Relationship Between Non-alcoholic Fatty Liver Disease and Decreased Bone Mineral Density: A Retrospective Cohort Study in Korea

  • Objectives: The aim of this retrospective cohort study was to investigate whether non-alcoholic fatty liver disease (NAFLD) was associated with incident bone mineral density (BMD) decrease. Methods: This study included 4536 subjects with normal BMD at baseline. NAFLD was defined as the presence of fatty liver on abdominal ultrasonography without significant alcohol consumption or other causes. Decreased BMD was defined as a diagnosis of osteopenia, osteoporosis, or BMD below the expected range for the patient's age based on dual-energy X-ray absorptiometry. Cox proportional hazards models were used to estimate the hazard ratio of incident BMD decrease in subjects with or without NAFLD. Subgroup analyses were conducted according to the relevant factors. Results: Across 13 354 person-years of total follow-up, decreased BMD was observed in 606 subjects, corresponding to an incidence of 45.4 cases per 1000 person-years (median follow-up duration, 2.1 years). In the model adjusted for age and sex, the hazard ratio was 0.65 (95% confidence interval, 0.51 to 0.82), and statistical significance disappeared after adjustment for body mass index (BMI) and cardiometabolic factors. In the subgroup analyses, NAFLD was associated with a lower risk of incident BMD decrease in females even after adjustment for confounders. The direction of the effect of NAFLD on the risk of BMD decrease changed depending on BMI category and body fat percentage, although the impact was statistically insignificant. Conclusions: NAFLD had a significant protective effect on BMD in females. However, the effects may vary depending on BMI category or body fat percentage.

  • Among aging diseases, the most basic problem is a decrease in bone mineral density. Patients with osteoporosis are steadily increasing in the world. This study was investigated to show the relationship between osteoporotic effects and oxidative damage. Water extracts of 15 medicinal plants and ethanol extracts of 14 medicinal plants with known anti-osteoporotic effects, were tested for their radical scavenging activity using DPPH, ABTS, SRSA and FRAP assay. Water extract of Cornus officinalis, Rubus coreanus and ethanol extract of Rubus coreanus, Viscium album var. coloratum, Cimicifuga heracleifolia showed about 15-20 mg/g of total phenolic contents. Water extract of Cornus officinalis, Rubus coreanus and Epimedium koreanum showed high radical scavenging activity. Ethanol extract of Drynaria fortunei, Cornus officinalis, Rubus coreanus, Gentiana scabra and Astragalus membranaceus showed high radical scavenging activity. Water extract of Drynaria fortunei, Cornus officinalis, Nelumbo nucifera, Epimedium koreanum, and Gentiana scabra showed very strong protective effect against oxidative damage to DNA. These results suggest the correlation between anti-osteoporotic effects and antioxidative effects.

Effect of Cinnamomum Cassia on Cartilage Protection in Rabbit and Human Articular Cartilage

  • Background & Objective: Articular cartilage is a potential target for drugs designed to inhibit the activity of matrix metalloproteinases (MMPs) to stop or slow the destruction of the proteoglycanand collagen in the cartilage extracellular matrix. The purpose of this study was to investigate the effects of Cinnamomum cassia in inhibiting the release of glycosaminoglycan (GAG), the degradation of collagen, and MMP activity in rabbit and human articular cartilage explants. Methods: The cartilage-protective effects of Cinnamomum cassia were evaluated by using glycosaminoglycan degradation assay, collagen degradation assay, colorimetric analysis of MMP activity, measurement of lactate dehydrogenase activity and histological analysis in rabbit cartilage explants culture. Results: Interleukin-1a (IL-1a) rapidly induced GAG, but collagen was much less readily released from cartilage explants. Cinnamomum cassia significantly inhibited GAG and collagen release in a concentration-dependent manner. Cinnamomum cassia dose-dependently inhibited MMP-1, MMP-3 and MMP-13 activities from IL-1a-treated cartilage explants culture when tested at concentrations ranging from 0.02 to 1 mg/ml. Conclusion : These results indicate that Cinnamomum cassia inhibits the degradation of proteoglycan and collagen through the down regulation of MMP-1, MMP-3 and MMP-13 activities of IL-1a-stimulated rabbit and human articular cartilage explants.

