Education of Bioterrorism Preparedness and Response in Healthcare-associated Colleges-Current Status and Learning Objectives Development (보건의료 교육기관에서 생물테러 관련 교육 현황조사 및 학습목표 개발)

  • Objectives: Bioterrorism (BT) preparedness and response plans are particularly important among healthcare workers who will be among the first involved in the outbreak situations. This study was conducted to evaluate the current status of education for BT preparedness and response in health care-related colleges/junior colleges and to develop learning objectives for use in their regular curricula. Methods: We surveyed all medical colleges/schools, colleges/junior colleges that train nurses, emergency medical technicians or clinical pathologists, and 10% (randomly selected) of them that train general hygienists in Korea. The survey was conducted via mail from March to July of 2007. We surveyed 35 experts to determine if there was a consensus of learning objectives among healthcare workers. Results: Only 31.3% of medical colleges/schools and 13.3% of nursing colleges/junior colleges had education programs that included BT preparedness and responses in their curricula. The most common reason given for the lack of BT educational programs was 'There is not much need for education regarding BT preparedness and response in Korea'. None of the colleges/junior colleges that train clinical pathologists, or general hygienists had an education program for BT response. After evaluating the expert opinions, we developed individual learning objectives designed specifically for educational institutions. Conclusions: There were only a few colleges/junior colleges that enforce the requirement to provide education for BT preparedness and response in curricula. It is necessary to raise the perception of BT preparedness and response to induce the schools to provide such programs.

Syndromic Surveillances based on the Emergency Department (응급실 증후군 감시체계의 운영 현황과 활성화 방안)

  • Due to heightened concerns regarding possible bioterrorist attacks, the Korea Center for Disease Control and Prevention introduced syndromic surveillance systems, which have been run by emergency departments in hospitals throughout Korea since 2002. These systems are designed to identify illness clusters before diagnoses are confirmed and reported to public health agencies, to mobilize a rapid response, and thereby to reduce morbidity and mortality. The Korea Center for Disease Control and Prevention performed drop-in syndromic surveillance successfully during the World Cup Football Games in 2002, the Universiad games in 2004, and the Asian Pacific Economic Cooperation meeting in 2005. In addition, sustainable syndromic surveillance system involving the collaborative efforts of 125 sentinel hospitals has been in operation nationwide since 2002. Because active data collection can bias decisions a physician makes, there is a need to generate an automatic and passive data collection system. Therefore, the Korea Center for Disease Control and Prevention plans to establish computerized automatic data collection systems in the near future. These systems will be used not only fur the early detection of bioterrorism but also for more effective public health responses to disease.

  • 이 연구는 9.11 테러 이후 발생한 뉴테러리즘 중 가장 잔혹한 생물테러리즘을 예방하기 위한 대비체계중 검증체계를 구축한 실태를 확인한 논문이다. 생물테러리즘을 위기로 인식하고 예방하기 위해서는 대비체제를 잘 구축하여 관리하면서 실질적으로 가동하는 여부를 확인하는 활동이 필요하다. 생물테러리즘을 예방하기 위한 체제는 우선 테러리즘을 예방하기위한 법체계가 구축되어야 하고, 생물테러리즘에 사용되는 전구물질을 통제하는 법체계가 갖추어져야 한다. 그리고 이러한 법체계를 수행하기 위한 각종 조직들이 갖추어지고, 조직들이 활성화되어야 한다. 본 연구결과 한국에는 테러리즘을 통제하는 상위법령이 없는 상태이나 국제적인 요구에 부응하기 위해, BWC의 협약에 언급된 직 간접적인 이전금지, 제조, 획득, 권유, 조장 등을 금지하기 위해 생물테러리즘을 예방하기 위한 여러 법령을 제정하고, 이를 시행하기 위한 조직이 구성되어 활동 중이다. 또한 생물테러리즘에 사용될 수 있는 물질과 장비들은 산업계와 학계에서 연구목적으로 또는 의료약품 생산을 목적으로 필요로 하는 것이므로 생물학 물자 및 장비의 유통질서를 확립하고 확인하는 것이 필요하다. 이를 위한 조직이 활동 중이다. 생물무기 보유여부의 신고, 이의 폐기, 평화적 용도로의 전환사용 등에 대해서도 추진 중이다. 또한 이를 국제적으로 확인하는 체제를 구축하기 위한 국제적 협력이 계속 되어야 할 것이다.

