폐내엽형 분리증 1례 치험 보고 (Intralobar Pulmonary Sequestration Report of one case)

  • A sequestrated Mass of ectopic non functioning pulmonary Tissue artery is an uncommon but clinically recognizable Entity. Pulmonary sequestrated, in general usage, designates an intralobar process intralobar pulmonary sequestration is a rare congenital malformation characterized by a cystic portion of the lung that derive its arterial blood supply through aberrant vessel directly of systemic circulation. As aberrant systemic vessel supplying the lung was reported by Hurber in 1777. We experienced a case of Intralobar pulmonary sequestration Pre-Operatively, confirm by Aortogram. The operative finding show that large Abscess cavity measuring 7x8 Well circumscribed, child fist sized Mass, and 4cm-length aberrant vessel arising from Descending aorta Just above the Diaphragm. The Anomalous systemic artery was ligatures & resection, and associated with left lower lobectomy was done. Post-Operative course was uneventful, and 7 days later discharged.

Total robotic right hepatectomy for multifocal hepatocellular carcinoma using vessel sealer

  • Rapid adoption of a robotic approach as a minimally invasive surgery tool has enabled surgeons to perform more complex hepatobiliary surgeries than conventional laparoscopic surgery. Although various types of liver resections have been performed robotically, parenchymal transection is challenging as commonly used instruments (Cavitron Ultrasonic Surgical Aspirator [CUSA] and Harmonic) lack articulation. Further, CUSA also requires a patient-side assistant surgeon with hepatobiliary laparoscopic skills. We present a case report of total robotic right hepatectomy for multifocal hepatocellular carcinoma in a 70-year-old male using 'Vessel Sealer' for parenchymal transection. Total operative time was 520 minutes with a blood loss of ~400 mL. There was no technical difficulty or instrument failure encountered during surgery. The patient was discharged on postoperative day five without any significant complications such as bile leak. Thus, Vessel Sealer, a fully articulating instrument intended to seal vessels and tissues up to 7 mm, can be a promising tool for parenchymal transection in a robotic surgery.

  • In modem city life, the citizen get insufficient exercise and has high levels of stress. Increased Stress causes such minor things as tiredness, disease and mental fatigue, and increase brain blood pressure too. In this paper trying to design the multi-functional blood pressure monitor with airo-dynamic brain hemokinesis improvement function. So this system have developed for the improvement of flows in the capillary blood vessel of head and limbs. Also This system is able to measure brain blood pressure(BBP) which need diagnosis circulation state of brain blood. So, we extracted correlativity of blood pressure(BP) and BBP through clinical experiment. We confirmed that compare factor of BP with BBP is more useful factors for diagnosis state of brain hemokinesis.

3차원 혈관 모델에서 협착 및 팽창 영역 탐색 방안 (Detecting Regions of Stenosis and Aneurysm in a 3D Blood Vessel Model)

  • 혈관 질환 검사는 일반적으로 혈관 조영술(angiography)과 CT 혈관 조영술(CT angiography) 등을 통해 이루어지며, 대부분 검사자의 육안을 통한 주관적 판단에 의존하여 진단이 이루어진다. 본 논문에서는 의료영상으로부터 재구성된 3차원 혈관 내벽 모델로부터 대표적 혈관질환에 해당하는 협착과 팽창 질환 의심 영역을 탐색하는 방안을 제안한다. 먼저, 의료영상에서 재구성된 3차원 혈관 내벽 모델로부터 혈관에 대한 골격 곡선(curve skeletons)과 외곽선(contours)을 생성하고, 생성된 골격 곡선을 가지 단위로 분할한 후, 가지에 속하는 각 노드에 대한 외곽선의 면적을 계산한다. 그런 다음, 계산된 외곽선들의 면적에 대해 평균 면적 및 최대/최소 면적, 그리고 인접 노드들 간의 외곽선 면적 차이를 고려하여 협착 및 팽창 질환의심 영역에 해당하는 노드들을 탐색한다. 다음으로 탐색된 의심 영역들을 적절하게 시각화함으로써 혈관질환의 진단을 지원한다. 제안된 방안을 구현하여 몇 가지 3D 인체 혈관모델에 적용한 결과 질환 의심 영역이 잘 찾아짐을 확인하였다. 이를 통해 제안된 방안의 유용성을 보인다.

