Development of International Versions of Pattern Identification Questionnaires using Cross-cultural Translation Methodology: Seven Emotions, Fatigue and Malaise, Phlegm, Food Retension, and Blood Stasis (칠정, 노권, 담음, 식적, 어혈 변증 설문지의 횡문화적 번역 연구)

  • Objectives This study aimed to perform a cross-cultural translation of 5 kinds of pattern identification questionnaires from Korean to English: questionnaires for seven emotions, fatigue and malaise, phlegm, food retention, and blood stasis. Methods We followed the strict guideline on the cross-cultural translation of healthcare evaluation tool. Total five stages of study were conducted. First, translations of two individual translators. Second, synthesizing of the two results. Third, two back translations from synthesized version to Korean. Fourth, expert committee reviewed with the original version, synthesized version, back translated versions to make a pre-final version. Last, with the pre-final version, 5 Americans evaluated face validity of the pre-final version. We made a final version after the above-mentioned 5 stages. Result and conclusion International versions of the 5 kinds of pattern identification questionnaires were completed. We can expect this versions are widly used for clinical usage and following academical researches.

Reference value of serum amyloid P : a systematic review (혈청 아밀로이드 P (SAP)의 정상 참고치 탐색을 위한 문헌 고찰)

  • Objectives : The aim of this study was to examine the reference value of biomarker serum amyloid p (SAP) to diagnose blood stasis objectively. Methods : Pubmed-Medline, Cochrane library, EMBASE were searched using the key words 'SAP' and 'serum amyloid p' in June 2018. Original articles of human adults that published in English, studies that recruited from the clinical research settings or well defined population based cohorts were only included. Results : A total of 12 studies were selected to extract the reference value of SAP. It was between 8.5 ng/mL (0.0085 mg/L) to 57.5 mg/L. Although the disease varied, most of them showed elevated SAP levels in the disease group (1.1-1.5 times). Conclusions : This study is meaningful in that it summarizes the results of previous researches of SAP, which has the potential to be diagnostic index of blood stasis.

Study on the Origin, Description and Composition of Sokmyeung-tang(續命湯) (속명탕(續命湯)의 출처(出處), 종류(種類) 및 조성(造成)에 대한 고찰(考察))

  • Sokmyeungtang was the representative prescription for Apoplexy under the rule of Tang and Song dynasty of which the cultures were thriving in the history of China. However, the clinical use of Sokmyeungtang has been gradually reduced since Geumwon dynasty of China because it was misunderstood that the dryness heat drugs of pungent in flavor and warm in property such as Ephedra, Pubescent Angelica Root, Chinese Cassia Tree-Bark, Divaricate Saposhnikovia Root, Prepared Aconite Root, Fresh Ginger, and Wildginger Herb included in the presciption for Apoplexy supplemented heat as damaging Yin flood. In fact, the drugs pungent in flavor and warm in property activate exterior and interior circulation, circulate channels and collaterals, promote blood circulation, and remove blood stasis with the side effect of relieving exterior syndrome with drugs warming channels. When treating Apoplexy with Sokmyeungtang, the cold drugs such as Gypsum, Baikai Skullcap Root, and Pueraria Root are prescribed to suppress fire of pungent dryness and to control excessive heat of people with Apoplexy as reducing the effects of hot drugs causing impairment of Yin. For treatment of Apoplexy, the above drugs accelerate blood and Qi circulation in channels and collaterals and then in necrotic tissue of human body as removing blood stasis. Consequently, these drugs improve disorders of capillary tube circulation. If Sokmyeungtang, an old prescription, is properly understood, it will be substantially helpful to all kinds of treatments in clinical cases

Clinical study on circulating blood and extinguishing blood stasis method in acute ischemic stroke patients (급성(急性) 뇌경색환자(腦硬塞患者)에서 활혈화어(活血化瘀) 치법(治法)의 응용(應用))

