폐경 후 류마티스 관절염 여성의 골량감소 판별요인 (Determinants of Reduced Bone Mass in Postmenopausal Women with Rheumatoid Arthritis)

  • This study was carried out to identify the important risk factors for reduced bone mass of postmenopausal RA patients and to develop discriminant function which can classify postmenopausal RA patients with either reduced or normal bone mass. Through the literature review, individual characteristics such as age, body weight, height, age of menarche, duration of menopause, gravity, parity, and breast feeding period and factors of life style such as milk consumption exercise habit, alcohol intake, cigarette smoking, coffee consumpt ion , disease activity, corticosteroid therapy were identified as influencing factors of reduced bone mass in RA patients Sixty eight postmenopausal women with rheumatoid arthritis aged between 42 and 76 were selected among those who checked bone mineral density in lumbar spine and femur from october, 1998 to Apr il, 1999 at Dong-a university hospital in Pusan. Assessment of disease activity, duration of disease and corticosteroid therapy were made by the same rheumatologist and included Ritchie articular index, erythrocyte sedimentation rate, and C-reactive protein on measuring bone mineral density. Cumulative steroid dosage was calculated from the daily dosage multiplied by t h e number of days received. The information of other risk factor including health assessment score, individual characteristics and life style factors were collected by questionnaire. Bone mineral density(BMD) was measured using DXA at lumbar spine and femoral Ward's triangle. Discriminant function(regression equation) was developed for estimating the likelihood of the presence or absence of reduced bone mass. The results are as follows: Among the subjects, thirteen(19.1%) exhibited osteoporosis in lumbar spine and twenty four(35.3%) exhibited osteoporosis in femoral Ward's triangle. For lumbar spine, the variables significant were age, body weight, health assessment score, while for femoral Ward's triangle, age, body weight, duration of disease. But disease activity and corticosteroid therapy were not signigicant to distinguish reduced bone mass from normal bone mass. When the discriminant function was evaluated by comparing the observed out come with predicted out come, the discriminant function correctly classified 85.4% of patients with reduce bone mass and 63.0% of patients with normal bone mass in the lumbar spine and 100% of patients with reduced bone mass and 9.1% of patients with normal bone mass in the femoral Ward's triangle. In summary, we found that osteoporosis in postmenopausal women with RA is more evident at the femur than the lumbar spine. Also the important discriminant factors of reduced bone mass postmenopausal women with RA were age, body weight , duration of disease and health disability. In nursing situation, the efforts to improve of functional capacity of postmenopausal women with rheumatoid arthritis should be considered to prevent osteoporosis and fractures. Also we recommend those postmenopausal women with RA who are classified as a group of the reduced bone mass in the discriminant function should examine the bone mineral density to further examine the usefulness of this discriminant function.

Region of Interest Localization for Bone Age Estimation Using Whole-Body Bone Scintigraphy

  • In the past decade, deep learning has been applied to various medical image analysis tasks. Skeletal bone age estimation is clinically important as it can help prevent age-related illness and pave the way for new anti-aging therapies. Recent research has applied deep learning techniques to the task of bone age assessment and achieved positive results. In this paper, we propose a bone age prediction method using a deep convolutional neural network. Specifically, we first train a classification model that automatically localizes the most discriminative region of an image and crops it from the original image. The regions of interest are then used as input for a regression model to estimate the age of the patient. The experiments are conducted on a whole-body scintigraphy dataset that was collected by Chonnam National University Hwasun Hospital. The experimental results illustrate the potential of our proposed method, which has a mean absolute error of 3.35 years. Our proposed framework can be used as a robust supporting tool for clinicians to prevent age-related diseases.

  • The cosmetic and behavioral aspects of aging have become increasingly evident over the years. Physical aging in people can easily be observed on their face, posture, voice, and gait. In contrast, bone aging only becomes apparent once significant bone degeneration manifests through degenerative bone diseases. Therefore, a more accurate and timely assessment of bone aging is needed so that the determinants and its mechanisms can be more effectively identified and ultimately optimized. This study proposed a deep learning approach to assess the bone age of an adult using whole-body bone scintigraphy. The proposed approach uses multiple inputs deep neural network architectures using a loss function, called mean-variance loss. The data set was collected from Chonnam National University Hwasun Hospital. The experiment results show the effectiveness of the proposed method with a mean absolute error of 3.40 years.

