기능성 발보조기의 족부형태별 균형유지에 미치는 영향 (The Effects of functional foot orthotics on the balance according to Foot Shape)

  • 기능성 발보조기는 신발 착용 시 발바닥과 직접 접촉되어 비정상적인 신체 분절의 교정 및 발의 기능을 향상시킬 뿐만 아니라 체중 및 균형을 유지시키고 약화된 부위를 지지함으로써 사람의 직립자세 유지를 위한 균형 유지에 많은 도움이 된다. 이에 본 사례 연구에서는 보행 안정성을 비롯한 충격 흡수 기능을 수용할 수 있는 기능성 발보조기가 족부 형태 및 재질에 따라 신체의 균형유지에 미치는 영향에 대하여 연구하였다. 사람의 평균 무게 중심점의 흔들림 속도(sway velocity)를 이용한 균형감각 자세유지도의 고찰을 통해 족부형태별, 기능성 보조기의 재질에 따른 신체의 균형능력에 미치는 영향을 비교 평가한 결과, 안정한 상태에서 눈을 뜬 자세에서만 유의(p<0.006)한 결과를 나타냈으며, 이 자세에서 발의 형태별로 구분하여 통계 처리한 결과, 정상족(p<0.010), 편평족(p<0.000), 요족(p<0.003)의 순으로 나타났다. 그리고 기능성 발보조기의 재질에 따른 발의 형태별 균형에 미치는 영향을 분석한 결과, 연성재질은 정상의 발에서 그리고 경성재질은 편평족과 요족에 있어서 균형유지에 효과가 있는 것으로 나타났다.

The Effects of Squat Exercises with Vertical Whole-Body Vibration on the Center of Pressure and Trunk Muscle Activity in Patients with Low Back Pain

  • Background: Patients with low back pain (LBP) experience misalignments in the center of pressure (COP) and muscle imbalances due to frequent onesided posture adjustments to avoid pain. Objectives: To identify the effects of Squat Exercises with Vertical Whole-Body Vibration on the Center of Pressure and Trunk Muscle Activity. Design: Randomized controlled trial. Methods: Thirty LBP patients with an imbalance in the COP were sampled and randomly assigned to an experimental group of 15 patients who under went an intervention involving squat exercises with vertical WBV and a control group of 15 patients who were treated via a walking intervention. As pretests before the interventions, the subjects' COP was identified by measuring their stability index (ST), and erector spinae, rectus abdominis, transverse abdominis, gluteus medius muscle activity was analyzed by determining the % reference voluntary contraction (%RVC) value using surface electromyography while sit to stand. After four weeks, a post test was conducted to remeasure the same variables using the same methods. Results: Statistically significant differences were found in the ST (P<.01) and trunk muscle (P<.05, P<.001) in the experimental group before and after the intervention. In terms of the differences between the left- and right-side (RL) muscle activity, only the transverse abdominis (TrA) and gluteus medius (GM) exhibited statistically significant increase (P<.05). A comparison of the groups showed statistically significant differences in the TrA with respect to muscle activity (P<.05) and in the RLTrA and RLGM in terms ofthe difference between left- and right-side muscle activity (P<.01). Conclusion: Squat exercises with vertical WBV produced effective changes in the COP of patients with LBP by reducing muscle imbalances through the delivery of a uniform force. In particular, strengthening the TrA and reducing an imbalance in the GM were determined to be important factors in improving the COP.

Effects of Dual Tasks on Balance Ability in Patients with Cerebellar Ataxia

  • Purpose: The purpose of this study was to examine the effects of dual tasks on balance and postural control during standing in patients with cerebellar ataxia (CA). It was hypothesized that CA patients would exhibit different sway characteristics of the center of mass (COM) depending on the complexity of the secondary cognitive tasks compared with normal control subjects. Methods: A total of 8 patients with CA and age-matched healthy control subjects participated in this study. They were instructed to perform two balance tasks (non-dual and dual movement) with 3 different complexity of dual tasks. Range, variability, and velocity of COMs were measured. Results: According to the results CA patients showed deficits in balance and postural control with increased dual-task complexity during the static balance task in saggital sway movements. However, there was no significant difference in static balance in frontal sway. With higher difficulty in the cognitive task, CA patients took longer to stabilize their body center, while normal control subjects showed no change between conditions. In addition, CA patients had a greater COM resultant velocity during recovery in the dual-task condition compared with the single-task condition. These findings indicate that CA patients had defendable compensatory strategies in performing dual tasks. Conclusion: In conclusion, CA patients appeared to manage the priority to balance and postural control. Particularly in a situation with a postural threat such as when potential consequences of the loss of stability increase, they appeared to prioritize the control of balance and posture over the performance of the secondary task.

