We cultured canine kidney(MDCK) cells stably expressing aquaporin-2(AQP2) on collagen-coated permeable membrane filters and examined the effect of extracellular ATP on arginine vasopressin(AVP)-stimulated fluid transport and cAMP production. Exposure of cell monolayers to basolateral AVP resulted in stimulation of apical to basolateral net fluid transport driven by osmotic gradient which was formed by addition of 500 mM mannitol to basolateral bathing solution. Pre-exposure of the basolateral surface of cell monolayers to ATP(100 ${\mu}M$) for 30 min significantly inhibited the AVP-stimulated net fluid transport. In these cells, AVP-stimulated cAMP production was suppressed as well. Profile of the effects of different nucleotides suggested that the $P2Y_2$ receptor is involved in the action of ATP. ATP inhibited the effect of isoproterenol as well, but not that of forskolin to stimulate cAMP production. The inhibitory effect of ATP on AVP-stimulated fluid movement was attenuated by a protein kinase C inhibitor, calphostin C or pertussis toxin. These results suggest that prolonged activation of the P2 receptors inhibits AVP-stimulated fluid transport and cAMP responses in AQP2 transfected MDCK cells. Depressed responsiveness of the adenylyl cyclase by PKC-mediated modification of the pertussis-toxin sensitive $G_i$ protein seems to be the underlyihng mechanism.