  • Objectives This study aimed to evaluate the correlation between pediatric weakness scale scores and growth indices. Furthermore, we intended to calculate the cut-off values for diagnosing Kidney Weak Children through growth indices. Methods A total of 193 patients, aged 6 - 12 years, underwent both the pediatric weakness scale and bone age assessment, and the correlations between the pediatric weakness scale and growth indices were analyzed. Moreover, the cut-off values for diagnosing Kidney Weak Children were calculated. Differences in bone age and predicted height between the normal group and Kidney Weak Children, classified according to the calculated cut-off values, were analyzed. Results The weakness of the Shin or kidney system was significantly correlated with the height percentile, weight percentile, differences in current height percentile and mid parental height percentile, differences in predicted height and mid parental height, and differences in bone age and chronological age. When the criteria for diagnosing Kidney Weak Children is set at "height percentile of ≤25% and weight percentile of ≤25%", the sensitivity and specificity of the cut-off value were maximized. The cut-off values for weakness of the Shin or kidney system were 9 in children aged 6 - 9 years and 10 in children aged 10 - 12 years. Conclusions To diagnose Kidney Weak Children, the cut-off values of weakness of the Shin or kidney system were 9 in children aged 6 - 9 years and 10 in children aged 10 - 12 years.

  • The author investigated the carpal bone and ephiphyses on the roentgenographic films. Several methods have been used in the study of carpal bones but I have calculated the numbers of ossification center. The subjects consisted of 259 boys and 239 girls from 3 to 13 years of age. The results are summerized as follows; 1. The number of ossification center were increased in aging both boys and girls. 2. Ossification tendency was more faster boys than girls and the difference of left and right hand were non significant. 3. The ossification of ephiphyses was completed in 4 years of age.

한의 성장 치료에 따른 성장 지표 변화 (Change of Growth Indicators by the Treatment of Korean Medicine)

  • Objectives We aimed to analyze changes in growth indicators before and after Korean medicine treatment in patients treated at the pediatric department of a hospital. Methods We analyzed the medical records of children and adolescents under 18 years of age who underwent growth assessment between January 1, 2017 and December 31, 2022. Results A total of 21 patients were selected for this study. After treatment, there was a significant increase in the height percentile, whereas bone age-chronological age (BA-CA) and predicted adult height (PAH) did not show significant changes. No major adverse reactions were observed during the treatment. Growth reassessment was conducted twice for 10 participants. When comparing the growth indicators between the assessment sessions, the height percentile showed an increasing trend between the initial and the first growth reassessment. However, there were no significant differences between BA-CA and PAH across the different assessment periods. Conclusions There is a need to establish evidence for the efficacy and safety of continuous Korean medicine growth treatment through the long-term observation of growth indicators in patients undergoing treatment for two or more periods, as well as observational studies on liver and renal function indicators.

Consequence of Synthetic Bone Substitute Used for Alveolar Cleft Graft Reconstruction (Preliminary Clinical Study)

  • Background The outcome of alveolar grafting with synthetic bone substitute (Osteon III) in various bone defect volumes is highlighted. Methods A prospective study was accomplished on 55 patients (6-13 years of age) with unilateral alveolar bone cleft. Osteon III, consisting of hydroxyapatite and tricalcium phosphate, is used to reconstruct the defect. Alveolus defect diameter was calculated before surgery (V1), after 3 months (V2), and finally after 6 months (V3) postsurgery. In the t-test, a significant difference and correlation between V1, V2, and V3 are stated. A p-value of 0.01 is considered a significant difference between parameters. Results The degree of cleft is divided into three categories: small (9 cases), medium (20 patients), and large (26 cases).The bone volume of the clefted site is divided into three steps: volume 1: (mean 18.1091 mm3); step 2: after 3 months, volume 2 resembles the amount of unhealed defect (mean 0.5109 mm3); and the final bone volume assessment is made after 6 months (22.5455 mm3). Both show statistically significant differences in bone volume formation. Conclusion An alloplastic bone substitute can also be used as a graft material because of its unlimited bone retrieval. Osteon III can be used to reconstruct the alveolar cleft smoothly and effectively.

재가 중.노년 여성의 골밀도와 낙상두려움 및 낙상효능감에 관한 연구 (Fear of Falling and Falls Efficacy with Bone Mineral Density in the Middle and Old Aged Women)

  • Purpose: The purpose of this study was to investigate bone mineral density(BMD) and fear of falling and falls efficacy in the middle and old aged women over 50 years. Methods: The subjects consisted of 409 women. One-way ANOVA, Pearson's correlations and multiple regression were used to test the BMD, fear of falling and falls efficacy scale by using SPSSWIN 12.0. The BMD of the calcaneus were measured with peripheral dual energy x-ray absorptiometry(DEXA). Results: The average age was 63 years old and the average T-score was -3.21 in patient with osteoporosis, -1.72 with osteopenia, and .13 with normal. There were significant differences in the status of the BMD according to age(p=.000), height(p=.000), weight(p=.000), married status(p=.000), age of menarche(p=.002), and menopause(p=.002). The fear of falling was related with falls efficacy(r=-.247, p=.01), BMD(r=-.337, p=.01). Falls efficacy($\beta$=-.21, p=.000)and BMD($\beta$=-.26, p=.000) were predicting variables of fear of falling. The model explained 13% of the variance in fear of falling(F=27.38, p=.000). Conclusion: Fear of falling and falls efficacy were related with the bone mineral density. Falls efficacy and BMD may be useful for the predicting fear of falling for women in middle and old age. Further studies with assessment of fall-related risk-factors and a longitudinal study are necessary to assess with falls efficacy, and BMD with age.

