Effect of Bladder Wall Thickness Through Change of Bladder Volume and Material Properties on Detrusor activity Study (체적의 변화를 통한 방광벽 두께와 기계적 재료상수 변화가 배뇨근 활동에 미치는 영향)

  • The structural and functional disorder of a detrusor induces a bladder hypertrophy and degenerates a bladder muscle gradually by preventing normal urination. Thus, the thickness of the bladder wall has been increased in proportion to the degree of bladder outlet obstruction. In this study, the mechanical characteristics of the detrusor is analyzed for the physical properties and the thickness changes of the bladder muscle using a mathematically analytic method. In order to obtain the mechanical property of the bladder muscle, the tensile test of porcine bladder tissue is performed because its property is similar to that of human. The result of tensile test is applied to the mathematically model as Mooney Rivlin coefficients which represent the hyperelastic material. The model of the bladder is defined as the spherical shape with the initial volume of 50ml. The principal stress and strain according to the thickness are analyzed. Also, computer simulations for three types of the material property for the model of the bladder are performed based on the fact that the stiffness of the bladder is weakened as the progress of the benign prostatic hyperplasia. As a result, the principal stress is 341kPa at the initial thickness of 2.2mm, and is 249kPa at 6.5mm. As the bladder wall thickness increases, the principal stress decreases. The principal stress and strain decrease as the stiffness of the bladder decreases under the same thinkness.

  • Transitional cell carcinoma(TCC) was identified by cytological and ultrasonographic findings in an 11-year-old, intact female Yorkshire terrier with intermittent hematuria for three years. In color-doppler ultrasound images there was marked irregular wall thickness of the bladder surface and well-defined hyperechoic focal lesions within the mass. Active vascular response was observed in the surroundings of hyperechoic lesions.

  • To investigate diagnostic imaging of cystitis and to apply it to the small animal practice, ultrasonogram of urinary bladder with moderate distension(4ml/kg) and with complete distension(9ml/kg) to evaluate the irregularity and thickness of bladder, radiography and histopathological examination were performed after experimental cystitis induction. On double contrast cystography, mucosal membrane of the urinary bladder was smooth and thickening urinary bladder wall was not found before cystitis induction. At 3rd day post induction(PI), mucosal irregularity was noted at the cranioventral region of the urinary bladder. Thickening of the urinary bladder wall and filling defect was observed as well. Cystographic findings of 7, 10, 15, 21 day PI were similar to that of 3rd day PI. On ultrasonographic findings of the mural thickness in induction group, it was ascertain that the mural thickness with moderate distention was more thickened than with complete distention at transverse scan. Ultrasonographic findings at longitudinal scan were similar to those of transverse scan. On ultrasonographic findings of mucosal irregularity in induction group, from PI to 7 day PI, mucosal irregularity with moderate distention was more irregular than mucosal irregularity with complete distention. At 10 day PI, there was similarity between moderate distention and complete distention. On histopathologic examination of a section of urinary bladder taken 3 day PI, the mucosal and submucosa were infiltrated by a mixture of thick inflammatory exudate which was composed of neutrophil, plasma protein, bacterial colony and necrotic cells. Congestion, hemorrhage and edema were also observed in the submucosa. At 7th day PI, the mucosal change was similar to that of 3rd day PI, but neovascularization and fibroplasia were observed in the submucosa. At 15th and 21th day PI, mild hyperplasia of mucosal epithelium was observed in the mucosa. Deposition of collagen, neovascularization and severe diffuse infiltration of lymphocyte were observed. These results suggest that ultrasonographic examination with moderate distention is considered to be a more simple, rapid, noninvasive, sensitive and useful diagnostic method than other diagnostic methods for the diagnosis of the cystitis and the differential diagnosis of urinary tract infection.

Air Pressure Regulation in Air Bladders of Ascophyllum nodosum(Fucales, Phaeophyceae)