Purpose: Using comprehensive and valid instrument, MDS-HC 2.0, this study aimed to analyze the functional status and to evaluate the care needs of the community-dwelling disabled with cerebral impairment. Method: With a convenient sample of 88 disabled with cerebral impairment, the data were collected at a community health center located in rural area in Choongchung providence in August 2005. Subject's functional status and care needs were evaluated using Minimum Data Set-Home Care version 2.0. Result: Significant proportion of subjects were totally dependent for locomotion-outdoor (26.1%), personal hygiene (24.1%), bathing (24.1%). For IADLs, over 40% of subjects were totally dependent for ordinary house work, managing finances, or shopping. Top five ranked care needs were preventive health care measures (100%), communication disorders (71.6%), visual function (55.7%), health promotion (52.3%), and pressure ulcers (48.9%). The proportion of triggered clinical assessment protocols were significantly higher in disability level I group for the risk of institutionalization (p=<.001), communication disorders (p=.004), cognitive problems (p=.001), pressure ulcers (p=<.001), skin and foot conditions (p=.010), and urinary incontinence and indwelling catheters (p=<.001). Conclusions: It is necessary to provide community based rehabilitation services that are individualized for their service needs thus enhance optimal level of functioning.
This is a study on the change of the Medievalmen's underclothes. Underclothing includes all such articles, worn by either sex, as were completely or mainly concealed from the spectator by the external costume. Throughout the Middle Ages, the underclothing of both sexes possessed characteristic inherited from earlier times. Its function was purely utilitarian. It was not used to express class distinction and only very indirectly to enhance sex attraction. The idea of underclothes was associated with the idea of the body's sinfulness. Underclothes could also symbolize the discipline of humility. To appear clad only in underclothes was a voluntary form of selfabasement often practised by pilgrims. The notion that underclothes might express the spirit of the changing times, in sympathy with the outer clothing, did not enter the medieval mind. The length of the shirt was less than that of the modern garment, especially after the middle of the fourteenth century. The sleeves were somewhat full, without cuffs, and cut straight. During the second half of the twelfth century, when braies become definitely an undergarment. In the fourteenth century braies became shorter and shorter and the shirt could no longer be tucked in but hung outside over the thighs. After 1340 the costume on the surface somewhat resembling modern 'tights', began to reveal display the shape of the male leg as a form of sex attraction. By the close of the Middle Ages, braies had become less like loincloths, and more like modern bathing trunks. The peasant and laborer did not attempt to keep pace with the changes of fashion but, continued, as a rule, to wear the long braies of their forefathers. The higher ranks how this garment reflected the changes of the mode of outer costumes.
This study finds out the aged consumers' needs on the developing elderly friendly and sensitive bathtub. This bathtub focuses on enhancement of multi-senses and enhancement of health, safety & convenience during bath. The enhancing multi-senses would have four functions; therapy with color and aroma, the massage with micro water bubbles, the visual and auditory with TV & cinema, and the auditory with music and radio. Also, enhancing others of bathtub would have seven functions; emergency bell, blood-pressure check system for health condition, water temperature indicator, keeping water warm, reading a book in that, and controlling of house entrance door and a getting the phone during bath. The aged consumers' needs find throughout a questionnaire survey, and 497 data was analyzed. The main results are as follow. They consider a full bath behavior as a healthcare. They have an intention to purchase or use the elderly and friendly bathtub, and they prefer using it in common facilities to using it at home. They highly need health related function of bathtub, which are massage (64.4%), blood-pressure check system (55.1%) and aroma therapy (45.7%). The emergency bell (67.0%), keeping water warm (62.2%), and water temperature indicator (49.7%), related to safety & information function of bathtub, are needed. Also, they think tolerance for error and low physical effort in bathtub design alternative, opening door and inside sitting support, to be important.
The objectives of this study were to characterize the health status, the requirement of home nursing care, and dependency of daily life of the elderly so that the result could be used to provide basic data necessary for establishing home nursing care practices. The study subjects were 249 elders over 65 years old who resided at home in low income areas of three districts in Pusan. The field survey was conducted from December 27, 1993 through January 27, 1994, with a structured questionnaire. Statistical analysis was done by X²-test and Student's T-test using SPSS/PC/sup +/ program. The result were as follows; 1. Alcohol drinking rate was 50.0% for men and 27.7% for women, and cigarette smoking rate was 77.8% for men and 27.1% for women. 92.9% of elders aged 65-75 exercised regularly as compared to 81.9% of elders aged 75 and over(p<0.05). 2. 63.9% of male and 85.9% of female elders had chronic diseases showing singnificant differences(p<0.01), and both sexes had 1.7 kinds of chronic diseases in average; musculoskeletal diseases were the most common in 25.7%, while 6.8% of the elderly had cardiovascular diseases. 3. Partially movable elderly was 6.9% for men and 6.8% for woman, showing 47.1% of causes being cerebrovascular accidents. 4. The home nursing requirement was the highest for medication(66.3%), intramuscular injection(53.8%), and measurement of blood pressure·body·temperature·pulse rate(47.0%). 5. Among Physical Activities of Daily Life, bathing(10.4%) and movement(4.0%) showed high dependency rates, and among Instrumental Activities of Daily Life, phone calling(25.3%), shopping(24.5%), going out by bus(22.5%) showed high dependency rates. 6. For five categories of home nursing showing high requirements such as physical health examination, blood pressure·temperature·pulas rate measurements, medication, and intravascular injection, the requirements were different by age groups, education, and marital status(p<0.05).
The WeeFIM is an outcome measure used worldwide to evaluate the functional abilities of children. The aim of this study was to evaluate the psychometric properties of WeeFIM in Korean children with cerebral palsy (CP) using the Rasch model. The mean age of the participants (92 boys and 53 girls) was 10.6 years (SD=2.3, range 5~15 years). The Winsteps software was used for analyzing the internal construct validity and reliability of WeeFIM. For analyzing the internal validity the motor and cognitive area items of the WeeFIM were analyzed both together and separately. When all 18 items were analyzed 4 were considered to be misfits; upper extremity dressing, lower extremity dressing, toileting, and comprehension. When only the 13 motor items were analyzed, toileting, bladder management, and bowel management were considered misfits. In addition, only comprehension was considered as a misfit among the 5 cognitive items. The most difficult motor items were stair climbing, and bathing. The simple ones were eating, bowel management, and bladder management. The most difficult cognitive item was problem solving, and the simplest one was comprehension. The person separation indexes and reliability for combined and divided instruments were reported as excellent. These results demonstrated the applicability of WeeFIM to Korean CP children with satisfactory reliability and validity. Further studies should include young children with CP and compare item difficulty among the different types of CP. In addition, the Korean normative data of nondisabled children should be used to compare the cultural differences between Korea and other countries.

Purpose: This study was performed to investigate prevalence related characteristics and management status in children with atopic dermatitis(AD) for developing health promotion strategies led by community. Methods: Data were collected from 6,562 children with AD in preschool and elementary schools in a district of Ulsan city. Results: The majority of children(44.4%) were onset in less than 2 years after the birth. Seventy six point seven percent of subjects had skin rash and the pathologic lesion of them were flexion of upper extremities and extension of lower extremities. The prevalence rate of the respondents was 30.3%. In bathing, the children in AD group had used soap more frequently than children in normal group. The most preference type of complementary therapy in frequency order were aloe, aroma oil and Pasture. Conclusion: More effective management strategy for alleviating symptom and preventing advanced disease such as asthma and allergic rhinitis should be developed. And also scientific evidence for using complementary therapy is necessary to be provided.

