부인암을 가진 여성에서 유방암의 선별검사로서의 디지털 유방단층 촬영술 (Digital Breast Tomosynthesis as a Breast Cancer Screening Tool for Women with Gynecologic Cancer)

  • 목적 본 연구는 부인암을 가진 여성에서 유방암의 선별검사로서의 디지털 유방단층 촬영술을 평가하였다. 대상과 방법 부인암을 가진 환자들 중 검진 목적으로 디지털 유방단층 촬영술을 촬영한 환자들을 대상으로 후향적 연구를 시행하였으며 유방암 발견율, 소환율, 민감도, 특이도, 양성예측도를 계산하였다. 양성예측도 1은 모든 양성 선별검사 중 1년 이내에 조직 검사에서 유방암을 진단받은 환자의 백분율로 정의되었다. 양성예측도 2는 진단 검사에서 조직검사의 필요 판정을 받은 후(그리고 선별 검사에서 Breast Imaging Reporting and Data System 카테고리 4, 5를 받은 후) 1년 이내에 조직검사에서 유방암을 진단받은 환자의 백분율로 정의되었다. 양성예측도 3은 실제로 조직검사를 시행 받은 환자 중 1년 이내에 조직검사에서 유방암을 진단받은 환자의 백분율로 정의되었다. 검진으로 발견된 암의 각 경우에 대해 환자의 나이, 부인암의 종류, 유방 밀도, 영상의 특징, 최종 Breast Imaging Reporting and Data System 평가, 조직학적 유형, T 및 N 병기, 분자아형 및 Ki-67 지수를 분석했다. 결과 전체 508명 중 7개의 유방암이 발견되었으며 유방암 발견율은 1000건 당 암 13.8이었다. 민감도는 100%, 특이도는 83.2%였으며 위음성률은 1000건 당 0이었다. 양성예측도 1, 양성예측도 2, 양성예측도 3은 각각 7.7, 31.8, 31.8이었으며 소환율은 17.9%였다. 결론 본 연구에서 디지털 유방단층 촬영술은 높은 유방암 발견율, 높은 민감도, 높은 양성예측도를 보이며 T, N 병기가 낮은 초기 암에 대해 높은 발견율을 보였다. 따라서 부인암 환자와 같은 고위험군에서 유방암의 선별검사로서 디지털 유방단층 촬영술이 유용할 수 있다.

항암치료 단계 유방암 환자의 대처방식, 사회적지지 및 외상 후 스트레스 장애와의 관계 (The Relationship among the Coping Style, Social Support, and Post-Traumatic Stress Disorder in Breast Cancer Patients Treated with Chemotherapy)

  • 목적: 본 연구는 항암치료 단계 유방암 환자의 PTSD, 대처방식, 사회적 지지의 관계를 파악하여 암환자의 PTSD에 관한 효과적인 간호중재의 기초자료를 제시하기 위함이다. 방법: J시 소재 G지역암센터 외과 외래를 방문한 만 30세 이상이며, 항암치료단계인 유방암 환자134명을 대상으로 조사하였다. 수집된 자료는 SPSS WIN 18.0프로그램을 이용하여 서술통계, ANOVA, Pearson correlation coefficients 등을 실시하였다. 결과: 항암치료 단계 유방암 환자의 외상 후 스트레스 장애는 26.9%가 PTSD 고위험군, 17.9%가 PTSD 위험군에 속하는 것으로 나타났다. 대상자의 PTSD 그룹(고위험군, 위험군, 정상)의 대처방식(적극적 대처, 소극적 대처), 사회적 지지의 상관관계는 PTSD 고위험군인 경우 적극적 대처와 소극적 대처가 유의미한 정적 상관관계를 가지는 것으로 나타났으며, PTSD 고위험군에서 사회적 지지와 적극적 대처는 유의미한 정적 상관관계를 가지고 있는 것으로 나타났다. 결론: 항암치료 단계 유방암 환자 중 26.9%가 PTSD 증상을 보이는 것으로 나타났으며, PTSD 고위험군에서 사회적 지지가 증가할수록 적극적 대처가 증가하는 것으로 나타났다. 암은 평생 자기관리가 필요한 질병임을 고려할 때 높은 사회적 지지는 자신의 건강관리 능력을 증진시킬 수 있는 계기가 될 수 있을 것이며, 더 나아가 PTSD 고위험군에게 있어 높은 사회적 지지는 환자의 스트레스를 줄이고 삶의 질을 높이는데 효과적인 역할을 할 것이라 여겨진다.

