Effects of Polycan on bone Metabolism in healthy Perimenopausal Women: a 12-week Randomized, Double-blind, Placebo-controlled study (폴리칸이 중년 여성의 골대사에 미치는 영향: 12주간의 무작위배정, 이중눈가림, 플라세보 대조 연구)

  • 배경: 골다공증은 골대사의 불균형으로 인해 골 흡수가 골 형성보다 많아져 골밀도가 감소함으로서 발생한다. 골다공증의 이상적인 치료목표는 골형성을 증가시키거나 골소실을 방지하여 골량을 현 상태로 유지하는 것이다. 따라서 향후 발생되는 골소실을 에방하는 것이 골다공증의 원칙적이고 효과적인 치료방법이 될 것이다. 본 연구에서는 흑효모 중 $Aureobasidium$ $pullulans$으로부터 유래한 폴리칸(베타-글루칸)이 중년여성의 골대사에 미치는 영향을 규명하고자 하였다. 연구방법: 골대사에 대한 폴리칸의 효과를 규명하기 위해 12주간의 무작위배정, 이중눈가림, 플라세보 대조 임상연구를 수행하였다. 총 60명(폴리칸 투여군30명, 플라세보 투여군 30명)의 중년 여성 피험자가 등록되어 이 중 총 58명의 피험자가 최종적으로 12주간의 임상연구를 종료하였다. 결 과: 폴리칸(150 mg/d) 투여 12주 후, 폴리칸 투여군은 요 중 Deoxypyridinoline (DPD) 농도가 유의적인 감소를 보였다($P$=0.014). 혈청 중 Osteocalcin(OSC) 농도는 두 군 모두에서 유의적으로 증가하였으며, bone-specific alkaline posphatase (bALP) 와 collagen type 1 cross-linked C-telopeptide (CTx)는 유의적 변화가 보이지 않았다. 폴리칸은 골밀도(BMD)와 혈청 부갑상선 호르몬(iPTH)에 대해 유의적인 변화를 보이지 않았으나, 24시간 요 중 Ca 배설량은 폴리칸 투여군에서 유의하게 감소되었다($P$=0.028). 또한 폴리칸 투여군에서 고밀도지단백 콜레스테롤(HDL-cholesterol) 농도의 증가 경향 및 중성지방(triglyceride)의 유의적인 감소가 보였다. 임상연구 기간 중에 발생한 이상반응은 두 군간에 유의적인 차이를 보이지 않았다. 결 론: 본 연구에서는 폴리칸이 골대사 및 지질에 대해 일부 개선효과가 있음을 보여주었다. 그러나, 골다공증 예방 측면에서 보다 장기적인 임상연구와 피험자 수를 확대하여 골대사 및 지질대사에 대한 폴리칸의 예방적 효과를 규명할 필요가 있을것으로 사료된다.

Preliminary Study on the Osteoporosis Improvement Effect of Compounds Isolated from Oryza sativa L. root Extract in the Ovariectomized Mouse Model (난소 적출 마우스 모델에서 Oryza sativa L.의 뿌리 추출물에서 분리된 화합물의 골다공증 개선 효과에 관한 예비연구)

  • Bisphosphonates, estrogen, and calcium supplements are commonly used medications for postmenopausal osteoporosis, but they are associated with various side effects such as vaginal bleeding, deep vein thromembolism, and breast cancer. In this study, we aimed to investigate the potential of a compound isolated from the roots of Oryza sativa L. to improve osteoporosis using an ovariectomized mouse model. We isolated and identified oryzativol A, a lignan compound, through chemical analysis of an ethanol extract using a bioassay-guided fractionation protocol. We also examined the metabolism, clearance, and CYP enzyme activity of oryzativol A, and found that it showed plasma stability of over 80% at all analysis times, and indicating a low likelihood of inactivation or excretion by the CYP3A4 enzyme. Our results showed that the high-dose group of oryzativol A exhibited a significant increase in bone mineral density compared to the control group. Although the ALP concentration did not differ significantly compared to the control group, it showed a tendency to increase in the high-dose group of oryzativol A. Furthermore, the abnormal ratio of serum Ca/P, caused by osteoporosis, was improved to a level closer to that of the normal group as the dosage of oryzativol A increased. Taken together, these findings suggest that oryzativol A is stable in vivo and has potential as a therapeutic agent for osteoporosis, particularly when administered in high doses.

