갑상선암 환자에서 관찰된 뇌수막종의 위양성 옥소 섭취 (False-positive I-131 Uptake in Meningioma)

  • We experienced a case with meningioma showing false positive I-131 uptake. A 55-years old female patient underwent high dose (150 mCi) radioactive iodine therapy to ablate remnant tissue after total thyroidectomy for papillary thyroid cancer. in addition to intense tracer uptake in thyroid bed, there was mild but focal abnormal uptake in left frontal lobe of the brain on post-therapy I-131 whole body scan. Subsequent brain MR imaging showed single mass lesion in left frontal lobe and the mass was resected under the impression of brain metastasis of thyroid carcinoma. Pathologic report confirmed meningioma from the surgical specimen.

사두증 환자를 위한 맞춤형 헬멧 몰드 디자인 시스템 개발 (Development of a Customized Helmet Design System for Patients with Plagiocephaly)

  • This study developed a three-dimensional helmet mold design software that can design helmets for treating the infant plagiocephaly (flat head syndrome) using three-dimensional head scan data. For this, the three-dimensional head data of sixth SizeKorea body measurement project as well as the data produced by a head modeling software were used to prepare the head shape data of plagiocephaly patients. A total of 14 landmarks and 10 dimensions of heads required for helmet mold shape design and plagiocephaly diagnosis were automatically measured using an anthropometric analysis software. Using the software developed in this study, plagiocephaly can be diagnosed not only visually by three-dimensional head data visualization but also quantitatively by calculating the medically defined indices such as cranial index, which measures the proportions of the head, and the cranial vault asymmetry index, which measures the asymmetry of the head. The basic shape of the helmet mold was automatically generated based on the head scan data. Additionally, it is possible to fine tune the shape of the mold to reflect individual characteristics by using a free form deformation technique. Finally, the designed helmet mold was converted into the data that can be printed on a three-dimensional printer for generating the actual prototype.

Simulation and assessment of 99mTc absorbed dose into internal organs from cardiac perfusion scan

  • Directly, it is not possible to measure the absorbed dose of radiopharmaceuticals in the organs of the human body. Therefore, simulation methods are utilized to estimate the dose in distinct organs. In this study, individual organs were separately considered as the source organ or target organ to calculate the mean absorption dose, which SAF and S factors were then calculated according to the target uptake via MIRD method. Here, 99mTc activity distribution within the target was analyzed using the definition and simulation of ideal organs by summing the fraction of cumulative activities of the heart as source organ. Thus, GATE code was utilized to simulate the Zubal humanoid phantom. To validate the outcomes in comparison to the similar results reported, the accumulation of activity in the main organs of the body was calculated at the moment of injection and cardiac rest condition after 60 min of injection. The results showed the highest dose absorbed into pancreas was about 21%, then gallbladder 18%, kidney 16%, spleen 15%, heart 8%, liver 8%, thyroid 7%, lungs 5% and brain 2%, respectively, after 1 h of injection. This distinct simulation model may also be used for different periods after injection and modifying the prescribed dose.

비파괴 초음파 검사법의 의학적 활용: 초음파 C 스캔 영상을 이용한 미세 골절의 진단 (Medical Application of the Nondestructive Ultrasonic Tests: Diagnosis of Micro Bone Fractures using Ultrasonic C Scan Images)

  • 비전리 방사선을 이용하는 초음파 탐상법은 안전하며 사용이 간편하기 때문에 비파괴검사에서 매우 선호되고 있는 방법이다. 의학적으로 초음파 탐상법은 동일한 원리를 이용하여 인체의 결함 즉 질병의 진단을 위해 유용한 수단 요로 활용되고 있다. 본 논문에서는 초음파 탐상법에 기반한 의료용 초음파 진단의 원리에 대해 개관하고, 그동안 임상적으로 별로 주목받지 못한 초음파 C스캔 영상을 이용한 치밀골의 미세 골절 진단 가능성을 실험적으로 평가하였다. 실험에서는 돼지 대퇴골에 3점 굽힘 힘 (2-4 kN)을 1 mm/min 속도로 가해 미세 골절을 유발하였다. 골절의 정도를 변화하면서 골절에 대한 엑스선 영상과 25 MHz 접속형 초음파 변환기롤 이용하여 C스캔 영상을 취득하였다. 실험 결과 초음파 C 스캔 영상은 기존의 골절 진단법인 엑스선 영상에서는 진단이 어려운 미세한 골절까지 감지할 수 있음을 확인하였다.

고관절 자기공명영상 검사 시 multitransmit 기법의 적용에 따른 검사시간 단축의 유용성 (An effectiveness of multitransmit parallel technique on scan time reduction in hip joint MRI)

  • 본 연구의 목적은 MRI 검사 시 multitransmit 기법의 적용으로 인한 검사시간 단축의 유용성을 알아보고자 하였다. 연구방법은 2015년 7월부터 동년 9월까지 고관절 MRI 검사를 시행한 환자를 대상으로 multitransmit 기법 적용 전-후 고관절의 T1, T2 강조영상을 획득하여 대퇴골두와 장요근의 SNR과 CNR, 그에 따른 검사시간을 비교 평가하였다. 연구결과, multitransmit 기법 적용 전-후 T1, T2 강조영상 모두 SNR과 CNR은 유의한 차이가 없었으며, 검사시간은 적용 전에 비해 T1 강조영상은 42.8%, T2 강조영상은 49.7%로 감소하였다. 결론적으로 MRI 검사 시 본 연구와 같이 multitransmit 기법을 적용하면, 기존의 검사시간 단축관련 연구들에서 문제가 되었던 영상의 질 저하 없이 검사시간을 획기적으로 줄일 수 있으며, 향후 다양한 검사에 multitransmit 기법을 적용한다면 MRI 검사의 고질적 문제인 검사시간을 단축할 수 있어 매우 유용하리라 사료된다.

