중년여성의 강인성, 폐경지식과 폐경관리에 관한 연구 (A Study on Hardiness, Knowledge of Menopause, Menopausal Management among Middle Aged Women)

  • The purpose of this study was to figure out related factors to the self-reported climacteric symptoms and the relationship among the health promoting behaviors, climacteric symptoms and degree of Sanhujori, the Korean traditional postpartal care. A cross-sectional survey design was employed in this study. The subjects were 108 middle-aged women who were non-hystrectomized and ranged in age from 40 to 60 years. They were selected in seoul and Kyoung-ki province, Korea, Data were collected from Oct.25 Nov. 10, 1997 by a structured questionnaire. The instruments used for this study were the revised health Promotion Lifestyle(HPLP) developed by Walker, Sechrist & Pender, and revised Climacteric Symptoms Scale developed by Chi, Sung Ai. the data were analyzed by the SPSS/$PC^+$ program using t-test, ANOVA and Scheffe test as a post hoc and Pearson Correlation Coefficient. The results of the study were as follows ; 1. The mean score of health promoting behaviors was low($2.42{\pm}0.35$). There were statistically significant differences in the score of health promoting behaviors according to the educational background, family income, marital satisfaction, whether or not taking a restorative food and degree of Sanhujori, especially the period (t=-2.07, F=2.60~7.57, p<0.05). 2. The mean score of score self-reported climacteric symptoms was 1.69%;99% of middle-aged women had symptoms. There were statically significant differences in the score of middle -aged women's self-reported climacteric symptoms according to the age, number of children, educational background, occupation, family income, marital satisfaction, whether or not receiving hormon replacement therapy (HRT) or consultation by a professional, perceived health status and self evaluation of Sanhujori(t=-2.04~3.69, F=2.87~11.63, p<0.05). 3. women's degree of Sanhujori was a positive correlation with health promoting behaviors(r=0.34, p=0.00) and negative correlation with the degree of self-reported climacteric symptoms(r=-0.19,p=0.03). 4. The influencing factors to the climacteric symptoms were self actualization, interpersonal support, and perceived health status among the health promoting behaviors with 57% of variance($R^2$=0.57). 5. The middle-aged women's type of coping pattern for the climacteric symptoms was classified as active behavioral coping, spiritual & psychological coping, and negative coping. In conclusion, to intervene the middle aged women's climacteric symptoms and develop nursing strategies for their health, health promoting behavior, especially ; self actualization, interpersonal support, and perceived health status should be considered. And, as the primary prevention strategy for women's health during the period of childbearing and also middle age, especially for the climacteric symptoms, Sanhujori should be reconsidered.

Performance assessment of buckling restrained brace with tubular profile

  • In recent years, there has been an upsurge for the usage of buckling restrained braces (BRB) rather than ordinary braces, as they have evidently performed better. If the overall brace buckling is ignored, BRBs are proven to have higher energy absorption capacity and flexibility. This article aims to deliberate an economically efficient yet adequate type of all-steel BRB, comprised of the main components as in traditional ones, such as : (1) a steel core that holds all axial forces and (2) a steel restrainer tube that hinders buckling to occurr in the core; there is a more practical detailing in the BRB system due to the elimination of a filling mortar. An investigation has been conducted for the proposed rectangular-tube core BRB and it is hysteric behavioral results have been compared to previous researches conducted on a structure containing a similar plate core profile that has the same cross-sectional area in its core. A loss of strength is known to occur in the BRB when the limiting condition of local buckling is not satisfied, thus causing instability. This typically occurs when the thickness of the restrainer tube's wall is smaller than the cross-sectional area of the core plate or its width. In this study, a parametric investigation for BRBs with different formations has been performed to verify the effect of the design parameters such as different core section profiles, restraining member width to thickness ratio and relative cross-sectional area of the core to restrainer, on buckling load evaluation. The proposed BRB investigation results have also been presented and compared to past BRB researches with a plate profile as the core section, and the advantages and disadvantages of this configuration have been discussed, and it is concluded that BRBs with tubular core section exhibit a better seismic performance than the ones with a plate core profile.

