Osteoma is a slow growing benign osteogenic lesion characterized by proliferation of compact or cancellous bone. Osteomas may be classified as peripheral, central, or extraskeletal. Central osteoma arise from the endosteum, pheripheral osteoma from the periosteum, and extraskeletal soft tissue osteoma within a muscle. Peripheral osteoma of the mandible is uncommon. They manifest as asymptomatic, fixed tumors of bony-hard consistency that may be sessile or pedunculated. Radiographically, a well circumscribed round or oval radiopaque mass is seen. Here, we report a case of a huge solitary peripheral osteoma of the buccal posterior mandible in a 40-year-old woman who was otherwise in good health. The patient visited at the dental clinic because the lesion makes esthetic problem. Treatment was performed by surgical esthetic recontouring and histological examination. The patient remains free of recurrence after surgical esthetic recontouring operation.
Epidermoid cyst is a cystic form of teratoma and believed to be derived from trapped embryonic cells along the lines of embryonic closure. A 28-year-old woman presented with a painless swelling over the left mandibular area. On panoramic view, the mandible revealed a $5.5{\times}2.0\;cm^2$ multilocular radiolucent lesion of the left mandibular body and a computed tomography scan showed expansion of both the buccal and lingual plates in the same area. Microscopy found stratified squamous epithelium of the cystic wall and cystic contents of keratinized material. The histological diagnosis wasan epidermoid cyst. The most common location of epidermoid cyst at the head and neck is in the orbit (47%), followed by the mouth floor (23%) and the cervical area (9~24%), but in the jaw bone, it is considered very infrequent. We report the uncommon epidermoid cyst in the mandibular body that had a good healing outcome after treatment with a conservative marsupialization during the 40 months follow-up.

Few cases of florid osseous dysplasia has been described as a condition that characteristically affects the jaws. It usually manifests as multiple radiopaque masses distributed throughout the jaws. Confusion exists about the relationship of florid osseous dysplasia, gigantiform cementoma, chronic sclersing osteomyelitis, sclerosing osteitis or multiple enostosis. Authors experienced a case of florid osseous dysplasia of the jaws in 52-year-old female on the basis of clinical, radiographic and histopathologic findings. The characteristic features are as follows : 1. In clinical examination, there was no clinical sign and symptoms except extracted area. And there was no facial asymmetry. 2. Radiograms show round or lobular dense radiopaque masses surrounded by radiolucent bands in lower molar teeth area bilaterally. And slight increased radiopacities in maxillary molar teeth area bilaterally. There was no expansion or thinning of buccal and lingual cortical bones. There is no displacement or resorption of involved teeth. In right side of mandible, mandibular canal is displaced inferiorly due to mass. 3. Photomicrograms show densely mineralized sclerotic acellular masses with empty lacunae. Pattern is suggestive of cementum, although it could be considered sclerotic bone. In the periphery, lesion consisting of moderately cellular fibrous tissue in which globular calcified products are deposited.
Purpose: This study was performed to investigate the characteristics of mandibular incisive canal (MIC) in Korean population. Materials and methods: A total of 97 subjects (60 males and 37 females) who underwent cone-beam computed tomography were included in the study. The anatomic features of MIC was assessed according to gender. Length, diameter and distance to inferior, lingual and buccal border were measured at the origin and the terminal. Also the distribution of MIC at each tooth position was evaluated. Results: Of 97 patients included, 75(77.3%) presented bilateral MIC and 13(13.4%) presented unilateral MIC. Of 194 hamimandibles, MIC was detected in 102(85%) sites in male and 61(82.4%) sites in female. Gender and side showed no statistically significant differences in detectability. The length, diameter and distance to adjacent structures were bigger in male than in female except the distance to lingual border. MIC travelled anteriorly in a slightly downward and lingual direction and usually terminated between the first premolar and the canine. On cross-sectional view, MIC showed individually scattered distribution both buccolingually and superoinferiorly. Conclusion: MIC is well detected with cone-beam computed tomography. Considered that the length and the location of MIC has large variations between individuals, its localization using cone-beam CT is highly recommended before performing surgical procedures such as implant placement and bone harvesting.
