Bilateral Oculomotor Nerve Palsy after Head Trauma: A Case Report

  • We introduce a patient who was suffered from isolated traumatic bilateral oculomotor nerve palsy after head trauma. The patient presented with bilateral ptosis and abnormal pupilary responses with slightly drowsy mentality at first. Performed images demonstrated some hematomas along subarachnoid, intraventricular, subdural spaces and multiple small supratentorial contusions. There was no bony abnormality or ligament injury. We assumed that small amount of interpeduncular hematoma might be the proper lesion associated with oculomotor nerve palsies, since the clinical symptom and signs presented bilaterally and the oculomotor neural fascicles run through the interpeduncular fossa.

  • Talus fracture is less common than most fractures, and bilateral talar neck fracture is extremely rare. Complications associated with talus fractures are generally deemed common because of the anatomical characteristics of the talus, but few reports have described the methods of treating such complications and the results of bilateral talar neck fracture. We report here a case of bilateral Hawkins type II talar neck fracture that had good clinical results without complications after early surgical treatment.

  • Regarding the bilateral vertebral artery [VA] dissecting aneurysms, treatment strategy remains controversial because there have not been enough cases to reach a conclusion on the best treatment. We present a patient underwent staged microsurgical trapping and endovascular coiling for each dissecting aneurysm of bilateral VA presenting subarachnoid hemorrhage [SAH]. The ruptured side was managed by VA trapping procedure without any neurological deficit. Postoperative cerebral angiography revealed patent right PICA without filling of previous right dissecting aneurysm and spontaneous occlusion of the left dissecting aneurysm one month after trapping procedure. However, follow-up angiography revealed recanalization and growing of the left VA dissecting aneurysm one year after the operation. The patient underwent endovascular embolization using GDC for the proximal occlusion of the left VA and postoperative course was uneventful.

  • In this paper, we proposed a teleoperation scheme of unmanned grounded vehicle to improve telepresence. Especially, bilateral control architecture for transmitting realistic steering feeling to the remote driver is investigated. System architecture of the teleoperated remote vehicle is introduced with visual, auditory and kinesthetic haptic channel. Several bilateral control architectures are proposed for transmitting remote steering feeling, and subject tests are done to evaluate the performance. Position-force bilateral control architecture with returning torque compensation algorithm shows best performance.

  • Bilateral teleoperation systems, connected to computer networks such as Internet have to deal with the time delay varying depending on factors such as congestion, bandwidth or distance. And the entire system is easy to become unstable due to irregular time delay. Passivity concept has been using as a framework to solve the stability problem in bilateral control of teleoperation. Acontrol scheme for teleoperation systems with varying time delay is proposed based on a passivity concept is proposed in this paper. One approach makinguse of the characteristic impedances is proposed to achieve a passive control. Since passive control does not mean that the system performance will be acceptable, another transmission scheme which focuses on both the passive feature and the acceptable performance is configured for varying time delay in this paper. The tracking performance has been proved through the computer simulation for varying time delay bilateral teleoperation system using Matlab Simulink.

  • In this paper, a depth image up-sampling method is put forward by using pixel classifying and jointed bilateral filtering. By analyzing the edge maps originated from the high-resolution color image and low-resolution depth map respectively, pixels in up-sampled depth maps can be classified into four categories: edge points, edge-neighbor points, texture points and smooth points. First, joint bilateral up-sampling (JBU) method is used to generate an initial up-sampling depth image. Then, for each pixel category, different refinement methods are employed to modify the initial up-sampling depth image. Experimental results show that the proposed algorithm can reduce the blurring artifact with lower bad pixel rate (BPR).

  • A male 17-year-old boy was tranferred from a private hospital for persistent bilateral pleural effusion.The effusion was confirmed as bilateral chylothorax by chest CT and lymphangiography. Persistent accumulation of the chylothorax was uncontrollable more 1000cc daily ever after pleuroperitoneal shunt operation and thoracoscopic thoracic duct ligation at Rt. side. Chemical pleurodesis with OK 432 into pleural cavity through thoracostomy tube was attempted as 1.5 KE-3 KE diluted in 50ml of normal saline for 3 consecutive days resulted dramatic reduction of the drainage amount. Chemical pleurodesis with OK 432 appeared to be very helpful for management idiopathic bilateral chylothorax.

  • In this paper, we propose a new post-processing technique that removes blocking and ringing artifacts in Block discrete cosine transformation (BDCT)-coded images using bilateral filtering. The selection of filter parameters is a key issue in the application of a bilateral filter because it significantly affects the result. An efficient method of selecting the bilateral filter parameters is presented. The experimental results show that the proposed approach alleviates the artifacts efficiently in terms of PSNR, MSDS, and SSIM.

Severe Symptomatic Vasospasm following Intraventricular Hemorrhage from Arteriovenous Fistula

  • The authors present a rare case of severe vasospasm following the rupture of arteriovenous fistula. On initial CT scan, hematoma in the corpus callosum and left inferior frontal region with surrounding cerebromalacia and all ventricles without apparent subarachnoid hemorrhage were seen. Angiograms showed arterivenous fistula but did not show cerebral vasospasm. Thirteen days after admission the neurological state of patient suddenly deteriorated and bilateral motor weaknesses developed. Following angiograms revealed severe narrowing on the supraclinoid portion of bilateral internal carotid arteries, bilateral anterior cerebral arteries and bilateral middle cerebral arteries. Transluminal angioplasty and intra-arterial papaverine infusion were performed. The patient remained stable with moderate neurologic deficits.

  • Competition among electric generation companies is a major goal of restructuring in the electricity industry, The trading system in an electricity market has been one of the most important issues in deregulated electricity market. This paper deals with comparisons of the major two types of the trading system: compulsory pool market and bilateral contract market. The two trading systems are compared quantitatively from the viewpoint of consumer's surplus and social welfare, This paper, also, proposes a unified model of Cournot and Bertrand for analyzing the mixed trading system of pool market and bilateral contract market. Nash equilibrium of the unified model is derived by criteria for participating in bilateral contract market. Numerical results from a sample case show that a mixed trading system of pool market and price-competitive bilateral market is beneficial to consumer from the view points of consumer's surplus.