  • In order to investigate the effect of circulating blood and extinguishing blood stasis method on acute ischemic stroke treatment, we compared muscle weakness in two groups. Dansamhwanotang was administered to experimental group and the Sopungtang was administered to comparison group. In prospective and consecutive study, 24 patients(male 14, female 10) were admitted to hospital within 6hours(median $4.21{\pm}2.45)$ after stroke attack. All of them were diagnosed computed tomography as acute cerebral infarction. We divided that patients into two groups. The experimental group was 13, took median $4.17{\pm}1.72hrs$ to admission after stroke attack and the comparison group 11, median $4.31{\pm}2.72hrs$ to admission after stroke attack. There was no statistical difference in time consumed from stroke onset to admission(P>0.05). Muscle weakness was measured on admission and 7 days later on AMA(American Medical Association) method. In the experimental group, muscle weakness on admission was $2.23{\pm}0.51$ and $2.79{\pm}0.72$ in upper and lower extremity, respectively. In comparison group, muscle weakness on admission was $2.17{\pm}0.43$ and $2.67{\pm}0.82$ in upper and lower extremity, respectively. There was no difference in muscle weakness(P〈0.05). In 7 days after, muscle weakness was $2.31{\pm}0.35$ in upper extremity and $3.15{\pm}0.12$ in lower extremity in experimental group, and $2.27{\pm}0.74$ in upper extremity and $3.45{\pm}0.48$, lower extremity in comparison group. There was no meaningful improvement statistically in upper extremity(p<0.05) but significant evolution in lower extremity(p<0.05). The muscle weakness comparison between admission time and 7 days later was as follows. Experimental group had improvement at the degree of $0.24{\pm}0.92$, $0.42{\pm}0.82$ in upper and lower extremity, respectively and comparison group, $0.12{\pm}0.82$, $0.27{\pm}0.97$ in same part(p<0.05). So, Dansamhwanotang administered group had more good muscle weakness improvement than Sopungtang administered group(P>0.05). From the above result, I suppose that circulating blood and extinguishing blood stasis method helps recover hemiparesis caused by acute ischemic cerabral disease, in acute stage at least.

