Three-dimensional observations of the incisive foramen on cone-beam computed tomography image analysis

  • Purpose: The purpose of this study was to utilize cone-beam computed tomography (CBCT) image analysis to obtain anatomical information related to the morphology of the incisive foramen to provide useful data regarding implant placement and clinical procedures such as anesthesia. Methods: The study included 167 patients who underwent CBCT scans over 20 years. Three components were measured: 1) the anteroposterior and mediolateral diameter of the incisive foramen, 2) the horizontal bone thickness anterior to the incisive foramen, and 3) the vertical bone height coronal to the incisive foramen. All measurements were expressed as mean±standard deviation and were analyzed by a single examiner. Results: The anteroposterior diameter of the incisive foramen was wider than the mediolateral diameter (P<0.001). The diameter of the incisive foramen in patients in whom the central incisors were present was smaller than that in those in whom at least one central incisor was absent, but no statistically significant difference between the groups was observed. The horizontal bone thickness in the patients with central incisors was statistically significantly larger than that in the patients without at least one central incisor (P<0.001). The same pattern was observed with regard to vertical height, but that difference was not statistically significant. Conclusions: The buccal bone thickness anterior to the incisive foramen was significantly decreased after central incisor loss. It is necessary to identify the morphology of the bone and the location of the incisive foramen via CBCT to avoid invasion of the incisive foramen and nasopalatine canal.

  • The purpose of this investigation was to evaluate the effect of the porous hydroxyapatite particles (Interpore $200^{(R)}$) and guided tissue regeneration membrane ($Gore-Tex^{TM}$ augmentation material) on amount and shape of generating new bone adjacent to implant. Implants were placed immediately after extraction in the bilateral 3rd, 4th premolars of the mandible of the adult dogs. In all experimental groups, artificial bony defects were formed at the buccal cortex area, 3.3mm in width and 3.0mm in depth. In the control group : sutured without HA particles & membranes after placing implants, the experimental group 1 : membrane was place over the artificial bony defect, the experimental group 2 : bony defect was filled with HA particles and covered with membrane. The examination of bone-implant interfaces using light microscope and fluorescent microscope concluded as follows. 1. In all three experimental groups, osseointegration was observed without epithelial migration. 2. In the healing degree of bony defect area, the experimental group 1, 2 showed more prominent healing than control group, and the experimental group 1 showed the most excellent bone formation. 3. In fluorescent microscopic finding, bone remodeling was observed in regenerated bone tissue at defect area of experimental group 1, but in experimental group 2, irregular, discontinuous linear fluorescence was observed at the lower portion of defect area and sign of bone remodeling was weak.

Radiographic patterns of periosteal bone reactions associated with endodontic lesions

  • Objectives: The formation of new bone by periosteum due to an insult is called periosteal bone reaction (PBR). This study assessed the cone beam computed tomography (CBCT) patterns of periosteal bone reactions associated with periapical inflammatory lesion (apical periodontitis/periapical rarefying osteitis). Materials and Methods: Twenty-two small field of view CBCT images of patients with PBR were selected from a database of a private practice limited to endodontics. The volume of the periapical inflammatory lesion, the presence of cortical fenestration, the distance of the root apices to the affected cortex, and the location, pattern, and longest diameter of the periosteal reaction were recorded. Statistical analysis was performed using Wilcoxon Ranksum, Fischer's exact, Spearman Correlation Coefficient, and paired t-test. Results: In all cases, periosteal bone reaction manifested as either parallel (90.9%) or irregular (9.1%). No correlation was found between periapical inflammatory lesion volume and the periosteal reaction's longest diameter (p > 0.05). Cortical fenestration was noted in 72.7% of the cases. In addition, the findings showed that periosteal reactions were located mostly on the buccal and were present 53.8% and 100% of the time in the mandible and maxilla, respectively. Conclusions: The periosteal reactions of endodontic origin had a nonaggressive form (i.e., parallel or irregular), and none of the lesions resulted in a periosteal reaction with an ominous Codman's triangle or spicule pattern.

