Comparison of ultrasonography guided serratus anterior plane block and thoracic paravertebral block in video-assisted thoracoscopic surgery: a prospective randomized double-blind study

  • Background: Various truncal block techniques with ultrasonography (USG) are becoming widespread to reduce postoperative pain and opioid requirements in video-assisted thoracoscopic surgery (VATS). The primary aim of our study was to determine whether the USG-guided serratus anterior plane block (SAPB) is as effective as the thoracic paravertebral block (TPVB) in VATS. Our secondary aim was to evaluate patient and surgeon satisfaction, block application time, first analgesic time, and length of hospital stay. Methods: Patients in Group SAPB received 0.4 mL/kg bupivacaine with a USG-guided SAPB, and patients in Group TPVB received 0.4 mL/kg bupivacaine with a USG-guided TPVB. We recorded the pain scores, the timing of the first analgesic requirement, the amount of tramadol consumption, and postoperative complications for 24 hours. We also recorded the block application time and length of hospital stay. Results: A total of 62 patients, with 31 in each group (Group SAPB and Group TPVB) completed the study. Between the two groups, there were no significant differences in rest and dynamic pain visual analog scale scores at 0, 1, 6, 12, and 24 hours after surgery. The total consumption of tramadol was significantly lower in the TPVB group (P = 0.026). The block application time was significantly shorter in Group SAPB (P < 0.001). Conclusions: An SAPB that is applied safely and rapidly as a part of multimodal analgesia in patients who undergo VATS is not inferior to the TPVB and can be an alternative to it.

  • Commercial transactions, one of the pillars of the capitalist economy, are occurring countless times every day, especially small and medium-sized businesses. However, small and medium-sized enterprises are bound to be the legal underdogs in contracts for commercial transactions and do not receive legal support for contracts for fair and legitimate commercial transactions. When subcontracting contracts are concluded among small and medium-sized enterprises, 58.2% of them do not apply standard contracts and sign contracts that have not undergone legal review. In order to support small and medium-sized enterprises' fair and legitimate contracts, small and medium-sized enterprises can be protected from legal threats if they can reduce the risk of signing contracts by analyzing various risks in the contract and analyzing and informing them of toxic clauses and omitted contracts in advance. We propose a risk prediction model for the machine reading-based legal contract to minimize legal damage to small and medium-sized business owners in the legal blind spots. We have established our own set of legal questions and answers based on the legal data disclosed for the purpose of building a model specialized in legal contracts. Quantitative verification was carried out through indicators such as EM and F1 Score by applying pine tuning and hostile learning to pre-learned machine reading models. The highest F1 score was 87.93, with an EM value of 72.41.

A Study on the Characteristics of the Spatial Distribution and the Disparities in the Provision of Public Libraries in Busan (부산지역 공공도서관 분포의 특성과 공급 불균형 양상 분석)

  • Public library usage is closely related to the accessibility to library facilities. Therefore, public library planning and development authorities should consider the policies for improving the library accessibility of community, for releasing disparities of the spatial accessibility and for increasing location efficiency of public libraries. In this regard, this study strives to analyze the spatial distribution of public libraries in Busan and to derive the regions that lack public libraries by identifying main characteristics using geographical information systems (GIS): identify the blind spot for public library service, analyze the hot and cold spot for the supply of libraries, and identify the vulnerable areas of library based on population density. The result of the study will contribute to understand the spatial distribution of public libraries in Busan and to prioritize sites where public library should be constructed in order to improve the accessibility to public library services.

Study on Safety Management Status and Policy Directions of Small and Medium-sized Building Construction Field in Seoul (서울시 중·소형 건축공사장의 안전관리 실태와 대책 마련 연구)

  • Seoul metropolitan government of South Korea has large population and facilities, and lots of building construction have been performed in urban area. Many safety accidents with causing lives and property damages occurred at construction sites in South Korea and Seoul. Thus, Korea government and Seoul have made efforts to secure the safety of construction sites. In this study, current law and measures by central department and Seoul were analyzed to find limitations and improvement points in construction safety management. Safety inspection results for construction sites in Seoul were investigated to demonstrate the main hazard work and safety risk, and interview surveys targeting safety managing persons were performed according to safety management, inspection, and education. Based on analytical results of current status on safety management, improvement measures were proposed for safety secure on construction safety in Seoul. The proposed measures include improvement of safety management systems, alleviation of safety blind spots according to construction size, strengthening efficiency of safety management, and autonomous safety engagement by construction sites centered.