Detection of Virulence-Associated Genes in Clinical Isolates of Bacillus anthracis by Multiplex PCR and DNA Probes

  • Anthrax is a zoonotic disease caused by Bacillus anthracis, and well recognized as a potential agent for bioterrorism. B. anthracis can be identified by detecting the virulence factors genes located on two plasmids, pXO1 and pXO2. The aim of the present study was to determine the presence of virulence genes in 27 isolates of B. anthracis isolated from clinical and environmental samples. For this purpose, multiplex PCR and DNA probes were designed to detect protective antigen (pag), edema factor (cya), lethal factor (lef), and capsule (cap) genes. Our results indicated that all the isolates contained all the above virulence genes, suggesting that the isolates were virulent. To the best our knowledge, this is the first study about the determination of virulence marker genes in clinical and environmental isolates of B. anthracis using multiplex PCR and DNA probes in India. We suggest that the above methods can be useful in specific identification of virulent B. anthracis in clinical and environmental samples.

Health Disaster Preparedness System for Response to Newly Emerging Terrorism (신종 테러리즘 대응을 위한 보건방재체계의 현황)

  • The disaster preparedness system in Korea has been developed in spite of many obstacles, but there are still many problems for response to newly emerging terrorisms due to the existing problems of disaster response system. The newly emerging terrorism in 21th century like biochemical terrorism has made us focus on terrorism preparedness, but health and medical aspect of terrorism has been overlooked. The health disaster system is more necessary for 21th terrorism response as well as the disaster engineering, economic and administrative aspects. The disaster preparedness system for bioterrorism has been developed by Ministry of Health with syndromic surveillance system and no case has been found in Korea yet and the resources of personnel and equipment as decontamination system in medical facilities are lacking in case of chemical terrorism. So through the multiple access method reflecting the risk factors in real terrorism field and human based health disaster concept, the disaster preparedness and response system to make up for the weak point should be suggested.

  • Injury is a serious problem that not only causes death but also significantly degrades the quality of life of the people and causes loss of socioeconomic opportunities and costs. Damage occurs as a result of an accident. Among them, natural disasters and artificial disasters take lives of many people in a short time and threaten their physical and mental health. The United States has responded to the disaster by establishing relevant laws and regulations and a response system with the recognition that health is recognised soon to be as national security in the wake of the 9/11 terrorist attacks and the Katrina disaster. It is necessary to build a knowledge infrastructure to train disaster response experts in public health area and to have health competence to cope with disasters.

  • 본 연구에서는 생물테러 발생 시에 대비한 대국민행동요령 교육 및 홍보자료를 개발함으로써, 국민들의 자발적 이해와 비상시 행동요령을 정확히 인지하게 하여 생물테러 발생에 의한 피해를 최소화하기 위하여, 일반 국민을 대상으로 생물테러감염병에 대한 행동요령을 개발하고 구체적 인지도 증가를 위한 매뉴얼의 컨텐트를 구축하였다. 본 연구를 통해 생물테러에 대한 대국민행동요령을 국민들이 숙지하고 생물테러감염병에 대한 인지도가 높아질 것이 기대되며, 생물테러 위기상황 발생 시 일반 국민들의 적절한 대응을 할 수 있고, 이러한 활동을 통하여 범국민적 안전문화 확산에 기여할 것이다.

  • Recently keeping pace with globalization, many international conferences and athletic games are being held in Korea. After 911 terror in New York in 2001, Korean government dispatched Zaytun Division in Iraq and this fact has also led to voice concerns that Korea should be prepared to protect from terrors of chemical, biological, radiological, nuclear, and explosive(CBRNE) emergency as soon as possible. It is important to develop the CBRNE emergency medical training for fire officers in Korea. So I propose the curriculum.