견갑대 운동 기능장애에 대한 치료 접근 (Treatment approach for the movement dysfunction of the shoulder girdle)

  • Functional stability is dependent on integrated local and global muscle function. Movement dysfunction can present as a local and global problem, though both frequently occur together. To good understand how movement induces pain syndrome, the optimal actions and interaction of the multiple anatomic and functional systems involved in motion must be considered. Minor alterations in the precision of movement cause microtrauma and, if allowed to continue, will cause macrotrauma and pain. These alteration of the movement result in the development of compensatory movement and movement impairment. Muscle that become tight tend to pull the body segment to which they are attached, creating postural deviation. The antagonistic muscles may become weak and allow postural deviations due to lack of balanced support. Both hypertonic and inhibited muscles will cause an alteration of the distribution of pressure over the joint(s) that they cross and, thus, may not only result from muscle dysfunction, but produce joint dysfunction as well. Alteration of the shoulder posture and movement dysfunction may sometimes result in compression of neurovascular structures in the shoulder and arm. There is a clear link between reduced proprioceptive input, altered motor unit recruitment and the neurovascular compression. This report start with understanding of the impaired alignment, movement patterns and neuromuscular compression of the shoulder girdle by movement impairment to approach method of the movement dysfunction.

(Community Care Preparation) Identification of Musculoskeletal Problems for the Elderly in Rural Areas and Presentation of Regional and Inter-university Health Management Models

  • Purpose: Compared to cities, rural areas are in a medical blind spot and face difficulties in accessing medical services due to inconvenient transportation facilities, lack of medical facilities, and the heavy burden of medical expenses. This study was carried out to identify the problems relating to the musculoskeletal system of the elderly in rural and fishing villages, which are medically vulnerable areas, and sought to present a regionally differentiated healthcare model. Methods: The study was conducted in 80 elderly people in two rural villages and two fishing villages after seeking inputs regarding medically vulnerable groups in the Gyeongnam Province. Postural balance and muscle flexibility were assessed and postural evaluation was conducted to identify musculoskeletal problems and gait stability. Strength and range of motion for each body segment were assessed for evaluating functional motion. Results: The elderly in both rural areas showed forward head posture characteristics. The strength level of the elderly in both rural areas was higher than the average, but their flexibility and balance ability were lower than the average. Conclusion: The musculoskeletal problems of the elderly in rural and fishing villages in this study did not show regional characteristics according to the area of residence. However, overall flexibility and balance ability appeared to be reduced. Therefore, a new management model connecting the region and the university is necessary in preparation for the coming era of community care.

전신진동(Whole body vibration)운동훈련을 통한 육상 투척선수의 하지관절 근육 기능변화에 관한 분석 (Analysis on the Changes in Muscle Function of the Leg Joint in Athletics Athletes Through by Whole Body Vibration Exercise Training)

  • 본 연구는 투척 선수의 하지 훈련 자세를 이용한 전신진동 운동 훈련 시 근육 변화와 대칭 지수를 살펴보는데 목적이 있다. 20 대 10 명(남성 6명 여성 4명, 나이:24.60±0.92세, 신장:177.90±7.40cm, 체중:92.90±22.97kg)의 투척 선수를 대상으로 스쿼트, 카프 레이즈, 런지 동작으로 하지 훈련 자세를 이용한 전신진동 운동 훈련을 실시하였다. 하지 관절 근육(대퇴이두근, 비복근 외측, 비복근 내측, 대퇴직근, 전경골근, 외측광근, 내측광근) 부위를 Tensiomyography(TMG) 변인 Time Delay(Td), Time Contraction(Tc), Time Sustain(Ts), Time Relaxation(Tr), Displacement Maximumal(Dm)을 통해 측정하여 근 활성도, 근 피로도, 좌우 대칭을 Wilconxon의 순위 검사로 비교하였고, 이때 유의수준은 α= .05로 분석하였다. 연구결과는 Td에서 왼쪽 RF, VL, 오른쪽 VM(p<.05), Tc에서는 VM(p<.05), Ts 에서는 GM(p<.05), Tr 에서는 왼쪽 RF, 오른쪽 TA(p<.05)에서 변화를 보였다. 따라서 스쿼트, 카프 레이즈, 런지 동작의 전신진동 운동 훈련이 근 수축 변화가 있는 효과적인 운동임을 입증되었고, 좌·우 근육 대칭으로 코어의 안정성이 확보되었다. 이로 인해 전신진동운동은 재활훈련 긍정적인 영향을 미칠 것이고, 경기력 향상을 도모할 수 있을 것으로 판단된다.

