Biomedicus pump를 이용한 개심술후 심실보조 (Postcardiotomy Ventricular Support with Biomedicus Pump)

  • 근래에 와서 심장수술 기법 및 심근보호법의 현저한 향상에도 불구하고 심장수술 환자들의 약 0.1~0.8 % 에서는 여전히 심장수술후 약물 치료나 대동맥 풍선펌프 등의 전통적인 치료법에 반응하지 않는 급성 심부전증이 발생하고 있다. 이런 환자들에서는 보다 적극적인 순환보조법으로 심실보조를 시행할 필요가 있는데, 현재 이런 목적으로 원심성 펌프가 비용이 비교적 저렴하고 사용하기 편리하다는 점에서 세계적으로 가장 많이 사용되고 있다. 그러나 아직 국내에서는 이러한 심실보조장치로서의 원심성펌프의 사용에 관한 문헌 보고가 매우 드문 실정이다. 1992년 1월 부터 1996년 1월 까지 서울대학병원에서는 총 2986례의 개심술이 시행되었는데, 이들 환자 중 모두 10명에서 심장수술후 전통적인 치료법에 반응하지 않는 심부전증이 발생하여 Biomedicus 원심성 펌프를 이용한 심실보조를 시행하였다. 10명의 환자중 남자가 8명 여자는 2명이였으며 평균 연령은 50$\pm$20세였다(범위; 9세~77세). 환자들이 받은 수술로는 6명의 환자가 관상동맥우회수술을 받았으며 이중2명의 환자는 관상동맥우회수술과 함께 대동맥판막치환수술을 시행받았다. 그밖에 대동맥박리증 수술 2례, 그리고 폐색전제거술 및 심장이식수술이 각각 1례씩 있었다. 심실보조 형태로는 전체 환자 중 5명의 환자에서 좌심실 보조를 시행하였으며 1명에서 우심실보조 그리고 4명에서 양심실보조를 시행하였다. 심실보조 기간은 평균 76$\pm$51시간이였다(범위; 24시간~175 시간). 10명중 7명에서 심실보조 장치 이탈이 가능하였고 이중4명이 생존하여 퇴원하였다. 사망 환자들의 사망 원인으로는 진행성 심부전증 2례, 그리고 다장기 부전증, 심실세동, 비가역적 뇌손상, 기계적 문제가 각각 1례씩이였다. 사망환자와 생존 퇴원환자간의 심폐바이패스시간, 대동맥차단시간, 심실보조시간에서 통계학적인 차이는 없었다. 원심성 심실보조장치 삽입에 연관된 주된 합병증으로는 출혈(7), 급성 신부전증(6), 감염(3), 그리고 신경학적 합병증(2)등이 관찰되었다. 결론적으로 원심성 펌프는 단기 순환보조용으로 비교적 만족한 결과로 사용될 수 있었다.

소아 결핵성 뇌막염의 임상적 고찰 (Clinical Study of Tuberculous Meningitis in Children)

