 존 컨트롤러는 번인 시험의 전반을 통제하는 한 워크스테이션 내에 내장되어 있으므로 이들의 추가는 물리적 공간의 제약을 받는다. 본 논문은 이런 문제들의 해결이나 완화를 위한 한 방법으로서 존의 슬롯 수를 동적으로 할당할 수 있는 동적 존 시스템을 제안한다. 이 시스템은 존의 슬롯 수를 가변시켜 빈 슬롯을 최소화함으로써 시스템의 운영 효율을 극대화한다. 또한 기존 시스템의 경우에는 정비를 위해 진행 중인 시험을 모두 중지시켜야 하지만, 동적 존 시스템의 경우에는 시스템에 전력을 공급하는 주 전원이 문제가 되지 않는다면 슬롯별로 개별적인 유지보수가 가능하다.

한의사의 간접구 이용실태에 대한 조사 연구 (Survey on the Status of Utilization of mediate Moxibustion by Korean Medicine Doctor)

  • Objectives : We examined the use of the moxibustion through survey on Korean Medicine Doctors (KMDs) on preparation of universal moxa product standard by fully reflecting opinions of practitioners and the industry. Methods : The questionnaire is composed of 25 questions including use of indirect moxa, side effects, complaints and improvements, general characteristics and so on, and were responded by 1,588 KMDs. For statistics, frequency analysis, chi-square analysis, T-test and ANOVA were conducted using SPSS 12.0 for windows, and the significance level was 0.05. Results : In this research, 91% of the responded KMDs was using moxibustion, and the proportion of using mediate moxibustion was shown as 63.2%. They used mediate moxibustion for side effects of direct moxa method such as risk of burn, ease of use, control of stimulation and so on. And it was mostly being conducted to women and 40 ages. Most respondents gave moxibustion treatments for the purpose of thermal stimulation on acupoint to musculoskeletal diseases. Most respondents were treating 1~2 pieces of moxibustion to patients by moxibustion points to those of 1~2 regions every time within 10 minutes. Regarding the frequency of side effects of mediate moxibustion, within 3% of respondents had experienced side effects, and the types were burn, flare and itchiness, odor and smoke. The dissatisfaction of mediate moxibustion products was moderate(2.00 out of 5 point scale), and went up public health doctors, the younger ages and the shorter clinical experience. Complaints on mediate moxibustion products were serious odor, poor attachment, risk of burn etc. Regarding ways to improve mediate moxibustion, the expansion of moxibustion's insurance cost carne out highest, followed by preparation of manual on efficacy, strengthening of training, acquiring evidence on efficacy, diversification of specifications, safety improvement, quality improvement of mediate moxibustion products. Conclusions: Many KMDs use for ease of use and give moxibustion treatments for thermal stimulation but they complain about safety, efficacy and quality of mediate moxibustion. After this it is considered that we have to elevate therapeutic effect through the improvement of mediate moxibustion products.