3차원 디지털 영상과 CAD/CAM 시스템을 활용한 전악 임플란트 수복 증례 (Full mouth rehabilitation with dental implant utilizing 3D digital image and CAD/CAM system: case report)

  • 본 증례는 상 하악 완전 무치악 환자에서 진단, 식립 그리고 보철물 제작에 디지털 방식을 활용하는 방법에 대해 제시하였다. 상악은 구개 부위, 하악은 기존 의치에 방사선 불투과성 마커를 부착한 후 구강스캔 및 CBCT 촬영을 시행하였다. 구강스캔 이미지와 CBCT 이미지를 중첩시켜 진단 및 수술가이드와 임시보철물을 제작하였다. 골유착 후에는 스캔바디를 이용한 구강스캔을 통해 최종보철물을 제작하였다. 임플란트를 이용한 전악수복에서 디지털 방식을 다양하게 활용하였으며 정확성과 환자의 불편감 최소화라는 목적에 만족할만한 결과를 얻었기에 이 증례를 보고하고자 한다.

Initial experience with intra-articular 188Re-tin colloid as a "radiation synovectomy" agent in various joints

  • Radiation synovectomy has been proposed as a promising palliative therapy for recurrent joint effusions for the last two or three decades. Ionizing radiations emitted by intrarticularly administered radiolabelled colloids. The aim of this study was to assess the effectiveness of radiation synovectomy (RSV) using $^{188}Re$-tin colloid in the treatment of recurrent joint effusions and chronic synovitis of knee joints. Three phase bone scan was acquired for the concerned joint prior to radiosynovectomy. $^{188}Re$-tin colloid was prepared as per the reported protocol. 9 patients, diagnosed with rheumatoid arthritis and suffering from chronic resistant synovitis of the knee, ankle or elbow joints were administered the radiopharmaceuticals, checked for radiochemical purity >95% by intraarticular route. A whole body scan was acquired 2 h post-radiosynovectomy. In all the 9 treatments, no leakage to non-target organs was visible in the whole body scan. Static scans of the joint revealed complete retention of $^{188}Re$-tin colloid in the joints post administration of the agent. Clinically all patients exhibited a complete or partial response. RSV with $^{188}Re$-tin colloid was safe and effective in patients with chronic synovitis of rheumatoid origin.

Transorbital Penetrating Intracranial Injury by a Chopstick

  • A 38-year-old man fell from a chair with a chopstick in his hand. The chopstick penetrated his left eye. He noticed pain, swelling, and numbness around his left eye. On physical examination, a linear wound was noted at the medial aspect of the left eyelid. Noncontrast computed tomography (CT) study showed a linear hypodense structure extending from the medial aspect of the left orbit to the occipital bone, suggesting a foreign body. This foreign body was hyperdense relative to normal parenchyma. From a CT scan with 3-dimensional reconstruction, the foreign body was found to be passing through the optic canal into the cranium. The clear plastic chopstick was withdrawn without difficulty. The patient was discharged home 3 weeks after his surgery. A treatment plan for a transorbital penetrating injury should be determined by a multidisciplinary team, with input from neurosurgeons and ophthalmologists.

  • 전기체간스캔법(EIS, electro interstitial scan)은 미세 직류전류를 인체의 사지말단 또는 국소적 인가를 통해 인체의 전류 반응을 기준으로 인체의 질병 유무를 판별할 수 있는 비침습적인 질병 스크리닝 방법으로 가능성을 보이고 있다. 프랑스에서 개발된 DDFAO는 다채널 EIS로 다양한 질병, 특히 당뇨와 같은 내분비 기관의 진단에 효과적이라고 주장한다. 본 연구에서는 RC 팬텀 모델을 이용하여 DDFAO의 측정의 반복성과 민감도를 확인하고, 정상군과 당뇨 질환군을 대상으로 임상적 유용성을 검정하였다. 그 결과, 팬텀을 이용할 경우, 반복 측정시 값의 변화가 없고 팬텀 특성이 변화될 때 DDFAO의 측정 결과도 변화되었다. 그러나 임상 측정에 있어서 6개의 표면 전극을 이용하여 비침습적 방법으로 정상인과 당뇨병 환자의 구별은 어려울 뿐만 아니라 반복성과 정확성도 우수하지 않았다. 그러나, 이러한 문제점에도 불구하고 측정 방법의 편리성과 간편성으로 인해 국소적인 영역보다 인체 전반적인 특성을 파악하는 스크린 장비로써의 개발 가능성은 여전히 보이고 있기에 EIS에 대한 다양한 기반 연구가 필요하다.

3D Scanner를 이용한 인체계측방법 및 플레어스커트의 착의형태평가방법에 관한 연구 (A Study on the Feasibility of the 3D Scanning Method of Body Measurement and Wearing Evaluation)

  • The study planned to testify the accuracy of the 3D scanner through analyzing the human body measurement method and the evaluation of flare skirt with 3D scanner. The results obtained from this study were as follows; First, the results of measurement for the dress form and human body by 3D scanner were more accurate than one or two dimensional measurement method. Second, from the results of the scan for the flare skirts as worn the human body, we could obtain very accurate horizontal section map. Third, in the clothing ergonomics viewpoint, the accuracy of the 3D scanner was very excellent and its utility was higher than that of the moir photography method. Therefore, the 3D scanner is very useful for the human body measurement and the wearing evaluation. In a nutshell, this study illustrates that we have to develop a program of automatic human body measurement system which will enhance the application of the 3D scanner.
