Endoscopic Management of Cranial Arachnoid Cysts Using Extra-Channel Method

  • Objective : Arachnoid cysts (ACs) can be cured by making the definite and wide communication between the cyst and arachnoid space using endoscopy, but often it is impossible only through the usual working-channel (intra-channel) procedures. We discuss and propose a more valuable endoscopic technique with the presentation of our series of cases. Methods : We treated 9 patients with cortical AC in various locations with extra-channel endoscopic techniques. The patients ranged in age from 3 years to 60 years (mean age, 37.2 yrs). The follow-up period ranged from 12 to 26 months (mean follow-up duration, 17.2 months). All patients had large AC compressing the adjacent brain with clinical symptoms or signs. The authors performed extensive fenestration via single burr hole with the aid of endoscope. Being bypassed the rigid endoscope, through the space between the shaft of endoscope and guiding cannula (extra-channel method), fenestration procedures were done in the dry fields. Results : Eight (88.9%) patients had been treated successfully with endoscope. One patient required shunt procedure. Among the eight patients who were treated with endoscopic procedure, 6 patients (66.7%) showed cyst reduction, and two (22.2%) showed disappearance of cyst. Conclusion : We suggest that extra-channel method will be simple and easy to perform using more valuable instruments with wider working area, and may promise better results compared to the conventional intra-channel endoscopic procedures.

컴퓨터단층촬영술과 자기공명영상을 이용한 두개강내 지주막낭종의 진단 4례 (Diagnosis of Intracranial Arachnoid Cyst using CT and MRI; A Study of Four Cases)

  • Arachnoid cysts are intra-arachnoid collections of cerebrospinal fluid. They may cause neurological deficit through expansion that can compress normal neural tissue and obstruct cerebrospinal fluid flow. Intracranial arachnoid cysts were found in 4 patients aged 10 months to 20 months(mean age 15 months). The presenting symptoms included circling(dog 2,3), seizure(dog 1), progressive weakness(dog 1,2,3), increasing scoliosis(dog 3), worsening spasticity(dog 3), and salivation(dog 3). One patient showed no symptoms from the intracranial arachnoid cyst. There were no significant findings in complete blood count and serum chemistry. Computed tomography(CT) with/without magnetic resonance imaging(MRI) scans were performed in these cases. Hypodense intracranial arachnoid cyst was observed on CT images in dog 4. Sagittal T1-weighted(T1W) and T2-weighted(T2W) images and transverse T1W and T2W images revealed a intracranial arachnoid cysts with syringohydromyelia(dog 3) and hydrocephalus(dog 3,4) in the brain. Clinical signs of three cases were improved by medication.

Toxoplasmic meningoencephalitis in a stray cat in Korea

  • A dead stray cat was necropsied for zoonotic feline disease monitoring. Grossly, there were no specific lesions. Major microscopic lesions included lymphocytic meningoencephalitis, malacia, and tissue cysts in the cerebral and cerebellar cortex. The size and shape of tissue cysts were identical to those of Apicomplexa including Toxoplasma (T.) gondii. Bradyzoites in the tissue cyst were strongly positive for T. gondii by immunohistochemistry. Electron microscopy revealed that bradyzoites within the tissue cyst were similar to the morphological features of T. gondii. Fresh tissue samples were examined by a polymerase chain reaction assay and resulted in a specific band of T. gondii only in the brain. Based on the results, this case was diagnosed as toxoplasmosis. This is the first case of toxoplasmic meningoencephalitis in a cat in Korea.

A role of carboxy-terminal region of Toxoplasma gondii-heat shock protein 70 in enhancement of T. gondii infection in mice

  • We investigated the role of recombinant Toxoplasma gondii heat shock protein (rT.g.HSP) 70-full length, rT.g. HSP70-NH2-terminal region, or rT.g. HSP70-carboxy-terminal region in prophylactic immunity in C57BL/6 mice perorally infected with Fukaya cysts of T. gondii. At 3, 4, 5, and 6 weeks after infection, the number of T gondii in the brain tissue of each mouse was measured by quantitative competitive-polymerase chain reaction (QC-PCR) targeting the surface antigen (SAG) 1 gene. Immunization with rT.g.HSP70-full length or rT.g.HSP70-carboxy-terminal region increased the number of T. gondii in the brain tissue after T. gondii infection, whereas immunization with rT.g.HSP70-NHa-terminal region did not. These results suggest that T.g. HSP70-carboxy-terminal region as well as T.g.HSP70-full length may induce deleterious effects on the protective immunity of mice infected with a cyst-forming T. gondii strain, Fukaya.

