Biceps long head tendon revisited: a case report of split tendon arising from single origin

  • A 27-year-old, right-hand-dominant woman with a posttraumatic anterior shoulder dislocation 3 months earlier after traYc accident presented because of pain and limited range of motion in the right shoulder. On physical examination, the patient had negative instability tests and a sulcus sign. On arthroscopic examination, a bifurcate long biceps tendon with two limbs was observed about 1 cm distal to the origin in the supraglenoid tubercle. We found no evidence of a tear in the long biceps tendon on probing, and the margin of each limb was smooth and round. Although this anatomic variant may be benign, its presence might be associated with other shoulder pathology. It is interesting to speculate whether the aberrant biceps anatomy in our patient contributed to transfer of injury at dislocation to the rotator cuff rather than to the classic anterior-inferior capsulolabral complex. In addition, recognition of the described anatomic variant on arthroscopy can aid the shoulder surgeon in focusing treatment on the actual pathology.

  • Lesion of the superior glenoid labrum and the insertion of the biceps tendon are a common cause for shoulder pain in patients performing overhead sports. Medial shearing stress during overhead throwing may cause biceps instability and associated rotator cuff problem aggravates the symptoms. Careful attention to the history and physical examination and may arouse suspicion of injury to the biceps tendon and the superior labral complex. Progress in shoulder arthroscopy has led to the identification of the lesion and normal variation. Treatment should be directed according to the type of lesion and related symptoms.

  • The purpose of this study is Muscle Activity for the Upper Extremity and Morto-Seoinage(Two-handed shoulder throw) according to performance levels and elite judo players (G1) and non-elite judo players, namely university players (G2). To achieve this purpose, three players in G1 whose main special skill was Morote-Seoinage and three university judo players(G2) were selected as comparative group. After analysing this study, conclusions were derived as follows. 1. Muscle activity of musculi membri superious to group of Kuzushi to Tsukuri was the highest in left biceps brachii and right biceps brachii in order. On the other hand, G2 was the highest in left biceps brachii and left flexer carpi randialis in order. 2. Intergral electromyogram of according to the phase showed right triceps 1P of left flexer carpi radialis. G1 and G2 of showed left biceps brachii was the highest in 1P and 2P. As a result, extra training of left flexer carpi radialis and left biceps brachii is needed.

Effects of Mental Practice on the Muscle Strengthening of the Hemiplegic Patients (상상연습이 편마비 환자의 근력증가에 미치는 영향)

  • The purpose of this study was to determine the effect of mental practice on biceps brachii muscle strengthening in 3 poststroke hemiplegia patients along with multiple baseline design across subjects. The mental practice adopted for this experiment involved imagery training to practice elbow flexion, which last 12 to 16 sessions with 30 minutes each session. Maximal muscle activities was measured pre- and post-mental practice to evaluate the strength of biceps brachii. The strength of biceps brachii was measured by surface-EMG. The results were: In the subject 1, 73.92 mV (pre-) and 127.56 mV (post-); in the subject 2, 147.60 mV (pre-) and 202.85 mV (post-); and in the subject 3, 20.75 mV (pre-) and 27.92 mV (post-). The results indicate that a simple mental practice is a useful method to strengthen biceps brachii muscle in hemiplegic patients.

Magnetic resonance imaging features of the biceps tendon rupture in a Labrador retriever dog

  • A five-year-old spayed female Labrador retriever presented with right forelimb lameness one day earlier. Radiographs and computed tomography of the right shoulder joint revealed conspicuous enthesophytes, bone cyst, and bone fragment at the intertubercular groove. Magnetic resonance imaging showed a loss of continuity of the biceps tendon and inhomogeneous hyperintense signal of the surrounding soft tissue and moderate synovial fluid on T2- and proton density-weighted images with contrast enhancement on the postcontrast T1-weighted images. The dog was diagnosed with tenosynovitis and biceps tendon rupture with osteoarthritis based on a comprehensive evaluation of the computed tomography and magnetic resonance imaging features.

Intraoperative periprosthetic humeral fracture during reverse shoulder arthroplasty: a sequelae of prior biceps tenodesis

  • Intraoperative periprosthetic humeral fractures are a rare but debilitating complication of reverse shoulder arthroplasty and can occur during multiple stages of the procedure. Prior biceps tenodesis has been found to reduce cortical humeral strength and predispose the patient to humeral fracture. We present a case of a 68-year-old female with a previous history of biceps tenodesis due to an irreparable rotator cuff tear. Months later, and after symptoms persisted, a reverse shoulder arthroplasty was performed. During the surgery and while performing final reduction, a fracture line was observed involving the hole used for the previous tenodesis procedure. The fracture was repaired, and the patient reported favorable outcomes. We report several factors that might have contributed to sustenance of this intraoperative fracture including prior biceps tenodesis, use of a press fit humeral stem, and the sex of the patient. The Effects of Lower Limb Muscle Activity on Postural Stability and Ground Type During Gait in Elderly Women (보행 시 여성노인의 자세안정성과 지면형태가 하지 근활성도에 미치는 영향)