지대주 연결 형태와 골질에 따른 저작압이 임프란트 주위골내 응력분포에 미치는 영향 (Study on the stress distribution depending on the bone type and implant abutment connection by finite element analysis)

  • Oral implants must fulfill certain criteria arising from special demands of function, which include biocompatibility, adequate mechanical strength, optimum soft and hard tissue integration, and transmission of functional forces to bone within physiological limits. And one of the critical elements influencing the long-term uncompromise functioning of oral implants is load distribution at the implant- bone interface, Factors that affect the load transfer at the bone-implant interface include the type of loading, material properties of the implant and prosthesis, implant geometry, surface structure, quality and quantity of the surrounding bone, and nature of the bone-implant interface. To understand the biomechanical behavior of dental implants, validation of stress and strain measurements is required. The finite element analysis (FEA) has been applied to the dental implant field to predict stress distribution patterns in the implant-bone interface by comparison of various implant designs. This method offers the advantage of solving complex structural problems by dividing them into smaller and simpler interrelated sections by using mathematical techniques. The purpose of this study was to evaluate the stresses induced around the implants in bone using FEA, A 3D FEA computer software (SOLIDWORKS 2004, DASSO SYSTEM, France) was used for the analysis of clinical simulations. Two types (external and internal) of implants of 4.1 mm diameter, 12.0 mm length were buried in 4 types of bone modeled. Vertical and oblique forces of lOON were applied on the center of the abutment, and the values of von Mises equivalent stress at the implant-bone interface were computed. The results showed that von Mises stresses at the marginal. bone were higher under oblique load than under vertical load, and the stresses were higher at the lingual marginal bone than at the buccal marginal bone under oblique load. Under vertical and oblique load, the stress in type I, II, III bone was found to be the highest at the marginal bone and the lowest at the bone around apical portions of implant. Higher stresses occurred at the top of the crestal region and lower stresses occurred near the tip of the implant with greater thickness of the cortical shell while high stresses surrounded the fixture apex for type N. The stresses in the crestal region were higher in Model 2 than in Model 1, the stresses near the tip of the implant were higher in Model 1 than Model 2, and Model 2 showed more effective stress distribution than Model.

치관/고정체 비에 따른 상악 구치부 임플란트 주변골의 응력 분포에 대한 3차원 유한요소법적 분석 (Stress distribution in bone surrounding maxillary molar implants under different crown-to-fixture ratio: A 3D FEM analysis)

  • 목적: 상악 구치부에서 자연치와 임플란트 보철시, 보철치관/고정체 비율에 따른 응력분포 양상을 비교하고자 했다. 재료 및 방법: 자연치 모델의 경우는 획득한 3차원 인체모델을 상악 좌측 제2소구치 및 제 1대구치가 포함된 상악골을 Box 형태의 3차원 유한요소모델로 변환하였고, 임플란트 모델은 3차원 인체모델에서 치아 부분을 제거하고 동일 부위에 임플란트 모델을 연결하는 과정을 거쳐서 임플란트가 삽입된 유한요소모델을 구성하였다. 치관/고정체 비율을 0.7:1, 1:1, 1.25:1이 되도록 골수준 (bone level)을 조정하였으며 각 모델의 치관 부위에 300 N의 수직 하중과 수평하중을 각각 가했다. 결과: 1. 모든 하중 조건하에서 자연치와 임플란트 모두에서 피질골과 인접하는 경부에 응력이 집중되는 양상을 보였다. 2. 치관/치근 (고정체) 비가 증가함에 따라 자연치와 임플란트 모두에서 교합면에 수직적 하중을 가한 경우에는 응력의 변화가 뚜렷하지 않았으나, 수평적 하중을 가한 경우에서는 응력이 증가하는 양상을 보였다. 3. 자연치의 경우에 치관/치근비가 증가함에 따라 splinting이 응력감소 효과를 보였고, 임플란트의 경우에는, 치관/고정체 비가 증가함에 따라 splinting이 수직 하중조건에 응력감소효과를 보였으나, 중심에서 벗어난 하중조건에는 최대응력이 오히려 증가하는 양상을 보였다. 4. 임플란트의 경우, 치관/고정체 비가 증가함에 따라 splinting이 수평하중조건 4에서 뚜렷한 응력감소 효과를 보이나, 수평 하중조건 5에서는 응력감소 효과가 감소되고, 특히 치관/고정체 비가 1.25:1인 경우에서는 오히려 응력의 증가를 보였다. 결론: 임플란트 보철물은 치관/고정체 비가 커질수록 더 큰 응력을 받게 되고, splinting의 효과도 감소하게 된다. 또한 교합하중이 임플란트의 장축을 벗어나거나 중심에서 벗어난 경우 응력이 커지는 것으로 사료된다.