Giant Sellar Xanthogranuloma after Surgical Treatment of Symptomatic Rathke's Cleft Cyst

  • Extremely massive sellar xanthogranuloma (XG) are rare, and the surgical outcome and prognosis are not well known. XG remain unknown whether they are derived from Rathke's cleft cysts (RCCs) or craniopharyngiomas (CPs) following extensive inflammation and metaplasia, to the point that no epithelium is readily identifiable. These lesions usually tend to occur in younger patients (mean 28.3 years), have a smaller diameter, and remain primarily intrasellar region with infrequent calcification. This 36-year-old man presented our hospital with visual deterioration. At the time of visit, there were no neurological problems other than visual field defect and hormonal disorder. He visited our hospital in 2007 due to headache and decreased vision, and underwent transphenoid surgery for pituitary RCC. Since then, he has received treatment at our hospital for postoperative hormonal disorders. Through preoperative imaging study, the author suspected CP and underwent surgery. During the operation, the adhesion of the tumor to the surrounding major neurovascular structures was severe in the naked eyes, but the tumor could be removed more easily than expected. The postoperative histological findings were confirmed as XG. The postoperative course was uneventful. Compared to the previous literature, this case is a case where the size of XG is very large in a sellar region and it can be proved that it originated from the RCC. And regular follow-up is necessary to confirm the prognosis after surgery.

Epidermoid Cyst Arising from the Corpus Callosum

  • Most of intradural epidermoid cyst arise as slowly growing extraaxial lesions but purely intracerebral epidermoid cysts are rare. A 39-year-old female presented with a headache during several months. Brain computed tomography [CT] scan showed a mass lesion in the both frontal lobe with heterogenous density approximately $5{\times}5cm$ in size. Magnetic resonance imaging [MRI] revealed a mass of heterogenous signal intensity on T1, T2-weighted image and faint enhancement with gadolinium [Gd]. Through the both interhemispheric approach, mass was removed subtotally except the calcified portion tightly attached to the corpus callosum. The patient discharged without neurological deficit. The authors report a case of epidermoid cyst in the corpus callosum and discuss the pathogenesis of the intraparenchymal epidermoid cyst.

A Ruptured Dermoid Cyst of the Cavernous Sinus Extending into the Posterior Fossa

  • Supratentorial dermoid cysts are uncommon to develop in the cavernous sinus. We present a ruptured dermoid cyst of the cavernous sinus extending into the posterior fossa. The patient was a 32-year-old female who complained occipital headache, blurred vision, and tinnitus over 4 years. Brain magnetic resonance (MR) imaging revealed an enhanced tumor in the right cavernous sinus extending into the right temporal base and the posterior fossa with findings of ruptured cyst. Surgical resection was performed, and pathological findings were confirmed to be a dermoid cyst. We report a second case with ruptured dermoid cyst of the cavernous sinus extending into the posterior fossa.

Neospora caninum에 자연 감염된 임신우로부터 유산된 태아의 병리조직학적 관찰 (Histopathologic Observation of the Aborted Fetus from Pregnant Dairy Cows Naturally Infected with Neospora caninum)

  • 충남 공주시와 연기군의 목장 4곳에서 Neospora에 자연 감염된 임신우로부터 유산된 태아를 병리조직학적, 전자현미경적으로 관찰하였다. 유산된 태아는 전신 부종과 함께 부검시 복강과 흉강은 혈액성 장액성 액체로 충만되어 있었다. 광학현미경적으로 심장과 간에서는 충 출혈 및 심근세포와 간세포의 괴사를 동반한 대식 세포, 임파구, 단핵구 세포로 구성된 많은 수의 염증세포 침윤이 관찰되었고, 간에서는 간세포의 세포질과 간질 조직에서 tachyzoite 집락이 관찰되었다. 뇌에서는 신경세포내와 주위에서 크기가 다양한 많은 수의 조직낭포가 관찰되었고, 조직낭포는 구형으로 두꺼운 막으로 싸여 있었으며 내부에는 많은 bradyzoite를 포함하고 있었다. 또한, 뇌조직에서는 충 출혈 및 국소성 괴사와 함께 광범위한 신경교증이 관찰되었고 국소성 괴사 부위 주위와 혈관주위에 소교세포로 구성된 많은 수의 염증세포 침윤이 관찰되었다. 전자현미경적으로 조직낭포의 막 두께는 대략 1 ${\mu}m$로 모양은 불규칙하였고, 내부에는 길이 2-5 ${\mu}m$, 폭 1-2 ${\mu}m$인 100여 개의 bradyzoite가 관찰되었다. Bradyzoite의 핵은 posterior tip으로부터 1-1.5 ${\mu}m$ 전방에 존재하였고, amylopectin 과립, 전자밀도가 높고 작은 구형인 과립과 전자밀도가 상대적으로 낮으나 큰 구형의 과립, 전자밀도가 균질한 rhoptry 및 zoite pellicle에 대해 수직방향으로 달리는 microneme이 핵과 posterior tip 사이의 세포질 내에서 관찰되었다. 이상의 본 연구는 ELISA검사를 통해 Neospora 항체에 양성반응을 나타내는 임신우로부터 유산된 태아에서 발견된 조직낭포를 전자현미경적으로 관찰하여 N. caninum임을 확인하였다.

