The effect of fibroblast growth factor receptor inhibition on resistance exercise training-induced adaptation of bone and muscle quality in mice

  • Aging in mammals, including humans, is accompanied by loss of bone and muscular function and mass, characterized by osteoporosis and sarcopenia. Although resistance exercise training (RET) is considered an effective intervention, its effect is blunted in some elderly individuals. Fibroblast growth factor (FGF) and its receptor, FGFR, can modulate bone and muscle quality during aging and physical performance. To elucidate this possibility, the FGFR inhibitor NVP-BGJ398 was administrated to C57BL/6n mice for 8 weeks with or without RET. Treatment with NVPBGJ398 decreased grip strength, muscular endurance, running capacity and bone quality in the mice. FGFR inhibition elevated bone resorption and relevant gene expression, indicating altered bone formation and resorption. RET attenuated tibial bone resorption, accompanied by changes in the expression of relevant genes. However, RET did not overcome the detrimental effect of NVP-BGJ398 on muscular function. Taken together, these findings provide evidence that FGFR signaling may have a potential role in the maintenance of physical performance and quality of bone and muscles.

Correlation of bone quality in radiographic images with clinical bone quality classification (방사선사진에서의 골질과 임상적으로 평가한 골질 분류의 상관관계)

  • Purpose : To investigate the validity of digital image processing on panoramic radiographs in estimating bone quality before endosseous dental implant installation by correlating bone quality in radiographic images with clinical bone quality classification. Materials and Methods : An experienced surgeon assessed and classified bone quality for implant sites with tactile sensation at the time of implant placement. Including fractal dimension eighteen morphologic features of trabecular pattern were examined In each anatomical sites on panoramic radiographs. Finally bone quality of 67 implant sites were evaluated in 42 patients. Results : Pearson correlation analysis showed that three morphologic parameters had weak linear negative correlation with clinical bone quality classification showing correlation coefficients of -0.276, -0.280, and - 0.289, respectively (p<0.05). And other three morphologic parameters had obvious linear negative correlation with clinical bone quality classification showing correlation coefficients of -0.346, -0.488, and -0.343 respectively (p<0.05). Fractal dimension also had a linear correlation with clinical bone quality classification with correlation coefficients -0.506 significantly (p<0.05). Conclusion : This study suggests that fractal and morphometric analysis using digital panoramic radiographs can be used to evaluate bone quality for implant recipient sites.

AN ANALYSIS OF STRESS DISTRIBUTION AROUND THE IMPLANT ACCORDING TO THE BONE QUALITY AND BITE FORCE: FINITE ELEMENT METHOD (저작압이 임프란트 주위골 내 응력분포에 미치는 영향에 관한 연구)

  • Since the early study about the osseointegration, lots of researches have been performed to increase the success rate and the stress around the implant in the jaw bone has been considered as one of the causes of failure. The purpose of this study was to examine the relationship between the implant failure and the stress by analysing the influence of different bone quality and bite force of some foods on the stress distribution around the implant, and to estimate the treatment result according to the bone quality and dietary pattern of patients. Bone quality was divided in 4 groups and models were drawn with the assumption that thread type implant(Nobel Biocare AB, Goteborg, Sweden) of 3.75mm diameter, 13mm length was installed to the bones. Various bite forces were applied to the occlusal surface of superstructure and the stress distributed around the implant were analysed with finite element analysis program. The results were as follows ; 1. The stress was changed proportionally to the bite forces of foods at all measuring points in all load cases. 2. The stress at the marginal bone was higher than that of the other measuring points in all load cases, and it was decreased at the first thread area. 3. The stress at the marginal bone was highest in type IV bone in all load cases. Especially it was twice those of other bone types at the bucco-lingual marginal bone and 50% higher at the mesio-distal marginal bone. 4. The stress at the bucco-lingual sides of the bone around the apical portions of implant showed little differences among the bone types, while type IV bone showed lower stress concentration than the other bone types in the mesio-distal sides. 5. Under the buccal oblique load ($15^{\circ}$ ), the stress at the lingual marginal bone was higher than that of buccal marginal bone, and the difference between the two points was almost same regardless of bone types.