  • 목 적 : 소아의 결핵성 뇌막염은 소아 결핵중 가장 심한 후유증을 남기므로 조기 발견과 치료를 요하는 질환으로 국내에서의 소아 결핵성 뇌막염 발생은 1980년대에 와서 전반적으 로 다른 폐외결핵과 함께 현저히 감소된 양상을 보이고 있다. 그러나 소아 결핵이 소멸되고 있지 않은 우리나라 실정에서 실제 결핵성 뇌막염은 지속적으로 발생되며, 발병이 낮아진 상황으로 인하여 임상 경험 결여와 무관심에 따른 문제점이 예견되고 있다. 이러한 관점에 서 전형적 또는 비전형적 소아 결핵성 뇌막염의 진단, 임상 특성, 합병증 및 예후 등에 관한 임상 연구는 요구되어 본 연구를 시행하게 되었다. 대상 및 방법 : 1985년부터 1996년 사이에 가톨릭의대부속 성모자애 병원, 성가 병원, 성 빈센트 병원, 의정부 성모 병원에 결핵성 뇌막염으로 진단되어 입원 치료 및 경과 관찰이 이루어진 44명의 환아를 대상으로 하였다. 이들 대상 환아들에서 결핵성 뇌막염의 진단적 기준, 초기 임상 특성, 신경학적 단계에따른 예후, 동반 합병증, 뇌 척수액 및 방사선 검사 소견들에 관한 내용을 의무 기록 내용을 통하여 후향적으로 연구하였다. 결과 : 1) 소아 결핵성 뇌막염의 발생은 1980년대 중반 이후에도 지속 되고 있으며, 3세 이하의 연령에서 높은 발병이 관찰되었다. 2) 이 질환의 진단은 활동성 결핵 환자와의 접촉력, 양성 결핵 반응, 항 결핵제 치료 반응, 뇌척수액 및 이외 가검물에서 결핵균 분리 또는 확인된 순으로 이루어 졌다. 이들 환아 들의 16%는 BCG 접종을 실시하지 않은 경우였고, 40%에서만 결핵 반응 검사상 양성이었다. 3) 내원시 주증세는 발열, 구토, 두통, 권면, 식욕 감소, 체중 감소, 경부 강직, 경련, 복통, 운동 장애순이었다. 4) 내원시 신경적 단계는 1단계 59%, 2단계 32%, 3단계 9%이었고, 합병증으로는 경한 신 경 손상이 29.5%, 중한 손상이 4.6% 그리고 사망한 경우가 6.8%이었다. 5) 뇌척수액 검사상 염증 세포가 평균 $239.5/mm^3$, 단백은 펑균 239.5mg%, 당은 40.7mg%이었다. 그러나 31.8%에서 염증 세포, 단백 및 당의 검사 소견이 이와같지 않은 비정형 뇌척수액 소견이 보였다. 6) 입원 약 4병일정도에서 SIADH 소견이 약 45.5%에서 보였으며, 이들중 반 정도에서 SIADH 임상 소견이 관찰되었다. 7) 뇌단층 촬영상 뇌수종, 뇌기저막 음영 증가, 혈류장애, 지주하막 염증순으로 이상 소견 이 34명에서 관찰되었다. 흉부 X선상 속립성 결핵(34.1%), 정상(29.5%), 폐침윤(11.4%), 석회 화 음영(9.1%)의 소견을 보였다. 결 론 : 국내의 소아 결핵은 과거에 비해 뚜렷한 감소를 보였으나 지속적 발생이 관찰되었고, 이들중 비정형 결핵성 뇌막염의 비율이 높아 광범위한 진단적 기준을 활용하여 조기 진단에 많은 고려가 있어야 하며 뇌 영상 검사의 적극적 활용도 필요함을 확인하였다. 그리 고 SIADH에 대한소견도 임상 경과중에 고려해야할 사항으로 인지해야 할 것이다.

목동맥 스텐트 삽입술 후 시상을 포함한 후대뇌동맥 영역에 발생한 뇌출혈 : 증례보고 (Posterior Cerebral Artery Territorial Hemorrhage Including Thalamus After Carotid Artery Stenting : A Case Report)

  • 목동맥 스텐트 삽입술은 목동맥 내막절제술에 적합하지 않은 환자에게 시행할 수 있는 목동맥 협착증의 대체 치료법으로 알려져 있다. 목동맥 내막절제술, 혈관성형술 또는 스텐트 삽입술 후에 드문 부작용으로 뇌내출혈이 발생할 수 있고, 이러한 출혈이 발생하는 원인은 대부분의 경우 재관류 손상과 관련이 있는 것으로 추정하고 있다. 이전의 연구에서는 내막절제술과 비교하여 목동맥 스텐트 삽입술 후 뇌내출혈의 빈도가 더 높다고 보고한 바 있다. 본 연구에서는 뇌경색으로 내원한 80세 남자환자를 대상으로 증례보고를 통해 동일 분야 연구에 활용하고자 자료 분석을 하였다. 80세 남자가 갑자기 발생한 오른 팔의 근력저하를 주소로 방문하였다. 왼쪽 속목동맥의 90% 협착이 발견되어 목동맥 스텐트 삽입술을 시행하였고, 시술 후 실시한 뇌 CT에서 시상을 포함하는 후대뇌동맥 영역의 뇌출혈이 뇌실내출혈까지 진행되어 있음을 관찰하였다. 이러한 출혈은 스텐트 삽입술이 시행된 동맥에서 공급될 가능성이 낮은 혈관 영역에서 발생했기 때문에, 이 경우에 내막절제술 시행 후 과다혈류에 의한 출혈과 다른 양상을 확인하였다.