에너지절감에 의해 감소되는 뉴로에너지의 증강에 관한 연구 (The Study about Improvement of Neuro Energy Decreased by Energy Saving)

  • 청각 및 후각의 다감각 자극을 통하여 에너지를 절감하기 위하여 불쾌해진 촉각을 보상함으로써 에너지는 절감하면서 근무자의 뉴로에너지(쾌적성, 집중력, 신체적 및 심리적 안정 등)는 증강시키는 연구를 하였다. 온도 $25[^{\circ}C]$, 상대습도 50[RH%], 조도 1,000[lux] 및 기류 속도 0.02[m/sec]의 항온 항습실에서 1/f의 변동리듬으로 청각을 자극하고 아로마 향기로 후각을 자극하여 실험을 진행하였으며, 접촉식으로 인체의 심리상태를 측정하는 뇌파, 학습능력 및 심박동변이도와 비접촉식으로 심리상태를 측정하는 진동이미지 방법을 사용하였다. 피험자는 신체적으로 활동하는데 지장이 없고, 심신의 안정과 집중력의 최대 상태를 가져오는 상대 ${\alpha}$파(8~13[Hz])의 활성도가 후두엽에서 높은 유형인 20대 대학생 8명(남성 4명, 여성 4명)으로 선정하였다. 측정자세는 쾌적한 열적 환경에서 의자에 착석해 안정을 취하고 있을 때의 활동량으로써 1met로 하였으며, 착의량 상태는 약 0.7clo로 통일하였다. 측정결과 쟈스민 향기와 변동리듬 a=1.106 음원의 다감각자극에서 감성 및 심신안정과 집중력이 가장 높아졌으며, 상대$M{\alpha}$파와 상대$M{\beta}$파가 각각 70.49[%], 89.72[%] 증가하였고, HRT는 38.09[%] 감소하였으며, 피로도와 긴장 불안은 각각 36.85[%], 15.54[%] 감소하였다.

Effects of Manual Intervention and Self-Corrective Exercise Models of the General Coordinative Manipulation on Balance Restoration of Spine and Extremities Joints

  • The purpose of this study was conducted in order to analyze the effects of the manual intervention and self-corrective exercise models of general coordinative manipulation(GCM) on the balance restoration of spine & extremities joints with distortions and mal-alignment areas. The subjects were the members who visited GCM Musculoskeletal Prevent Exercise Center from March 1 2012 to December 31 2013 because of spine & extremities joints distortion and mal-alignments, poor posture, and body type correction. All subjects were diagnosed with the four types of the GBT diagnosis. And according to the standards of the mobility vs stability types of the upper & lower body, they were classified into Group 1(40 persons) and Group 2(24 persons). For every other day for three times a week, GCM intervention models were applied to all subjects for four weeks, adding up to 12 times in total. Then the balance restoration effects were re-evaluated with the same methods. The results are as follows. 1) Balance restoration effects of VASdp(Visual analysis scale pain & discomfort) and ER(Equilibrium reaction: ER) came out higher in GCM body type(GBT) II III IV of Group 1. 2) In case of balance restoration effects in Moire and postural evaluation areas, Group 1 was higher and cervical and scapular girdle were higher in Group 2. The balance restoration of the four GBT types was significant in all regions(p<.05), and the scapular girdle came out as high in the order of GBTII IV I. 3) In case of thoracic-lumbar scoliosis and head rotation facial asymmetric cervical scoliosis ribcage forward, the balance restoration effects of the upper body postural evaluation areas came out the highest in Group 1 and Group 2, respectively. The balance restoration effects of the four GBT types were significant in all regions(p<.05), and came out the highest in lumbar scoliosis GBTIII I, ribcage forward and thoracic scoliosis GBTII IV. 4) The balance restoration effects of the lower body postural evaluation areas came out higher in Group 1 and Group 2 for pelvis girdle deviation patella high umbilicus tilt and hallux valgus foot longitudinal arch: FLA patella direction, respectively. The balance restoration effects of the four GBT types were significant in all regions(p<.05), and came out the highest in pelvis girdle deviation GBTIII I and patella high-direction GBTIV II I. 5) The balance restoration effects between the same GBT came out significant (p<.05) in all evaluation areas and items. The conclusions of this study was the manual intervention and self-corrective exercise models of the GCM about the mal-alignment of the spine & extremities joints across the whole body indicated high balance restoration effects(p<.05) in spine & extremities joints in all evaluation areas.

