The Stress and Adaptation of the Spouses of Patients with Gynecological Cancer (여성생식기암 환자 배우자의 스트레스와 적응)

  • Purpose: This study was undertaken to obtain the basic data on the spouses‘ stress and adaptation caring their wives affected by gynecological cancer. In addition, moderating variables for adaptation, family resource and situational definition, were also investigated. Method: This study was descriptive survey research work by questionnaire on the 71 spouses hospitalized in an university hospital located on Gwangju from Sep. 2001 to Oct. 2002. Results: The mean score of stress was 2.27, and the mean score of adaptation was 3.12. The correlation between stress and adaptation was significant (r = -.692, p = .000). The adaptation score of high stress group with high family resource was higher (p = .041) than the group with low family resource. The adaptation score of high stress group with high situational definition was higher (p = .033) than the group with low situational definition. Conclusion: This study showed that the spouses experienced a similar level of stress compared with the level of families with other type of cancer. This study also showed that the stress and adaptation levels were different depending upon the family resource and situational definition. This aspect should be considered for the development of nursing intervention program for the spouse of patients with gynecological cancer.

A Comparative Study on Nurses' Organizational Culture and Job Satisfaction according to the Hospital Size Differences (병원규모에 따른 간호사의 조직문화유형과 직무만족 비교연구)

  • Purpose: This study was conducted to compare the characteristics of organizational structure and occupational satisfaction among nurses in general hospital and small to medium-sized hospital and to investigate the affecting factors on their occupational satisfaction. Methods: The study was based on the cross-sectional descriptive survey. A self-report questionnaire was used to collect data from 343 nurses between June and July, 2010. Data were analyzed by ${\chi}^2$-test, t-test, ANOVA, Scheffe's test, Pearson's correlation coefficient, and multiple regression using the SPSS/WIN 14.0 program. Results: Hierarchical structure was dominant in general hospital whereas relationship was highly valued in small to medium-sized hospital. Occupational satisfaction was positively correlated with work environment built on relationship, innovation and tasks. Factors significantly influencing on occupational satisfaction in general hospital included innovative work environment, nurses' income and their health status ($R^2$=40.3%). For the small to medium-sized hospital, they included innovative work environment, satisfaction in life, tasks and professionalism ($R^2$=40.4%). Conclusion: Organizational structure, especially innovative work environment and relationship-oriented attitude had a significant influence on nurses' occupational satisfaction. Therefore, nursing administrators have to develop and consider organizational structure to improve occupational satisfaction.

A Survey on Physical Health Status and Health Behavior Practice in Elderly People (일 지역사회 노인의 신체적 건강상태와 건강행위에 관한 연구)

  • Purpose: The purpose of this study was to Identify the physical health status and health behavior practice of elderly people in order to provide basic data for effective nursing interventions to promote health and quality of lift. Method: The participants for this study were 299 elderly persons in D city. Data were collected by interview with a questionnaire. Results: Average score for the physical health status of the participants was 3.98. There was a significant difference in average scores for physical health status for the variables age, sex, marital status, education level, religion, monthly income, source of living expense, perceived health status, alcohol use and type of household. The average score for the health behavior practice of the participants was 99.52, which means that elderly persons have good health behavior. There was a significant difference in average scores for health behavior practice for the variables age, sex, education level, perceived health status and type of household. Perceived health status, education level and alcohol use explained 50.6% of the variance for physical health status. Perceived health status and education level explained 27.4% of the variance for health behavior practice. Conclusion: To promote health behavior in elderly people, it is necessary to develop nursing interventions that take into consideration sociocultural traditions and demographic characteristics.