뇌성 마비를 동반한 악안면 기형 환자의 치험례 (TREATMENT OF DENTOFACIAL DEFORMITY PATIENT WITH CEREBRAL PALSY)

  • Cerebral palsy(CP) is one of the most common motor disease, due to brain injury during fetal and neonatal development which results in neuromotor paralysis and associated neuromuscular symptoms. Features of CP include motor disability due to the lack of muscle control, often accompanied by sensory disorders, mental retardation, speech disorders, hearing loss, epilepsy, behavior disorders, etc. There are increasing chances of treatment of dental patients with cerebral palsy, as the occurrence of CP is increasing with the decrease in infant mortality and an increase in immature birth and premature birth and also, there is a trend to pursue of higher quality of life. Reports on the relationship between CP and maxillofacial deformity are uncommon, but it is well known that the unbalance and discontrol of the facial muscles, lip, tongue and the jaws leads to malocclusion and temporomandibular joint disorders, and statistics show that class 2 relationship of the jaws and open bite is frequently reported. However, it is difficult to perform maxillofacial deformity treatment, which consists of orthodontic treatment, maxillofacial surgery and muscle adaptation training, due to difficulties in communication and problems of muscle adaptation caused by difficulties in motor control which leads to a high recurrence rate. This case report is to trearment of maxillofacial deformity in CP patient. A 26 year old female patient came to the department with the chief complaint of prognathism of the mandible and facial asymmetry. According to the past medical history, she was diagnosed as cerebral palsy 1 week after birth, classified as GMFC, classII accompanied with left side torticollis. The patient's intelligence was moderate, and there were no serious problems in communication. For two years time, the patient underwent lingual frenectomy, pre-operation orthodontic treatment and then bimaxillary orthognathic surgery to treat mandibular prognathism and facial asymmetry followed by rehabilitatory exercise of facial muscle. After 6 months of follow up, there was a good result. This is to report to the typical signs and symptoms of DFD in CP patient and the limitation of the usual method of the treatment of DFD in CP patient with literature review.

Alteplase와 pamiteplase에 의한 MMPs 조절 효과 비교 (Comparison of Effects between Alteplase and Pamiteplase on MMPs Regulation)

  • 뇌졸중 환자에 사용되는 tPA치료법은 혈전을 용해시키고 혈액의 흐름을 용이하게 해 주기 때문에 매우 중요하게 사용되고 있다. 동물 실험모델을 이용한 이전의 실험결과에 따르면 tPA뿐 아니라 tPA의 일종인 pamite-plase 역시 30분 이내 탁월한 혈전 용해의 효과를 보여주었다. 그러나 fPA치료법은 단시간 처리와 출혈, 부종과 같은 여러 가지 부작용이 수반될 수 있으며 이들은 MMP-9의 활성 조절과 깊은 관련이 있는 것으로 보고되고 있다. 본 연구에서는 임상에 사용되는 tPA의 한 종류인 alteplase와 이러한 부정적인 효과를 극복하기 위해 개발된 pamiteplase의 MMP-9활성에 미치는 효과를 비교 분석하였다. 랫트의 뇌로부터 추출한 신경세포에서 alteplase의 처리는 농도의존적으로 MMP-9의 발현을 촉진시켰고 활성화된 형태의 MMP-2역시 증가되는 양상을 보였다. 반면, pamiteplase의 경우 MMP-2와 MMP-9의 발현양상에 영항을 미치지 않았다. 유사한 효과는 뇌신경계를 구성하는 다른 세포인 성상세포에서도 관찰되었다. 즉 대뇌의 성상세포를 분리, 배양하여 이들의 효과를 확인한 결과 신경세포에서와 마찬가지로 alteplase의 경우 농도의존적으로 증가하였고 pamiteplasse의 경우 변화를 나타내지 않았다. Pamiteplase는 뇌신경구성세포에서 출혈과 부종을 유도하는 데 관련이 있는 것으로 보고된 MMP-9의 활성에 영향을 미치지 않는 것으로 보아 alteplase에 비해 보다 효과적인 치료제로서의 가능성을 보여주고 있다.

무심폐기 관상동맥 우회술에 있어서 이식편의 대동맥부 근위부 문합 방법에 따른 수술 후 신경학적 합병증 발생의 비교: 하트스트링을 이용한 방법 대 고식적인 수기 문합술 (Comparison of the Neurologic Outcome according to the Method of Proximal Graft Anastomosis at the Aortic Side during Off-pump Coronary Artery Bypass Grafting: The Heartstring Sealing System versus Conventional Manual Anastomosis)

