Development of RPG-based Cognitive Behavioral Group Therapy for Reducing Delinquency in Adolescents (청소년의 비행 문제 감소를 위한 롤플레잉게임형식 인지행동 집단치료 프로그램의 개발)

  • The purpose of this study was to develop a program that reduces adolescent delinquency and enhances adolescents' social problem-solving, emotional regulation, and self-control skills and to assess the effectiveness of the program. The program was administered in the form of a role-playing game ("RPG") to increase the participants' motivation to participate in and the effectiveness of the therapy. The subjects in this study were 36 adolescents who engaged in delinquent behavior and their 18 homeroom teachers in middle schools located in Seoul and Gyeonggi Province. The subjects were randomly assigned to the experimental group or the comparison group. The experimental group received the proposed RPG-based cognitive behavioral group therapy while comparison group received problem-solving group therapy. Teachers in the experimental group were educated about how to guide their students following each therapy session while teachers in the comparison group did not receive any education. The dependent variables in this study were incidences of delinquent behavior and social problem-solving, emotional regulation, and self-control skill level. Each variable was assessed before treatment, immediately after the end of the full treatment program, and two months after the end of the full treatment program. At the end of the full treatment program, the experimental group engaged statistically significantly in fewer delinquent behaviors and displayed statistically significantly higher levels of social problem-solving, emotional regulation, and self-control skills than the comparison group. These differences persisted until the assessment two months after the end of the full treatment program. Moreover, the experimental group reported higher levels of satisfaction with the treatment program than the comparison group. The findings of this study suggest that this RPG-based cognitive behavioral group therapy program is effective at reducing adolescent delinquency, improving adolescents' social and emotional management techniques and strategies for avoiding delinquency, and in motivating delinquent adolescents to engage more actively in treatment.

The Children's Needs for the Child Psychiatric Therapy Environment as to the Type of Problem (아동의 문제 특성에 따른 상담치료환경에 대한 요구)

  • The purpose of this study is to propose how the interior of the child psychiatric therapy Environment should be designed according to different problems that children have. For the purpose, this research has been conducted with a literature review and an empirical survey. The subjects of the survey were 100 children, 58 male and 42 female, who were using child psychiatric therapy centers located in Busan. Findings are as follows: (1) Children with behavior problem($C_b$) showed a significantly higher satistaction with the entrance and the information in a given child psychiatric therapy space than children with emotional problem ($C_e$) did. (2) Spatial attitude of $C_e$ and $C_b$ were significantly differed. (3) Colors that were being preferred by $C_e$ were cold colors, for example, blue and indigo. While, $C_b$ were preferring warm colors such as red and yellow. (4) There was difference in the image of a entrance and classrooms at the child psychiatric therapy center that children had in mind, the two groups of children were quite differently feeling about the size and noise of the therapy space.

Pain Relieving Effect of Yakson Therapy for Infants (신생아 통증완화를 위한 약손요법 적용 효과)

  • Purpose: The purpose of this study was to identify the effects of Yakson therapy as a pain management tool oil the physiologic and behavioral reponses of infants with a painful heelstick procedure. Method: Infants were randomly assigned to a group that underwent a series of Yakson therapy and a control that received nothing before a heelstick. Heart rate, oxygen saturation, and NIPS were compared between the experimental (n=16) and control (n=16) infants during an undisturbed baseline and after a standard heelstick procedure Yakson therapy consisted of laying a hand on the back, and caressing the abdomen by hand for 5 minutes. Result: The pain scores of the Yakson group were lower than the control group. Foroxygen saturation, there were statistically significant differences between groups. For heart rate, there were no statistically significant differences between groups. Conclusion: This data suggests that Yakson therapy had a pain relief effect in behavior responses and $SaO_2$. Accordingly, Yakson therapy should be used as a nursing intervention for simple pain management for a heel prick.

