외상환자에서 수혈과 사망의 연관성 (The Relationship between Blood Transfusion and Mortality in Trauma Patients)

  • Purpose: Using a propensity analysis, a recent study reported that blood transfusion might not be an independent predictor of mortality in critically ill patients, which contradicted the results of earlier studies. This study aims to reveal whether or not blood transfusion is an independent predictor of mortality in trauma patients. Methods: A total of three hundred fifty consecutive trauma patients who were admitted to our emergency center from January 2004 to October 2005 and who underwent an arterial blood gas analysis and a venous blood analysis were included in this study. Their medical records were collected prospectively and retrospectively. Using a multivariate logistic analysis, data on the total population and on the propensity-score -matched population were retrospectively analyzed for association with mortality. Results: Of the three hundred fifty patients, one hundred twenty-nine (36.9%) received a blood transfusion. These patients were older (mean age: 48 vs. 44 years; p=0.019) and had a higher mortality rate (27.9% vs. 7.7%; p<0.001). In the total population, the multivariate analysis revealed that the Glasgow coma scale score, the systolic blood pressure, bicarbonate, the need for respiratory support, past medical history of heart disease, the amount of blood transfusion for 24 hours, and hemoglobin were associated with mortality. In thirty-seven pairs of patients matched with a propensity score, potassium, new injury severity score, amount of blood transfusion for 24 hours, and pulse rate were associated with mortality in the multivariate analysis. Therefore, blood transfusion was a significant independent predictor of mortality in trauma patients. Conclusion: Blood transfusion was revealed to be a significant independent predictor of mortality in the total population of trauma patients and in the propensity-score-matched population.

A Structural Analysis on the Leaflet Motion Induced by the Blood Flow for Design of a Bileaflet Mechanical Heart Valve Prosthesis

  • This paper presents a structural analysis on the rigid and deformed motion of the leaflet induced by the blood flow required in the design of a bileaflet mechanical heart valve (MHV) prosthesis. In the study on the design and the mechanical characteristics of a bileaflet mechanical heart valve, the fluid mechanics analysis on the blood flow passing through leaflets, the kinetodynamics analysis on the rigid body motion of the leaflet induced by the pulsatile blood flow, and the structural mechanics analysis on the deformed motion of the leaflet are required sequentially and simultaneously. Fluid forces computed in the previous hemodynamics analysis on the blood flow are used in the kinetodynamics analysis on the rigid body motion of the leaflet. Thereafter, the structural mechanics analysis on the deformed motion of the leaflet follows to predict the structural strength variation of the leaflet as the leaflet thickness changes. Analysis results show that structural deformations and stresses increase as the fluid pressure increases and the leaflet thickness decreases. Analysis results also show that the leaflet becomes structurally weaker and weaker as the leaflet thickness becomes smaller than 0.6 mm.

Changes in the blood electrolyte, BUN and glucose values in diarrheic Hanwoo neonatal calves

  • Calf diarrhea is a major health concern in the livestock industry that requires accurate analysis for appropriate treatment. Dehydration and electrolyte imbalance are the most significant consequences of diarrhea in calves. Until now, the reference values of blood analysis of Hanwoo neonatal calves have not been well known. Recently, portable blood analyzers have made it possible to immediately analyze blood in the farm and provide suitable treatment for the patients. We analyzed electrolytes, blood urea nitrogen (BUN) and glucose in the blood of 179 Hanwoo neonatal calves, including 79 with diarrhea. These 179 calves were divided into 3 groups based on their age. Values of sodium (Na+ ) and glucose were significantly lower in diarrheic calves (1~30 days), while potassium (K+ ) and chloride (Cl- ) values were higher in diarrheic calves (1~30 days). BUN values, which are closely related to dehydration due to diarrhea, increased in diarrheic calves of all ages. Therefore, these data can be used as reference values for blood analysis and treatment of healthy or diarrheic Hanwoo neonatal calves within 30 days of age.

  • A cerebral aneurysm is a ballooning arising from a weakened area in the wall of a blood vessel in the brain. In this study, the flow analysis of the blood vessel, including the cerebral aneurysm and the stent, was conducted to investigate the effect of the stent shape on reduction of blood flowing into the cerebral aneurysm. We used the Carreau model to describe Non-Newtonian behaviors of blood flow. Two kinds of stents, which had different cross angles were modelled by the commercial CAD program and the geometries of those stents were reflected in the analysis domain of the blood vessel. From the results of the flow analysis, we found that the blood flowing into the cerebral aneurysm was decreased as the cross angle of the stent was decreased.