Working with Vulnerable Families: A Nurse Home Visiting Perspective (취약계층의 방문간호 서비스 요구 특성)

  • Purpose: This study defines a vulnerable group in a community that has become the main target of a national health project also, it is descriptive research to suggest an evidence-based direction to meet their deficit health-related needs, Method: This research examined 833 families and 1,835 family members of the financially vulnerable class that was registered in a home visiting program of a public health center. Among them, 892 persons who had health problems, and their family members were examined in detail to find out their characteristics of vulnerability and health needs by assessment during a nurses home visit. Frequency distribution, stepwise-regression and factor analysis were used to analyze the data. Result: The vulnerable group that was defined with social indexes set as standards, involved substantial characteristics of vulnerability. The characteristics of demand showed tendencies of being clustered in 5 factors needs of intensive nursing care, chronic nursing care problems and helplessness, maintenance of family functioning with a disability, deficient problem solving ability, and simple financial fragility. Conclusion: Categorization of needs is an evidence-based estimator of workload in nurse home visiting services, and can be used as a basic resource for direction to meet the deficit needs of a vulnerable group.

Effectiveness of the Self-determination Theory based a Motivational Interviewing YOU-TURN Program for Smoking Cessation among Adolescents (자기결정성 이론 기반 동기면담 유턴 프로그램이 청소년 금연에 미치는 효과)

  • Purpose: In this study, the effectiveness of a motivational interviewing smoking cessation YOU-TURN program for adolescents was examined. The program was based on the self-determination theory. Methods: The study was carried out with a nonequivalent control group pretest-posttest design. Participants in the present study were 136 high school students living in D city. The students were assigned to the experimental group (n=52) who participated in the motivational interviewing smoking cessation YOU-TURN program based on self-determination theory, or to the control group (n=84) who participated in a general smoking cessation program. Data were collected from September 1, 2013 through April 30, 2014. Collected data were analyzed using SPSS PC+ 21.0 with Chi-square test, Fisher's exact test, t-test, Mann-Whitney U test, Repeated Measures ANOVA, and MANOVA-Wilk's Lambda. Results: The experimental group had a significant increase in basic psychological needs, and duration of quitting-smoking in comparison with the control group. The experimental group had a significant decrease in cigarettes smoked per day and cotinine in urine in comparison with the control group. Conclusion: The motivational interviewing YOU-TURN program, when delivered to adolescents who smoked, was effective in discouraging smoking, and can be utilized as an effective nursing intervention for adolescents who smoke.

Development of a Measurement of Stress for Hospitalized Schizophrenic Patient (조현병 환자의 입원 스트레스 측정도구 개발)

  • Purpose: This study was done to develop a measurement for stress experienced by patients with schizophrenia during hospitalization. Methods: The preliminary tool was developed through in-depth interviews and a validity verification test of content. For data collection, 15 inpatients with schizophrenia were selected as participants for in-depth interviews and 195 patients admitted to one of eight psychiatric hospitals in four provinces were recruited as participants to test reliability and validity of the preliminary tool. Results: The questionnaire was developed as a four-point Likert-type scale in a self-report form with 28 items. Factor analysis showed 28 items in six factors. Factors were named 'Unjust human rights infringement', 'Futureless life', 'Alienation from other family members', 'Infringement of basic needs', 'Infringement of personal preference' and 'Inconvenience of shared living'. The six factors explained 63.5% of the total variance. Cronbach's alpha for the total items was .93 and for the factors ranged from .65 to .87. Conclusion: A tool to measure stress in patients hospitalized with schizophrenic was developed based on identified hospitalization stress experiences. Study results indicate that this tool can be used to evaluate hospitalization stress in these patients and will contribute to establishing nursing interventions for relief of hospitalization stress.

A Study of Care Burden, Burnout, and Quality of Life among Family Caregivers for the Elderly (노인 주 부양자의 부양부담감, 소진감 및 삶의 질에 대한 연구)

  • Purpose: This study was a survey to provide basic data about nursing interventions for improving the quality of life among family caregivers for the elderly by examining their care burden, burnout, and quality of life, and by confirming the correlation between each of them. Method: The subjects were 215 people in Seoul and Gyeonggi Province who understood the purpose of this study and participated voluntarily from April 1 to June 4, 2007. Data was analyzed by the SAS program. Result: 1. Concerning primary caregivers of the elderly, it was found that their care burden was slightly high, burnout was high and quality of life was good on the whole. 2. When it comes to the correlation among care burden, burnout, and quality of life among family caregivers, it was found that burnout increases in proportion to care burden, quality of life decreases as care burden increases, and bigger burnout leads to a lower quality of life. Conclusion: The quality of life among primary caregivers should be improved by reducing their care burden and burnout. Thereby, a priority might be considered for admission to nearby, comfortable nursing homes or failing that, home visiting services, rather than family support in the home.

