Clinical observation was done on 1092 cases of cerebrovascular disease which were confirmed by Brain CT scan in Oriental Medical Hospital in Kyung Hee Univ. from May 1987 to May 1988. Specially, clinical prognosis of 250 patients who had been hospitalized for over 4 weeks, were obserbed. The results were obtained as follows; 1. In this study, Occlusive CVD was 77.9%, Cerebral hemorrhage was 18.8%, Subarachnoid hemonhage was 0.8%. 2. The ratio of male to female was 1.4:1. In the age distribution, 60th decade, 50th decade, 40th decade, 30th's, 20th's were in order of frequency and specially 60th decade was 35.53% over 70th decade was 17.1% in ratio. 3. The most common preceding disease of CVD was hypertension (54.21%) and diabetes mellitus (19.96%) was second. 4. Almost, the duration of hospitalization was 2-4 weeks in 34.8%, within 4 weeks in 78.02%. 5. Primary attack was 75.7%, 2nd attack was 17.9%, over 3rd attack was 3.1% in ratio of recurrence. 6. The level of consciousness was Grade I in 96.4%, Grade II in 3.2%, Grade III in 1% at attack. 7. A few complications of C.V.A. were observed in the studies: pneumonia was noted frequently in 3.2%, bed sore, urinary tract infection, gastro intestinal bleeding in order of frequency. 8. The ratio of neurologic deficiency in occlusive CVD decreased from 51.9% to 29.3% in upper limb, 52.6% to 24.4% in lower limb, and that in cerebral hemorrhage decreased from 69.5% to 25% in upper limb, 50% to 20% in lower limb. 9. The ratio of left side hemiplegia to right was 1.04:1 in male, 1:1.18 in female. 10. The herb medications for C.V.A. were various Chungg-Paesagantang, Sunghanggeonggisan were used most frequently to Chungyeold, Geopung, Soongi, Haldam and Chungsimtang, the drugs for Bogiheol were used as discharge. In these oriental medical therapy of C.V.A. objective diagnosis and more various therapeutic method must be obtained through east-west medical co-operation.
Purpose : To evaluate clinical usefulness of facial soft tissue thickness measurement using 3D computed tomographic images. Materials and Methods : One cadaver that had sound facial soft tissues was chosen for the study. The cadaver was scanned with a Helical CT under following scanning protocols about slice thickness and table speed; 3 mm and 3 mm/sec, 5 mm and 5 mm/sec, 7 mm and 7 mm/sec. The acquired data were reconstructed 1.5, 2.5, 3.5 mm reconstruction interval respectively and the images were transferred to a personal computer. Using a program developed to measure facial soft tissue thickness in 3D image, the facial soft tissue thickness was measured. After the ten-time repeation of the measurement for ten times, repeated measure analysis of variance (ANOVA) was adopted to compare and analyze the measurements using the three scanning protocols. Comparison according to the areas was analyzed by Mann-Whitney test. Results : There were no statistically significant intraobserver differences in the measurements of the facial soft tissue thickness using the three scanning protocols (p>0.05). There were no statistically significant differences between measurements in the 3 mm slice thickness and those in the 5 mm, 7 mm slice thickness (p>0.05). There were statistical differences in the 14 of the total 30 measured points in the 5 mm slice thickness and 22 in the 7 mm slice thickness. Conclusion : The facial soft tissue thickness measurement using 3D images of 7 mm slice thickness is acceptable clinically, but those of 5 mm slice thickness is recommended for the more accurate measurement.

Distribution of Western medicine treatment ; fluid therapy 163(35.5%), antihypertensiv 100(21.8%), antibiotics 53(11.6%), oral hypoglycemic agents and insulin preperations 46(10.1%) and the others 95(20.8%) 7. Distribution of disease of consult from Western medicine to Oriental medicine ; obesity 14(34.2%), liver disease 13(31.7%), lumbago 8(19.5%) and the others 6(14.6%) Distribution of Oriental medicine treatment ; Acupuncture and Moxibustion 25(38.5%), astarvation cure 22(33.8%), Oriental herbs 18(27.7%) 8. Distribution of the admission period ; 1-10 days 148(37.6%), 11-20 days 105(26.6%), 21-30 days 69(17.5%) and the others 72(18.3%).