  • Objective : The purpose of this study was to analyze the effects of lower limb muscle activity on postural stability and ground type in elderly women subjects. Method : Forty two subjects participated in the experiment (high group - age: $74.29{\pm}4.13yr$, height: $152.44{\pm}5.54cm$, weight: $57.43{\pm}6.16kg$, BMI: $24.77{\pm}2.99$, low group - age: $77.67{\pm}5.16yr$, height: $151.40{\pm}3.93cm$, weight: $60.92{\pm}6.40kg$, BMI: $26.59{\pm}2.57$). Wireless EMG with eight channels was used. Ground types were classified as flat and cushion. Results : In the double-support phase, left and right rectus femoris, left biceps femoris, left and right tibialis anterior, and left gastrocnemius did not show a significant difference in postural stability according to ground type. However right biceps femoris and gastrocnemius showed higher muscle activity in the elderly women group with lower postural stability. In the single-support phase, left and right rectus femoris, right biceps femoris, and left and right tibialis anterior did not show a significant difference in postural stability according to ground type. In addition, left biceps femoris had higher muscle activity in the elderly women group with lower postural stability. Left gastrocnemius had higher muscle activity in the elderly women group with higher postural stability and right gastrocnemius had higher muscle activity in the elderly women group on cushion ground. Conclusion : In a dynamic postural stability and cushion ground, biceps femoris and gastrocnemius muscle activity were high. As a result, biceps femoris and gastrocnemius muscle strengthening exercise on cushion ground could be beneficial in the prevention of falling.

Effects of Different External Loads on the Activities of the Gluteus Maximus and Biceps Femoris during Prone Hip Extension in Healthy Young Men

  • PURPOSE: This study examined the effects of different external loads on the muscle activities around the hip during prone hip extension with knee flexion (PHEKF) exercise in healthy young men. METHODS: Sixteen healthy adult males participated in the study. A pressure biofeedback unit was used to provide feedback to the participants during the abdominal drawing-in maneuver (ADIM) with PHEKF. Sandbags (0 kg, 1 kg, 2 kg, and 3 kg) were used to provide external resistance. The quadriceps was contracted to maintain knee flexion 90° against resistance. Each resistance condition using a sandbag weight was given in random order. Surface electromyography (sEMG) was used to measure the electrical activity of the gluteus maximus, biceps femoris, and erector spinae during PHEKF. RESULTS: The muscle activity of the gluteus maximus was highest with the 3 kg resistance and lowest with 0 kg (F = 128.46, P = .00). The muscle activities of the biceps femoris and erector spinae were highest with 0 kg and lowest with 3 kg (F = 29.49, P = .00). The muscle activity rate of the gluteus maximus/biceps femoris was highest with 3 kg and lowest with 0 kg (F = 37.49, P = .00). CONCLUSION: The activity of the gluteus maximus was increased using a higher external weight load during PHEKF, while the activity of the biceps femoris decreased. These findings suggest that an external weight is needed during hip extensor exercise to strengthen the gluteus maximus and inhibit the biceps femoris.

An Analysis of Muscular Activity of the Long and Short Heads of Biceps Brachii Muscle According to the Elbow Flexion Angle (위팔두갈래근의 긴갈래와 짧은갈래의 팔굽관절 굽힘 각도에 따른 근활성도 분석)

  • Purpose: This study aimed to investigate the impact of resistance on the muscle activities of the long and short heads of the biceps brachii, according to the elbow angle in supination, and the difference in muscle activity between the long and the short heads. Methods: This study was conducted with 22 men in their 20s who voluntarily agreed to participate. With the glenohumeral joint neutral in a position of supination, the elbow angle was randomly moved to 0°, 30°, 60°, and 90°. Using an 8-channel surface EMG while the participants held a 2-kg. dumbbell, the muscle activities of the long and the short heads of the biceps brachii were measured. The measured data were statistically processed using SPSS for Windows 12.0. For the activities of the short and the long heads according to the angle, a one-way ANOVA was conducted, and subsequently, to check the results of an analysis of the difference between groups, an LSD post-hoc test was conducted. An independent t-test was used to compare the activities of the long head and the short head according to the angles. Results: The analysis of the impact of the load of the dumbbell at each elbow angle on the muscle activities of the long and short heads of the biceps brachii revealed significant differences in both heads (p < 0.05). The result of the post-hoc analysis showed significant differences in both heads at angles between 0° and 30°, between 0° and 60°, and between 0° and 90°. Analysis of the impact of the load of the dumbbell in supination on the muscle activities of the long and short heads showed a significant difference at the angle between 0° and 30° (p < 0.05). Conclusion: The long head of the biceps brachii mainly acts in supination; however, in supination at elbow-bending angles of 60° and 90°, there was no difference in muscle activity between the short head and the long head.

Comparison of Number of Repetitions and Repetition Rate in 5 Sets of 65%1RM Bench Press and Biceps Curl Exercise

  • The purpose of this study is to investigate the changes in the number of repetitions and the repetition rate according to the exercise mode when performing 65%1RM resistance exercise at the 1-minute rest interval and the 3-minute rest interval. Sixteen healthy male subjects were treated with Bench press and Biceps curl of 65%1RM intensity at 1 and 3 minute rest intervals. The number of repetitions for each set of 1 minute rest interval showed a significant decrease from 1set to 5set in bench press. biceps curl showed a significant decrease from 1set to 4set. The repetition rate according to the exercise mode with a 1-minute rest interval showed a significant difference from 2sets to 4sets. In the repetition rate for each set, bench press showed a significant decrease from 1set to 5set. biceps curl showed a significant decrease from set 1 to set 4. The number of repetitions according to the exercise mode with a 3-minute rest interval showed a significant difference from 2sets to 5sets. In the number of repetitions for each set, bench press showed a significant decrease from 1set to 5set. biceps curl showed a significant decrease from 1set to 4set. In summary, the decrease in the number of repetitions according to the set progression in the resistance exercise of the endurance depends on the exercise mode, and the increase of the rest interval or the decrease of the weight-intensity should be considered when aiming for more exercise.