  • The result were as follows: 1. Yu Wan So(劉完素) regarded the cause of surgical disease as the heat(熱). He used the three method to drain(疏通), promoting pus drainage(托裏) and the balance between ying-energy(營氣) and wei-energy(衛氣). 2. Jang Jong Jeung(張從正) only refered to medicines without entire theory. 3. Lee Dong Won(李東垣) regarded the cause of surgical disease as the greasy diet(膏梁厚味), damp air(濕氣), wetness heat(濕熱). For each treatment he used the method of Yu Wan So's treatment and emphasized the balance between ying-energy(營氣) and wei-energy(衛氣), stomach energy(胃氣). 4. Ju Jin Heung(朱震亨) distinguished the cause of surgical disease from enternal(內), external(外) and channel(經). For each treatment he used the method to drain(疏通), promoting pus drainage(托裏) and the balance between ying-energy(營氣) and wei-energy(衛氣). The first stage used the clearing away heat and toxic materials(淸熱解毒) and the last stage used the invigoration and dispersion(補托). 5. Yu Wan So(劉完素) used to be very busy of Moschus(麝香), Alumen(白礬), Olibanum(乳香), Coptidis Rhizoma(黃連), Minium(黃丹) and Scutel1ariae Radix(黃芩) In classification of the medical action, great part of the medicine are activating blood circulation to dissipate blood stasis(活血祛瘀), clearing away heat and toxic materials( 淸熱解毒) and external application(外用). 6. Jang Jong Jeung(張從正) used to be very busy of Olibanum(乳香), Angelicae Gigantis Radix(當歸), Minium(黃丹), Myrrha(沒藥), Calomelas(輕粉), Rhei Radix Et Rhizoma(大黃) and Phellodendri Cortex(黃柏). In classification of the medical action, great part of the medicine arc activating blood circulation to dissipate blood stasis(活血祛瘀), clearing away heat and toxic materials(淸熱解毒) and diverged wind-cold evil(發散風寒). 7. Lee Dong Won(李東垣) used to be very busy of Forsythiae Fructus(連翹), Scuteliariae Radix(黃芩), Angelicae Gigantis Radix(當歸). Astragali Radix(황기), Glycyrrhizae Radix(炙甘草), Bupleuri Radix(紫胡), Phellodendri Cortcx(黃柏), and Coptidis Rhizoma(黃連). In classification of the medical action, great part of the medicine are activating blood circulation to dissipate blood stasis(活血祛瘀), regulating vital energy(理氣) and clearing away heat and toxic materials(淸熱解毒). 8. Ju Jin Heung(朱震亨) used to be very busy of Glycyrrhizae Radix(甘草), Astragali Radix(황기), Angelicae Gigantis Radix(當歸), Phellodendri Cortex(黃柏), Scutellariae Radix(黃芩), Ginseng Radix(人蔘), Qlibanum(乳香).   • The cause that the increase of animality fat intakes, under exercise, fatness, adding the stress, advanced age etc., the occurrence rate of the circulation system disease has been increased. And the thrombosis importantly came to the front as the risk factor of these circulation system's disease. Nowadays, the ischemic disease has especially discussed, for example the angina or myocardial infarction, originated in thrombosis that came from the platelet aggregation. In the western medicine, as the cure and prevention, using the aspirin or ticlopidine for platelet aggregation suppressant. But in the , the curing method must be used properly according to the pectoralgia or heartache's kind, state, grade. The platelet do not attache to the normal hemangioendothelial cell. But when it stimulated by endothelium peronia and so on, it attache to the injury endothelium or rise aggregation between the platelet. On this time, it secrete the platelet aggregation inducer as like ADP, thromboxane A2 from the inside of platelet. So it has first defensive function through the aggregation augment that prevent the celerity consumption of blood. But the activation of abnormal platelet occur the platelet grume and thrombogenesis. So it bring up the occlusive angiosis, so to speak, cardiovascular disease, cerebrovascular disease, arterial sclerosis. In oriental medicine, the thrombosis in the category of blood stasis and this blood stasis present the generalise or local blood circulation disturbance that generated by all kinds of pathological fact or blood stream retention accompanying with a series of syndrome. As the syndrome, stabbing pain fixed at certain region, squamous and dry skin, fullness and pain of the chest and hypochondrium, firmness and fullness of the lower abdomen, black stool, dark purple tongue or with ecchymoses and petechiae etc.. has been created. And it becomes the pathopoiesis cause that the convulsion and palpitation, severe palpitatiion, tympanites, the symtom complex with a mass or swelling in the abdomen, insanity, stricken by wind etc.. Moreover, it used the drugs for invigorating blood circulation and eliminating blood stasis or drugs for removing blood stasis for all kinds of syndrome through the blood stasis. And the drugs for activating the blood circulation, such as Salviae Radix, Angelicae Sinensis Radix, Persicae Semen, Achyranthis Radix, Cnidii Rhizoma, Carthami Flos are used for that. And it is used to the herbs of insects that has strong effect about the disintergrating blood stasis such as Hirudo, Scolopendrae Corpus, Buthus, Lumbricus etc.. On this study, It used Geijibokryunghwan(GBH) and the consisting herbs to investigate the influence of platelet aggregation about drugs that used to improvement various symptoms created by the thrombosis in oriental medicine. GBH formula has as formula recorded in the , action of 'eleminating the evil and not impairment of healthy energy' and 'promoting the flow of QI and cold and heat, so used for the expel blood stasis herbs from the ancient. Therefore we investigated the restraint effect of GBH and the consisting herbs about the platelet agregation induced to the ADP, AA or collagen. The conclusion is following. 1. When it added the aggregation inducer after that it added GBH and individual consisting herbs in the PRP, GBH showed the (+) inhibition effect on the platelet aggregation and it showed the (+) inhibition effect in the individual consisting herbs as like Paeoniae Radix and Moutan Cortex Radicis. 2. It showed the (+), (+,++) inhibition effect on the platelet aggregation in Paeoniae Radix Hoelen, Paeoniae Radix Moutan Cortex Radicis, Hoelen Moutan Cortex Radicis etc. 3. In the aggregation inhibition activating on the difference of density, GBH showed strong inhibition effect to the aggregation state induced to collagen, and it showed the inhibition effect in the individual consisting herbs as like Paeoniae Radix and Moutan Cortex Radicis about the aggregation induced by the collagen. 4. It showed the strong inhibition effect about the aggregation induced by the collagen in Paeoniae Radix Hoelen, Paeoniae Radix Moutan Cortex Radicis, Hoelen Moutan Cortex Radicis etc Like this, as confirm GBH and the individual consisting herb's inhibition effect of platelet aggregation, We considerated that GBH and the individual consisting herbs have practical applicational value of clinical trial in the thrombosis caused by platelet aggregation.