Chiari malformation은 하부 뇌간과 소뇌가 대공 속으로 들어가 하향 편위의 소견을 보이는 중추 신경계의 기형으로 3가지 type으로 분류된다. 본 증례는 7세 10개월 된 여환이 10일전부터 칫솔질시 상악 우측 어금니가 아프고 얼굴이 약간 붓고 아프다는 주소로 내원하였는데, 임상구강검사결과 상악 우측 제1대구치 원심에 치은낭이 형성되어 있었고 동요도와 동통이 존재하였다. 악골 방사선사진검사결과 상악 구치부의 골밀도가 정상에 비해 낮으며, 특히 좌우측 제1대구치 하방의 골의 부재가 보였다. 유년성 치주염의 가진 하에 치주치료 중 전신질환과의 연관성 문진으로 Chiari malformation의 의과적 병력을 확인한 후 3차원 전산화단층촬영을 시행하였다. 두개골이 전반적으로 않으며 다수의 골결손이 관찰되고 대공이 다소 커져있으며 후두골과 상악골의 골밀도가 감소되어 있었고 특히 상악 구치부의 치조골이 거의 없어 상악 좌우측 제1대구치가 부유치처럼 보였다. 이에 본원 신경외과로 협의진료를 의뢰하여 계속적으로 관찰하고 있는 중이다. 소아에서 치주염이 의심될 때 leukemia, hystiocytosis X, hypoposphatasia 등의 잠재된 전신질환과 연관되어 있을 수 있으므로 적절한 검사를 시행해야 하며 전신질환의 근본적인 원인을 치료할 수 있도록 고려해야 한다.
In order to find the effects of physical therapy on the recovery of motor functions and mental state, a survey was conducted to 63 patients who, diagnosed as stroke by brain CT, had been hospitalized in a university medical center located in Jeonbuk provincial area from December 2000 to August 2001. The outcomes of the survey are as follows: 1. The subjects of the study were composed of 35 males(55.6%) and 28 females(44.4%), with 34(54.0%) below 60 in age and 29(46.0%) older than 60 years. 2. As to type of lesion, the surveyed patients were divided into 28(44.4%) with cerebral infarction, 24(38.1%) with cerebral hemorrhage, and 11(17.5%) with subarachnoid hemorrhage. In terms of size of lesion, they were divided into 29(46.0% with less than tan 1cm, 15 (23.8%) with $1{\sim}2cm$ and 19(30.2%) with longer than 2cm. Thirty-five patients(55.7%) reported paralysis in their right side, while 28(44.8%) complained paralysis in their left side. No recidivation was reported by 51(81.1%) while recidivation was asserted by 12(19%). Among them, 27(42.9%) had no past history but 36(57.1%) had such. 3. Regarding the time hour onset to admission to physical therapy, 46patients(73.0%) experienced it for less than one month, while 17(27.0%) for longer than one month. In terms of period of physical therapy, 30(47.6%) underwent the therapy for less than one month, 18(28.6%) for 1-2 months and 15(23.8%) for longer than 2 months. As is shown in the above study, the longer the period of physical therapy is, the more changes might occur in points of MMSE-K and MMAS. It may, therefore, be concluded that sufficiently longer period of physical therapy ensures the increased recovery of physical functions from stroke.