Impact of Lifestyle and Health Examination Results on Sick Leave in Commercial Airline Pilots (운항승무원의 생활습관과 건강검진 결과가 병가에 미치는 영향)

  • Purpose: This study aims to compare the general characteristics, life-style, health examination results, and sick leave days by airmen medical examination decision and to investigate factors affecting sick leave days. Methods: We obtained data from 2,361 Korean pilots who worked for a commercial airline. Comparison of the results by airmen medical examination decision (Fit or waver) was conducted using the 𝝌2 test or Fisher's exact test. Factors affecting sick leave days were analyzed using logistic regression. Results: Age, smoking history, blood pressure, obesity, and fasting blood sugar level were significantly different between the Fit and Waver groups. Rate of using sick leave long-term was higher in the Waver than in the Fit. Sick leave days were significantly associated with age, habits of drinking, and smoking in the Fit group. Conclusion: This study demonstrated the health risk factors that affect the number of sick leave days. By providing basic data for the health care of workers, it is expected to be applicable to the provision of health promotion and disease prevention programs for workers.

A Study on the Relationship of Marital Intimacy, Depression, and Menopausal Symptoms in Middle-aged Women (중년여성의 부부친밀감, 우울과 갱년기 증상의 관계)

  • Purpose: This study aims to identify the relationship between marital intimacy, depression, and menopausal symptoms, and to provide basic data for the health promotion of middle-aged women. Methods: A sample of 231 middle-aged women was recruited from four cities in Korea. The survey was conducted with participants using self-reported questionnaire from July 9 to November 11, 2011. The data were analyzed using descriptive statistics, t-test, ANOVA, and Correlation Analysis. Results: There were differences of marital intimacy scores that were statistically significant in educational background (F=6.88, p=.001), religion (t=3.96, p<.001), own job (F=4.47, p=.012), own salary (F=3.01, p=.019), spouse's salary (F=6.72, p<.001), ownership of residence (t=4.00, p<.001), leisure with spouse (t=-5.05, p<.001), current treatment of disease (t=2.22, p=.027), and current disease (t=2.53, p=.012). Marital intimacy was correlated with depression (r=-0.61, p<.01) and menopausal symptoms (r=-0.39, p<.01). Conclusion: Marital intimacy in middle-aged women is closely related to both depression and menopausal symptoms. The nursing program that improves marital intimacy will be useful to promote the health of middle-aged women.

Factors associated with Hepatitis A Preventative Behaviors among University Students (일 지역 대학생의 A형간염 관련 실태와 예방행위에 영향을 미치는 요인)

  • Purpose: The purpose of this study is to identify health beliefs and knowledge related to hepatitis A vaccination (HAV). Preventative behaviors related to HAV were also examined. Methods: The convenience sample of 332 students were drawn from a university in Chung-nam province. The results were analyzed using descriptive statistics, t-test, ANOVA, Scheff$\acute{e}$ test, Pearson's correlation coefficient, and stepwise multiple regression with SPSS for Windows 21.0 software. Results: Vaccination rates for hepatitis A were 23.4%. The mean scores of health beliefs, knowledge and preventative behaviors related to hepatitis A were $2.38{\pm}0.25$, $0.34{\pm}0.30$, and $3.15{\pm}0.40$ respectively. The factors found to be related to hepatitis A preventative behaviors were HAV, having the HAV antibody and health beliefs. Conclusion: An experience of HAV, having HAV antibody, and positive health beliefs related to hepatitis A may be necessary to increase voluntary hepatitis A preventive behaviors among university students. It is essential to develop the strategy of educating university students about HAV and having HAV antibody as well as reinforcing health beliefs about hepatitis A which prevent the hepatitis A occurrence.