The effects of Sa-Mul-Tang (Si-Wu-Tang), a Traditional Chinese Medicine, on Phenylhydrazine-induced Anemic Rats

  • Sa-Mul-Tang (Si-Wu-Tang, SMT), a kind of Chinese medicine, has been used for the hemato-deficient disease for hundreds of years. In this work, investigations on the anti-anemic activity of an aqueous extract of SMT were undertaken in order to find the pharmacological basis for the ethnomedical use of the formulation. Three kinds of Angelicase species, such as Angelica sinensis, Angelica acutiloba, and Angelica gigas, were used for preparing the water extracts of SMT. Anemic model rats were induced by the treatment of phenylhydrazine (40 mg/kg/day, i.p.) for 4 days. After the treatment of phenylhydrazine, rats were divided into several groups for their different treatment of three kinds of SMT. Red blood cell (RBC), hemoglobin (Hg), and hematocrit (Hct) were determined on the day 0, 3, 6, 10, 14 after the treatment of SMTs and erythrocytes deformabilities were also determined at the end of experiments. Oral administration of SMT (1g/kg/day) for 14 days did not ameliorate drug-induced anemic states evaluated by RBC counts, Hg contents, and Hct values. However, the erythrocyte deformabilities were improved in phenylhydrazine-treated group by the administragest that SMTs (p<0.05). Especially, these effects were high in the Angelica acutiloba group. These results suggest that SMTs have an ameliorative effect on blood rheology related to the blood stasis syndrome in oriental diagnostics not on the blood deficient states related to the anemic syndrome.

Curative Effects of Typhae Pollen on the Diet-Induced Hyperlipidemia in Rats (포황이 흰쥐의 식이성 고지혈증 치료에 미치는 영향)

  • In Oriental Medicine, hyperlipidemia is considered as the phlegm stagnancy(痰飮) and blood stasis(瘀血). Typhae Pollen helps remove blood stasis and hemorrhage and it also has an effect of relieving pains caused by blood stasis. The purpose of this study is to investigate the curative effects of Typhae Pollen on diet-induced hyperlipidemia in rats. Thirty-two rats were divided into normal, control and treated group. Control and treated group were fed with 1.5% cholesterol diet during 4 weeks, and fed with normal diet during next 4 weeks. Treated group was also administrated with extract of Typhae Pollen during last 4 weeks. Blood samples were collected from each of animals at 4, 6 and 8weeks, and used for the blood chemical analysis. Animals were sacrificed at 8 weeks, and chemical analysis was performed on the collected liver samples. The serum total cholesterol(47.82±2.03㎎/㎗), LDL-cholesterol(11.09±1.27㎎/㎗), HDL-cholesterol (19.64±1.23㎎/㎗), triglyceride(85.09±3.48㎎/㎗) values of treated group were significantly decreased compared with the serum total cholesterol(73.73±5.34㎎/㎗), LDL-cholesterol(22.09±3.09㎎/㎗), HDL-cholesterol(25.18±1.45㎎/㎗l), triglyceride(124.27±6.72㎎/㎗) values of control group at 8weeks. The liver total cholesterol and triglyceride values of treated group show no statistically significant differency compared with those values of control group. On the basis of these results, it is confirmed that Typhae Pollen has the curative effects on the hyperlipidemia in rats induced by hyper-cholesterol diets.

