There has been increasing recognition of pediatric bipolar disorder in the psychiatric field during the past 10 years. The clinical presentation of this disorder in preadolescent is greatly debated and few studies have been conducted in Korea. The authors report 3 cases of children with bipolar I disorder whose clinical symptoms were improved after pharmacotherapy. The authors also review current concepts, debates and treatment of pediatric bipolar disorder.
Purpose: Mood disorders such as depression and bipolar disorder are a major mental health problem in college students. We investigate the prevalence of depression and bipolar disorder and the relevance of risk factors for these mood disorders among one college freshmen. Methods: The subjects were 2,865 college students who entered one university located in Seoul and Ansung in 2009. We used BDI (Beck Depression Inventory) for depression assessment and K-MDQ (Mood Disorder Questionnaire) for bipolar disorder assessment. Demographic and socioeconomic factors were measured by questionnaire. Height, weight, blood pressure, total cholesterol, complete blood cell count, and liver function test data were obtained by physical examination for freshmen. Chi-square test and multiple logistic regression were performed to analyze the possible risk factors for depression and bipolar disorder. Results: With different BDI cutoff value, 16 and 21, the prevalence of depression was 8.7% (male: 7.6%, female: 10.1%) and 2.4% (male: 2.5%, female: 2.3%), separately. 'Low economic status', 'urban birth place', and 'low grade at entrance' were significantly associated with depression. Using the original cutoff criterion, defined as clustering of 7 or more symptoms that caused moderate or severe problems, the prevalence of bipolar disorder was 1.3% (male: 1.4%, female: 1.1%). The risk factor of bipolar disorder was academic fields (art fields). Conclusion: Depression and bipolar disorder are common disease in college freshmen. Therefore, Campus-based mental health service program is needed to help with prevention of and early intervention of these mood disorders.
Objective:This study was performed to investigate the prescribing patterns of antimanic agents in the treatment of acute bipolar disorder inpatients in Korea from 1990 through 2000. The results will serve as the basic data for the practice guideline for the pharmacotherapy of bipolar disorder patients in Korea. Method:Retrospective chart review of bipolar disorder inpatients of Soonchunhyang Medical Center in Seoul and Chun-An was conducted for each of the year 1990, 1995, and 2000. The following data are collected ; 1) demographic data, 2) history of bipolar disorder, 3) length of hospital stay, 4) detailed drug titration records of antimanic agents and antipsychotic agents. Results:During the last decade, the frequency of lithium monotherapy was decreased obviously. Instead, more than half of the patients in 2000 were on combination therapy of lithium and anticonvulsants. Lithiumvalproate combination was the preferred strategy and the use rate of carbamazepine has been decreased. In addition, most of the patients were given antipsychotic agents during the last 10 years. And recently, atypical antipsychotics were increasingly prescribed. These changes in the field of pharmacology of bipolar disorder have resulted neither in shorter hospital stays nor lower dosages of concurrent neuroleptics. Conclusions:The results indicate the trends in the prescribing of antimanic agents for the treatment of bipolar disorder in Korea across the past 10 years. Mostly, the change seems to correspond to the international practice guideline. More systematic research is needed to find out the clinical benefits of the anticonvulsants in the real practice of treatment of bipolar disorder.

Objectives: Mood disorder is highly comorbid with attention deficit/hyperactivity disorder (ADHD), and comorbid ADHD symptoms are associated with suicide risk. The aim of this study was to examine the association between comorbid ADHD symptoms and the number of suicide attempts among patients with unipolar and bipolar depression. Methods: In this cross-sectional study, 100 patients with either unipolar or bipolar depression constituted the final sample. Pearson's correlation and multiple regression analyses were conducted to examine the association between comorbid ADHD symptoms and suicide risk. Results: Among patients with bipolar depression, the number of suicide attempts was significantly correlated with ADHD symptoms (r=0.324, p<0.01). ADHD symptoms significantly predicted the number of suicide attempts (β=0.249, p<0.05). Conclusion: Our findings underscore the importance of screening and assessing ADHD symptoms in male young adults with depressive disorders.
Circadian rhythm is a periodic and continuous change in physiological, behavioral, and mental characteristics that occurs in most organisms on the Earth, because the Earth rotates in a 24-hour cycle. The circadian system regulates daily rhythms of physiology and behavior, such as the sleep/wake cycle, body temperature, hormonal secretion, and mood. The influence of circadian rhythm is very powerful, but limited research has addressed its effects. However, many recent studies have shown that circadian dysregulation may play an important role in the pathogenesis of bipolar disorder. This review study examined current and noteworthy studies, including the authors' own works, and proposes a possible clinical application of bipolar disorder based on evidence that circadian rhythm dysregulation in bipolar disorder may be a key pathogenetic mechanism.
Subsyndromal bipolar symptoms are common during maintenance treatment and appear to be associated with relapse into an episode of the same polarity. This implies subsyndromal symptoms are an important problem in recurrent bipolar disorder and require more additive and infallible therapeutic intervention. Undetected, untreated subsyndromal states lead patients to have poor prognosis and quality of life. The combination of a long undetected illness and significant psychosocial impairment renders early identification and intervention vital for the treatment of bipolar disorders. Methods for early identification includes finding prodromes, using screening tools such as the HCL-32 (Hypomania Checklist-32) and the BSDS (bipolar spectrum diagnostic scale). Various augmentation treatment methods would be needed to reduce subsyndromal symptoms, especially, psychosocial treatment has the potential to help patients address the multiple psychosocial problems associated with this chronic illness. To overcome difficulties of diagnosing subsyndromal disorder and to treat it appropriately, a staging system was suggested by some researchers. It assumes that earlier stages have better prognosis and require simpler therapeutic regimens. Staging may assist in treatment planning and prognosis of bipolar disorder, and emphasize the importance of early intervention. Further research is required in this exciting and novel area.

