The Diagnosis and Ancillary Tests of Brain Death (뇌사의 진단과 진단을 위한 보조적 검사)

  • Brain death is defined as the irreversible cessation of all brain function. The diagnosis of brain death is primarily based on a careful neurological examination demonstrating unresponsiveness, absent of brain stem reflexes, and no respiratory activity. Several conditions which may mimic brain death must be excluded. In some cases investigations such as electroencephalography, angiography, transcranial doppler or evoked potentials can contribute to the diagnosis. The brief review will introduce medical technologist and clinician to the key issues in the definition and management of brain death.

  • Prospective memory (PM) is related to remember to carry out a previously intented behaviour. The purpose of this study was to develop a paradigm for measuring PM function to diagnosis in mild cognitive impairment 1 or brain injury in patients 2. among brain injured patients Thirty-eight normal healthy subjects participated in current study. The paradigm was composed of four conditions: a baseline and three intention conditions (expectation, execution 1 and 2). In the expectation condition, subjects were asked to make a new response to intented stimuli during ongoing task, but the intented stimuli never occurred. In the execution 1 (one type of expected stimulus) and 2 (two types of expected stimuli), the intended stimuli did occur in 20% of trials. The reaction time and error rate were calculated in each condition. Repeated measures using ANOVA of subject's mean reaction times (RTs) and mean error rates (ERs) showed main effects of conditions during ongoing task. The comparison of PM tasks in executive condition 1 and 2 also showed significance in RTs and ERs. This paradigm reflects sufficiently the performance of prospective memory function during ongoing task in normal individuals. Thus, we suggest that the paradigm will be helpful to study neural network of PM function using brain imaging techniques and diagnosis of PM dysfunction.

  • It is very difficult to evaluate sleep disorders by simple history taking, because which covers very comprehensive areas such as psychobiosocial fields. Although polysomnography is used for the method of final diagnosis, systemic history taking and sleep question-aires are still critically important especially in evaluation of insomnia. Proper informations through sleep questionnaires can provide very precise data for effective treatment as well as exact diagnosis. Sleep questionnaires consist of largely four kinds of questionnaires, which are screening questionnaire of sleep disorders, sleep diary and questionnaire of sleep hygine, diagnostic questionnaire for specific sleep disorder and questionnaire of special symptoms of sleep disorders including insomnia, daytime sleepiness, cognitive function, mental symptom and personality, parasomnia, physical illness and sexual function. However, for more conclusive diagnosis especially in excessive daytime sleepiness nocturnal polysomnography and multiple sleep latency test should be performed.

  • In modem city life, the citizen get insufficient exercise and has high levels of stress. Increased Stress causes such minor things as tiredness, disease and mental fatigue, and increase brain blood pressure too. In this paper trying to design the multi-functional blood pressure monitor with airo-dynamic brain hemokinesis improvement function. So this system have developed for the improvement of flows in the capillary blood vessel of head and limbs. Also This system is able to measure brain blood pressure(BBP) which need diagnosis circulation state of brain blood. So, we extracted correlativity of blood pressure(BP) and BBP through clinical experiment. We confirmed that compare factor of BP with BBP is more useful factors for diagnosis state of brain hemokinesis.

  • Evaluation of dementia in patients with early symptoms of cognitive decline is clinically challenging, but the need for early, accurate diagnosis has become more crucial, since several medication for the treatment of mild to moderate Alzheimer' disease are available. Many neurodegenerative diseases produce significant brain function alteration even when structural imaging (CT or MRI) reveal no specific abnormalities. The role of PET and SPECT brain imaging in the initial assessment and differential diagnosis of dementia is beginning to evolve vapidly and growing evidence indicates that appropriate incorporation of PET into the clinical work up can improve diagnostic and prognostic accuracy with respect to Alzheimer's disease, the most common cause of dementia in the geriatric population. in the fast few years, studios comparing neuropathologic examination with PET have established reliable and consistent accuracy for diagnostic evaluations using PET - accuracies substantially exceeding those of comparable studies of diagnostic value of SPECT or of both modalities assessed side by side, or of clinical evaluations done without nuclear imaging. This review deals the role of functional brain imaging techniques in the evaluation of dementias and the role of nuclear neuroimaging in the early detection and diagnosis of Alzheimer's disease.