Demineralized Bone Matrix (DBM) as a Bone Void Filler in Lumbar Interbody Fusion : A Prospective Pilot Study of Simultaneous DBM and Autologous Bone Grafts

  • Objective : Solid bone fusion is an essential process in spinal stabilization surgery. Recently, as several minimally invasive spinal surgeries have developed, a need of artificial bone substitutes such as demineralized bone matrix (DBM), has arisen. We investigated the in vivo bone growth rate of DBM as a bone void filler compared to a local autologous bone grafts. Methods : From April 2014 to August 2015, 20 patients with a one or two-level spinal stenosis were included. A posterior lumbar interbody fusion using two cages and pedicle screw fixation was performed for every patient, and each cage was packed with autologous local bone and DBM. Clinical outcomes were assessed using the Numeric Rating Scale (NRS) of leg pain and back pain and the Korean Oswestry Disability Index (K-ODI). Clinical outcome parameters and range of motion (ROM) of the operated level were collected preoperatively and at 3 months, 6 months, and 1 year postoperatively. Computed tomography was performed 1 year after fusion surgery and bone growth of the autologous bone grafts and DBM were analyzed by ImageJ software. Results : Eighteen patients completed 1 year of follow-up, including 10 men and 8 women, and the mean age was 56.4 (32-71). The operated level ranged from L3/4 to L5/S1. Eleven patients had single level and 7 patients had two-level repairs. The mean back pain NRS improved from 4.61 to 2.78 (p=0.003) and the leg pain NRS improved from 6.89 to 2.39 (p<0.001). The mean K-ODI score also improved from 27.33 to 13.83 (p<0.001). The ROM decreased below 2.0 degrees at the 3-month assessment, and remained less than 2 degrees through the 1 year postoperative assessment. Every local autologous bone graft and DBM packed cage showed bone bridge formation. On the quantitative analysis of bone growth, the autologous bone grafts showed significantly higher bone growth compared to DBM on both coronal and sagittal images (p<0.001 and p=0.028, respectively). Osteoporotic patients showed less bone growth on sagittal images. Conclusion : Though DBM alone can induce favorable bone bridging in lumbar interbody fusion, it is still inferior to autologous bone grafts. Therefore, DBM is recommended as a bone graft extender rather than bone void filler, particularly in patients with osteoporosis.

폐경 후 정상여성과 폐경 후 류마티스 관절염환자의 골밀도 및 골다공증 위험요인 비교 (Comparison of Bone Mineral Density and Risk Factors of Osteoporosis between Normal and Rheumatoid Arthritis in Postmenopausal women)

  • This study was carried out to compare the bone mineral density and risk factors of osteoporosis between normal and rheumatoid arthritis in postmenopausal women. Sixty-eight postmenopausal patients with rheumatoid arthritis(RA) were compared with 124 postmenopausal normal women. Data were collected from october, 1998 to April, 1999 at Dong-a university hospital in Pusan. From all subjects, individual characteristics such as age, body weight, height, age of menarche, duration of menopause, gravity, parity, and breast feeding period and factors of life style such as milk consumption, exercise, alcohol intake, cigarette smoking, coffee consumption were identified as influencing factors of osteoporosis by questionnaire. From RA patients, health assessment score, Ritchie articular index, erythrocyte sedimentation rate, C-reactive protein and steroid dosage were measured by rheumatologist on measuring bone mineral density. Bone mineral density was measured at the Lumbar spine, femoral neck, femur Ward's triangle, and femur trochanter using dual x-ray absorptiometry. The data was analyzed by using a frequency, t-test, Chi-square, ANCOVA with SPSS PC program. The results could be summarized as follows : 1) There was a significant difference in age and breast feeding period between RA patients and normal women. 2) RA patients took less calcium in the past and practiced less regular exercise in past and present than normal women. 3) There was no difference in lumbar bone mineral density between RA patients and normal women. 4) There was a significant difference in femur Ward's triangle and femur trochanter between RA patients and normal women after adjustment for age and breast feeding period. 5) The prevalence of osteoporosis of all subjects was the highest in femur Ward's triangle. In summary, our findings suggest that the bone mineral densities of femur Ward's triangle and trochanter in postmenopausal women with RA is significantly lower than normal women. Also the exercise participation rate of postmenopausal women with RA is lower than normal women. For the further study, we recommend to develop exercise program that improve the bone mineral density in femur Ward's triangle and trochanter and to test the effect of that exercise program.