$\imath$--Muscone의 실험관계에 관한 약리연구 (Pharmacological Actions of $\imath$--Muscone on Cardiovascular System)

  • In order to investigate the pharmacological properties of ι-muscone, effects of ι-muscone and musk were studied on the cardiovascular system with various experimental models. In isolated rat aorta, ι-muscone and musk made the relaxation of blood vessels in maximum contractile response to phenylephrine (10$^{-6}$ M) in endothelium-containing rings of the rat aorta, but not in endothelium-denuded rings. However, ι-muscone and musk in the presence of the inhibitor of NO synthase and guanylate cyclase did not make the relaxation of blood vessels. In spontaneously hypertensive rats (SHRs), ι-muscone and musk slightly reduced blood pressure but significantly decreased heart rate. In the isolated perfused rat hearts, ι-muscone and musk did not affect significantly on LVDP, contractile force, coronary flow and (-dp/dt)/(+dp/dt). These results suggest that ι-muscone and musk have weak cardiovascular effects with relaxation of blood vessel and decrease of heart rate, but without significant cardiac functions.

  • Recently, the high blood pressure is appeared the most frequent disease of a man's the whole sickness. But, to treat this disease is uncertain and produces an adverse reaction of a medicine therapy. Also, a patients are burden high price for doctor's treatment. Therefore, we studied effectively and financially to execute the treatment of the high blood pressure using the Chinese Medicine Theorem that recently arousing a great interest of the people. The main theories of this paper are Blood Vessel Theory and Acupuncture. The composed circuits are low-frequency generating circuit, charging-discharging circuit. The former will substitute a needle that use in the acupuncture therapy method and the latter will charge and discharge the bioelectric action potential of a patient in Ear Acupuncture Point. Also, all circuits will be controlled a micro-computer circuit.

A dual path encoder-decoder network for placental vessel segmentation in fetoscopic surgery

  • A fetoscope is an optical endoscope, which is often applied in fetoscopic laser photocoagulation to treat twin-to-twin transfusion syndrome. In an operation, the clinician needs to observe the abnormal placental vessels through the endoscope, so as to guide the operation. However, low-quality imaging and narrow field of view of the fetoscope increase the difficulty of the operation. Introducing an accurate placental vessel segmentation of fetoscopic images can assist the fetoscopic laser photocoagulation and help identify the abnormal vessels. This study proposes a method to solve the above problems. A novel encoder-decoder network with a dual-path structure is proposed to segment the placental vessels in fetoscopic images. In particular, we introduce a channel attention mechanism and a continuous convolution structure to obtain multi-scale features with their weights. Moreover, a switching connection is inserted between the corresponding blocks of the two paths to strengthen their relationship. According to the results of a set of blood vessel segmentation experiments conducted on a public fetoscopic image dataset, our method has achieved higher scores than the current mainstream segmentation methods, raising the dice similarity coefficient, intersection over union, and pixel accuracy by 5.80%, 8.39% and 0.62%, respectively.

변형후 형상의 최적화를 위한 동맥과 PTFE 문합의 직경비 연구 (The Study on the Diameter Ratio of the Artery-PTFE Anastomosis for the Optimized Deformed Shape)