Effects of Hominis Placenta Pharmacopuncture and Electroacupuncture Neuroprotection in Contused Spinal Cord of Rats (척수손상 흰쥐에서 자하거 약침과 침전기 자극이 신경성장인자 발현에 미치는 영향)

  • This study was designed to investigate the effects of Hominis placenta pharmacopuncture treatment and electroacupuncture therapy on the functional recovery and histological change, protein expression in spinal cord injury(SCI) rats. Experimental groups were divided into the Group I(normal control rat), Group II(Non-treatment after spinal cord injury induction), Group III(Hominis placenta pharmacopuncture treatment after SCI induction), Group IV (Electroacupuncture therapy after SCI induction), Group V(Hominis placenta pharmacopuncture treatment and electroacupuncture therapy after SCI induction). After operation, rats were tested at modified Tarlov test at 1 to 3 days with divided into 4 groups, and motor behavior test(BBB locomotor rating scale, Grid walk test) was examined at 3, 7, 14, and 21 days. For the observation of damage change and size of the organized surface in muscle and spinal cord, histopathological studies were performed at 21 days by H & E stain, and BDNF & NT-3 protein expression studies were performed at 21 days. Acco rding to the results, Hominis placenta pharmacopuncture treatment and electroacupuncture therapy can play a role in facilitating recovery of locomotion following spinal cord injury. Specially, Hominis placenta pharmacopuncture treatment and electroacupuncture combimed therapy after SCI induction was most improvement in functional recovery, BDNF, and NT-3 protein synthesis.

Effects of Participation in Physical Group Occupational Therapy on Cognitive Function, Behavioral and Psychological Symptoms of Dementia(BPSD), Social Behavior Function in the Elderly with Wandering Dementia (배회가 있는 치매노인에게 신체 그룹 작업치료 참여가 인지기능, 심리행동증상 및 사회적 행동기능에 미치는 효과)

  • Objective : The purpose of this study was to effects of the Participation in physical group occupational therapy on cognitive function, neuropsychiatric, social behavior in elderly with wandering dementia. Methods : Thirteen elderly with wandering dementia who live in nursing home in this study. Assessment tools were made by using Cognitive function scale(CFS), Algase Wandering Scale-V2(AWS-2), Neuropsychiatric Inventory-Questionaire(NPI-Q), social-behavioral function measure(SBFM). physical group occupational therapy performed for forty minute, once a week for 9weeks. the assessment tools were made twice: before, after the intervention. Results : Significant differences between pre-treatment and post-treatment were found in the AWS(p<0.05) score and SBFM(p<0.05) score, NPI-Q behabioral and psychological level(p<0.05) score. The cognitive function and neuropsychiatric number showed improvement. but score was not significantly(p>0.05) difference after physical group occupational therapy Conclusion : The findings suggest that the physical group occupational therapy used in this study may be helpful and effectiveness in elderly with wandering dementia, further studies with more subjects and longer treatment period, including will be performed to justify these findings.

Preliminary Study on Developing Protocol for Music Therapy Assessment for Cognitive and Emotional-Behavioral Domain using Rhythm (MACED-Rhythm) (인지 및 정서행동 영역에서의 음악치료 사정을 위한 리듬 프로토콜(MACED-Rhythm) 개발 예비 연구)

  • Assessment in music therapy is a vital part for both the therapist and client in the process of therapy. Based on what is assessed, objectives are identified to formulate specific action procedures and strategies. The existing assessment tools involve lists of skills and behaviors in developmental domains without the music assessment protocol. In this study, the authors attempted to develop an assessment protocol using rhythm production for assessing skills in cognitive and emotional-behavior domain, namely Music Therapy Assessment for Cognitive and Emotional Behaviors (MACEB). The test items of the MACEB-Rhythm were developed using rhythmic patterns varying in terms of item difficulty, which are based on the various degree of clarity in the grouping/gestalt, saliency in part-whole relationship, and complexity in repetition vs. variability. Also the developed tool purported to examine one's level of emotional behavior trait by analyzing performance of musical parameters such as tempo, pacing, and loudness in the reproduced output. In order to verify the logical sequencing of test items, firstly 61 subjects participated in verifying the item difficulty for the selected 15 pilot items. The test items were revised and re-sequenced based on the gathered scores of item difficulty. In the second procedure, seven experts in the fields of music education, music therapy and music psychology whose research interest lie in music cognition revised the developed rhythm protocol items focusing on learning sequence, cognitive process and feasibility for skills assessment. The study attempted to provide foundations for using rhythm as an assessment protocol prior to its verification of assessment validity and reliability.

