심박변이도를 이용한 중환자실 입원 환자의 자율신경계 일중변동성에 대한 예비연구 (A Preliminary Study on the Circadian Rhythm of Autonomic Nervous System in Patients Admitted to Intensive Care Unit, Using the Heart Rate Variability)

  • Objectives A normal circadian rhythm of autonomic nervous system function stands for the daily change of sympathetic and parasympathetic modulation, which can be measured by heart rate variability (HRV). Generally, patients in the intensive care unit (ICU) are prone to sleep-wake cycle dysregulation, therefore, it may have an influence on the circadian rhythm of autonomic nervous system. This study was designed to interpret possible dysregulation of autonomic nervous system in ICU patients by using HRV. Methods HRV was assessed every 3 hours in 21 ICU patients during a 7-minute period. The statistical differences of HRV features between the morning (AM 6 : 00-PM 12 : 00), and the afternoon (PM 12 : 00-PM 18 : 00) periods were evaluated in time domain and frequency domain. Results Patients showed significantly increased normalized power of low frequencey (nLF), absolute power of low frequencey (LF)/absolute power of high frequencey (HF) in the afternoon period as compared to the morning period. However, normalized power of high frequency (nHF) was significantly decreased in the afternoon period. There was no statistically significant difference between the morning period and the afternoon period in the time domain analysis. Conclusions The increased sympathetic tone in the afternoon period supports possible dysregulation in the circadian rhythm of autonomic nervous system in ICU patients. Future studies can help to interpret the association between autonomic dysregulation and negative outcomes of ICU patients.

노인요양병원에 입원한 경도인지장애 노인의 휴식-활동 일주기 리듬에 관한 탐색적 연구: 타액 알파 아밀라제와의 관련성을 중심으로 (Rest-activity circadian rhythm in hospitalized older adults with mild cognitive impairment in Korea and its relationship with salivary alpha amylase: an exploratory study)

  • Purpose: This study aimed to evaluate the rest-activity circadian rhythm (RAR) using data obtained from wearable actigraph devices in hospitalized older adults with mild cognitive impairment (MCI), and to investigate its relationship with salivary alpha amylase (sAA). Methods: This secondary data analysis used data from the Hospitalized Older Adults' Cognition and Physical Activity Study. Actigraph data for 3-4 days were analyzed for RAR. RAR indices such as interdaily stability (IS), intradaily variability (IV), activity level during the most active 10-hour period and during the most least active 5-hour period, and relative amplitude (RA) were calculated. Data on sAA collected in the morning and general characteristics, including body mass index (BMI), were analyzed. Results: Data from 92 hospitalized older adults with MCI were analyzed. The IS, IV, RA were 0.23, 0.73, 0.88, respectively. The average level of sAA was 77.02 U/mL, and a higher level of sAA was significantly associated with better IS and RA in the regression analysis, while age, BMI, and cognitive level were not. BMI showed positive correlations with IS and RA. Conclusion: RAR in the hospitalized older adults with MCI was attenuated, showing especially low IS, which implies they failed to maintain regular and repetitive 24-hour RAR. Increased sAA and BMI were associated with robust RAR. Nurses need to pay attention to maintain robust RAR in hospitalized older adults with MCI, and strategies should be developed to improve their RAR.

혈장(血漿) MHPG와 HVA 농도(濃度)의 24시간(時間) 주기성(週期性) 변화(變化)에 관한 연구(硏究) (Circadian Rhythms of Plasma MHPG and HVA Concentrations in Healthy Young Adults)

  • We invesigated whether the circadian rhythms of plasma MHPG and HVA concentrations exist in 11 healthy young adults, and analyzed the patterns of their circadian rhythms. The results were as follows : 1) The change in the mean plasma MHPG concentrations analyzed by repeated measures ANOVA was not statistically significant. Five subjects showed significant circadian rhythms of plasma MHPG concentrations of each individual, and 4 of those had the acrophases between 17PM and 24PM. 2) The change in the mean plasma HVA concentrations analyzed by repeated measures ANOVA was not statistically significant. Six subjects showed significant circadian rhythms of plasma HVA concentrations of each individual, and 4 of those had the acrophases between 21PM and 6AM. In conclusion, the circadian rhythm pattern of plasma HVA concentrations in our result is consistent with the previous study. It was suggested that plasma MHPG and HVA concentrations should be measured more frequently, and the physical activities of subjects be controlled more strictly for the following study.

  • 지난 10년간 분자 식물 일주기성에 대해 분자 생물학적, 생화학적인 연구가 많이 진행되었다. 본 연구에서는 식물의 Arabidopsis, rice 그리고 algae에서 지금까지 발표된 연구들을 종합하고 고찰해보려 했다. 그 결과, 아직까지도 주기성 대사의 모든 부분을 설명하기엔 부족한 부분이 많다는 것을 알수 있었다. 최근 주기성 단백질들의 전사후, 번역 그리고 번역후 변형과정에 대해 많은 연구자들이 관심을 갖기 시작했다. 이러한 부분에서 다량의 단백질을 한번에 볼 수 있는 2-DE gel electrophoresis와 MS/MS 기술이 절실히 요구된다고 할 수 있겠다.

  • 빛은 인간과 동물에서 일주 리듬과 신경내분비 활동을 조절하는 자극이다. 망막-시상하부 경로는 주/야 주기와 신경내분비 활동에 관여한다. 특히 멜라토닌 분비는 빛에 의해 영향을 받으며, 다양한 측면의 건강 문제와 관련이 있다는 것이 알려져 왔다. 본 논문은 생물학적 리듬과 신경내분비 활동에 있어서 빛이 수행하는 역할과, 빛과 건강 문제와의 관련성, 망막-시상하부 경로의 특성을 소개하고자 한다.