Purpose: The aim of this study is to identify the influence of visual and hearing impairment on the activities of daily living of community dwelling elderly. Methods: Data were collected by home visiting interviewers from 452 older people aged 65 years or older living in community. Resident Assessment Instrument MDS-HC(2.0version) was used for data collection. Data analysis for descriptive statistics, Chi-square test and multiple regression was made by SAS 6.2 Results: 34.7% of the subject had hearing impairment and 64.3% had visual impairment Among IADL. one half of them were dependent in ordinary house work and meal preparation. In the case of ADL. 13.9% of subjects were dependent in bathing and 8.9% in personal hygiene. There was significant difference in IADL performance by visual and hearing impairment On the other hand, ADL performance showed the significant difference. only in the case of hearing impairment. As the result of input of visual and hearing impairment in the process of regression. variances were increased from 3% to 11%. Conclusions: Large proportions of older people living in the community have visual and hearing impairment. It could be confirmed that hearing and vision were significant factors influencing on IADL performance of older people. Intervention and support policy for elderly needs to focus on improvement of visual and hearing impairment.

According to industralization the percentage of working women with children is on the continuous increase. There has been a growing concern over important health problems of middle-aged women, especially with working women. But we didn't know how to care of themselves yet even if it was importment to fint out that. So this reseach was an attempt to identify the nature of experience of their health management so that it is to be contributed to be knowledge development of women' health management strategy. The phenomenological approach in qualitative studies was used to serve this purpose. Subjects were 10 middle-aged working women having children in well-being state. The result of this study was as follows : 'Diet pattern' was to unbalanced and irregular but they tried to correct balanced and harmony dietary life. 'Exercise' was very simple and irregular. They have closed-minded of kinds of exercise. 'Controlling the environment and their minds' was to obtain emotional stability and be free from stress. They put emphasis on refraining from being overtiring and watching their condition carefully. They always tried to keep the peaceful mind. 'Reducing fatigue' was to be bathing, hot streamed-bath, finger pressure therapy, massage, singing, communication with others and maintenance of personal relationships. 'Identification of self-esteem' was very key point to maintain their health. Working regulary gave them to please and value of the life. 'Work oriented life pattern' was to be harm their health state so that it need to be available clinics to check up their health state after work. The rest of this study showed that it need to be social support for working women to work with comfort. Further study regarding working women engaging in the lower level is nesessary.

Especially in NICU(Neonatal Intensive Care Unit), handwashing is an important factor in decresing nosocomial infections due to reduced immunity, prematurity and various invasive procedures. The purpose of this study was to investigate the basic characteristics related to handwashing by NICU nurses. It was composed of three parts ; questionnaire survey I (genoral characteristics of handwashing), questionnaire survey II(the awareness degree of handwashing importance) and actual performance. This study was performed from Oct. 8 to 14 in 1994. The results of this study were as follows ; 1. Frequency of handwashing were different by shift of working about 47.1% of the respondents. They were washing most frequently during day-shift about 75% of above. According to self evaluation to handwashing frequency, 64.7% of the respondents said their frequency of handwashing was inadequate, because of being too busy.(45.1%), bothering caused by detergent or disinfectant(17.6%), and too far from handwashing facilities(17.6%). 2. The most common handwashing agent was soap(88.2%), 52.9% of the subjects experienced adverse effects after handwashing ; rough hand(44.5%), dryness(33.3%) All subjects washed their hands with running water, and 70.6% of the subjects washed upto wrist. In duration of handwashing, 52.9% of subjects washed hands 5 to 10 seconds, 29.5% of them did 11 to 15 seconds. 3. 29.4% among subjects had participated to take in the handwashing educational program. About 60% responded they would like to take the course of handwashing if possible. 4. The important nursing activities that need handwashing were chaning of caring ostomy, suctioning, chaning IV dressing site. On the other hand, they responded handwashing was not essential before dealing with omitus, before and after transfering machine, before chaning diaper(stool) 5. Handwashing performance was 61.7%. Among seven nursing activities, suctioning(73.4%) was the highest, the next was dealing with discharge or sampling(71.1%), the lowest was bathing(34.6%). The performance was better after(70.2%) nursing activities that before(52.5%), and day(63.6%) or evening(68.3%) shift than night(56.7%)