Daily localization of partial breast irradiation patients with three-dimensional ultrasound imaging

  • Purpose: Accurate localization of the lumpectomy cavity during accelerated partial breast radiation (APBR) is essential for daily setup to ensure the prescribed dose encompasses the target and avoids unnecessary irradiation to surrounding normal tissues. Three-dimensional ultrasound (3D-US) allows direct visualization of the lumpectomy cavity without additional radiation exposure. The purpose of this study was to evaluate the feasibility of 3D-US in daily target localization for APBR. Materials and methods: Forty-seven patients with stage I breast cancer who underwent breast conserving surgery were treated with a 2-week course of APBR. Patients with visible lumpectomy cavities on high quality 3D-US images were included in this analysis. Prior to each treatment, X-ray and 3D-US images were acquired and compared to images from simulation to confirm accurate position and determine shifts. Volume change of the lumpectomy cavity was determined daily with 3D-US. Results: A total of 118 images of each modality from 12 eligible patients were analyzed. The average change in cavity volume was 7.8% (range, -24.1% to 14.4%) on 3D-US from simulation to the end-of-treatment. Based on 3D-US, significantly larger shifts were necessary compared to portal films in all three dimensions: anterior/posterior (p = 7E-11), left/right (p = 0.002), and superior/inferior (p = 0.004). Conclusion: Given that the lumpectomy cavity is not directly visible via X-ray images, accurate positioning may not be fully achieved by X-ray images. Therefore, when the lumpectomy cavity is visible on US, 3D-US can be considered as an alternative to X-ray imaging during daily positioning for selected patients treated with APBR, thus avoiding additional exposure to ionizing radiation.

Prognostic factors for survivals from first relapse in breast cancer patients: analysis of deceased patients

  • Purpose: This study was performed to evaluate prognostic factors for survival from first relapse (SFFR) in stage I-III breast cancer patients. Materials and Methods: From June 1994 to June 2008, 3,835 patients were treated with surgery plus postoperative radiotherapy and adjuvant chemotherapy for stage I-III breast cancer at Samsung Medical Center. Among them, a total of 224 patients died by June 2009, and 175 deaths were of breast cancer. Retrospective review was performed on medical records of 165 patients who met the inclusion criteria of this study. Univariate and multivariate analysis were done on survivals according to variables, such as age, stage, hormone status of tumor, disease-free interval (DFI), sites of first failure, number of organs involved by recurrent disease (NOR), application of salvage treatments, and existence of brain or liver metastasis (visceral metastasis). Results: Patients' median overall survival time was 38 months (range, 8 to 123 months). Median SFFR was 17 months (range, 5 to 87 months). Ninety percent of deaths occurred within 40 months after first recurrence. The patients with SFFR ${\leq}1$ year had tendency of triple-negativity, shorter DFI (${\leq}2$ years), larger NOR (>3), visceral metastasis for first relapse than the patients with SFFR >1 year. In multivariate analysis, longer DFI (>2 vs. ${\leq}2$ years), absence of visceral metastasis, and application of salvage treatments were statistically significant prognosticators for longer SFFR. Conclusion: The DFI, application of salvage treatments, and visceral metastasis were significant prognostic factors for SFFR in breast cancer patients.