The Effects of Parent's Nutritional Education for Body Weight Control of Obese Children (부모의 영양교육이 비만아동의 체중조절에 미치는 효과)

  • The purpose of this study was to investigate the effects of parent's nutritional education for body weight control of obese children. The weight control program include nutritional education, exercise and behavioral therapy during 20 weeks. Twenty- three children completed this program, the children were divided into two groups by control group and parent's nutritional education group. Parents volunteered to participate in a 4 week nutritional education program for parents and contact the therapist at least once per week to help their obese children. The results from this study were as follows. There were not significant differences in anthropometric values after weight control program between two groups. Triglyceride(TG) level in serum was decreased after weight control program in group of parent's nutritional education, but there was not significant difference. Parent's nutritional education did not add improvements in weight and fitness, but the children of parent's nutritional education group showed increased general self-worth upon completing the program(p<0.05) whereas the other children of control group did not. Also there were desirable changes of exercise and life habits in group of parent's nutritional education group.

Effect of Bilateral Arm Movement on Brain and Muscle Activity in Chronic Stroke Patients (양손 운동이 만성 뇌졸중 환자의 뇌활성도와 근활성도에 미치는 영향)

  • PURPOSE: This study investigated the neurophysiological and behavioral adaptation during one or both hands movement in chronic stroke patients. METHODS: The study included sixteen hemiplegic stroke patients. Neurophysiological data (brain activation and muscle activation) were examined by electroencephalography (EEG) and electromyography (EMG), and behavioral adaptation was examined by wrist extension angle during wrist extension with one hand or both hands. Outcome variables of one hand or both hands were; mu rhythm of the EEG, EMG amplitude of wrist extensor and flexor muscles, and wrist angle of Myomotion 3D motion analysis. RESULTS: Our results revealed that wrist extension angle was significant increased during both hands movement compared to one hand movement (p<.05). Furthermore, in affected sensorimotor area, there was significant increase in the brain activation during both hands movement compared to one hand movement (p<.05). However, there was no significant different between one hand and both hands movement in muscle activation (p>.05). CONCLUSION: According to the findings of this experiment, bilateral arm movement improved brain activity on affected sensorimotor area and wrist extension angle. Therefore, we suggest that bilateral arm movement would positive effect on stroke rehabilitation in terms of increase in brain activation on affected motor area and wrist extension during bilateral arm movement.

  • There have been many trials of clinical efficacy of multi component and single component treatments for irritable bowel syndrome(IBS). We reviewed effects of non phamacological treatments in the IBS. Though the efficacy of multi-component approaches was unclear, several results suggest that cognitive behavioral therapy was effective in improving gastrointestinal symptoms of IBS. As a single component, cognitive therapy and relaxation with or without biofeedback could improve the symptoms and psychological health of IBS patients. Yoga, meditation, self-help information and hypnotherapy could be applicable to IBS.

Treatment of Morsicatio Buccarum by Oral Appliance: Case Report

  • Morsicatio buccarum is a condition caused by chronic cheek biting. It means frictional hyperkeratosis by repetitive cheek biting, which may be associated with obsessive compulsion. Clinically it presents as rough, shaggy, whitish, often peeling surface. We report a 9-year-old female patient who complained oral ulceration on both buccal mucosa. The initial diagnosis was oral candidiasis because of whitish plaques which were peeled off. Topical antifungal agent was ineffective and the symptom did not disappear. By incisional biopsy, she was diagnosed with morsicatio buccarum. A soft oral appliance was placed in the patient's oral cavity. Although morsicatio buccarum improved significantly when the appliance was used, the oral lesion had a tendency to recur, when the patient stopped using the appliance. In case of recurrence, cognitive behavioral therapy for obsessive-compulsive disorder is needed for the fundamental treatment.