Primary sinonasal mucosal melanoma simulated as cystic lesions: a case report

  • Sinonasal mucosal melanoma (SNMM) in the maxillary sinus is a rare disease condition. Compared to oral mucosal melanoma, SNMM has a bulky, exophytic, and polypoid appearance, is weakly pigmented, and associated with unspecific symptoms. Due to these features, SNMM in the maxillary sinus has been misdiagnosed as nasal polyps and chronic sinusitis. In this case report, we described SNMM occurring in the right maxillary sinus simulated as a cystic or benign lesion. Cortical bone thinning and expansion were observed around the mass. The excised soft mass was encapsulated and weakly pigmented. The mass was clearly excised and covered with a pedicled buccal fat pad graft. Diagnosis using immunohistochemistry with S-100 and homatropine methylbromide-45 (HMB-45) is critical for proper treatment.

The preliminary study for three-dimensional alveolar bone morphologic characteristics for alveolar bone restoration

  • Background: The concept of the ideal morphology for the alveolar bone form is an important element to reconstruct or restore the in maximizing esthetic profile and functional alveolar bone restoration. The purpose of this preliminary study is to evaluate the normal alveolar bone structure to provide the standard reference and guide template for use in diagnosing for implant placement, determining the correct amount of bone augmentation in actual clinical practice and producing prostheses based on three-dimensional imaging assessment of alveolar bone. Methods: This study was included 11 men and 11 women (average age, 22.6 and 24.5 years, respectively) selected from among 127 patients. The horizontal widths of alveolar bone of maxilla and mandible were measured at the crestal, mid-root, and root apex level on MDCT (multi-detector computed tomography) images reconstructed by medical imaging software. In addition, tooth dimensions of the central incisors, canines, second premolars, and first molars of maxilla and mandible, including the horizontal width of the interdental alveolar bone crest, were also measured and statistically analyzed. Results: The horizontal alveolar bone width of the palatal side of maxilla showed a distinct increment from the alveolar bone crest to the apical region in both anterior and posterior areas. The average widths of the maxillary alveolar ridge were as follows: central incisor, 7.43 mm; canine, 8.91 mm; second premolar, 9.57 mm; and first molar, 12.38 mm. The average widths of the mandibular alveolar ridge were as follows: central incisor, 6.21 mm; canine, 8.55 mm; second premolar, 8.45 mm; and first molar, 10.02 mm. In the buccal side, the alveolar bone width was not increased from the crest to the apical region. The horizontal alveolar bone width of an apical and mandibular border region was thinner than at the mid-root level. Conclusions: The results of the preliminary study are useful as a clinical guideline when determining dental implant diameter and position. And also, these measurements can also be useful during the production of prefabricated membranes and customized alveolar bone scaffolds.

미니스크류 식립 시 상악동의 골두께에 대한 고려 (Consideration of maxillary sinus bone thickness when installing miniscrews)

  • 본 연구는 전산화 단층촬영 영상으로 상악동의 형태를 파악하여 교정용 미니스크류를 안전하게 식립할 수 있는 위치를 찾고자 시행되었다. 남자 20명, 여자 20명의 3차원 전산화 단층촬영 사진을 촬영한 후 3차원 영상프로그램을 이용하여 상악동의 형태를 계측하여 통계 분석하였으며 다음과 같은 결과를 얻었다. 상악동저에서 백악법랑경계까지의 골두께는 제1대구치와 제2대구치 사이에서 유의하게 얇았고, 제1소구치와 제2소구치 사이에서 유의하게 두꺼웠다. 상악동저에서 협측 상악골 외연까지의 두께는 제1대구치와 제2대구치에서 유의하게 두꺼웠으며, 제1소구치와 제2소구치 사이에서 유의하게 얇았다. 이상의 결과를 바탕으로 안전하게 미니스크류를 식립하여 교정치료의 영역을 넓힐 수 있을 것이다.