The Prognostic Impact of Synchronous Ipsilateral Multiple Breast Cancer: Survival Outcomes according to the Eighth American Joint Committee on Cancer Staging and Molecular Subtype

  • Background: In the current American Joint Committee on Cancer staging system of breast cancer, only tumor size determines T-category regardless of whether the tumor is single or multiple. This study evaluated if tumor multiplicity has prognostic value and can be used to subclassify breast cancer. Methods: We included 5,758 patients with invasive breast cancer who underwent surgery at Samsung Medical Center, Seoul, Korea, from 1995 to 2012. Results: Patients were divided into two groups according to multiplicity (single, n=4,744; multiple, n=1,014). Statistically significant differences in lymph node involvement and lymphatic invasion were found between the two groups (p<.001). Patients with multiple masses tended to have luminal A molecular subtype (p<.001). On Kaplan-Meier survival analysis, patients with multiple masses had significantly poorer disease-free survival (DFS) (p=.016). The prognostic significance of multiplicity was seen in patients with anatomic staging group I and prognostic staging group IA (p=.019 and p=.032, respectively). When targeting patients with T1-2 N0 M0, hormone receptor-positive, and human epidermal growth factor receptor 2 (HER2)-negative cancer, Kaplan-Meier survival analysis also revealed significantly reduced DFS with multiple cancer (p=.031). The multivariate analysis indicated that multiplicity was independently correlated with worse DFS (hazard ratio, 1.23; 95% confidence interval, 1.03 to 1.47; p=.025). The results of this study indicate that tumor multiplicity is frequently found in luminal A subtype, is associated with frequent lymph node metastasis, and is correlated with worse DFS. Conclusions: Tumor multiplicity has prognostic value and could be used to subclassify invasive breast cancer at early stages. Adjuvant chemotherapy would be necessary for multiple masses of T1-2 N0 M0, hormone-receptor-positive, and HER2-negative cancer.

유방암 환자에서 액와부 림프절 전이를 시사하는 자기공명영상 소견 (MRI Findings Suggestive of Metastatic Axillary Lymph Nodes in Patients with Invasive Breast Cancer)

  • 목적 유방암 환자의 수술 전 자기공명영상에서 림프절 전이를 시사하는 소견들에 따른 진단 성적을 알아보고자 한다. 대상과 방법 수술 전 유방 자기공명영상을 촬영하고 유방암 수술을 시행한 192명의 환자를 후향적으로 분석하였다. 영상 소견에서 림프절의 크기와 장경/단경의 비율, 피질의 두께와 모양, 변연, 수질의 소실, 비대칭성, T2 강조영상에서의 신호강도, 이른 조영증강의 정도, 조영증강의 역학을 조사하였다. 수신자판단특성곡선 분석, 카이 분석과 t-검정, 맥니마 검정을 이용하여 통계분석을 시행하였다. 결과 단경의 증가, 피질의 불규칙한 모양과 피질 두께의 증가, 수질의 소실, 비대칭성, 피질의 불규칙한 변연 그리고 T2 강조영상에서의 낮은 신호강도는 전이를 시사하는 의미 있는 소견이었다. 이중 단경과 피질의 두께에 대해 수신자판단특성곡선 분석으로 각각 8.05 mm와 2.75 mm로 절단값을 얻었다. 2.75 mm 이상의 피질 두께, 피질의 불규칙한 모양은 맥니마 검정으로 다른 소견들과 비교할 때 민감도의 유의한 차이를 보였다. 피질의 불규칙한 변연(100%)은 가장 높은 특이도를 보였다. 결론 유방 자기공명영상의 전이 림프절 분석에서 2.75 mm 이상의 피질 두께와 피질의 불규칙한 모양은 다른 소견들보다 높은 민감도를 보이고 피질의 불규칙한 변연은 가장 높은 특이도를 보이는 소견이다.

Safety of long-term subcutaneous free flap skin banking after skin-sparing mastectomy

  • Background A persistent problem in autologous breast reconstruction in skin-sparing mastectomies is skin restoration after skin necrosis or secondary oncological resection. As a solution to facilitate reconstruction, skin banking of free-flap skin has been proposed in cases where the overlying skin envelope must be resected, as this technique spares the patient an additional donor site. Herein, we present the largest series to date in which this method was used. We investigated its safety and the possibility of skin banking for prolonged periods of time. Methods All skin-sparing mastectomies and immediate autologous breast reconstructions from December 2009 until June 2013 at our institution were analysed. Results We identified 31 patients who underwent 33 free flap reconstructions in which skin banking was performed. Our median skin banking period was 7 days, with a maximum duration of 171 days. In 22.5% of cases, the banked skin was used to reconstruct overlying skin defects, and in 9.6% of cases to reconstruct the nipple-areolar complex. Microbiological and histological investigations of the banked skin revealed neither clinical infections nor malignancies. Conclusions In situ skin banking, even for prolonged periods of time, is a safe and cost-effective method to ensure that skin defects due to necrosis or secondary oncological resection can be easily reconstructed.