  • Diurnal and age-related changes in air pressure were measured in air bladders of Ascophyllum nodosum from the Atlantic coast of Nova Scotia. Exterior and interior bladder volumes vary significantly with 4 and 6 y bladders being about 40% larger than 2 y bladders (p < 0.01). Freshly collected bladders yielded a mean pressure of 40.8 ± 6.5 cm H2O. Overnight (20 h) dark treatment at 15°C generated pressure reductions by 80% in 2 y bladders but only by about 30% in 4 and 6 y bladders. Furthermore, in 2 y bladders 8 out of 11 bladders were reduced to atmospheric pressure. Pressure losses were inversely related to pressure recovery after 2.5 h in natural daylight, but after 5 h in daylight there was no significant difference in pressure within the age groups. Even under 25% of full illumination, bladders inflated to full pressure after 5 h. The results show that differences in bladder age and bladder wall thickness have roles in diurnal patterns of bladder inflation and deflation. These results confirm that bladder inflation is based on photosynthetic O2 production and not on partial pressures of O2 in the surrounding medium as was suggested for Sargassum. Chemical analyses of fluid recovered after the interior of bladders were washed with saline showed no evidence for the occurrence of surfactant that might be responsible for maintaining the air-liquid interface.

  • One consideration of radiation delivery in cervical cancer is the complication of critical organs, e.g., bladder and rectum. The absorbed dose of bladder and rectum in HDR intracavitary brachytherapy is measured indirectly with TLD dosimetry A method for the complication reduction of bladder and rectum is suggested. For two-hundred cervical cancer patients, follow-up MRI images were reviewed and distances from cervical central axis to bladder and rectum and vaginal wall thickness were measured. The sealed TLDs were placed upon the gauze packing of the ovoids and the distances to the TLDs from the ovoid center were measured in the simulation film and actual doses of bladder and rectum were calculated. From published data, maximal tolerance doses of bladder and rectum were derived and based on the permissible doses per fraction in HDR brachytherapy the packing thicknesses were determined in both directions. The required minimal packing thicknesses for bladder and rectum were 0.43 and 0.92 cm, respectively. The results were compared with computer calculation using the Meisberger polynomial approach. It is our hope this study can be used for a guideline for users in clinic in estimating critical organ dose in bladder and rectum in HDR brachytherapy in vivo dosimetry.

The Investigation Regarding the Dose Change due to the Heterogeneity of Prostate Cancer Treatment with IMRT (전립선암의 세기조절 방사선치료 시 불균질부에 의한 선량변화에 관한 고찰)

  • Purpose: The pelvic phantom was fabricated in the following purposes: (1) Dose verification of IMRT plan using Eclipse planning computer, (2) to study the interface effect at the interface between rectal wall and air. The TLD can be inserted in the pelvic phantom to confirm the dose distribution as well as uncertainty at the interface. Materials and Methods: A pelvic phantom with the dimension of 30 cm diameter, 20 cm height and 20 cm thickness was fabricated to investigate the dose at the rectal wall. The phantom was filled with water and has many features like bladder, rectum, and prostate and seminal vesicle (SV). The rectum is made of 3 cm-dimater plastic pipe, and it cab be blocked by using a plug, and film can be inserted around the rectal wall. The phantom was scanned with Philips Brillance scanner and various organs such as prostate, SV, and rectal wall, and bladder wall were delineated. The treatment parameters used in this study are the same as those used in the protocols in the SNUH. TLD chips are inserted to the phantom to evaluate the dose distribution to the rectal wall (to simulate high dose gradient region), bladder wall and SV (to simulate the high dose region) and 2 spots in anterior surface (to simulate the low dose region). The TLD readings are compared with those of the planning computer (ECLIPSE, Varian, USA). Results: The target TLD doses represented as the prostate and SV show excellent agreements with the doses from the RTP within +/-3%. The rectal wall doses measured at the rectal wall are different from the those of the RTP by -11%. This is in literatures called as an interface effect. The underdosages at the rectal wall is independent of 3 heterogeneity correction algorithm in the Eclipse RTP. Also the low dose regions s represented as surface in this study were within +/-1%. Conclusion: The RTP estimate the dosage very accurately withihn +/-3% in the high dose (SV, or prostate) and low dose region (surface). However, the dosage at the rectal wall differed by as much as 11% (In literatures, the underdosage of 9$\sim$15% were reported). This range of errors occurs at the interface, for example, at the interface between lung and chest wall, or vocal cord. This interface effect is very important in clinical situations, for example, to estimate the NTCP (normal tissue complication probability) and to estimate the limitations of the current RTP system. Monte-carlo-based RTP will handle this issue correctly.