캔틸레버의 길이와 하중이 하악 임플랜트지지 고정성 보철물의 응력 분산에 미치는 영향 (EFFECT OF CANTILEVER LENGTH AND LOAD ON STRESS DISTRIBUTION OF FIXED IMPLANT-SUPPORTED PROSTHESES)

  • The purpose of this study was to evaluate the effect of cantilever length, load, and implant number on the stress distribution of implant supported fixed prosthesis. In the replica of an edentulous human mandible, four or five implants were placed and spaced evenly between the mental foramina and symmetrical gold alloy cast superstructures with cantilever were fabricated. Strain gauges were placed in buccal and lingual side of implants. 9, 15, 21kg of loads at varying cantilever lengths were applied to the occlusal surface of fixed prostheses. The strains were recorded from each gauge and principal stresses were calculated The results were as follows : 1. Increasing the length of the cantilever increased the stresses on the bone supporting implants. and the ratio of increase became high as increasing the load. 2. In the model with four implants, the highest compressive stress was measured on lingual side of the first implants nearest loading point and the highest tensile stress was measured on buccal side of the second implants. 3. In the model with five implants, the highest compressive stress was measured on lingual side of the first implants nearest loading point. And the highest tensile stress was measured on buccal side of the second implants, and lingual side of the third implants. 4. There was no significant change of the magnitude of stress on the most distal imp]ant of non cantilevered side as increasing the cantilever length or load. 5. In general, the superstructure supported by five implants reduced the stress and was less affected by cantilever length compared to the support provided by four implants.

탈회 동결 건조골과 탈단백 우골의 백서 치조골 천공결손에 대한 치유 효과 (Healing Effects of Demineralized Freeze - Dried Bone Allograft and Deproteinized Bovine Bone Mineral on Periodontal Fenestration Defect in Rats)

  • The present study was performed to compare effects of demineralized freeze-dried bone allograft(DFDBA) with deproteinized bovine bone mineral(DBBM) on periodontal fenestration defect in rats. Twelve adult male rats weighing 500 to 540 grams were used in this study. Periodontal fenestration defects were surgically created with tapered fissure bur(${\Phi}1mm$) at the left side of buccal surface of the mandible. The defect size was from anterior border of the first molar to anterior of the ascending ramus mesiodistally and from just below the alveolar crest to apically 1.5-2mm area apicocoronally with 2mm in depth. Rats were divided into control group, test group I and II. Four defects were assigned to the test group I grafted with DBBM and other 4 defects were assigned to the test group II grafted with DFDBA. The rest of defects were the negative control group. At 10 days and 35 days after surgery, 12 rats were sacrificed through intracardiac perfusion and specimens were obtained prepared with Hematoxylin-Eosin stain for light microscopic evaluation. The results of this study were as follows : 1. In the control group, new bone, osteoid, dense connective tissue were observed in the defects at 10 days. new bone formation was not found but loose connective tissue was formed in the defect and fibrous encapsulation of graft materials was shown in two test groups at 10 days. 2. In all groups, new bone formation was shown in the defect at 35 days. And in the control group, bone formation increased at 35 days than at 10 days. 3. In the test group I and II at 35 days, graft materials were combined with new bone and joined host bone. There was very close contact between new bone, graft materials, and host bone with no gaps. 4. In the test group I and II, new bone formation was similar to that in the control group but not exeeded. In conclusion, in the test group I new bone formation was similar to that in the test group II at 35 days, but there was infiltration of inflammatory cells at 10 days. DFDBA and DBBM were considered as the biocompatible graft materials and effective in the regeneration of new bone.