소아청소년 집단에서 집단 따돌림의 유병률과 이와 관련된 정신병리 현상 (Prevalence of School Bullying and Related Psychopathology in Children and Adolescents)

  • Objectives : This study examined the current prevalence rate of school bullying and its related psychopathology. Methods : A total of 3,550 elementary/middle school students and their parents were recruited for this study. A self-report questionnaire on perpetration and victimization in school bullying was used for collection of data regarding prevalence and the present state of school bullying. For evaluation of associated psychopathology, self report forms, including the Children's Depression Inventory (CDI), State-Trait Anxiety Inventory for Children (STAIC), attention-deficit hyperactivity disorder Rating Scale (ARS), Korean-Child Behavior Checklist (K-CBCL), Korean-Youth Self Report (K-YSR), and the Korean Eppendorf Schizophrenia Inventory (K-ESI) were applied. Samples were classified according to four subtype groups (control, victim, perpetrator, and victim-perpetrator) and characteristics of each group were compared. Results : Overall, the prevalence rate for bullying perpetration was 64.4% and the prevalence rate for bullying victimization was 63.4%, indicating involvement of more than half of students in school bullying. Bully-victims reported high social immaturity and depressive and suicidal tendency, whereas bully-perpetrators reported less social immaturity and more externalizing problems. Among the subtype groups, the victim-perpetrator group showed the most prominent depressive/anxiety tendency and behavioral problems. Conclusions : Both victimization and perpetration of bullying are common problems for child and adolescent groups and several psycho-social problems were found to be related. The results of this study will guide direction of future study and development of strategies for prevention of bullying.

Validating the Structural Behavior and Response of Burj Khalifa: Synopsis of the Full Scale Structural Health Monitoring Programs

  • New generation of tall and complex buildings systems are now introduced that are reflective of the latest development in materials, design, sustainability, construction, and IT technologies. While the complexity in design is being overcome by the availability and advances in structural analysis tools and readily advanced software, the design of these buildings are still reliant on minimum code requirements that yet to be validated in full scale. The involvement of the author in the design and construction planning of Burj Khalifa since its inception until its completion prompted the author to conceptually develop an extensive survey and real-time structural health monitoring program to validate all the fundamental assumptions mad for the design and construction planning of the tower. The Burj Khalifa Project is the tallest structure ever built by man; the tower is 828 meters tall and comprises of 162 floors above grade and 3 basement levels. Early integration of aerodynamic shaping and wind engineering played a major role in the architectural massing and design of this multi-use tower, where mitigating and taming the dynamic wind effects was one of the most important design criteria established at the onset of the project design. Understanding the structural and foundation system behaviors of the tower are the key fundamental drivers for the development and execution of a state-of-the-art survey and structural health monitoring (SHM) programs. Therefore, the focus of this paper is to discuss the execution of the survey and real-time structural health monitoring programs to confirm the structural behavioral response of the tower during construction stage and during its service life; the monitoring programs included 1) monitoring the tower's foundation system, 2) monitoring the foundation settlement, 3) measuring the strains of the tower vertical elements, 4) measuring the wall and column vertical shortening due to elastic, shrinkage and creep effects, 5) measuring the lateral displacement of the tower under its own gravity loads (including asymmetrical effects) resulting from immediate elastic and long term creep effects, 6) measuring the building lateral movements and dynamic characteristic in real time during construction, 7) measuring the building displacements, accelerations, dynamic characteristics, and structural behavior in real time under building permanent conditions, 8) and monitoring the Pinnacle dynamic behavior and fatigue characteristics. This extensive SHM program has resulted in extensive insight into the structural response of the tower, allowed control the construction process, allowed for the evaluation of the structural response in effective and immediate manner and it allowed for immediate correlation between the measured and the predicted behavior. The survey and SHM programs developed for Burj Khalifa will with no doubt pioneer the use of new survey techniques and the execution of new SHM program concepts as part of the fundamental design of building structures. Moreover, this survey and SHM programs will be benchmarked as a model for the development of future generation of SHM programs for all critical and essential facilities, however, but with much improved devices and technologies, which are now being considered by the author for another tall and complex building development, that is presently under construction.

반복하중을 받는 비보강 확장 단부판 접합부의 해석 및 실험적 연구 (Analytical and Experimental Study of an Unstiffened Extended End-Plate Connection)

  • 확장단부판 접합부는 강구조물의 보-기둥 접합부 혹은 변단면 부재로 구성된 PEB 구조시스템에 적용되는 접합부의 한 형태이다. 확장단부판 접합부는 접합부를 구성하는 단부판의 두께, 고장력볼트의 게이지 거리, 고장력볼트 축부의 직경, 고장력볼트의 개수 등의 영향으로 상이한 거동특성을 나타낸다. 확장단부판 접합부는 미국 및 유럽 등지에서는 다양한 형태로 강구조물의 기둥-보 접합부에 적용되고 있으나 우리나라에서는 널리 적용되고 있지 않다. 이러한 이유로는 확장단부판 접합부에 대한 설계강도식 제안, 접합부상세 제안, 내진성능 평가, 제작 및 시공지침서 개발 등이 적절히 이루어지지 못하고 있기 때문이다. 따라서 이 연구는 비보강 확장단부판 접합부의 국내 적용을 위한 기초자료를 제공하기 위하여 진행하였다. 이를 위하여 두께 12mm의 비보강 확장단부판에 대한 비선형 유한요소해석 및 실험을 수행하였다.