  • 배경: 관상동맥 우회술의 대동맥부 근위부 문합을 위한 상행 대동맥 부분 겸자는 대동맥 내벽에 존재하는 죽상반의 색전에 의한 뇌경색의 위험성을 높일 뿐 아니라, 겸자에 따른 직접적 대동맥 손상의 원인이 될 수 있다. Heartstring proximal sealing system (Guidant Corporation, Santa Clara, Calif)을 이용한 근위부 문합은 대동맥 부분 겸자를 피할 수 있어, 이러한 잠재적 위험을 방지할 수 있는 것으로 제시되어 왔다. 이 연구는 무심폐기 관상동맥 우회술의 근위부 문합에 있어서 대동맥 부분 겸자를 통한 수기 문합과 Heartstring system을 이용한 문합간의 수술 후 신경학적 합병증을 비교하고자 하였다. 대상 및 방법: 2003년 1월부터 2008년 8월까지 499명의 환자가 무심폐기 관상동맥 우회술을 시행받았다. 그 중 182명의 환자가 Heartstring system을 이용한 이식편 대동맥 근위부 문합술을 시행받았고(I군), 317명의 환자가 고식적 수기 문합술을 시행 받았다(II군). 이 두 군간의 수술 후 신경 손상을 비롯한 주요 합병증 및 사망율을 비교하였다. 결과: 나이, 성별, 좌심실 구출율, 신장 기능, 당뇨, 고혈압, 흡연력 등의 수술 전 인자들에 있어서 두 군간에 유의한 차이는 없었다. 수술 전 뇌허혈의 과거력은 환자는 두 그룹간에 유의한 차이가 없었으나(I군 31.3%, II군 34.4%, p=0.48), 뇌혈관 MRI상에서의 내경동맥의 협착이 75%가 넘는 환자는 I군이 81명(44.5%), II군이 95명으로(30.0%) 두 군간의 유의한 차이를 나타냈다(p=0.003). 수술 후 새로 발생한 신경 이상의 경우 I군에서 3명, II군에서 2명으로, 두 군간에 유의한 차이를 보이지 않았으나(p=0.258), I군의 경우 모두 일과성 허혈이었던 것에 반해 II군의 경우 모두 색전에 의한 다발성 뇌경색으로 나타났다. 그 외 수술 후 발생한 주요 합병증(p=0.47)및 사망(p=0.40)에 있어서 두 군간에 유의한 차이를 보이지 않았다. II군에서 한 명의 환자에서 수술 중 근위부 분합에 따른 대동맥 박리증이 발생하여 상행대동맥 치환술을 시행하였다. 결론: 비록 수술 후 신경학적 합병증의 빈도가 낮아 통계학적 유의성에 다다르진 못했으나, Heartstring system을 이용한 근위부 문합술은 고식적 문합에 비해 색전에 의한 수술 후 뇌졸중의 합병 및 대동맥 손상의 위험도를 낮출 것으로 기대해 본다.

정상인에서 기능적 뇌 자기공명영상과 확산텐서영상 합성기법을 이용한 피질척수로의 위치에 따른 정량적 분석 (Quantitative Evaluation of the Corticospinal Tract Segmented by Using Co-registered Functional MRI and Diffusion Tensor Tractography)

  • 목적 : 기능적 뇌 자기공명영상 (fMRI)과 확산텐서영상(DTI) 합성기법을 이용하여 피질척수로의 여러 부위에서 정량적 특성을 연구하고자 하였다. 대상 및 방법 : 신경학적 이상이 없는 정상인 10명 (남: 8, 여: 2, 평균연령: 30세, 연령분포 : 22 -38세)을 대상으로 하였다. fMRI는 1.5T를 이용하였으며, 손의 쥐기 펴기를 수행하였다. fMRI와 확산텐서섬유로(DTT)의 합성이 가능한 DtiStudio 프로그램을 이용하여 피질척수로를 3차원 영상화하였다. 이때, 시작 관심영역은 2차원 분할 비등방성(fractional anisotropy, FA) 색지도(color map)에서 fMRI의 운동 수행 시 활성부위가 가장 많은 곳으로, 목표 관심영역은 하부 전방 뇌교의 피질척수 부위로 설정하였다. 정량적 분석을 위하여 관심영역을 부채살부터 연수까지 좌우 각각 5곳에 설정하여 분할 비등방성과 현성 확산계수(ADC)를 측정하였다. 결과 : 모든 대상자는 fMRI에서 일차 감각운동 영역이 주로 활성화되었다. 확산텐서 영상에서 피질척수로의 경로는 일차 감각운동 영역부터 연수까지 주행하였다. 피질척수로의 FA 값은 모든 대상자에서 중뇌와 내측 섬유띠의 후지가 타 부위보다 높았다. 결론 : fMRI와 DTT의 합성기법은 피질척수로 상태의 3차원 영상화 및 각 부위에서 FA와 ADC값을 이용한 정량적 분석이 가능하였다. 앞으로, fMRI와 DTT 합성기법은 뇌손상 환자에서 피질척수로의 명확한 상태를 연구하는 데 유용하게 이용될 것으로 사료된다.