The aim of this systematic review was to evaluate the effectiveness and complications of corticotomy and piezocision in canine retraction. Five electronic databases (PubMed, SCOPUS, Web of Science, Embase, and CENTRAL) were searched for articles published up to July 2017. The databases were searched for randomized control trials (RCTs), with a split-mouth design, using either corticotomy or piezocision. The primary outcome reported for canine retraction was either the amount of tooth movement, rate of tooth movement, or treatment time. The secondary outcome was complications. The selection process was based on the PRISMA guidelines. A risk of bias assessment was also performed. Our search retrieved 530 abstracts. However, only five RCTs were finally included. Corticotomy showed a more significant (i.e., 2 to 4 times faster) increase in the rate of tooth movement than did the conventional method. For piezocision, both accumulative tooth movement and rate of tooth movement were twice faster than those of the conventional method. Corticotomy (with a flap design avoiding marginal bone incision) or flapless piezocision procedures were not detrimental to periodontal health. Nevertheless, piezocision resulted in higher levels of patient satisfaction. The main limitation of this study was the limited number of primary research publications on both techniques. For canine retraction into the immediate premolar extraction site, the rate of canine movement after piezocision was almost comparable to that of corticotomy with only buccal flap elevation.
A variety of therapeutic modalities can be used for the endodontic treatment of a traumatized tooth with internal root resorption (IRR). The authors present a case report of the successful restoration of a traumatized upper central incisor that was weakened due to severe IRR and subsequent periapical lesion formation. A 20-year-old female patient was referred to our clinic with severe internal resorption and subsequent periapical pathosis destroying the buccal bone wall. Root canal treatment had been initiated previously at another dental practice, but at that time, the patient's condition could not be managed even with several treatments. After cone-beam computed tomography imaging and proper chemomechanical cleaning, the tooth was managed with a mineral trioxide aggregate plug followed by root canal filling using short fiber-reinforced composite, known as the Bioblock technique. This report is the first documentation of the use of the Bioblock technique in the restoration of a traumatized tooth. The Bioblock technique appears to be ideal for restoring wide irregular root canals, as in cases of severe internal resorption, because it can uniquely fill out the hollow irregularities of the canal. However, further long-term clinical investigations are required to provide additional information about this new technique.
Objective: When an alveolar cleft is too large to close with adjacent mucobuccal flaps or large secondary fistula following a primary bilateral palatoplasty exists, a one-stage procedure for bone grafting becomes challenging. In such a case, we used the tongue flap to repair the fistula and cleft alveolus in the first stage, and bone grafting to the cleft defect was performed in the second stage several months later. The purpose of this paper is to report our experiences with the use of an anteriorly-based Y-shaped tongue flap to fit the palatal and labial alveolar defects and the ultimate result of the bone graft. Patients: A series of 14 patients underwent surgery of this type from January 1994 to December 1998.The average age of the patients was 15.8 years old (range: 5 to 28 years old). The mean period of follow-up following the 2nd stage bone raft operation was 45.9 months (range: 9 to 68 months). In nine of the 14 cases, the long-fork type of a Yshaped tongue flap was used for extended coverage of the labial side alveolar defects with the palatal fistula in the remaining cases the short-forked design was used. Results: All cases demonstrated a good clinical result after the initial repair of cleft alveolus and palatal fistula. There was no fistula recurrence, although Partial necrosis of distal margin in long-forked tongue flap was occurred in one case. Furthermore, the bone graft, which was performed an average of 8 months after the tongue flap repair, was always successful. Occasionally, the transferred tongue tissue was bulging and interfering with the hygienic care of nearby teeth; however, these problems were able to be solved with proper contour-pasty performed afterwards. No donor site complications such as sensory disturbance, change in taste, limitations in tongue movement, normal speech impairments or tongue disfigurement were encountered. Conclusion: This two-stage reconstruction of a bilateral cleft alveolus using a Y-shaped tongue flap and iliac bone graft was very successful. It may be indicated for a bilateral cleft alveolus patient where the direct closure of the cleft defect with adjacent tissue or the buccal flap is not easy due to scarred fibrotic mucosa and/or accompanied residual palatal fistula.