  • 본 연구에서는 end-to-end 문합시 변형된 직경의 불일치로 인하여 발생하게 되는 혈관질환을 방지하기 위하여 기계역학적 거동을 유한요소 법을 이용하여 해석한 결과를 나타내었다. 이 연구에서는 서로 다른 직경을 가지는 동맥과 인공혈관인 PTFE의 문합시 봉합으로 인한 예변형을 고려하였으며, 봉합된 문합부에 수축기혈압인 120mmHg(16.0KPa)을 작용시켜 혈관의 변형을 분석하였다. 변형 후 최종 문합부의 형상은 동맥과 PTFE의 초기 직경비(R$_{I}$)와 PTFE의 두께에 대하여 분석하였다. 그리고 동맥과 PTFE의 초기 직경비가 문합부에서 발생되는 응력에 어떠한 영향을 미치는지에 대하여 해석하여 다음과 같은 결과를 얻었다. 1. 혈관내막의 증식등을 고려하지 않고 봉합으로 인한 예변형과 수축기 혈압만을 고려할 경우 가장 이상적인 초기 직경비(R$_{I}$)는 1.073이다. 2. 상당응력과 원주방향응력은 초기 직경비(R$_{I}$) 증가에 따라 증가하며 모두 접합부에서 PTFE측으로 0.4mm 떨어진 지점에서 최대값이 발생하였다.

열입혈실증(熱入血室證)에 대한 소고(小考) (A Study on the Pattern of 'Heat Entering The Blood Chamber')

  • Objective : One of the unique syndromes in Asian traditional medicine named 'heat entering the blood chamber(熱入血室, HEBC)' first appeared in Shanghanlun(傷寒論) and Jinguiyaolue(金匱要略) written by Zhangzhongjing(張仲景) who is the most famous doctor in ancient China. Method : Through comparison between Huangdineijing(黃帝內經), Shanghanlun(傷寒論), Jinguiyaolue(金匱要略) and other medical texts, the correct meaning, causes and mechanisms of HEBC can be analysed and organized to suggest new view of HEBC in modern society. Result : This syndrome is usually occurred in women during catching cold and menstruation, accompanying alternation of chillness and fever, pseudo-malaria, delirium, raveled chest(結胸), uterine hemorrhage, etc. The main sign of this syndrome, delirium belongs to the category of liver disease and fever in Huangdineijing(黃帝內經) which is a document more early published than Shanghanlun. Although there are still many other comprehensions about what blood chamber is, it could be the same as uterus according to Huangdineijing, it is relevant to the control of menstruation and emotions, and the function of liver and thoroughfare vessel(衝脈). Conclusion : HEBC is a syndrome exclusive to women, caused by their unique physical and psychological characteristics. It's beginning can be found in Huangdineijing, and by Shanghanlun and Jinguiyaolue, its concept as a single disease pattern becomes established. In other words, HEBC is a complex disease related to menstruation and its related hormonal dysfunctions, closely related to PMS, menopausal syndrome of today. Physical symptoms accompanied by psychological anxiety and fear is characteristic of this condition. Therefore gynecological approaches as well as socio-cultural issues related to women in modern society must be adopted when dealing with HEBC.

중심정맥관 삽입 시 발생하는 공기유입량의 예측: 실험연구 (Prediction of air inflow during central venous catheter insertion: experimental study)

  • Objective: This study examined the incidence and amount of air inflow during central venous catheter (CVC) insertion. Methods: This study was an experimental study aimed at designing an apparatus to implement blood vessel and blood flow in the human body. A 1.5-m long core tube with a Teflon tube, suction rubber tube, and polyvinyl chloride tube were made. This core tube was assumed to be the blood vessel of the human body. Blood was replaced with a saline solution. The saline solution was placed higher than the core tube and flowed into the inside of the tube by gravity. The CVC was injected 15-cm deep into the core tube. The air was collected through a 3-way valve into the upper tube. The experiments were carried out by differentiating the pressure in the tube, CVC insertion step, and diameter of the end of the catheter. The experiment was repeated 10 times under the same conditions. Results: The amount of air decreased with increasing pressure applied to the tube. Air was not generated when the syringe needle was injected, and the amount of air increased with increasing size of the distal end catheter. Conclusion: To minimize the possibility of air embolism, it is necessary to close the distal end catheter at the earliest point as soon as possible.
