Intracardiac Foreign Body (Bone Cement) after Percutaneous Vertebroplasty

  • Percutaneous vertebroplasty is a relatively easy and minimally invasive procedure used in treating vertebral fractures. However, the procedure has many complications, one of which is bone cement leakage, which happens frequently. Leakage to the paravertebral venous system, in particular, may lead to especially devastating consequences. Here we report a case of a 65-year-old male patient with an intracardiac foreign body (bone cement) that generated a perforation on the right ventricle, and result in hemopericardium after percutaneous vertebroplasty. We performed open heart surgery to remove the foreign body.

Acellular dermal matrix and bone cement sandwich technique for chest wall reconstruction

  • The authors performed rigid reconstruction using the sandwich technique for full-thickness chest wall defects by using two layers of acellular dermal matrix and bone cement. We assessed six patients who underwent chest wall reconstruction. Reconstruction was performed by sandwiching bone cement between two layers of acellular dermal matrix. In all patients, there was no defect of the overlying soft tissue, and primary closure was performed for external wounds. The average follow-up period was 4 years (range, 2-8 years). No major complications were noted. The sandwich technique can serve as an efficient and safe option for chest wall reconstruction.

과립형 β-삼인산칼슘을 사용한 Brushite계 골시멘트의 보관 조건에 따른 경화 특성 (Effect of Storage Conditions on the Setting Properties of Brushite Bone Cement Containing Granular β-Tricalcium Phosphate)

  • In the bone cement composed of dense granules of $\beta-Ca_3(PO_4){_2}(\beta-TCP)$ and $Ca(H_2PO_4){_2}H_2O$, the compressive strength, setting time and temperature rise were measured to observe the degradation of cement with respect to the stored days before setting. Decreases of compressive strength and temperature rise were observed, while setting time increased with respect to the stored days. The similar trends were repeated with the increase of temperature of storage. Such a change virtually meant the fading of the character of cement and it took place only when the two starting materials were mixed during storage. The degradation could be mitigated taking advantage of granular $\beta$-TCP instead of powdery one. The formation of $CaHPO_4$, which resulted from reaction with ambient humidity, was attributed to the degradation observed during storage. Dependence of the degradation behavior on mixing and temperature during storage was discussed in terms of the driving force for reaction of cement.

Setting Behavior and Drug Release from Brushite Bone Cement prepared with Granulated Hydroxyapatite and β-Tricalcium Phosphate

  • Calcium phosphate bone cement was prepared to contain antibiotics for release after setting using granulated ${\beta}$-tricalcium phosphate (${\beta}$-TCP) and hydroxyapatite (HA). Gentamicin sulfate (GS) solution was infiltrated within the interconnected pores of the granule to avoid affecting the setting reaction and for protection of GS during the setting. Consequently, the setting time and the temperature increase were not affected, regardless of the loading of GS. The release of the GS from the cement was estimated by measuring the concentration at regular intervals from the cement dipped solution. The ${\beta}$-TCP granule loaded with GS exhibited the saturation of accumulative concentration at 16 h. In contrast, the HA granule with GS exhibited steady increase in accumulative concentration of over $10{\mu}g/ml$ at 144 h. Thus, the granulated cement could release the GS greater than the minimum inhibitory concentration of staphylococcus during the prescription peroid of the oral antibiotics.

  • In orthopedic surgery and more especially in total arthroplastie of hip, the fixing of the implants generally takes place essentially by means of constituted surgical polymer cement. The damage of this materiel led to the fatal rupture and thus loosening of the prosthesis in total hip, the effect of over loading as the case of tripping of the patient during walking is one of the parameters that led to the damage of this binder. From this phenomenon we supposed that a remain of bone is included in the cement implantation. The object of this work is to study the effect of this bony inclusion in the zones where the outside conditions (loads and geometric shapes) can provoke the fracture of the cement and therefore the aseptic lousing of the prosthesis. In this study it was assumed the presence of two bones -type inclusions in this material, one after we analyzed the effect of interaction between these two inclusions damage of damage to this material. One have modeled the damage in the cement around this bone inclusion and estimate the crack length from the damaged cement zone in the acetabulum using the finite element method, for every position of the implant under the extreme effort undergone by the prosthesis. We noted that the most intense stress position is around the sharp corner of the bone fragment and the higher level of damage leads directly the fracture of the total prosthesis of the hip.