Purpose: Following tooth extraction caused by severe periodontitis, alveolar ridge dimension lose their original volume. To reduce the alveolar ridge dimension, the ridge preservation technique has been introduced and tested in many clinical studies with membrane alone or membrane plus graft, achieving reduced ridge loss compared to extraction only. The aim of the present clinical study was to compare the post-extraction dimensional changes in the membrane exposure group to non-exposure group during healing period following ridge preservation technique. Methods: Ridge preservation was performed in 44 extraction sites. After extraction, deproteinized bovine bone mineral coated with synthetic oligopeptide (Ossgen-$X15^{(R)}$) or deproteinized bovine bone mineral (Bio-$Oss^{(R)}$) was implanted into the socket. A collagen membrane (Bio-$Gide^{(R)}$) was trimmed to cover the socket completely and applied to the entrance of the socket. Four clinical parameters were compared between baseline and 6 months. Results: During healing period, membrane exposure was observed at 19 sites. At the re-entry, hard newly formed tissue were observed at the ridge preservation site. The grafted socket sites were well preserved in their volume dimension. In both groups, horizontal ridge width was reduced and vertical height was increased. There were not statistically significant differences in horizontal (-1.32 mm vs -1.00 mm) and vertical ridge change (2.24 mm vs 2.37 mm at buccal crest, 1.36 mm vs. 1.53 mm at lingual crest) between two groups. Conclusions: The ridge preservation approach after tooth extraction effectively prevented resorption of hard tissue ridge in spite of membrane exposure during healing period.
Objective: The purpose of this study was to investigate the stability of mini-implants in relation to loading time. Methods: A total of 48 mini-implants (ORLUS, Ortholution, Korea) were placed into the buccal alveolar bone of the mandible in 8 male beagle dogs. Orthodontic force (200-250gm) was applied immediately for the immediate loading group while force application was delayed for 3 weeks in the delayed loading group. For the subsequent loading periods (3, 6, 12 weeks), BIC (bone implant contact) and BV/TV (bone volume/total volume) and mobility test were carried out. Results: The immediate loading group showed no changes in BIC from 3 to 12 weeks, while the delayed loading group showed a significant increase in BIC between 3 and 12 weeks (p<0.05). The BV/TO of the delayed loading group significantly increased from 6 to 12 weeks of loading (p<0.05), while the BV/TV of the immediate loading group decreased from 3 to 12 weeks of loading. However, there was no significant difference in BV/TV between experimental groups. The mobility of the immediate loading group was not significantly different from that of the delayed loading group after 12 weeks of loading (p<0.05). Conclusions: These results showed that immediate loading does not have a negative effect on the stability of mini-implants compared to the early loading method in both the clinical and histomorphometric point of view.

Trauma commonly leads to ankylosis for the tooth whose periodontal ligaments have been injured. In growing patients, an ankylosed tooth can disrupt alveolar development, resulting in infraocclusion of the affected tooth. Consequently, this causes aesthetic problems during adolescence, interferes with prosthetic treatment due to the inclination of adjacent teeth, and complicates orthodontic dental movement. When the infraoccluded ankylosed tooth is extracted, a considerable amount of alveolar bone is lost, especially in the maxillary anterior region. Moreover, depression of the thin buccal alveolar bone compromises aesthetic restoration. In order to prevent alveolar bone loss, the ankylosed tooth should be treated by decoronation. In the present study, the traumatically injured maxillary incisors of 9-year-old and 10-year-old patients that present infraocclusion accompanied by ankylosis were treated with decoronation procedures. Decoronation procedures were performed when the affected teeth were 2-3 mm below relatively to the adjacent teeth. Moreover, the patients were treated before they get to 16 years old considering the maximum growth peak of the korean adolescents. As the results, there are favorable clinical results associated not only with preservation of horizontal alveolar volume but also with increase in vertical alveolar height after decoronation in growing individuals.