아동기 처치관련 관심전환요법: 국내 아동 중재연구의 비평적 고찰 (Distraction Techniques for Children Undergoing Procedures: A Critical Review of Korean Intervention Research)

  • 목적 본 연구의 목적은 아동의 처치관련 통증을 경감시키기 위한 관심전환중재를 적용한 국내 중재연구들을 분석, 평가하고, 추후 아동의 처치관련 통증을 경감시키기 위한 관심전환중재 연구의 발전방향을 제시하기 위한 근거자료를 제공하고자 함이다. 방법 본 연구는 통증과 관련하여 아동을 대상으로 실시된 관심전환요법의 국내 간호연구 동향을 파악하고 실무 및 연구의 발전방향을 모색하기 위한 비판적 고찰 연구이다. 결과 국내에서 실시된 관심전환을 중재로 적용한 연구는 대부분이 학령전기 및 학령기 초기 아동을 대상으로 실시한 유사실험연구 설계였으며, 시청각 자극을 통해 관심을 전환하는 방법을 가장 많이 사용하였다. 관심전환중재는 아동의 통증과 두려움 등의 행동반응 결과변수의 변화에는 분석 연구의 대부분에서 효과가 있는 것으로 그리고 생리적 반응 결과변수의 변화에는 분석 연구의 46.7%에서 효과가 있는 것으로 나타났다. 그러나 분석에 포함된 논문들의 연구설계, 시행, 보고 등에 있어서 방법론적인 엄격성이 부족하였다. 또한 각 연구들은 시행한 관심전환중재의 절차에 대한 제한된 정보만을 제시하여 다른 연구자들이 반복연구를 시행한다거나 중재를 실무로 중개하는 데 한계가 있었다. 결론 관심전환요법이 아동의 절차관련 통증과 두려움을 경감시키는 데 효과가 있는지에 대한 근거를 좀 더 확실하게 파악하기 위해서는 방법론적 엄격함이 요구된다. 추후 연구에서는 관심전환중재의 임상적 활용성을 파악하기 위해 비용과 시간 효과성 그리고 보호자와 건강관리 제공자의 만족도를 밝히기 위한 연구가 요구된다.

폭발 후 화재하중 시나리오에 따른 2방향 프리스트레스트 콘크리트 패널부재의 손상도 평가 (Damage Evaluation of Bi-directionally Prestressed Concrete Panels under Blast-fire Combined Loading)

  • 각종 사고 및 테러로 인한 폭발, 충돌, 화재 사고가 발생함에 따라 사회적인 안전 불감증이 더욱 고조되고 있으나, 실제 극한하중에 대한 구조물의 방호 설계가 반영되지 못하고 있는 실정이다. 특히, 원전격납구조물, 가스탱크 등과 같은 주요 시설물에 적용되고 있는 2방향 프리스트레스트 콘크리트(PSC) 구조물에 대한 폭발, 그리고 폭발로 인한 2차적으로 발생 가능한 화재에 대한 연구가 미흡함에 따라 복합손상 시나리오에 대한 구조물의 검토가 필요하다. 따라서, 본 연구에서는 폭발 후 화재하중 복합손상을 분석하기 위하여 $1,400mm{\times}1,000mm{\times}300mm$의 2방향 비부착 프리스트레스트 콘크리트를 제작하여, 폭발하중은 ANFO 25 kg의 장약량을 1.0 m 이격거리로 실험을 구성하고, 화재하중은 5분 이내에 $1,200^{\circ}C$의 화재하중을 가할 수 있는 RABT 화재시나리오를 적용하여 극한저항성능을 검토하였다. 본 연구는 향후 국내외 프리스트레스트 콘크리트에 대한 방호설계 및 폭발해석 등 관련 연구분야의 중요한 자료가 될 것이라 판단된다.