Purpose: Doxylamine is antihistamine drug that is used as a hypnotic. It is also used for suicidal attempts because it can be easily purchased at the pharmacy without a prescription. There were many articles about the complications after doxylamine intoxication such as a rhabdomyolysis, but only a few articles have reported on seizure. We reviewed the cases of doxylamine intoxication with seizure that were treated in the emergency department. Methods: We reviewed the medical records of the patients who were over 15 years old and who were intoxicated by doxylamine at 3 emergency medical centers from January 2006 to June 2010. We reviewed the patients' age, gender, the dose of doxylamine ingested, if gastrointestinal decontamination was done, the time from intoxication to hospital arrival, the seizure history, treatment of seizure, the electroencephalography (EEG) results, the brain computed tomography (CT) results and the blood test results. Results: There were 168 patients who were intoxicated by doxylamine during the study period. Twelve patients had a seizure episode. The differences between the patients who developed seizure and the patients who did not were the dose and the serum levels of sodium and creatinine. The only clinically meaningful difference was the amount of doxylamine. The amount of doxylamine ingested (>29 mg/kg) predicted the development of seizure with a sensitivity of 75% and a specificity of 92% on the ROC curve. One patient among the seizure patients expired in the emergency department. Conclusion: In case of doxylamine intoxicated patients, there is close relationship between seizure and ingested amount, so close observation needs to be done for the patients who ingest too much because doxylamine can cause death. Further prospective studies are needed for doxylamine intoxicated patients with a seizure episode.
Objective : Penumbra ACE68 reperfusion catheter is a new large bore aspiration catheter used for reperfusion of large vessel occlusion. The objective of this study was to investigate the efficacy of this catheter in comparison to that of previous Penumbra catheters in patients with acute ischemic stroke related to internal carotid artery (ICA) occlusion. Methods : Data of all eligible patients who received endovascular treatment (EVT) for ICA occlusion using Penumbra aspiration catheters between January 2015 and December 2018 were retrospectively reviewed. After dividing into two groups according to use of penumbra ACE68, baseline characteristics of patients, successful recanalization rate, puncture to recanalization time, and switch to stent base technique rate were assessed. Successful recanalization was defined by a thrombolysis in cerebral infarction (TICI) score ${\geq}2b$ and favorable functional outcome was defined according to modified Rankin scale (score, 0-2). Results : ACE68 reperfusion catheter was used in 29 of 75 eligible patients (39%). The puncture to recanalization time was significantly shorter ($26{\pm}18.2$ minutes vs. $40{\pm}24.9$ minutes, p=0.011) and the rate of switch to stent-based retrieval was significantly lower (3% vs. 20%, p=0.046) in ACE68 catheter group. Moreover, although not statistically significant, the successful recanalization rate was higher (83% vs. 76%, p=0.492) in ACE68 catheter group. Favorable functional outcome was observed in 48% of patients treated with ACE68 reperfusion catheter and in 30% of patients treated using other Penumbra systems (p=0.120). Baseline Alberta Stroke Program Early CT Scores ${\geq}8$ (odds ratio [OR], 9.74; 95% confidence interval [CI], 1.72-54.99; p=0.010) and successful recanalization (OR, 10.20; 95% CI, 1.13-92.46; p=0.039) were independent predictors of favorable outcome. Conclusion : EVT using ACE68 reperfusion catheter can be considered a first-line therapy in patients with acute ICA occlusion as it can achieve rapid recanalization and reduce the frequency of conversion to stent-retrieve therapy.
1. Purpose : The purpose of this study was done to compare the sex, past history, hematologic relationship between the Cerebral infarction patients and the Cerebral hemorrhage patients. 2. Methods : We selected the two study groups. The one is 20 Cerebral infarction patients and the other 20 Cerebral hemorrhage patients with confirmed by Brain CT or MRI. We made an investigation into past history. We examined and made a comparative study of CBC, LFT, Na, K, Cl in the both group. 3. Result & Conclusion : In the Cerebral hemorrhage group, there were many patients with Hypertension and Diabetes mellitus than the Cerebral infarction group. In the Cerebral infarction group, there were many patients with abnormal RBC count with no significant. Also, ESR is higher than the other group. Between the Cerebral infarction and the other group, we discovered significant cases with abnormal triglyceride, ALP in the Cerebral infarction group(p<0.05). Abnormal triglyceride is known one of important risk factor of Cerebral infarction, but this study was significant in the Cerebral hemorrhage group. Also, ALP was significant in the Cerebral hemorrhage group. Therefore more extensive research is needed.