Effect of soy isoflavones supplement on climacteric symptoms, bone biomarkers, and quality of life in Korean postmenopausal women: a randomized clinical trial

  • BACKGROUND/OBJECTIVES: Soy isoflavones are expected to improve menopausal symptoms and osteoporosis in women. However, their efficacy is still inconclusive, and there was limited data for postmenopausal women in South Korea. We examined the effects of soy isoflavones on climacteric symptoms, bone biomarkers, and quality of life in Korean postmenopausal women. SUBJECTS/METHODS: A randomized, double-blind study design was used. Eighty-seven participants who had undergone natural menopause were randomly administered either 70 mg/day isoflavones (n = 43) or placebo (n = 41) for 12 weeks. We assessed the Kupperman index for climacteric symptoms and the menopause-specific quality of life (MENQOL) questionnaire for quality of the life. Biomarkers of bone metabolism were also measured in serum bone-specific alkaline phosphatase (BALP), osteocalcin (OC), N- and C-terminal cross-linking telopeptides of type Ι collagen (NTx, CTx), and urine-deoxypyridinolin (u-DPD). RESULTS: Scores of the Kupperman index were decreased in both the isoflavones group ($-7.0{\pm}15.8$, P = 0.0074) and placebo group ($-6.3{\pm}14.6$, P = 0.0064) during the intervention, but no significant difference was noted between the groups. Regarding the bone formation markers, the level of serum BALP increased by $6.3{\pm}4.1%$ (P = 0.004) and OC increased by $9.3{\pm}6.2%$ (P < 0.001), meanwhile those of the placebo were not changed. For the bone resorption markers, NTx, CTx, and u-DPD were not significantly different in either group. MENQOL was significant decreased in the isoflavone group ($-0.6{\pm}0.5$) and placebo group ($-0.6{\pm}0.4$), with a significant difference between groups (P = 0.0228). CONCLUSIONS: Our study suggests that 70 mg isoflavones supplement has beneficial effects on bone formation markers; however, it showed no benefit compared to the placebo on climacteric symptoms or quality of life.

Evaluation of Hormone Deficiency in Vertebral Body: Analysis of Bone Structure and Quality (호르몬 결핍이 척추체에 미치는 영향 평가: 골의 구조학적 및 질적 요소 분석)

  • This study evaluated the structure and quality of osteoporotic vertebral bone. To induce osteoporosis, eight rats were ovariectomized (OVX). All rats were divided into two groups (Normal group: 4, OVX group: 4). Total lumbar vertebrae for each rat were scanned by in-vivo ${\mu}CT$ at 0, 4 and 8 weeks. Morphological characteristics (BV/TV, Tb.Th, Tb.N, Tb.Sp and SMI) were calculated by in-vivo ${\mu}CT$ image analyzer. Three dimensional finite element models were analyzed to investigate bone strength of OVX and Normal groups. Moreover, the elastic modulus was quantitatively analyzed to evaluate the quality changes of osteoporotic bone. In the OVX group, BV/TV, Tb.Th and Tb.N were significantly decreased at all the lumbar over time (p<0.05). We also investigated a contrary tendency in Tb.Sp and SMI, compared to the above results in each group. A degree of alteration of mechanical characteristics in OVX group was decreased over measuring time (p<0.05). Bone quality presented by distribution of elastic modulus was improved in the Normal group more than OVX group. The findings of the present study indicated that both bone structure and quality of whole lumbar could be tracked and detected by analyzing the morphological and biomechanical characteristics of bones, based on a nondestructive method.

  • Ultrasonic has been proposed as an attractive means of detecting bone loss. There have been several commercial ultrasound devices developed for measuring the heel to predict fracture at other bones. However, these devices select only single point of heel bone as measurement site. It causes poor assessment of bone quality due to the error of transducer positioning. In an effort to improve current ultrasound systems, we evaluated the linear scanning method which provides better prediction of bone quality and an accurate image of bone shape. The system used in this study biaxially scans a heel bone using automated linear scanning technique. The results demonstrated that the values of ultrasound parameters varied with different positions within bone specimen. It has been also found that the linear scanning method could better pre야ct bone quality, eliminating the error of transducer positioning.