음곡 육종용 약침이 난소적출 생쥐의 골다공증에 미치는 영향 (Effects of Herbal-Acupuncture with Cistanches Herba Extract at KI10 (Eumgok) on Osteoporosis in Ovariectomized Mice)

  • Objective & Methods : This study aimed to evaluate the effects of Cistanches Herba herbal-acupuncture (CH-HA) at KI10 (Umgok) on osteoporosis induced by ovariectomy in mice. The results were as follows; Results : CH-HA at KI10 significantly inhibited the overgrowth of tibia, the elevation of serum osteocalcin level, the reduction of tibial Ca and P level, the increase of tibial osteoclast like cells, the reduction of tibial TBV (trabecular bone volume), the overgrowth of tibial GPL (growth plate length) in ovariectomized mice. NP at KI10 significantly inhibited the reduction of BMD (bone mineral density) in ovariectomized mice, but CH-HA did not. Conclusion : Taken together, CH-HA at KI10 has a protective or therapeutic effect for osteoporosis in ovariectomized mice. Therefore, it is suggested that CH-HA may be an useful therapeutics in clinical filed after further researches.

Protective Effect of Vascular Endothelial Growth Factor on Focal Cerebral Ischemia in Rats

  • This study aimed to investigate the cerebroprotective effect of vascular endothelial growth factor (VEGF) on permanent focal cerebral ischemia in Sprague-Dawley rats. Right middle cerebral artery (MCA) was occluded for 6 and 24 hours by an intraluminal monofilament technique. An open cranial window was made on the right parietal bone for determination of continuous changes in regional cerebral blood flow (rCBF) by laser-Doppler flowmetry. The infarct size was morphometrically determined using the 2,3,5-triphenyltetrazolium chloride technique. Brain edema was determined by measuring brain water content. In normal rats, rCBF was significantly increased by intravenous infusion of VEGF for 10 minutes. The VEGF-induced increase in rCBF was significantly inhibited by pretreatment with suramin, a heparin-binding growth factor inhibitor as well as $N^{\omega}-nitro-L-arginine$, a nitric oxide synthase inhibitor. In focal cerebral ischemic rats, the amplitude of decrease in rCBF during ischemic period was significantly less in VEGF-treated group, compared with that in vehicle-treated group. The cerebral infarct size was reduced by VEGF in a dose-dependent manner. The brain edema formation was dose-dependently reduced by VEGF in 24-hour MCA occlusion group but not in 6-hour MCA occlusion group. It is suggested that VEGF not only improves the rCBF during cerebral ischemic period but also reduces the brain edema formation, and thereby exert a protective effect on focal cerebral ischemia in rats.

Deficiency of Formyl Peptide Receptor 2 Retards Hair Regeneration by Modulating the Activation of Hair Follicle Stem Cells and Dermal Papilla Cells in Mice

  • Hair loss is one of the most common chronic diseases, with a detrimental effect on a patient's psychosocial life. Hair loss results from damage to the hair follicle (HF) and/or hair regeneration cycle. Various damaging factors, such as hereditary, inflammation, and aging, impair hair regeneration by inhibiting the activation of hair follicle stem cells (HFSCs) and dermal papilla cells (DPCs). Formyl peptide receptor 2 (FPR2) regulates the inflammatory response and the activity of various types of stem cells, and has recently been reported to have a protective effect on hair loss. Given that stem cell activity is the driving force for hair regeneration, we hypothesized that FPR2 influences hair regeneration by mediating HFSC activity. To prove this hypothesis, we investigated the role of FPR2 in hair regeneration using Fpr2 knockout (KO) mice. Fpr2 KO mice were found to have excessive hair loss and abnormal HF structures and skin layer construction compared to wild-type (WT) mice. The levels of Sonic hedgehog (Shh) and β-catenin, which promote HF regeneration, were significantly decreased, and the expression of bone morphogenetic protein (Bmp)2/4, an inhibitor of the anagen phase, was significantly increased in Fpr2 KO mice compared to WT mice. The proliferation of HFSCs and DPCs was significantly lower in Fpr2 KO mice than in WT mice. These findings demonstrate that FPR2 impacts signaling molecules that regulate HF regeneration, and is involved in the proliferation of HFSCs and DPCs, exerting a protective effect on hair loss.