Effect of Calcium Extracted from Salted Anchovy (Engraulis japonicus) on Calcium Metabolism of the Rat (멸치젓에서 추출한 칼슘이 흰쥐의 칼슘대사에 미치는 영향)

  • This study was conducted to examine the effect of calcium extracted from salted anchovy (Engraulis japonicus) on the calcium metabolism of rats. Sprague-Dawley male rats were fed low-calcium diets (0.15%) for 2 weeks after the adjustment period. Rats were divided into five groups and were fed experimental diet for four weeks. Experimental diets were low calcium (LC, 0.15% $CaCO_3$), 0.5% $CaCO_3$ (CC), seaweed calcium (SC), calcium lactate (LC), anchovy calcium (AC). The low-calcium diet group (LC) showed the lowest weight gain and had no differences among the groups with adequate calcium intake. Calcium retention was lowest in the LC group and higher in the CL, SC, AC groups than in SC groups. Serum alkaline phosphatase (ALP) level was highest in LC group, and significantly low in the CC and AC groups (p<0.05). Parathyroid hormone and osteocalcin levels showed no differences among experimental groups. The urine deoxypyridinoline (DPD) level was lower in AC and CC groups compared to the LC group (p<0.05). The dry weight of the femur showed no significant differences among normal calcium groups. The bone mineral density of the femur in AC and CC group were significantly higher than the LC group (p<0.05). From these results, calcium extracted from salted anchovy can be useful as a calcium supplement comparable with calcium carbonate.

Effects of the Oriental Medicinal Prescriptions, Jahyulyangeuntang and Yanghyuljangeunkeonbohwan, on Calcium Absorption in the Human Colon Carcinoma Cell Line (Caco-2 Cells) (자혈양근탕 및 양혈장근건보환이 인체 소장 상피세포주 (Caco-2) 모델에서 칼슘 흡수에 미치는 영향)

  • Effects of the two oriental medicinal prescriptions, Jahyulyangeuntang (JH) and Yanghyuljangeunkeonbohwan (YH), on intestinal calcium absorption were examined in the human colon carcinoma tell line, Caco-2 cells. Intestinal calcium absorption was evaluated at the level of Ca uptake into the cells across the brush border membranes, as well as at the level of net Ca transport (implying the amount of intestinal Ca transported into the blood stream). When the Caco-2 cells were incubated for 4, 8, 16 and 24 days post seeding, the cells were differentiated continuously, and showed progressively increased activities of Ca uptake (1.13 $\pm$ 0.04, 1.19 $\pm$ 0.02, 1.94 $\pm$ 0.03, and 2.40 $\pm$ 0.12 nmole.mg protein$^{-1}$ .30 min$^{-1}$ , respectively). Pretreatment of confluent Caco-2 cells with 50 $\mu\textrm{g}$/ml of YH for 24 hours resulted in a 30% increase in Ca uptake (p < 0.07), while pretreatment of the cells with the same concentration of JH for 6 hours resulted in a 24% increase (p < 0.05) in Ca uptake, compared to the value for the control cells (2.34 $\pm$ 0.10 nmole.mg protein$^{-1}$ .30 min$^{-1}$ ). When the cells were pretreated with varied concentrations (5-100 $\mu\textrm{g}$/ml) of the test samples for 6 hours, maximal increases in Ca uptake were observed in the cells pretreated with 100 $\mu\textrm{g}$/ml of YH (a 23% increase), and 50 $\mu\textrm{g}$/ml of JH (a 28% increase), respectively : however, no influence was seen on the net Ca transport activity. These results show that pretreatment with JH or YH, the two oriental medicinal prescriptions commonly used for improvement of bone metabolism, could possibly increase Ca accumulation inside the cells. but not the intestinal Ca net transport in vitro.