경피적 척추성형 후 발생한 우심방 내 골 시멘트 색전의 도관경유제거술: 시술 중 두 동강나 우폐동맥으로 이동한 색전 (Transcatheter Removal of Bone Cement Embolism in the Right Atrium after Percutaneous Vertebroplasty: The Embolus Broke in Half and Migrated to the Right Pulmonary Artery Intraoperatively)

  • 골 시멘트 색전은 경피적 척추성형 후 드물지 않게 발생하는 합병증이다. 이는 보통 작은 폐동맥 색전이고 대부분 무증상이기 때문에 치료를 필요로 하지 않는다. 하지만 심장 내에 생긴 골시멘트 색전은 매우 드물며, 치명적인 결과를 야기할 수 있다. 이전 보고된 증례들은 심장 내 골시멘트 색전을 주로 개심수술을 통해 제거하였으며, 중재시술로 제거한 것은 3개 증례만 보고되어 있었다. 따라서 저자들은 경피적 척추성형 후 발생한 우심방 내 6 cm 크기의 골 시멘트 색전을 도관경유로 안전하게 제거한 1예를 보고하고자 한다. 색전이 시술 중에 두 동강나 우폐동맥으로 이동하였으나 두 개의 스네어와 하나의 필터 제거 장치를 이용하여 서로 반대 방향에서 접근시켜 우폐동맥 내의 색전 조각들을 조심스럽게 잡아 끌어낸 후 쉬스와 일렬로 만들어 제거하였으며, 시술 후 합병증은 발생하지 않았다.

Preliminary Report of Three-Dimensional Reconstructive Intraoperative C-Arm in Percutaneous Vertebroplasty

  • Objective : Percutaneous vertebroplasty (PVP) is usually carried out under three-dimensional (2D) fluoroscopic guidance. However, operative complications or bone cement distribution might be difficult to assess on the basis of only 2D radiographic projection images. We evaluated the feasibility of performing an intraoperative and postoperative examination in patients undergoing PVP by using three-dimensional (3D) reconstructive C-arm. Methods : Standard PVP procedures were performed on 14 consecutive patients by using a Siremobil Iso-$C^{3D}$ and a multidetector computed tomography machine. Post-processing of acquired volumetric datasets included multiplanar reconstruction (MPR) and surface shaded display (SSD). We analyzed intraoperative and immediate postoperative evaluation of the needle trajectory and bone cement distribution. Results : The male : female ratio was 2 : 12; mean age of patients, 70 (range, 77-54) years; and mean T score, -3.4. The mean operation time was 52.14 min, but the time required to perform and post-process the rotational acquisitions was 7.76 min. The detection of bone cement distribution and leakage after PVP by using MPR and SSD was possible in all patients. However, detection of the safe trajectory for needle insertion was not possible. Conclusion : 3D rotational image acquisition can enable intra- or post-procedural assessment of vertebroplasty procedures for the detection of bone cement distribution and leakage. However, it is difficult to assess the safe trajectory for needle insertion.

Cement Type 맞춤형 인공고관절의 설계 변수 변화에 따른 Cement 두께 변화와 이에 대한 응력 분석 (The Analysis of Stress and Variation of Cement Thickness as the Variation of Design Variables for Custom-Made Hip Implant)

  • The choice of suitable hip implant is one of important factors in the total hip arthroplasty (THA). In clinical view point, an improper adaptation of hip implant might induce abnormal stress distribution to the bone, which can shorten the lifespan of replaced hip implant. Currently, interest in the custom-designed hip implants has increased as studies reveal the significance of geometric shape of patient's femur in modeling and designing the implants. In this study, we have developed custom-designed hip implant models with various sizes, and analyzed the stress distribution in the bone and bone cement using the Finite Element Method. It was found that minimizing the gap between implant stem and femoral cavity is crucial to minimize the stress concentration in the bone.

Frontotemporal Craniotomy for Clipping of Unruptured Aneurysm Using a Diamond-Coated Thread Wire Saw and Reconstruction Using Calcium Phosphate Cement without Metal Fixation

  • Metal fixation systems for cranial bone flaps cut by a drill are convenient devices for cranioplasty, but cause several complications. We use modified craniotomy using a fine diamond-coated threadwire saw (diamond T-saw) to reduce the bone defect, and osteoplasty calcium phosphate cement without metal fixation. We report our outcomes and tips of this method. A total of 78 consecutive patients underwent elective frontotemporal craniotomy for clipping of unruptured intracranial aneurysms between 2015 and 2019. The follow-up periods ranged from 13 to 66 months. The bone fixation state was evaluated by bone computed tomography (CT) and three-dimensional CT (3D-CT). The diamond T-saw could minimize the bone defect. Only one wound infection occurred within 1 week postoperatively, and no late infection. No pain, palpable/cosmetically noticeable displacement of the bone flap, fluid accumulations, or other complications were observed. The condition of bone fixation and the cosmetic efficacy were thoroughly satisfactory for all patients, and bone CT and 3D-CT demonstrated that good bone fusion. No complication typical of metal fixation occurred. Our method is technically easy and safety, and achieved good mid-term bone flap fixation in the mid-term course, so has potential for bone fixation without the use of metal plates.