VR을 이용한 균형능력 측정장비의 성능평가 (Performance Evaluation of Balance Ability Equipment Using VR)

  • Purpose : Conventional Balance Measurement can only measure the center of gravity and the shaking movement of the body. As a result, it has the disadvantages of not responding to visual changes and blocking functions of variables. This study was carried out to evaluate the performance of new equipment that measures the balance of the body using changes in body segment and pressure using the acceleration sensor to compensate for the disadvantages of the existing equipment. Methods : To this end, balance ability was measured in 43 healthy male/female adults without orthopedic injuries and nervous system damage in the last 6 months. in a situation where the visual information was restricted by Virtual Reality (VR) gear, all subjects measured and evaluated the balance ability utilizing the new equipment. Balance measurement (Prime Medilab, Korea) and Wii fit (Nintendo, Japan) were used to measure the balance ability of the subjects, and the balance ability test was performed in 4 postures using each device for data acquisition. The test duration for each posture was 30 seconds. For data acquisition, the average value of three experiments measured using each equipment was analyzed, and the statistical test was performed using the independent sample and the corresponding sample t-test, and the significance level was set to α=.05. Results : As a result of measuring the balance ability using individual equipment, blocking visual information using VR gear, the average speed, maximum speed, and moving area of the COP increased equally. It was found that the obtained absolute size of the result in Wii was somewhat larger than that of BM. Conclusion : It is considered that in the future research, it is necessary to measure changes in the body's center of gravity through image analysis, etc., to make clear comparison and evaluation of the usability.

8주간의 척추 안정화 운동 프로그램이 만성 경부통 환자의 NDI, 균형 능력 및 자세 변화에 미치는 영향 (The effects of 8-week spinal stabilization exercise program on NDI, postural balance and body shape change in patients with chronic neck pain)

  • 이 연구는 8주간의 척추 안정화 운동프로그램이 경부 장애 지수, 균형 능력 및 자세 변화에 어떠한 영향을 주는지를 알아보는 것이다. 운동 프로그램은 8주간 주 3회 척추 안정화 운동을 시행하였으며, 총 60분으로 구성하였다. 연구의 대상자는 6개월간 경부통을 호소하는 만성 경부통 환자 16명을 대상으로 운동 집단(n=8명)과 통제 집단(n=8명)으로 분류하여 운동 프로그램 사전, 사후의 결과를 관찰하여 다음과 같은 결과를 도출하였다. 경부 장애 지수는 시기, 집단, 상호작용에서 유의차가 나타났다(p<.05). 균형 능력은 집단 및 시기 간에서 유의한 상호작용 효과가 나타났다(p<.05). 자세 변화는 골반 경사각에서 시기, 상호작용에서 유의차가 나타났고(p<.05), 경부 및 어깨 위치각은 집단, 시기, 집단, 상호작용 모두 유의차가 나타났다(p<.05). 이상의 결과 척추 안정화 운동이 만성 경부통 환자에게 경부 장애 지수, 균형 능력 변화, 자세 변화에서 유의하게 개선된 결과를 나타내었다. 추후 연구에서는 방사선 영상을 통한 척추 구조 변화에 대한 구체적인 변화를 분석하여 척추 안정화 운동의 타당성을 높이는 연구가 필요할 것으로 사료된다.