Smart City Policy Promotion and Gyeonggi-do Strategy (스마트시티 정책추진과 경기도 광역자치단체 전략)

  • The purpose of this study is to investigate the role of Gyeonggi provincial governments in smart city policy. In order to satisfy the purpose of the study, we closely identified how may local governments(si. gun) have been taking participated in smart city project of the central government, and its characteristics. Then we know the detailed strategies that Gyeonggi provincial governments should do. Gyeonggi-do provincial governments enacted smart city-related ordinances in 2020 and participated in the Smart City Local Council. In the future, Gyeonggi privincial government will propose amendments to the law, review participation in the central government smart city policy project, the "integrated platform construction project of the metropolitan government model", and propel Gyeonggi-do's own smart city policy based on the blind spots of the policy.

The Effect of Comfrey on Enoxaparin-Induced Bruise in Patients with Acute Coronary Syndrome: A Randomised Clinical Trial

  • Objectives: Bruising is an important side effect of enoxaparin injection. Comfrey (Symphytum officinale) is used to treat different types of wounds, bone fractures, and bruising in traditional medicine in many countries for centuries. This study aimed to determine the effects of the Comfrey ointment on the bruise size and color following enoxaparin injection. Methods: This double-blind randomized clinical trial was conducted on 80 patients with acute coronary syndrome (ACS). The participants were randomly divided into two groups of 40, namely Comfrey and Placebo. Changes in bruise size and color in both groups were assessed daily before and after the intervention. Results: The Comfrey and Placebo groups were homogeneous in demographic and clinical variables. A downward trend was observed in the bruise size in both groups throughout the study. However, the bruise size was smaller in the Comfrey group than the Placebo group on day 2-5 of the intervention. Moreover, there was a significant difference in bruise color between the groups, with a shorter healing course in the Comfrey group. Conclusion: The Comfrey ointment accelerated the healing process of enoxaparin injection-induced bruising in patients with ACS. It is recommended as a safe and simple approach for these patients.

Chamomile Extract versus Clotrimazole Vaginal Cream in Treatment of Vulvovaginal Candidiasis: A Randomized Double-Blind Control Trial

  • Objectives: Vulvovaginal candidiasis (VVC) treatment is advised for all women due to its symptoms and complications. In this study, the standard treatment, clotrimazole, was compared with chamomile extract cream in outpatient clinics. Methods: We recruited 73 women with VVC, who were randomly allocated into two groups, clotrimazole versus chamomile extract cream. After two weeks of treatment with the same criteria, cheese-like vaginal discharge, itching and burning sensations, strawberry cervix, and recovery percentage was evaluated. Results: Thirty patients in each group were analyzed. There was no significant difference in age and number of pregnancies between groups (p = 0.85 and 0.09, respectively). Comparing before and after treatment, cheese like discharge (p < 0.001), itching (p < 0.001), burning (p < 0.001) had significantly improved in both groups. Further, the recovery percentage was not significantly different between groups (88.9% vs 75% in the chamomile vs clotrimazole groups, respectively). Conclusion: Chamomile is as effective as clotrimazole in VVC treatment; a higher percentage of women who used this medication recovered, although this did not reach significance. In addition, no complications were reported in either group.

The effect of wall squat exercise according to the difference in the support surface on the muscle thickness and balance of the trunk (지지면 차이에 따른 월 스쿼트 운동이 몸통 근두께와 균형에 미치는 영향)