  • 뼈의 역학기능을 예측하는 인자(predictive factor) 로서 골밀도(BMD)만으로는 충분하지 않다는 최근의 임상결과는, 골밀도 이외의 새로운 뼈의 강도 및 골절리스크를 지배하는 인자의 중요성을 보여준다. 이와 같은 골역학기능에 대한 골밀도 이외의 부가적인 지배인자를 골질(bone quality)이라고 하는데, 다양한 골질관련인자(bone quality-related factor) 중 하나의 지표로서 뼈의 주성분인 생체아파타이트(BAp)의 결정학적 방향성에 주목, 대표적인 경조직 질환을 해석하였다. 파골세포결손에 의해 대리석증을 유발하는 op/op마우스는 골밀도의 변화뿐만 아니라, 골질의 유의한 변화가 있었다. 즉, 이와 같은 결과는 파골세포결손에 의한 조골세포의 활성저하의 의해 골질이 저하됨을 시사하는 결과이며, 파골세포 과잉의 의해 골다공증을 유발하는 OPG-KO마우스는 골밀도가 급격히 저하됨과 동시에, BAp배향성도 급격히 낮아졌다. 즉, 골대사회전의 상승에 따른 섬유성골(woven bone)의 형성에 의해 BAp의 결정성장이 억제되며, 그 결과 BAp배향성이 저하된다고 사료된다. 이상, 본 연구에서는 대표적인 골 질환조직을 각각의 정상골과 비교함으로써, 골양(BMD)의 변화뿐만 아니라 골질(BAp배향성)의 변화를 발견하였다. 이와 같은 변화는 골질지표로서 BAp배향성이 유효하다는 것을 강하게 시사한다. 따라서 본 연구에 의해 얻어진 견해는 경조직 질환의 병리해명에 적용 가능함과 동시에, 경조직 질환의 진단 응용이나 치료약 개발, 임플란트 개발 등 폭넓은 분야에 유용하다고 할 수 있다.

Effects of Beef Bone Extracts on Quality of Baeck Kimchi (사골 국물이 백김치의 품질에 미치는 영향)

  • This study was conducted to investigate the effect of beef bone extracts on the organoleptic quality and fermentation of Baeck Kimchi. For this study, the organoleptic characteristics of Baeck Kimchi, with addition beef bone extracts were tested, and the changes in the fermentation factors, such as pH, acidity, reducing sugar and total microbial count, were observed during fermentation at 10$^{\circ}C$ and 20$^{\circ}C$. Greater amounts of lipids and soluble nitrogen were extracted to increasing the boiling time for the preparation of the beef bone extracts. However, 1% of the mixed beef bone extracts, prepared by boiling for 5, 7 and 9 hours, was the most acceptable for the Baeck Kimchi fermentation, as assessed from the results of sensory consumer tests. The beef bone extracts enhanced the umami taste of the unfermented Baeck Kimchi. The carbonated taste, sourness and fresh flavor of the fermented Baeck Kimchi were also enhanced. The fermentation rate was increased by the addition of beef bone extracts to Baeck Kimchi.

Quality of life of patients with nasal bone fracture after closed reduction

  • Background: Closed reduction is the standard treatment for nasal bone fractures, which are the most common type of facial bone fractures. We investigated the effect of closed reduction on quality of life. Methods: The 15-dimensional health-related quality of life survey was administered to 120 patients who underwent closed reduction under general anesthesia for nasal bone fractures from February 2018 to December 2019, on both the day after surgery and 3 months after surgery. Three months postoperatively, the presence or absence of five nasal symptoms (nose obstruction, snoring, pain, nasal secretions, and aesthetic dissatisfaction) was also evaluated. Results: The quality of life items that showed significant changes between immediately after surgery and 3 months postoperatively were breathing, sleeping, speech, excretion, and discomfort. Low scores were found at 3 months for breathing, sleeping, and distress. There were 31 patients (25.83%) with nose obstruction, 25 (20.83%) with snoring, 12 (10.00%), with pain, 11 (9.17%) with nasal secretions, and 29 (24.17%) with aesthetic dissatisfaction. Conclusion: Closed reduction affected patients' quality of life, although most aspects improved significantly after 3 months. However, it was not possible to rule out deterioration of quality of life due to complications and dissatisfaction after surgery.