yoxoplusmg leondii의 세포막 단백 성분과 그 항원성 (Membrane Proteins and Their Antigenicity of Toxoplasma gondii)

  • Toxoplasmn gondii이 강독주인 RH주와 조직내 cyst 형성주인 Fukaya주의 세포막 단배 성분을 SDS 존재하에 서 전기영동하여 분석하였다. 먼저 RH tachyzoite와 Fukaya의 cyst를 각각 마우스의 복강액과 뇌조직으로부터 분리하였는데, 불연속 Percoll density-gradient서 원심분리하여 tachygoite는 50 U와 605 Percoll용액 경계면에서, cyst는 40%와 50%의 경계면 및 50%와 60 % 경계면에서 얻었으며, cyst는 저장액으로 처리하여 bradyzoite를 얻었다. Lactoperoxidase를 촉매로 세포막에 방사성 요오드를 표지시킨 후 자가방사표지그림을 얻었을 때, bradyzoite 는 15 KDa와 14 KDa의 분자량을 가진 단백질이 주요 단백질로 나타났으며, tachyzoite에서는 30 KDa 단백질이 주요 단백질로 나타났다. 또, 당단백질의 존재를 파악하기 위해서 lectin blotting을 시행하였는데, concanavalin A는 bradyzoite에서 200K∼50KDa의 여러 단백질을, .그리고 tachyzoite에서는 52KDa 단백질을 주로 하는 33K∼20 KDa단백질을 검출하였으며, phytohemagglutinin은 두·유형에서 아무런 단백질도 검출하지 못하였다. 한편, 이들을 효소면역이적법으로 항 Fukfya항체와 항 RH항체로 반응시켰을 때, 많은 교차 반응을 보였으나, bradyzoite에서는 15 KDa 단백질이, 그리고 tachyzoite에서는 52 KDa, 30 KDa 및 25 KDa 단백 짙이 각각 유형 특이 항원 단백으로 나타났다. 위의 결과들로, bradyzoite에서는 15 KDa 단백질이 당단백질은 아니지만 특이 항원성을 갖는 주요 백으로 나타났으며, tachyzoite에서는 지금까지 주요 세포막 단백으로. 알려진 P3O외에 당단백질이며 성을 갖는 세포막 단백으로 SaKDa 단백 (gps2)을 확인할 수 있었다.

Toxoplasma 감염 마우스에 있어서 trimethoprim-sulfamethoxazole의 투여 효과 (Prophylactic effects of trimethoprim-sulfamethoxazole in Toxoplasma-infected mice)

  • Toxoprqsmagonnii의 강독주(RH주) 혹은 약독주(Beverley주)를 마우스에 감염시킴과 동시에 trimethoprim-sulfamethoxazole(TMP-SMZ)을 투여하여 Toxoplasma 감염 마우스에 있어 서 TMP-SMZ의 감염 예방효과를 마우스의 생존기간과 생존율. 면역혈청학적 반응 및 뇌조직내 씨스트의 수로 평가한 후 감염대조군 및 spiramycin 투여군과 비교하였다. T. gondii의 RH주를 감염시킨 마우스(마우스당 $1{\;}{\times}{\;}10^5$개의 tachyzoite를 감염)에 TMP-SMZ을 투여시 마우스의 생존기간은 감염대조군이나 spiramycin 투여군보다 유의하게 증가하였으며(p < 0.01), TMP-SMZ의 투여량을 증가시킴에 따라 생존기간 및 생존율이 증가하였고 마우스당 24 mg/day 이상 15일간 투여시 100% 생존하였다. T. gondii의 Beverley주를 감염시킨 마우스(마우스당 10개의 씨스트를 감염)의 혈청내 IgG 및 IgM 항체가는 TMP-SMZ 투여시 유의하게 감소하였으며(p < 0.05) TMP-SMZ을 마우스당 24 mg/day 이상 투여시 뇌조직내에서 씨스트를 발견할 수 없었다. 그러나 spiramycin 투여군은 감염대조군과 유사한 항체가를 보였으며 감염 후 20일부터 씨스트가 발견되었다 이상의 성적으로 보아 TMP-SMZ은 T. gondii의 강독주나 약독주 감염 마우스에 투여시 감염 예방효과가 있었다.