Comparisons of Discriminant Analysis Model and Generalized Logit Model in Stroke Patten Identifications Classification (중풍변증분류에 사용되는 판별분석모형과 일반화로짓모형의 비교)

  • In this study, when a physician make a diagnosis of the Pattern Identifications(PIs) of stroke patients, the development methods of the PIs classification function is considered by diagnostic questionnaire of the PIs for stroke patients. Clinical data collected from 1,502 stroke patients who was identically diagnosed for the PIs subtypes diagnosed by two clinical experts with more than 3 years experiences in 13 oriental medical hospitals. In order to develop the classification function into PIs using the 44 items-Fire&heat(19), Qi-deficiency(11), Yin-deficiency(7), Dampness phlegm(7)- of them was significant statistically by univariate analysis in 61 questionnaires totally, we make some comparisons of the results of discriminant analysis model and generalized logit model. The overall diagnostic accuracy rate of the PIs subtypes for discriminant model(74.37%) was higher than 3% of generalized logit model(70.09%).

Performance Evaluation of YOLOv5s for Brain Hemorrhage Detection Using Computed Tomography Images (전산화단층영상 기반 뇌출혈 검출을 위한 YOLOv5s 성능 평가)

  • Brain computed tomography (CT) is useful for brain lesion diagnosis, such as brain hemorrhage, due to non-invasive methodology, 3-dimensional image provision, low radiation dose. However, there has been numerous misdiagnosis owing to a lack of radiologist and heavy workload. Recently, object detection technologies based on artificial intelligence have been developed in order to overcome the limitations of traditional diagnosis. In this study, the applicability of a deep learning-based YOLOv5s model was evaluated for brain hemorrhage detection using brain CT images. Also, the effect of hyperparameters in the trained YOLOv5s model was analyzed. The YOLOv5s model consisted of backbone, neck and output modules. The trained model was able to detect a region of brain hemorrhage and provide the information of the region. The YOLOv5s model was trained with various activation functions, optimizer functions, loss functions and epochs, and the performance of the trained model was evaluated in terms of brain hemorrhage detection accuracy and training time. The results showed that the trained YOLOv5s model is able to provide a bounding box for a region of brain hemorrhage and the accuracy of the corresponding box. The performance of the YOLOv5s model was improved by using the mish activation function, the stochastic gradient descent (SGD) optimizer function and the completed intersection over union (CIoU) loss function. Also, the accuracy and training time of the YOLOv5s model increased with the number of epochs. Therefore, the YOLOv5s model is suitable for brain hemorrhage detection using brain CT images, and the performance of the model can be maximized by using appropriate hyperparameters.

  • Clinical neuropsychological tests were developed originally for the diagnosis of neurological and neuro-surgical diseases. Recently, these tests are being introduced to psychiatric patients. Authors had the experience to use these tests in pychiatric outpatient clinic. Results were as follows. There was a significantly increase in language and attentional function in residual schizophrenia compared to normal control. In chronic neurosis, as visuospatial function was reduced, language and attentional functions were enhanced. With these results, authors suggest that application of neuropsychological tests in psychiatric patients may be very helpful in classifying the subgroups of disease, in selecting the modality of treatment, and in expecting prognosis.

  • Bioelectrical measurement has long been an important area in medical researches and practices, and contributed valueable data to the field of human engineering. In particular, the measurement of EEG has been widely used for the study of brain function as well as for the diagnosis of various brain disorders. The present study tried to improve conventional measurements of EEG in that FFT algorithm with microcomputer machine language was applied to facilitate the computation of various aspects of the EEG.

Study on the Ischemic penumbra concept in stroke patient by case study (허혈경계부의 감소를 보인 뇌졸중 환자에 대한 증례보고)

  • I have treated one ischemic stroke patient in acute stage with Seonghyangjeonggi-san, and observed remarkable reduction of the size ischemic portion in brain CT, notable improved motor power of patient. So I report this case of stroke patint. The ischemic penumbra, simply stated, is the part of the brain that is sandwiched brain regions committed to die and those that receive enough blood to communicate. Therefore, it is ischemic brain tissue that has just enough to communicte and function. The life expectancy of the penumbrais short. Although the penumbra is an elegant concept, in practice, it has been a difficult one to exploit. Up to now, a lot of research worker have tried to develop the method to make a accurate diagnosis. and then we know that PET and Xenon CT is available for the diagnosis for the ischemic penumbra. But those are not perfect to diagnose of penumbra. The case in my case report was confirmed as ischemic penumbra with CT. I know that CT is not prefect to diagnose penumbra, but I just want to raise the interest in penumbra of oriental medicine researcher and my report will be benificial to the penumbra researcher.
