Dural Arteriovenous Fistula Presenting with Cerebral Infarction

  • We report on a diabetic 52-year-old man who complained ocular floating sensation, headache and dizziness, in whom a left parieto-occipital dural ateriovenous fistulas [DAVFs], fed by bilateral superficial temporal arteries and occipital artery, drained into the cortical vein of the left parieto-occipital convexity. Because the patient's chief complaint was ocular symptom for diabetic retinopathy, we initially didn't consider an DAVFs until brain magnetic resonance imaging [MRI] was done. Diffusion-weighted brain MRI revealed acute cerebral infarction and microhemorrhage in the lesion. Transarterial embolization with mixture of glue and lipiodol obliterated the DAVFs completely. Although the DAVFs fed by multi-arteries, the fistulous portion has been disappeared after embolization via an only left occipital artery Endovascular embolization of the fistula led to symptomatic improvement, except ocular discomfort.

흉벽에 발생한 Sparganosis;1례 보고 (A Case of Sparganosis in the Chest Wall)

  • Human sparganosis caused by Sparganum mansoni, the larval plerocercoid worm of the genus Spirometra, is not uncommon in Korea and is mostly found in subcutaneous or adipose tissue of the abdominal, thoracic wall and inguinal region, but is rarely found in the orbital cavity, brain and breast. It, at present, is a surgical disease because its diagnosis depends almost on the demonstration of the larva[e] from lesion or finding the worm section in surgical pathology specimens. We experienced a case of human sparganosis from a 48 years old woman who had a history of eating a raw frog. We report the case and review the related literatures.

  • Foreign Accent syndrome refers to segmental and suprasegmental changes of speech characteristics following brain lesion which is perceived by listeners as a foreign accent. Change in dialect after a stroke, however, have rarely been reported. We describe a patient who showed prominent change of accent from one to another Korean dialect and discuss about the alteration of prosodic patterns and the changes in segmental level of speech.

Recurrent transient amnesia: a case of transient epileptic amnesia misdiagnosed as transient global amnesia

  • Transient epileptic amnesia and transient global amnesia both exhibit temporary memory loss. The lack of clues of epileptic events and the absence of epileptiform abnormalities in electroencephalography, a clear brain lesion, and interictal cognitive decline can make diagnoses challenging. Here we present a middle-aged female who experienced long-term recurrent transient epileptic amnesia with subtle epileptic features over a period of 3 years.

전산화단층영상 기반 뇌출혈 검출을 위한 YOLOv5s 성능 평가 (Performance Evaluation of YOLOv5s for Brain Hemorrhage Detection Using Computed Tomography Images)

  • 뇌 전산화단층촬영은 비침습성, 3차원 영상 제공, 저방사선량 등의 장점 때문에 뇌출혈과 같은 질병 진단을 위해 시행된다. 하지만 뇌 전산화단층영상 판독을 위한 전문의의 인력 공급 부족 및 막대한 업무량으로 인해 수많은 판독 오류 및 오진이 발생하고 있다. 이와 같은 문제를 해결하기 위해 객체 검출을 위한 다양한 인공지능 기술이 개발되고 있다. 본 연구에서는 뇌 전산화단층영상으로부터 뇌출혈 검출을 위한 딥러닝 기반 YOLOv5s 모델의 적용 가능성을 확인하였다. 또한 YOLOv5s 모델 학습 시 초매개변수를 변화시켜 학습된 모델의 성능을 평가하였다. YOLOv5s 모델은 backbone, neck 및 output 모듈로 구성하였고, 입력 CT 영상 내 뇌출혈로 의심되는 부위를 검출하여 출력할 수 있도록 하였다. YOLOv5s 모델 학습 시 활성화함수, 최적화함수, 손실함수 및 학습 횟수를 변화시켰고, 학습된 모델의 뇌출혈 검출 정확도 및 학습 시간을 측정하였다. 연구결과 학습된 YOLOv5s 모델은 뇌출혈로 의심되는 부위에 대한 경계 박스 및 해당 경계박스에 대한 정확도를 출력할 수 있음을 확인하였다. Mish 활성화함수, stochastic gradient descent 최적화함수 및 completed intersection over union 손실함수 적용 시 YOLOv5s 모델의 뇌출혈 검출 정확도 향상 및 학습 시간이 단축되는 결과를 확인하였다. 또한 YOLOv5s 모델의 뇌출혈 검출 정확도 및 학습 시간은 학습 횟수에 비례하여 증가하는 결과를 확인하였다. 따라서 YOLOv5s 모델은 뇌 전산화단층영상을 이용한 뇌출혈 검출을 위해 활용할 수 있으며, 최적의 초매개변수 적용을 통해 성능을 향상 시킬 수 있다.