Enhanced Healing of Rat Calvarial Bone Defects with Hypoxic Conditioned Medium from Mesenchymal Stem Cells through Increased Endogenous Stem Cell Migration via Regulation of ICAM-1 Targeted-microRNA-221

  • The use of conditioned medium from mesenchymal stem cells may be a feasible approach for regeneration of bone defects through secretion of various components of mesenchymal stem cells such as cytokines, chemokines, and growth factors. Mesenchymal stem cells secrete and accumulate multiple factors in conditioned medium under specific physiological conditions. In this study, we investigated whether the conditioned medium collected under hypoxic condition could effectively influence bone regeneration through enhanced migration and adhesion of endogenous mesenchymal stem cells. Cell migration and adhesion abilities were increased through overexpression of intercellular adhesion molecule-1 in hypoxic conditioned medium treated group. Intercellular adhesion molecule-1 was upregulated by microRNA-221 in mesenchymal stem cells because microRNAs are key regulators of various biological functions via gene expression. To investigate the effects in vivo, evaluation of bone regeneration by computed tomography and histological assays revealed that osteogenesis was enhanced in the hypoxic conditioned medium group relative to the other groups. These results suggest that behavioral changes of endogenous mesenchymal stem cells through microRNA-221 targeted-intercellular adhesion molecule-1 expression under hypoxic conditions may be a potential treatment for patients with bone defects.

탈북자의 사회적응 지원을 위한 종합형 사례관리 모형의 제시와 그 실천 (Development and Application d A Comprehensive Case Management Model for Helping North Korean Refugees' Psycho-Social Adjustment in South Korea)

  • 본 연구는 지역 사회복지관의 사회복지사가 중심이 되어 운영할 수 있는 탈북자의 사회 적응을 위한 종합적 사례관리 서비스 모형을 제시한 후 그 것을 탈북자를 대상으로 실천해 보면서 향후 탈북자를 위한 서비스의 모형 수립과 관련한 사항을 제안하였다. 그 과정을 세별하면, 첫째, 프로그램을 통해 주로 개입해야할 사회 심리적 내용이 무엇인지를 알아보기 위해 연구자가 11명의 탈북자에 대한 비구조화된 개별 면접을 총 20회 실시하였다. 둘째, 탈북자와의 면접과 문헌연구를 통해 확인된 문제들을 소개하고 이러한 문제들에 대처하기 위한 프로그램 구성 요소를 치료적 측면과 사례관리 서비스적 측면에서 나누어 탐색하였다. 셋째, 탈북자의 사회적 적응 및 심리적 안정과 자립 정착 지원을 연계하며 지속적으로 실천할 수 있는 종합적 사례관리 프로그램을 제시하였다. 넷째, 제시된 프로그램을 탈북자 대상으로 적용 실천해 보았다. 프로그램 실천 대상자는 일차적으로 면접한 탈북자 중 사회 심리적 부적응 현상을 보임과 동시에 복합적 생계 문제를 표출시켜 개입이 필요하다고 생각되는 두 사람으로 하였다. 마지막으로 실천과정에서 드러난 문제점과 향후 보완책, 그리고 제안점들을 제시하였다.

자폐증 유사증상 발현 마우스의 점액 면역에 대한 연구 (Evaluation of Mucosal Immunity in BTBR T+tf/J Mice Resembling Autism Spectrum Disorder)

  • Objectives: This study was undertaken in order to evaluate a potential mechanism involved in gastro-intestinal problems observed in autistic subjects and uses an animal model of autism investigation. Methods: BTBR T+tf/J, a mouse strain with typical socio-behavioral characteristics of autistic subjects and FVB mice with highly social behaviors as the control strain were used. Both genders of mice aged three weeks and six months were used from four separate litters for each strain. Serum was prepared following cardiac puncture, and mesenteric lymph nodes were collected for in vitro stimulation and enumeration of major immune cell proportion. Results: The level of serum IgA was significantly enhanced in six-month-old BTBR mice compared with three-week-old BTBR, which was not observed with the FVB control mice. The serum IgE level was also higher among BTBR mice than among age-sex matched FVB mice, respectively. Considering the ratio of interleukin-4 vs interferon-gamma production from mesenteric lymph node T cells, skewedness toward type-2 reactivities was observed. In addition, the proportion of B cells in mesenteric lymph nodes was significantly higher in BTBR mice than in FVB mice. Conclusion: Upregulation of mucosal immunity related with enhanced type-2 immune reactivity observed in BTBR mice could be involved with the etiology of gastro-intestinal abnormalities in autism.
