Purpose: The Dyke-Davidoff-Masson syndrome is a rare disease entity that was first reported in 1993, and it is characterized by not only the cerebral hemiatrophy that is accompanied by the ipsilateral ventriculomegaly and ipsilateral compensatory osseous hypertrophy, but also the overgrowth of the paranasal sinuses. No studies have attempted to examine it from perspectives of the skull deformity and plastic surgery. Here, we report our case with a review of the literatures. Methods: A 45-year-old man with Dyke-Davidoff-Masson visited our medical institution with nasal bone fracture. Based on the previously taken brain MRI scans, we measured the degree of craniofacial deformity, and the horizontal distance, which is based on the margin of the skull, as well as the falx cerebri. Results: We made a comparison of the degree of craniofacial deformity. This showed that the mean horizontal distance on the axial view was shorter by approximately 28.46%, as compared with that of the left unaffected side. Conclusion: The Dyke-Davidoff-Masson is characterized by a concurrent presence of the atrophy of the cerebral hemisphere, with the cranial deformity. For the reconstruction of the bone and soft-tissue deformity with Dyke-Davidoff-Masson syndrome, it is needed to perform objective assessments.
소뇌 운동실조는 점차 진행되는 신경퇴행질병이며 운동 조절을 위한 기능의 상실을 동반하기에 환자의 삶을 심각하게 저하시킨다. 소뇌 운동실조 환자는 운동제어 과정에서 부적절한 폐회로 소뇌 반응으로 인해 운동 명령이 제한된다. 본 논문에서는 최근 뇌-컴퓨터 인터페이스 기술을 이용하여 소뇌의 이상으로 인한 운동실조 환자들이 외부기기를 제어할 수 있도록 운동상상 기반의 뇌파의 특성을 분석하고 이를 이용한 뇌-컴퓨터 인터페이스 기법을 제안한다. 뇌파 기반의 뇌-컴퓨터 인터페이스의 효용성을 검증하기 위하여 소뇌 운동실조 환자와 정상인 그룹에서 운동상상에 따른 뮤밴드 파워를 조절하는 능력을 비교하였다. 이를 통하여 소뇌 운동실조 환자에의 뇌-컴퓨터 인터페이스의 가능성을 보여준다.
심정지(Cardiac arrest)는 원인과 관계없이 심장의 박동이 정지되어 발생하는 일련의 상태를 말한다. 심장질환에 의한 심정지는 매년 우리나라 3대 사망원인에 포함되며 예측이 불가하다. 심정지 발생시 환자의 생명을 구하기 위한 유일한방법 중 하나는 심폐소생술이며 이 술기를 통하여 순환을 유지시킬 수 있다. 시각으로 받아들여진 정보는 시신경을 통하여 뇌로 전달되며, 그중 거울은 자신의 움직임과 자신의 형태를 볼 수 있게 하고, 교정과 분석이 가능하여 다양하게 활용 할 수 있다. 이에 거울을 활용한 시각적 정보에 따른 가슴압박의 질을 비교하였다. 연구결과 거울 사용에 따라 가슴압박의 평균 깊이가 유의한 차이가 있었고($48.93{\pm}6.76$, $53.86{\pm}4.56$, <0.001), 압박 대 이완 비율에도 차이를 보였다($0.87{\pm}0.13$, $0.96{\pm}0.10$, <0.002). 또한 자세를 의식하는 정도에서도 유의한 차이를 보였다($4.93{\pm}0.85$, $8.14{\pm}1.38$, <0.001).