Magnetic Resonance Findings in Two Episodes of Repeated Cerebral Fat Embolisms in a Patient with Autologous Fat Injection into the Face

  • We report magnetic resonance image (MRI) and magnetic resonance spectroscopy (MRS) findings in a patient of cerebral fat embolism (CFE) occurred in a 26-year-old woman after an autologous fat injection into the face. After initial neurologic symptom onset, MRI and MRS data were obtained two times to investigate repeated CFE. We obtained the MRS data in the two different time intervals and two different echo times to compare the lesions with normal brain parenchyma. The results of MRS data showed that a decrease in N-acetyl-aspartate, an increase in lactate and a very high early peak of free lipids between 0.9 and 1.4 ppm were obtained at the acute infarcted lesion as compared with normal brain parenchyma. In addition, these findings were more clearly detected on short echo time spectrum rather than long spectrum. A close relationship between the clinical manifestations and MRI and MRS findings of the brain can helpful to distinguish CFE with other conditions and to evaluate the cause materials of infarctions rather than conventional MRI or diffusion-weighted imaging.

광화학적 허혈성 뇌졸중 모델에서 사심탕(瀉心湯)의 뇌세포 손상 보호효과 (The Protective Effects of Sasim-tang on the Brain Cell Damage in Photothrombotic Ischemia Mouse Model)

  • Objectives : This study was designed to investigate the effects of Sasim-tang (SST) on proinflammatory cytokine production in a photothrombotic ischemia mouse model. Methods : Photothrombotic ischemia was induced in stereotactically held male Balb/c mice using rose bengal (10 mg/kg) and cold light. The target of photothrombotic ischemic lesion was 1 mm anterior to bregma and 3 mm lateral to midline with 2 mm in diameter, which are decreased by oral administration of SST. Results : SST protected ischemic death of brain cells through inhibition of pro-inflammatory cytokines production and catalytic activation of caspase-3 protease in photothrombotic ischemia mouse model. Conclusions : The results of this study suggest that SST can have protective effects on brain cell damage in a photothrombotic ischemia mouse model.

기저핵부 뇌실질내 출혈에 대한 수술후 뇌농양으로 오인한 다형성 교아세포종 (Glioblastoma Misdiagnosed as Brain Abscess after Surgical Evacuation of Spontaneous Basal Ganglia Hemorrhage)

  • A 51-year-old woman presented with sudden severe headache, vomiting, and right hemiparesis at first admission. Computed tomography(CT) scans revealed an hemorrhagic density at left basal ganglia. Preoperative cerebral angiography showed no vascular lesion. Under the diagnosis of hypertensive intracerebral hemorrhage(ICH), total extirpation of hematoma was done. The postoperative neurological condition improved gradually and discharged without any neurological sequelae. Two months later, she revisited with headache, vomiting and progressive right hemiparesis. CT scans at second admission showed an irregular rim enhanced mass with central low density with surrounding edema at the initial bleeding area. Repeated craniotomy was performed and the mass was partially removed. The histopathological diagnosis of the specimen was confirmed as glioblastoma. The authors report a glioblastoma, which occurred at initial ICH site and regarded as a brain abscess with literature review.

Alveolar Soft Part Sarcoma Metastasized to Both the Skull and the Brain

  • Alveolar soft part sarcoma (ASPS) with skull and brain metastases is extremely rare. A 53-year-old patient diagnosed as skull metastasis of ASPS visited our clinic complaining of an outgrowing scalp mass in spite of radiation therapy. Past medical history revealed that the patient had been diagnosed and treated for ASPS of the thigh 4 years ago. Magnetic resonance imaging revealed a hyperintense ovoid mass on the T2-weighted image, an isointense on the T1-weighted image, and a homogeneous enhanced mass with gadolinium. Another small-sized enhanced mass with mild peritumoral swelling was found at the deep white matter of the left frontal lobe. A gross total resection of the skull lesion with cranioplasty was performed for the surgical defect. A histologic examination of the specimens revealed metastatic ASPS involving the skull. Surgery with a total removal of the lesions may be effective for improving a patient's symptoms especially from neurological dysfunction.

  • A 3-year-old female, 5kg, Shih-tzu developed an acute onset of depression, disorientation, hypersalivation, nystagmus after falling down 2 meter height place. In plain skull radiography, there was fracture line in the frontal and parietal bones and next day magnetic resonance imaging examination was performed. Magnetic resonance imaging of the brain was performed with 3.0 Tesla unit. Under general anesthesia, the dog was placed in prone with its head positioned in a birdcage coil. Transverse, sagittal and coronal fast spin echo images of the brain were obtained with the following pulse sequences: T1 weighted images (TR = 560 ms and TE = 18.6 ms) and T2 weighted images (TR = 3500 ms and TE = 80 ms). Magnetic resonance imaging showed epidural hematoma in the left frontal area resulting in compression of the adjacent brain parenchyma. Left lateral ventricle was compressed secondarily and the longitudinal fissure shifted to the right, representing mass effect. The lesion was iso-to slightly hyperintense on T1 weighted image and iso-slightly hypointense signal on T2 weighted image. At necropsy, there was a skull fracture and epidural hematoma in the left frontal area. Magnetic resonance imaging of epidural hematoma is reviewed.