대동맥 박리에서 전방성 뇌 관류와 역행성 뇌 관류의 신경학적 분석 (Analysis of Neurological Complications on Antegrade Versus Retrograde Cerebral Perfusion in the Surgical Treatment of Aortic Dissection)

  • 대동맥 박리에서 완전 순환 정지 하에 대동맥 궁의 확인 및 치환이 필요한 경우가 많다. 이런 경우 뇌의 보호를 위해 역행성 뇌 관류 또는 전방성 뇌 관류를 사용하게 된다. 최근에는 역행성 뇌관류의 한계를 극복하기 위해 전방성 뇌 관류가 보편화되고 있는 실정이다. 저자들은 이 두 가지 방법에 대해 신경학적 이상에 중점을 두고 비교하였다 대상 및 법: 2000년 5월부터 2004년 5월까지 대동맥 박리 환자 중 뇌관류를 시행한 40명을 대상으로 하였으며, 회복과정 및 신경학적 합병증에 관해 비교하였다. 결과: 동맥관 삽관은 전방성 뇌 관류군의 15예 중 10예에서 액와동맥에 시행하고, 역행성 뇌 관류군의 25예 중 24예에서 대퇴 동맥에 시행하였다. 완전 순환정지 시식도 온도와 직장 온도를 비교하여 보면, $17.2^{\circ}C,\;22.8^{\circ}C$ (전방성 뇌 관류군)와 $16.1^{\circ}C,\;19.7^{\circ}C$ (역행성 뇌 관류군)로 전방성 뇌 관류 군이 통계적으로 높게 나타났다 이는 수술시간 및 체외순환시간을 의미 있게 단축시켰다. 회복에서는 통계적 차이가 없었다. 신경학적 합병증의 발생에서도 11예와 13예로 통계적 차이가 없었다. 하지만 영구적 신경계 이상을 진단받은 각 군의 5예를 비교하여 보면, 전방성 뇌 관류군에서 영구적 신경계이상을 보인 5명의 환자 모두가 일상생활에 지장을 받지 않는 상태로 회복되었지만, 역행성 뇌 관류군에서의 5명 모두는 일상생활의 장애를 가진 채 퇴원하였다. 결론: 전방성 뇌 관류는 정방향성 혈류를 유지함으로써 중등도의 체온 저하, 수술시간 및 체외순환시간을 단축시키고 뇌의 기능을 보다 더 보호할 수 있는 것으로 생각된다. 저자들은 향후 전방성 뇌 관류법이 보다 적극적으로 사용되어져야 할 것으로 생각한다..96$이었다. 관상동맥 우회술시 다른 수술이 동반되었던 경우가 총 10예$(6.5\%)$였다. 수술 후 대동맥내 풍선펌프는 21예$(13.6\%)$에서 이용하였으나, 비체외순환하 수술에서는 4예$(5.1\%)$로 줄었으며, 전체 사망환자는 12명$(7.9\%)$이었으나, 2001년 이후에는 111예 중 5명$(4.5\%)$으로 감소하였다. 수술합병증으로 수술 전후의 심근경색증 9예$(5.8\%)$, 저심박출증 17예$(11\%)$, 부정맥 30예$(19.5\%)$등이었다. 결론: 국립의료원 흉부외과에서는 관상동맥 우회술을 시작한 이래 수술경험의 축적, 비체외순환하 관상동맥 우회술의 도입, 내흉동맥 및 요골동맥으로의 이식편 이용 변화에 따라 수술성적이 향상되었음을 알 수 있으며, 향후 더 많은 임상경험의 축적 및 장기 추적 관찰이 필요하다고 사료된다.보였으며, 난중, 난황색, 난백고 및 Haugh unit는 처리 간 차이(p>0.05)가 없었다.이고, 환자 1인당 Wedge filter의 교체작업이 $1{\sim}2$회일 때 10MV의 경우 연간선량이 $0.08{\sim}0.4mSv$로 평가되었으며, 15MV의 경우 $0.27{\sim}1.36mSv$로 평가되어 작업종사자의 연간 허용선량인 20mSv에 비해 안전한 것으로 평가되었다.서 정상조직이 적게 조사되었다. 결과 : 기존의 ICRU계획은 그 효과 및 안전성이 입증되었음에도 불구하고 CT를 이용한 CTV계획 등을 적용 한다면 잔류종양이 적은 경우 정상조직에 대한 조사를 줄이면서 잔류종양에 목표선량을 조사할 수 있을 것이다. 다만 잔류종양이

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure.