Amyloid peptide (A${\beta}$) is the major component of senile plaques found in the brain of patient of Alzheimer's disease. ${\beta}$-amyloid peptide (25-35) (A${\beta}$25-35) is biologically active fragment of A${\beta}$. The three-dimensional structure of A${\beta}$25-35 in aqueous solution with 50% (vol/vol) TFE determined by NMR spectroscopy previously adopts an ${\alpha}$-helical conformation from $Ala^{30}$ to $Met^{35}$. It has been proposed that A${\beta}$(25-35) exhibits pH- and concentration-dependent ${\alpha}-helix{\leftrightarrow}{\beta}$sheet transition. This conformational transition with concomitant peptide aggregation is a possible mechanism of plaque formation. Here, in order to gain more insight into the mechanism of ${\alpha}$-helix formation of A${\beta}$25-35 peptide by TFE, which particularly stabilizes ${\alpha}$-helical conformation, we studied the secondary-structural elements of A${\beta}$25-35 peptide by molecular dynamics simulations. Secondary structural elements determined from NMR spectroscopy in aqueous TFE solution are preserved during the MD simulation. TFE/water mixed solvent has reduced capacity for forming hydrogen bond to the peptide compared to pure water solvent. TFE allows A${\beta}$25-35 to form bifurcated hydrogen bonds to TFE as well as to residues in peptide itself. MD simulation in this study supports the notion that TFE can act as an ${\alpha}$-helical structure forming solvent.

High quality publicly-available transcriptomic data representing relationships in gene expression across a diverse set of biological conditions is used as a context network to explore transcriptomics of the CNS. The context network, 18367Hu-matrix, contains pairwise Pearson correlations for 22,215 human genes across18,637 human tissue samples1. To do this, we compute a network derived from biological samples from CNS cells and tissues, calculate clusters of co-expressed genes from this network, and compare the significance of these to clusters derived from the larger 18367Hu-matrix network. Sorting and visualization uses the publicly available software, MetaOmGraph (http://www.metnetdb.org/MetNet_MetaOm-Graph.htm). This identifies genes that characterize particular disease conditions. Specifically, differences in gene expression within and between two designations of glial cancer, astrocytoma and glioblastoma, are evaluated in the context of the broader network. Such gene groups, which we term outlier-networks, tease out abnormally expressed genes and the samples in which this expression occurs. This approach distinguishes 48 subnetworks of outlier genes associated with astrocytoma and glioblastoma. As a case study, we investigate the relationships among the genes of a small astrocytoma-only subnetwork. This astrocytoma-only subnetwork consists of SVEP1, IGF1, CHRNA3, and SPAG6. All of these genes are highly coexpressed in a single sample of anaplastic astrocytoma tumor (grade III) and a sample of juvenile pilocytic astrocytoma. Three of these genes are also associated with nicotine. This data lead us to formulate a testable hypothesis that this astrocytoma outlier-network provides a link between some gliomas/astrocytomas and nicotine.
Adenoviral vectors are among the most promising vectors available for human gene therapy. However, the use of recombinant adenoviral vectors, including replicationcompetent adenovirus (RCA), raises a variety of safety concerns in relation to the development of new therapies based on gene therapy. To examine how organic compounds change in rat plasma following the injection of adenovirus, $\beta$-galactosidase expressing recombinant adenovirus (designated rAdLacZ) or RCA, we investigated the content of fatty acids (FAs), which are important biochemical indicators in pathological conditions. Pattern recognition analysis on the level of FAs in rat plasma is described for the visual discrimination of adenovirus infection groups from normal controls. Plasma FAs from four control rats (normal group), and from four rats with rAdLacZ infection and six rats with RCA infection (the two abnormal groups), were examined by gas chromatography-mass spectrometry in selected ion monitoring modes as their tert-butyldimethylsilyl derivatives. In total, 20 FAs were positively detected and quantified. The results of the Student's t-test on the normal mean of two abnormal groups, the levels of three FAs (p<0.05) from rAdLacZ group and eleven FAs (p<0.05) from RCA group were significantly different. When star symbol plotting was applied to the group mean values of 20 FAs after normalization to the corresponding normal mean values, the resulting eicosagonal star patterns of the two infected groups were distorted into similar shapes, but were distinguishable from each other. Thus, these approaches will be useful for screening and monitoring of diagnostic markers for the effects of infection following the use of adenoviral vectors in gene therapy.