파골세포 분화에 복령 추출물이 미치는 영향 (Effect of Hoelen in RANKL-induced Osteoclast Differentiation)

  • Osteoporosis is an important public health issue in postmenopausal women. It is a major public health concern and is widely believed that osteoporosis results from imbalance between bone resorption and bone formation. Recently natural products from plants have been extensively studied as therapeutic drugs to treat and prevent various diseases. Hoelen (scientific name, Poria cocos) is a mushroom that is used in traditional Chinese medicine. Hoelen exhibits anti-inflammatory activity and has a protective effect on tumor progression. However, the effect of hoelen in osteoclast differentiation remains unknown. Thus, we examined the effect of hoelen in receptor activator of nuclear factor-${\kappa}B$ ligand (RANKL)-induced osteoclast differentiation. Hoelen significantly inhibited RANKL-induced osteoclast differentiation in bone marrow-derived macrophages (BMMs) in dose dependent manner without toxicity. Also, we showed that hoelen significantly inhibited the mRNA expression of tartrate-resistant acid phophatase (TRAP) and nuclear factor of activated T cells 1 (NFATc1) in BMMs treated with RANKL. In Particular, hoelen greatly inhibited the protein expression of NFATc1. Ectopic expression of NFATc1 partially reverses hoelen-mediated inhibition of osteoclast differentiation. Taken together, our results demonstrated that hoelen may be useful treatment option of bone-related disease such as osteoporosis, reumatoid arthritis, and periodontitis.

난소 절제한 흰쥐에서 도라지 추출물이 골 대사에 미치는 영향 (Effect of Platycodon Grandiflorum A. Extract in Bone Metabolism in Ovariectomized Rats)

  • 여성의 폐경에 의한 estrogen의 감소는 골다공증을 유발시킨다고 알려져 있다. 최근 새로운 식품으로 각광받고 있는 도라지 추출물을 인위적 갱년기 장애를 유발시킨 흰쥐에 투여하여 혈 중 ALP활성 변화와 골 중의 collagen의 함량 및 collagen 가교물질 pyridinoline과 deoxypyridinoline 함량 변화를 측정하여 그 효과를 검토해 보았다. 도라지 추출물을 갱년기를 유도한 흰쥐에 투여하여 혈 중 ALP활성 변화에 미치는 영향을 검토한 결과 난소절제에 의하여 ALP활성이 증가하였으나, 도라지 추출물 투여한 군에서 ALP활성이 억제되는 것을 확인할 수 있었다. 또한 난소 절제에 의해 연골의 collagen 함량이 감소되었는데, 이는 도라지 추출물을 투여함으로써 정상적인 수치로 증가하는 경향을 나타내어 도라지 추출물이 골 대사와 관련된 골 대사 질환에 유효한 효과를 나타낼 것으로 추측된다. 또한 collagen 가교물질로써 골대사의 biomaker인 pyridinoline 및 deoxypyridinoline의 골 중의 함량은 난소절제에 의해 감소하였고, 도라지 추출물 투여에 의해 증가하여 estrogen부족으로 인한 골 손실에 도라지 추출물이 유익한 효과를 가지는 것으로 보이며, 이는 도라지 중에 함유되어 있는 phytoestrogen에 의한 것으로 추측되어 앞으로 구체적인 검토가 필요한 것으로 사료된다.

초생추에서의 납의 독성과 철·구리·아연 및 단백질과의 상호작용 (Effects of Over-dosed Lead and its Interaction with Iron, Copper, Zinc or Protein Supplement in Chicks)

  • The protective effects of high levels of dietary iron, copper, zinc or protein on lead toxicity were studied In chicks. Growth retardation, reduction of feed intake, anemia and accumulation of lead in the bone and kidney were observed in chicks fed a diet containing 500mg lead as chloride per kg of feed for 42 days. Early changes due to ingested lead were inhibition of red blood cell ${\delta}$-aminolevulinic acid dehydrase at all doses and no effect of iron, copper, zinc or protein addition were observed. Tibia lead accumulation was reduced in chicks receiving additional dietary iron or zinc compared to the lead only group but increased in chicks given supplementary protein. Decreased body weight gain was overcome by supplementary zinc or protein in chicks fed lead but not by supplementary iron. Overall the results of this study show that lead poisoning can be partly reduced by providing supplementary iron, zinc or protein, but the interaction of these element remained to be elucidated.

Effects of Aralia cordata Thunb. on Proteoglycan Release, Type II Collagen Degradation and Matrix Metalloproteinase Activity in Rabbit Articular Cartilage Explants

Aralia cordata Thunb. significantly inhibited GAG and collagen release in a concentration-dependent manner. Aralia cordata Thunb. dose-dependently inhibited MMP-3 and MMP-13 expression and activities from IL-1a-treated cartilage explants cultures when tested at concentrations ranging from 0.02 to 0.2 mg/ml. Aralia cordata Thunb. had no harmful effect on chondrocytes viability or cartilage morphology in cartilage explants. Histological analysis indicated that Aralia cordata Thunb. reduced the degradation of the cartilage matrix compared with that of IL -1a-treated cartilage explants.