Ultradian Rhythms in the Hypothalamic Arcuate Nucleus Kisspeptin Neurons and Developmental Processes

  • Numerous physiological processes in nature have multiple oscillations within 24 h, that is, ultradian rhythms. Compared to the circadian rhythm, which has a period of approximately one day, these short oscillations range from seconds to hours, and the mechanisms underlying ultradian rhythms remain largely unknown. This review aims to explore and emphasize the implications of ultradian rhythms and their underlying regulations. Reproduction and developmental processes show ultradian rhythms, and these physiological systems can be regulated by short biological rhythms. Specifically, we recently uncovered synchronized calcium oscillations in the organotypic culture of hypothalamic arcuate nucleus (ARN) kisspeptin neurons that regulate reproduction. Synchronized calcium oscillations were dependent on voltage-gated ion channel-mediated action potentials and were repressed by chemogenetic inhibition, suggesting that the network within the ARN and between the kisspeptin population mediates the oscillation. This minireview describes that ultradian rhythms are a general theme that underlies biological features, with special reference to calcium oscillations in the hypothalamic ARN from a developmental perspective. We expect that more attention to these oscillations might provide insight into physiological or developmental mechanisms, since many oscillatory features in nature still remain to be explored.

Investigation of Visual Perception Under Zen-Meditation Based On Alpha-Dependent F-VEPs

  • Variation of brain dynamics under Zen meditation has been one of our major research interests for years. One issue encountered is the inaccessibility to the actual meditation level or stage as a reference. In this paper, we propose an alternative strategy for investigating the human brain in response to external flash stimuli during Zen meditation course. To secure a consistent condition of the brain dynamics when applying stimulation, we designed a recording of flash visual evoked potentials (F-VEPs) based on a constant background EEG (electroencephalograph) frontal $\alpha-rhythm$ dominating activities that increase significantly during Zen meditation. Thus the flash-light stimulus was to be applied upon emergence of the frontal $\alpha-rhythm$. The alpha-dependent F-VEPs were then employed to inspect the effect of Zen meditation on brain dynamics. Based on the experimental protocol proposed, considerable differences between experimental and control groups were obtained. Our results showed that amplitudes of P1-N2 and N2-P2 on Cz and Fz increased significantly during meditation, contrary to the F-VEPs of control group at rest. We thus suggest that Zen meditation results in acute response on primary visual cortex and the associated parts.

Circardian rhythm of cardiac nonlinear dynamics in healthy human

  • In this study, we investigated the circardian rhythm of complexity of cardiac dynamics in humans. Dynamic 24-hour electrocardiographic recordings were obtained from 30 healthy ambulant subjects aged 41 to 50 years. or each recordings, normalized low frequency (0.04-0.1 hertz) and high frequency (>0.15 hertz) component are calculated. our different indexes obtained from separate algorithms of nonlinear dynamics - approximate entropy, correlation dimension, Lyapunov exponent and fractal dimension - were calculated. During early morning, low frequency component rose rapidly with concomitant withdrawl of high frequency component. All the our indexes of nonlinear dynamics showed remarkably same circardian rhythm: an early morning dip preceded by a steep decline during late night, a gradual recovery during evening and a peak around midnight. These data indicate that the simultansous losses of all of the our different mechanisms of nonlinear control of heart rate during early morning, concomitent with the surge of symapathetic activity and reduction of vagal activity, may contribute to the increased incidence of cardiovascular events during morning hours.

  • Living organisms are influenced by many external rhythms and they have adapted their physiology to periodically changing conditions. These adaptive strategies are controlled by endogenous innate programs of behavior and physiology which are determined by external signals ("Zeitgeber"). There are many biological rhythms, each with its own characteristic functional adaptation. Among them, the presence of endogenous time control of feeding and drinking becomes obvious. There are increasing evidences that the control of food intake, food selection, and drinking are regulated by the endogenous rhythms including a circadian rhythm. However, there have been many restrictions in understanding the endogenous control of food intake itself and its mechanism. To broaden our know ledges of the endogenous time control of feeding and drinking, the author reviwed the characteristics of the endogenous timing for food intake, the influence of circadian pacemakers and food-entrainable oscillators, the interaction between the circadian control and the external and internal conditions in the control of food intake, the conseqences of feeding, the circadian control of food selection, and the biological cycles in energy balance.

LMS 적응필터를 이용한 직교 함수 기반의CPR 잡음 제거 알고리즘 설계 (Design of CPR Artifact Removal Algorithm Based on Orthogonal Function using LMS Adaptive Filter)

  • This study proposes an algorithm for removal of CPR artifact in order that automated external defibrillator (AED) can effectively diagnose ECG rhythm during cardiopulmonary resuscitation (CPR). Current AED required to interrupt chest compression for reliable rhythm analysis to avoid the effect of artifacts produced by CPR. However even temporarily interruption of chest compression during CPR adversely affects the probability of restoration of spontaneous circulation (ROSC) and survival after the delivery of the shock. Therefore, we proposed a method for removal of CPR artifacts using least mean square (LMS) filter. The removal of the CPR artifacts would enable compressions to continue during AED rhythm analysis, thereby increasing the likelihood of resuscitation success. It was tested on 31 segments of shockable and 300 segments of non-shockable ECG signals recorded from three pigs during CPR. In the result, sensitivity (Se) and specificity (Sp) analysis on the test segments showed values of Se = 3.2%, Sp = 66.0% and Se = 96.8%, Sp = 98.7% in the case of unfiltered and filtered signals during CPR. In conclusion, it was shown that the proposed method can be a useful tool to exactly diagnose the ECG rhythm during the CPR.