Fitting Cure Rate Model to Breast Cancer Data of Cancer Research Center

  • Background: The Cox PH model is one of the most significant statistical models in studying survival of patients. But, in the case of patients with long-term survival, it may not be the most appropriate. In such cases, a cure rate model seems more suitable. The purpose of this study was to determine clinical factors associated with cure rate of patients with breast cancer. Materials and Methods: In order to find factors affecting cure rate (response), a non-mixed cure rate model with negative binomial distribution for latent variable was used. Variables selected were recurrence cancer, status for HER2, estrogen receptor (ER) and progesterone receptor (PR), size of tumor, grade of cancer, stage of cancer, type of surgery, age at the diagnosis time and number of removed positive lymph nodes. All analyses were performed using PROC MCMC processes in the SAS 9.2 program. Results: The mean (SD) age of patients was equal to 48.9 (11.1) months. For these patients, 1, 5 and 10-year survival rates were 95, 79 and 50 percent respectively. All of the mentioned variables were effective in cure fraction. Kaplan-Meier curve showed cure model's use competence. Conclusions: Unlike other variables, existence of ER and PR positivity will increase probability of cure in patients. In the present study, Weibull distribution was used for the purpose of analysing survival times. Model fitness with other distributions such as log-N and log-logistic and other distributions for latent variable is recommended.

Perception, Attitudes, Preparedness and Experience of Chemotherapy-Induced Alopecia among Breast Cancer Patients: a Qualitative Study

  • Objectives: Regardless of its negative impact on quality of life, little is known about the importance of alopecia from the patients' perspective. This study aimed to explore the whole experience of chemotherapy-induced alopecia among Korean breast cancer patients including perception, attitudes, preparedness, and changes after alopecia. Methods: Patients expected to experience or had experienced alopecia were recruited at a tertiary hospital in Seoul, Korea. Semi-structured in-depth interviews were performed in 21 patients. Recurrent issues were identified and placed into thematic categories. Results: All patients think that appearance is important and they pay attention to how they look like. They had negative perceptions about alopecia. Patients were not well prepared for alopecia, and experienced substantial physical, psychological and social distress. Lack of information and limited social support combined with negative images of cancer made it difficult for patients to overcome the trauma and deterred them from usual daily activities resulting in poor quality of life. Conclusions: Patients were not well prepared for alopecia and negative perceptions, lack of preparedness, and limited social support and resources increased alopecia-related distress. Educational programs for preparing patients to cope with alopecia distress and advocate activities to change people's negative perception about alopecia are needed to reduce the burden imposed by alopecia in cancer patients.

Circulating Tumor DNA in a Breast Cancer Patient's Plasma Represents Driver Alterations in the Tumor Tissue

  • Tumor tissues from biopsies or surgery are major sources for the next generation sequencing (NGS) study, but these procedures are invasive and have limitation to overcome intratumor heterogeneity. Recent studies have shown that driver alterations in tumor tissues can be detected by liquid biopsy which is a less invasive technique capable of both capturing the tumor heterogeneity and overcoming the difficulty in tissue sampling. However, it is still unclear whether the driver alterations in liquid biopsy can be detected by targeted NGS and how those related to the tissue biopsy. In this study, we performed whole-exome sequencing for a breast cancer tissue and identified PTEN p.H259fs*7 frameshift mutation. In the plasma DNA (liquid biopsy) analysis by targeted NGS, the same variant initially identified in the tumor tissue was also detected with low variant allele frequency. This mutation was subsequently validated by digital polymerase chain reaction in liquid biopsy. Our result confirm that driver alterations identified in the tumor tissue were detected in liquid biopsy by targeted NGS as well, and suggest that a higher depth of sequencing coverage is needed for detection of genomic alterations in a liquid biopsy.