Preventive Effects of Typhae Pollen on the Diet-induced Hyperlipidemia in Rats (흰쥐의 식이성 고지혈증에 대한 포황의 예방 효과)

  • Objectives : In Oriental Medicine, hyperlipidemia is considered as phlegm retention (痰飮) and blood stasis (瘀血). Typhae pollen helps remove blood stasis and hemorrhage, and it also has an effect of relieving pain caused by blood stasis. The purpose of this study is to investigate the preventive effects of Typhae pollen on diet-induced hyperlipidemia in rats. Methods : Thirty-two male Sprague-Dawley rats were divided into normal, control and treated groups. The control and treated groups were fed with a 1.5% cholesterol diet over 4 weeks, and extract of Typhae pollen was administered to the treated group during the same period. Blood samples were collected from each animal at 0 and 4 weeks and used for the blood chemical analysis. Animals were sacrificed at 4 weeks and chemical analysis was performed on the collected liver samples. Results : The serum total cholesterol ($122.45{\pm}4.96mg/dL$), LDL-cholesterol ($54.27{\pm}4.95mg/dL$), and triglyceride ($122.00{\pm}7.33mg/dL$) values of the treated group were significantly lower compared with the serum total cholesterol ($147.91{\pm}7.22mg/dL$), LDL-cholesterol ($81.91{\pm}8.34mg/dL$), and triglyceride ($169.27{\pm}12.34mg/dL$) values of the control group. The liver total cholesterol (TEX>$230.18<{\pm}18.81mg/dL$) values of the treated group were significantly higher compared with the liver total cholesterol ($178.54{\pm}$10.11mg/dL) values of the control group. Conclusions : On the basis of these results, it is confirmed that Typhae pollen has preventive effects on diet-induced hyperlipidemia in rats.

Literature Review on Parkinson's Disease in Oriental Medicine (파킨슨병의 한의학적 고찰 -병인병리(病因病理)와 침구요법(鍼灸療法)을 중심으로-)

  • Objective: In order to find oriental medical therapies on Parkinson's disease and to make a fundamental basis for clinical application, this study was performed. Methods: We reviewed 35 kinds of the ancient and modern text, and related articles. Results: Parkinson's disease is an extrapyramidal disease characterized by akinesia, tremor at rest, rigidity, and slowness of movement. In old oriental medical text, Parkinson's disease is described as Jinjeon(振顫), Chi, Gyeong (痙), Gyejong, Ryeon(攣) and Pyeongo(偏枯), Tantan, Jungbu(中腑症) of Jungpung(中風). According to the text, major pathological causes were Wind(風), Fire(火), Phlegm(痰), and Blood stasis(瘀血). And Parkinson's disease can be classified into four clinical types as liver & kidney yin-deficiency, qi & blood deficiency, retention of phlegm-heat, and qi-stagnation & blood stasis. Standardized acupuncture points are GV20, GB20, GV14 on head, CV12, ST25 on abdomen, GV26, ST7, GB1, S14, LI20 on face, LI4, LI11, TE5, SI3, HT3, LI15, SI6 on upper extremity, and ST36, GB34, SP6, LR3, KI1, GB30, BL40 on lower extremity. Other methods, such as scalp acupuncture, electro-acupuncture, and herb-acupuncture, can be applied to treat Parkinson's disease. Conclusions: We find out that there are oriental medical concepts related with Parkinson's disease such as Jinjeon(振顫), Chi, Gyeong (痙), Gyejong, Ryeon(攣) and Pyeongo(偏枯), Tantan, Jungbu(中腑症) of Jungpung(中風), of which major causes are Wind(風), Fire(火), Phlegm(痰), and Blood stasis(瘀血). We can also apply many effective acupuncture points and acupuncture therapies according to differential diagnosis, for example, liver & kidney yin-deficiency. qi & blood deficiency, retention of phlegm-heat, and qi-stagnation & blood stasis.