Assessment of bovine blood sample stability for complete blood count and blood gases and electrolytes analysis during storage

  • Delayed arrival of blood samples from the field and a large number of samples delivered often causes delay in sample analysis leading to inaccurate measurements. Therefore, this study aimed to assess whether prolonged storage in refrigerator could influence the stability of cattle blood samples and to establish an optimal time limit for complete blood count (CBC) parameters and blood gas and electrolyte (BGE) parameters analyses. Samples collected from healthy cows were tested immediately for CBC and BGE using automated hematology, blood gas and electrolyte analyzers. Samples were kept in refrigerator at 4℃ and analyzed after 6 h, 12 h, 24 h, 48 h, 72 h, 120 h, and 192 h of storage. Mean differences between observations were assessed at 5% significance level using ANOVA and Duncan's multiple range test. Total CBC parameters and the platelet profile remained stable for 192 h, except for MCHC. Among leukocyte-related counts, NEU and EOS remained stable for 192 hours. WBC and LYM, and MONO values produced inconsistent measurements which recovered its initial measurement after 12 h and 24 h of storage, respectively, then remained stable until 120 h. Among the blood gas indices, PCO2, PO2, tCO2, and BE showed declining and significant changes over time, but pH, tHb, and SO2 remained stable for 192 h. Electrolyte status in the blood showed that ions are unstable and tend to change in as early as 6 h of storage. This study established that cattle blood specimens for CBC analysis can be stored for 120 h at 4℃, but specimens for BGE analyses must be tested within 6 to 24 h.

A Numerical Analysis on the Curved Bileaflet Mechanical Heart Valve (MHV): Leaflet Motion and Blood Flow in an Elastic Blood Vessel

  • In blood flow passing through the mechanical heart valve (MHV) and elastic blood vessel, hemolysis and platelet activation causing thrombus formation can be seen owing to the shear stress in the blood. Also, fracture and deformation of leaflets can be observed depending on the shape and material properties of the leaflets which is opened and closed in a cycle. Hence, comprehensive study is needed on the hemodynamics which is associated with the motion of leaflet and elastic blood vessel in terms of fluid-structure interaction. In this paper, a numerical analysis has been performed for a three-dimensional pulsatile blood flow associated with the elastic blood vessel and curved bileaflet for multiple cycles in light of fluid-structure interaction. From this analysis fluttering phenomenon and rebound of the leaflet have been observed and recirculation and regurgitation have been found in the flow fields of the blood. Also, the pressure distribution and the radial displacement of the elastic blood vessel have been obtained. The motion of the leaflet and flow fields of the blood have shown similar tendency compared with the previous experiments carried out in other studies. The present study can contribute to the design methodology for the curved bileaflet mechanical heart valve. Furthermore, the proposed fluid-structure interaction method will be effectively used in various fields where the interaction between fluid flow and structure are involved.

  • In this study, a numerical analysis has been performed for a three-dimensional pulsatile blood flow associated with the elastic blood vessel and curved bileaflet for multiple cycles in terms of fluid-structure interaction. Here, blood has been assumed as a Newtonian, incompressible fluid. Pressure profiles have been used as boundary conditions at the ventricle and the aorta. From this analysis, the motion of the leaflet has been observed with fluttering phenomenon and rebound, and the flow fields of blood have been obtained with recirculation and regurgitation. The results can contribute to the development of design methodology for the curved bileaflet mechanical heart valve.

  • Objectives : The purpose of this study was to investigate the effect of Intravascular Laser Irradiation of Blood(ILlB) by the live blood analysis. Methods : We had analysed the changing forms of the live blood samples with Ultra Darkfield Microscope before and after Intravascular Laser Irradiation of Blood. Results : 1. Somatid did not showed significant change. 2. In the rouleau of red blood cells was decreased significantly. 3. In the morphological change of red blood cells, Burr cell, Ovalocyte and Poikilocyte were decreased significantly, but Acanthocyte and Target cell were increased significantly. 4. In the abnormal matters in plasma, the Cholesterol cristal did not showed significant change, but the Aggregation of platelet, Lipids, Spicule, Leucocyte, Uric acid cristal did showed a little significant decrease. Conclusion : These findings suggest that live blood analysis is useful to judge the effect of treatment and diagnosis in oriental medicine, and with the effect of Intravascular Laser Irradiation of Blood, it had showed significant effect on rouleau of red blood cells, morphological change of red blood cells and abnormal matters in plasma.

원심형 혈액펌프의 최적화 수력설계 및 성능해석 (Hydraulic Design Optimization and Performance Analysis of a Centrifugal Blood Pump)

  • This paper presents the hydrodynamic design and performance analysis method for a miniaturized centrifugal blood pump using three-dimensional computational fluid dynamics (CFD) code. In order to obtain the hydraulically high efficient configuration of a miniaturized centrifugal blood pump for cardiopulmonary circulation, a well-established commercial CFD code was incorporated considering detailed flow dynamic phenomena in the blood pump system. A prototype of centrifugal blood pump developed by the present design and analysis method has been tested in the mock circulatory system. Predicted results by the CFD code agree very well with in vitro hydraulic performance data for a centrifugal blood pump over the entire operating conditions. Preliminary in vivo animal testing has also been conducted to demonstrate the hemodynamic feasibility for use of centrifugal blood pump as a mechanical circulatory support. A miniaturized centrifugal blood pump developed by the hydraulic design optimization and performance prediction method presented herein shows the possibility of a good candidate for intra and extracorporeal cardiopulmonary circulation pump in the near future.