Effects of Talc on Cyclophosphamide-Induced Cystitis in the Rat (Cyclophosphamide유발 방광염에 대한 활석의 효능)

  • We investigated the effects of aqueous extract from talc for suppression in the process of cyclophosphamide-induced cystitis in the rat. The weight of urinary bladder was increased in the cyclophosphamide-injected rat compared with normal, but the increase of weight was arrested by intake of talc. More similar results showed in the uric test involving nitrate content and blood cell number and serum analysis involving the content of blood urea nitrogen and uric acid in the talc challenged rat compared with control one. More severe histological changes of urinary bladder such as edema, wall thickness, bleeding, vacuolation in mucosal epithelium were demonstrated in the rat injected with cyclophosphamide compared with normal. Fewer scores of these changes such as edema and bleeding were observed in rats treated with talc. In the immunohistochemical analysis, the expression of inflammatory-related protein examined tended to increase in the urinary bladder of cyclophosphamide-injected rat, especially COX-2 and $TNF-{\alpha}$ in mucosal epithelium and iNOS and $IL-1{\beta}$ in mucosal and muscular layer. The decline of these immunoreation were observed in the talc treated rat, significant decrease of COX-2 was detected in mucosal epithelium and iNOS in submucosal layer. These results suggest that talc may use as a useful therapeutic agent for noninfectious cystitis.

Opisthorchis felineus and Metorchis bilis Metacercariae in Cyprinid Fish Leuciscus idus in Nura-Sarysu River, Kazakhstan

  • Aim of the present study was to provide presence of opisthorchiid metacercariae in cyprinid fish Leuciscus idus in Nura-Sarysu river, Kazakhstan. Infection rate of the ides by the metacercariae was 42%. The metacercariae, similar morphologically to those of the liver flukes, were found: elliptical in shape, $0.19-0.25{\times}0.15-0.22mm$, oral and ventral suckers nearly equal size, and excretory bladder O-shape with black content, occupying posterior part of the body. The metacercariae were divided into 2 groups with differences in size and thickness of cyst wall. Adult flukes were recovered from the Syrian hamsters infected with the opisthorch metacercariae and identified with morphological characters to Opisthorchis felineus and Metorchis bilis. DNA sequences of ITS1, ITS2, and cox1 supported the taxonomic assignment.

Echinostoma revolutum: Metacercariae in Filopaludina Snails from Nam Dinh Province, Vietnam, and Adults from Experimental Hamsters

  • We detected metacercariae of Echinostoma revolutum in Filopaludina sp. snails purchased from a local market in Nam Dinh Province for the first time in Vietnam. Adult flukes were harvested from experimentally infected hamsters at days 14 and 17 post-infection. The metacercariae were round, $170-190{\mu}m$ (n=15) in diameter, with a cyst wall thickness of about $12{\mu}m$. A total of 37 collar spines were arranged around the head collar, and large excretory granules were seen in 2 canals of the excretory bladder. The 14-day old adult flukes were elongated, ventrally curved, and $5.0-7.2{\times}0.8-1.3mm$ (n=20). The head collar had a total of 37 collar spines arranged in 2 alternating rows, including 5 corner spines on each side. The cirrus sac contained a saccular seminal vesicle, a prostatic gland, and an unarmed cirrus. Two tandem testes were smooth or slightly lobed. Eggs were ovoid to elliptical, $110-118{\times}70-75{\mu}m$. These morphological characters were similar to those of E. revolutum and E. jurini. We tentatively identified it as E. revolutum because the validity of E. jurini remains to be elucidated. The taxonomic relationship of E. revolutum and E. jurini is discussed.

Indochinamon ou (Crustacea: Potamidae) as a New Second Intermediate Host for Paragonimus harinasutai in Luang Prabang Province, Lao PDR

  • Paragonimus harinasutai metacercariae were found in a species of freshwater crab, Indochinamon ou, collected in a small stream of Namback District, Luang Prabang Province, Lao PDR. Adult flukes were recovered after experimental infection of the metacercariae to dogs. Metacercariae were round or slightly elliptical, $0.666{\times}0.626\;mm$ in average size, and had a thin cyst wall of about $20{\mu}m$ in thickness, a black excretory bladder, convoluted ceca, and some pinkish materials in the body. Adults were somewhat elongated, $95.2{\times}36.5\;mm$ in average size, covered with single-tipped tegumental spines, had a smaller oral sucker than the ventral sucker, a moderately branched ovary, and 5-6 lobulated testes. Eggs were ovoid and bilaterally symmetrical in shape, $79{\times}45{\mu}m$ in average size, and had a uniformly thickened shell. By the present study, it has been confirmed that I. ou is a new second intermediate host for P. harinasutai.