  • Background: The purpose of this study is to investigate the effect of wall squat exercise according to the difference in the support surface on the thickness change of external oblique, internal oblique and transverse abdominalis of the trunk muscles and the change in distance of center of pressure when the eyes are opened and closed. Design: Randomized controlled trial. Methods: The subjects were 26 healthy adults, 18 males and 8 females. The composition of each group using the single-blind method was 13 people in the unstable side wall squat exercise group and 13 people in the stable side wall squat exercise group. Experimental measurements were divided into before, 3 weeks, and 6 weeks after the experiment, and changes in muscle thickness and balance were confirmed. Muscle thickness was measured using ultrasinic, and static balance change was measured using Bio-rescue. Results: There were significant differences in the thickness changes of external oblique, internal oblique and transverse abdominalis according to the wall squat exercise method by period and in the interaction between the period and the group (p<0.05). As a result of the post-hoc analysis, there was a significant difference in the change between the two groups in external oblique muscle after 6 weeks. And there was a significant difference in the distance of the pressure center between eyes open and closed eyes (p<0.05). Conclusion: In terms of instability, wall squat exercise was positive for changes in trunk muscle thickness. It suggests the possibility of using it for trunk muscle strengthening training in the future.

Quality Evaluation of Ultrasonographic Equipment Using an ATS-539 Multipurpose Phantom in Veterinary Medicine

  • The purpose of this study is to examine the status of quality control using multipurpose phantom of ultrasound equipment used in hospital of veterinary college in South Korea by using ATS-539 multipurpose phantom so as to examine quantitative and objective new image evaluation method. Specialists discussed and analyzed multipurpose phantom images acquired by using convex transducer of 10 ultrasound imaging devices, currently used in 9 veterinary colleges, at 4.0-6.0 MHz. Total 8 items that can be measured with ATS-539 multipurpose phantom including dead zone, vertical and horizontal measurement, axial/lateral resolution, sensitivity, focal zone, functional resolution and gray scale/dynamic range were evaluated. For qualitative evaluation, valid decisions were made based on dead zone, axial/lateral resolution, and gray scale/dynamic range which are resolution index, and coefficient of variation (COV) and blind referenceless image spatial quality evaluator (BRISQUE) were found to increase objectivity. As a result of experiment, all the targeted ultrasonic devices were found appropriate from qualitative evaluation items of dead zone, axial/lateral resolution, and gray scale/dynamic range. In other evaluation items, they were found to be appropriate from focal zone and vertical measurement of quantitative evaluation while inappropriate from horizontal measurement, sensitivity, and functional resolution. COV value was 0.12 ± 0.04, and BRISQUE value was 47.77 ± 2.77, both analysis results show that the noise level of all ultrasonic devices was located within tolerance range. Upon image examination using ATS-539 multipurpose phantom, they were 100% appropriate with inspection standards of dead zone, axial/lateral resolution, and gray scale/dynamic range, and besides, focal zone and functional resolution can be used as evaluation items. In the field of veterinary medicine, 8 standard items using ATS-539 multipurpose phantom and image evaluation items using COV and BRISQUE can be used as standards for quality control of ultrasonography machine.

Effect of Marham-i-Raal on Episiotomy Wound Healing: A Single-Arm pre-and post-treatment study

  • Episiotomy is the commonest obstetrics intervention in the world to reduce severe perineal injuries. Its prevalence is 43% to 100% in primiparous women in Asia. Further, worldwide approximately 10-95% of pregnant women undergo episiotomy incision during birth. Delay in wound healing probably increases the risk of wound infection, changes the muscular structure, and ultimately causes muscle tone loss. So, wound care is of specific significance to postnatal maternal outcomes. Hence, this case study aimed to evaluate the effect of Marham-i-Raal in episiotomy wound healing and pain relief. The study was conducted in eleven postpartum primi or multipara pregnant women aged between group 19 and 35 years with term gestational age, singleton pregnancy in a cephalic presentation who had a normal vaginal delivery with mediolateral episiotomy, without a perineal tear, and perineal hematoma. Externally, application of Marham-i-Raal 2g on episiotomy incisional wound, twice a day for 10 days was advised. Wound healing of episiotomy and pain intensity was assessed with REEDA ["redness, oedema, ecchymosis, discharge and approximation of the edges"] scoring and VAS scoring for pain intensity respectively. At one hour (baseline), the REEDA mean score of eleven patients was 3.90±1.04 whereas on day 7-10 it was 0.18±0.40 with statistically significant difference (p<0.001). The VAS mean score at one hour was 6.90±1.22 whereas on day 7-10 it was 0.72±0.78 with a statistically significant difference (p<0.001). Marham-i-Raal would be effective in episiotomy wound healing and reducing pain intensity. Further, randomized double-blind controlled trials in large sample size are recommended.