This study was conducted to investigate the use of medical equipment in patients receiving home care service. The subjects of this study were 88 patients cared by seven home care nursed who were registered in the Seoul Nurses Association. Data was collected from Aug. 1, 1998 to Dec. 30, 1998. The findings are as follows. 1. The sample was found to be 55.7% female : 51.7% over 65 years old. 75% with neurologic disease including CVA, brain tumor, ICH, Parkinsonism & Spinal stenosis and 78.4% living in Seoul. The Clinical experience of the home care nurses was greater than five years. 2. Medical equipment which the patients possessed were foley catheters(61.4%), L-tubes(59.1%) and tracheostomy tubes(51.1%). 3. Technical difficulties in use of medical equipment were related to home care ventilators(60.0%), L-tubes(3.8%) and tracheostomy tubes(2.2%). 4. Most of the medical equipment were obtained from the hospital where they had been admitted previously or from medical equipment companies. 5. Complications from the use of this equipment were infection through invasive techniques including wound drainage tubes(50%), and IV injections(22.2%), The complications were resolved through referral to the doctor of the hospital where they were previously admitted or through community health centers. 6. Most of the equipment was disposable, and equipment was disinfected by using various methods including boiling and soaking in antiseptic solutions. These findings suggest that consistant policy on the management of medical equipment is necessary for the safety of home care patients.
In this paper, we describe implementation of a computer access device for the severly motor-disability. Many people with severe motor disabilities need an augmentative communication technology. Those who are totally paralyzed, or 'locked-in' cannot use conventional augmentative technologies, all of which require some measure of muscle control. The forehead is often the last site to suffer degradation in cases of severe disability and degenerative disease. For example, In ALS(Amyotrophic Lateral Sclerosis) and MD(Muscular dystrophy) the ocular motorneurons and ocular muscles are usually spared permitting at least gross eye movements, but not precise eye pointing. We use brain and body forehead bio-potentials in a novel way to generate multiple signals for computer control inputs. A bio-amplifier within this device separates the forehead signal into three frequency channels. The lowest channel is responsive to bio-potentials resulting from an eye motion, and second channel is the band pass derived between 0.5 and 45Hz, falling within the accepted Electroencephalographic(EEG) range. A digital processing station subdivides this region into eleven components frequency bands using FFT algorithm. The third channel is defined as an Electromyographic(EMG) signal. It responds to contractions of facial muscles and is well suited to discrete on/off switch closures, keyboard commands. These signals are transmitted to a PC that analyzes in a time series and a frequency region and discriminates user's intentions. That software graphically displays user's bio-potential signals in the real time, therefore user can see their own bio-potentials and control their physiological signals little by little after some training sessions. As a result, we confirmed the performance and availability of the developed system with experimental user's bio-potentials.
이 연구는 장애인 대상 치과진료 필요자 중 치과의료 서비스 미충족율과 그 이유를 조사하고, 미충족 치과치료에 따른 구강상태를 분석하였다. 국민건강영양조사 참여 대상 중 만 19세 이상 장애등록 응답자 444명을 선정하였고, 그 중 구강검진과 설문조사를 완료한 대상자 375명을 최종분석하였다. 장애유형은 6종류(지체장애, 뇌병변장애, 감각장애, 발달장애, 정신장애, 내분비장애)로 분류하였다. 연구대상자 중 미충족 치과치료 대상자는 128명이었으며, 최근 1년 동안 치과진료를 받지 못한 가장 주된 이유는 '경제적인 이유로'에 대한 항목이 49.2%으로 가장 많았으며, '다른 문제에 비해 덜 중요하다고 느껴서'에 대한 항목이 18.0%이었다. 치주질환과 영구치우식경험을 가진 장애인 중 미충족 치과치료 빈도와 진행정도가 높은 것으로 확인하였다. 치과진료 자원의 가용성 측면에서 장애인 수 대비 적절한 치과진료 서비스를 충분히 제공하여 치과진료 서비스의 원활한 이용이 요구된다.