성인기의 양극성장애와 정신분열병 환자에서 아동기에 주의력결핍-과다활동 증상을 조사하여 서로의 연관성을 알아보고자 하였다. 저자들은 40세 미만의 남자 중 원주기독병원에서 추적진료중인 26명의 양극성장애군, 29명의 정신분열병군, 그리고 27명의 정상대조군에서 Wender Utah Rating Scale(WURS), WURS 25개 항목(WURS-25), Parent Rating Scale(PRS)을 이용하여 아동기의 주의력결핍-과다활동 증상을 비교하였다. 양극성장애군은 정상대조군과 비교하여 WURS의 평균득점이 의미있게 높았다. WURS-25와 PRS의 평균득점도 양극성장애군이 다른 두 군에 비해 의미있게 높았다. 그러나 정신분열병군에서 WURS, WURS-25, PRS 각각의 평균 득점은 정상대조군과 유의한 차이가 없었다. WURS와 PRS는 양극성장애군과 정상대조군에서 상관관계를 보였지만 정신분열병군에서는 상관관계가 없었다. 양극성장애군에서 정신분열병군과 정상대조군에 비해 아동기의 주의력결핍-과다활동 증상이 더 많았으며, 이 결과는 양극성장애와 주의력결핍-과다활동장애의 연관성을 시사한다.

소아, 청소년기에 발생하는 양극성장애는 주의력결핍/과잉행동장애, 행동장애 뿐만 아니라 불안장애도 중복진단 비율이 높다. 불안장애 중 강박장애, 사회공포증, 공황장애, 분리불안장애가 많이 공존한다고 보고 되었지만 불안장애와 양극성장애 발생사이의 시간적 관련성에 관한 전향적이면서 믿을만한 연구는 드물다. 본 저자는 양극성장애에 선행한 불안증상에 대해 항우울제를 투여하였을 때 발생한 조증삽화 2예를 보고하고자 한다. 기분변화의 과거력이나 양극성장애의 가족력이 없어도 선행된 불안증상에 대해 항우울제를 사용할 때는 조증삽화의 가능성을 염두에 두고 조심스럽게 처방해야 될 필요성이 있으며 향후 소아, 청소년 양극성장애와 불안장애 사이의 시간적 연관성과 약물학적 치료 지침에 대해 포괄적이며 전향적인 연구가 필요할 것이다.
저자들은 뇌졸중후 우울증보다 매우 드물게 발생하는 뇌졸중후 양극성장애 증례를 경험하여 보고하였다. 좌반구 기저핵 뇌경색이후 뇌졸중후 우울증을 앓아오던 환자에서 조증 에피소드가 발병하여 원인을 규명하는 과정에서 자기공명영상을 시행한 결과, 이차성 양극 성장애와 연관되는 것으로 보고되어온 우반구 기저핵의 새로운 뇌경색이 밝혀졌다. 한 환자에서 좌반구 뇌졸중이후 우울증, 우반구 뇌졸중이후 양극성장애의 발생이 시간적 연관성을 보였으며, 한편 뇌졸중후 우울증과 이차성 양극성장애 각각이 병변 위치와 특이한 연관성을 보여주고 있다. 문헌 고찰에 의하면, 이차성 조증이 주로 피질의 병변과 연관성을 보이는 반면, 양극성 장애는 피질하 병변에서 호발하는 것으로 보고되어왔으며, 이차성 조종의 위험인자로는 정신장애의 가족력과 경한 피질하 위축이 제시되었다. 이 증례에서도 우반구 피질하 병변과 경한 피질하 위축이 관찰되었다. 뇌졸중 병변위치와 기분장애의 상관성 및 위험인자에 대한 연구는, 일차성 기분장애의 신경생물학적 연구에 기여할 수 있을 것이며, 임상에서도 뇌졸중 환자의 정신증상 치료에서 조중 유발의 위험성을 인식하는데 도움이 될수 있을 것이다.
Objectives : Autobiographical memory (ABM) is a special type of episodic memory, containing events that have occurred in a personal life. Overgeneral tendency of ABM refers to the retrieval of memory with only general and categorical descriptions rather than specific events. ABM specificity in depression and posttraumatic stress disorder is a robust finding with relation to cognitive vulnerability, affect regulation, problem-solving ability. It is also implicated in bipolar disorder with frequent relapses. In this study, we investigated whether ABM specificity was related to manic or euthymic mood states in patients with bipolar disorder. Methods : Forty bipolar patients with manic and euthymic episodes and 25 healthy controls participated in this study. Prompted by 5 positively and 5 negatively valenced emotional cue words, each participant was instructed to recall positive or negative memories and describe them in detail. The One-way ANOVA was used to compare ABM scores and post-hoc analyses were done. Results : Comapred to the healthy persons, the bipolar patients reported significantly more general than specific negative memories in both manic and euthymic episodes (p = 0.003). However, there was no significant difference between manic and euthymic patients (p = 0.074). Conclusions : These results suggest that overgeneral tendency of negative ABM may be a trait abnormality in bipolar disorder. Moreover, this phenomenon might be related to underlying cognitive deficits or affect regulation irrespective of the mood state.
